Pseudohypoaldosteronism*

Aneurysm of the vein of Galen is a rare midline arteriovenous malformation, usually presenting with cardiac failure in infancy or with hydrocephalus and raised intracranial pressure in older children. We experienced a case of the vein of Galen aneurysm diagnosed with computed tomographic (CT) features and magnetic resonance image (MRI), a new imaging modality. Our case was a 3 day-old male neonate and had a nonspesific symptom of high fever without cardiac failure. A brief review of related literature was made.

BACKGROUND: The number of patients of doxylamine overdose has increased, but there were little clinical reports. The purpose of this study is review the patients of doxylamine overdose for aid clinical decision making of patients disposition. METHODS: We reviewed medical records of patients who ingested doxylamine succinate retrospectively from Jan. 1, 1996 to Dec. 31. 1996. Data that was recorded were followed: age, sex, psychiatric history, amounts of ingestion, time interval from ingestion to arrival to hospital, initial vital signs and symptoms, EKG, chest PA, Lab. findings, treatment, patients disposition, length of hospital stay, complication. RESULTS : 1) Total number of patients was ninety and average dosage of ingestion was 1062mg. 2) Tachycardia was the most frequent anticholinergic symptom(46%). 3) Routine lab. findings was not abnormal except one patient who was diagnosed as Rhabdomyolysis. 4) Gut decontamination was performed in patients who had arrived at hospital in 2-3 hours, and who had ingested large dose of drug. 5) Five patients were admitted to Psychiatric dept. and one patient was admitted to emergency dept. for treatment of rhabdomyolysis. 6) The mean duration of hospital stay was 6.5 hours. CONCLUSION: The patients who ingested overdose of doxylamine was managed safely in emergency department, but we should warn against the possibility of rhabdomyolysis.

Mucocutaneous Lymph Node Syndrome(MLNS), a recently recognized entity was first reported by Dr. T. Kawasaki in Japan in 1967 and then about 12,000 cases have been reported since 1967; it is now being found in the United States, but it's etiology is still unknown. The characteristic symptoms is high fever, hypermia of conjunctivae and sclera, reddening of lips and oral cavity, palmer and solar erythema with desqumation and swelling of cervical lymph nodes. Authors insisted that this entity was quite distinct from periarteritis nodosa, Steven-Johnson syndrome, scarlet fever, erythema multiforme and in many ways. We experienced four cases of it and all cases were male and three or them were under the age of two years. All cases had the episodes of high fever, and three of them showed the characteristic symptoms of desquamation, indurtive edema and cervical lymphnodes swelling. In our cases, the fever was continued for about 13 days and desquamation appeared near the end of second weeks of illness. This disease has been treated with various antibiotics and aspirin, but the key point of treatment is how to prevent the complications. From these cases, it may be an interesting disease entity which might be evaluated by careful investigations and follow up study.

Different types of aphasia resulting from stroke, as well as their anatomical correlates, are well characterized. In contrast, the literature on ictal aphasia is relatively scant. A 58-year-old woman had 4 times of transient motor aphasia. No other behavioral abnormalities and focal sensorimotor neurological deficits were revealed. Brain MRI showed a small meningioma at the left sphenoid ridge. Electroencephalogram showed occasional sharp waves on the left inferior frontal area. We report a rare case of ictal aphasia associated with meningioma.

We report a case of polycythemia vera combined with coagulation disorder. The patient was 54 years old man who complained of continuous bleeding after incision of skin abscess 20days ago. Laboratory tests were revealed prolonged aPTT and slightly prolonged PT. Coagulation factor, I, VIII, IX, XI and fibrinogen decreased, however FDP did not increased. It appears that patient with polycythemia vera have chronic activation of coagulation system, probably initiated by activation of factor XII. Platelet aggregation test to ADP, collagen, epinephrine was also revealed poor response.

Cavernous lymphangioma in mediastinum is a rare congenital malformation of lymphatic system and is caused by lack of adequate drainage from the sequestrated lymphatic vessels to the venous system due to insufficiency or atresia of the efferent lymphatic channels The authors experienced 10 month-old male patient with cavernous lymphangioma in mediastinum documented by chest CT and open biopsy The review of the literature was made briefly.

Objective: The study aimed to investigate whether mindfulness could mitigate the impact of job and interpersonal demands arising from stressful organizational cultures in predicting depression during the COVID-19 pandemic. Methods: A total of 280 Korean physical therapists participated in this study. To verify the mediated moderation model, this study implemented an SPSS 26.0 PROCESS Macro (v.3.5) that was based on the verification of Hayes' conditional indirect effect. Results: The study confirmed the moderated mediation effect of mindfulness on the mediating relationship between stressful organizational culture, job demands/interpersonal demands, and depression. Moreover, the conditional indirect effect of mindfulness was found to be statistically significant. However, the impact on interpersonal stress differed from anticipated outcomes. Conclusions Mindfulness, applicable within the framework of the Job Demands-Resources model theory, served as a resource variable and moderated the impact of organizational culture on members' depression. Amidst ongoing debates on mindfulness effectiveness, this study partially confirmed its potential within the Job Demands-Resources model, suggesting a role in mitigating the influence of organizational culture on members' depression as a resource variable.
