1.The results of combined therapeutic modalities for hepatoblastoma.

In hepatoblastoma, encouraging cure rates have been achieved with recent advances in chemotherapy and surgical techniques. The aim of this study is to evaluate the role of combined therapeutic modalities and surgical resection in hepatoblastoma. Fifteen cases of hepatoblastoma were treated from January 1993 to August 2000. Six patients had resectable tumors at initial diagnosis. All underwent surgical resection and in four patients postoperative adjuvant chemotherapy was needed. Nine out of 15 patients had unresectbale tumors at initial diagnosis, and preoperative chemotherapy was applied. There was one operative mortality and 14 patients showed good prognosis after surgery. Although various treatment modalities should be combined for the unresectable hepatoblastoma, surgical resection remains the major curative procedure.

2.Preoperative Chemotherapy in Gastric Cancer.

Gastric cancer is the most prevalent cancer in Korea and comprises the second cause of cancer death. Surgery only can provide chance of cure, but most locally advanced cancers recur after a curative resection, even though important advances in the surgical and nonsurgical treatments of gastric cancer have taken place. Preoperative chemotherapy theoretically can provide the advantages of reducing the bulk of tumor, which might improve the R0 resection rate, and of treating micrometastases early. Also, preoperative chemotherapy is expected to render unresectable tumors resectable without increasing postoperative morbidity and mortality. There are many new chemotherapeutic agents available for the treatment of advanced gastric cancer, but still the most effective agent, the optimal time and number of cycle for administration are still not known. The addition of postoperative chemotherapy through an intraperitoneal route and/or radiotherapy might affect the outcome of surgery favorably, but that hasn't been proved yet. A multicenter prospective randomized phase III trial should be performed to answer for those questions and to improve the curability of gastric cancer treatment.

3.Endophthalmitis.

Endophthalmitis is a rare but severe form of ocular inflammation due to infection of the intraocular cavity that can lead to irreversible visual loss if not treated properly and timely. It can be classified as exogenous or endogenous based on the transmission route of the infectious source. Exogenous endophthalmitis occurs when infecting organisms gain entry into the eye via direct inoculation, while endogenous endophthalmitis occurs when infectious agents hematogenously spread into the eye from a distant focus of infection. The diagnosis of endophthalmitis depends mostly on the clinical findings on ophthalmological examination. Delayed diagnosis of endogenous endophthalmitis can lead to not only visual loss, but also increased risk of mortality. Since ocular and systemic symptoms of endophthalmitis are usually non-specific, early diagnosis relies on the alertness of clinicians. Early diagnosis and proper treatment are keys to saving the eye. Following advances in vitreoretinal pharmacotherapy and surgical technology, early surgical intervention is the current trend in the management of endophthalmitis.

4.Pharmacotherapy for Obesity in Mood Disorders.

The prevalence of obesity and overweight is increasing in mood disorder, and it is connected to an increased cardiovascular mortality. Because of them, treatment for obesity may be an essential part of mood disorder treatment. Similar to the general population, non-pharmacological treatment such as correction of life habits should be considered first of all. If this approaches are fail, pharmacological treatment for obesity would be required as next step. Any drug for obesity is not approved officially in mood disorder. So approved drugs in general population, and drugs supported by several studies are prescribed in clinical settings. Several treatment guidelines for mood disorder and studies support that orlistat, metformin, topiramate and bupropion is effective and safe.

5.A Case of Nasal Type NK/T-cell Lymphoma.

Nasal type natural killer (NK)/T-cell lymphoma is rare but clinically aggressive, with a high rate of mortality. It is characterized by the expression of the NK-cell antigen CD56, frequent extranodal spreading and a strong association with the Epstein Barr virus. It commonly appears as deep nodules, or an infiltrative or ulcerative plaque. We report a case of nasal type NK/T-cell lymphoma, clinically showing subcutaneous panniculitis. It recurred after complete response to chemotherapy.

Cardiovascular (CV) toxicity associated with anti-cancer treatment is commonly encountered and raises critical problems that often result in serious morbidity or mortality. Most cardiac toxicities are related to the cumulative dose of chemotherapy; however, the type of chemotherapy, concomitant agents, and/or conventional CV risk factors have been frequently implicated in CV toxicity. Approximately half of the patients exhibiting CV toxicity receive an anthracycline-based regimen. Therefore, serologic biomarkers or cardiac imagings are important during anti-cancer treatment for early detection and the decision of appropriate management of cardiotoxicity. However, given the difficulty in determining a causal relationship, a multidisciplinary collaborative approach between cardiologists and oncologists is required. In this review, we summarize the CV toxicity and focus on the role of cardiac imaging in management strategies for cardiotoxicity associated with anti-cancer treatment.

9.Surgical Management of Recurrent Cervical Cancer.

The majority of patients with recurrent cervical cancer are incurable and treatment is based on the type of primary therapy delivered. Only a very small percentage of the patients with recurrent cervical cancer following primary radiotherapy will have central pelvic recurrences that are amenable to surgical resection and curable by pelvic exenteration. These procedures should be undertaken only after the completion of exhaustive attempts to exclude extrapelvic disease.

10.Prognostic value of statin for cancer patients: A Meta-analysis.

OBJECTIVE:
To systematically evaluate the effect of statins on prognosis for patients with cancers.
METHODS:
Literature on PubMed, EMbase and Cochrane library was screened from the establishment of databases to March, 2015 to find relevant studies. Random-effects models were used to calculate the pooled hazard ratios (HR) and 95% confidence interval (CI) for the association between statin use and all-cause mortality and cancer-specific mortality.
RESULTS:
A total of 25 studies covered 523 193 patients were identified and included in this Meta-analysis. The pooled effect showed that statin application was associated with a reduction in all-cause mortality in cancer patients (HR, 0.82; 95% CI: 0.76 to 0.89). A significantly decreased mortality in prostate cancer was revealed in subgroup by cancer sites (HR, 0.66; 95% CI: 0.52 to 0.83). In addition, sensitivity analysis demonstrated a weakened association between statin application and all-cause mortality after excluding the studies with shorter follow-up duration (HR, 0.91; 95% CI: 0.75 to 1.10).
CONCLUSION
A beneficial effect of statin on all-cause mortality and cancer-specific mortality is presented in patients with cancer. However, further studies are needed to confirm the long term effect.
