1.The effect of Xintong Changluo complex prescription ShensuⅡon renal interstitial fibrosis and TGF-β1 expression in FSGS rats

Objective To observe and discuss the effect of the traditional Chinese drug complex prescription ShensuⅡfrom Xintong Changluo therapeutic principle on renal interstitial fibrosis (RIF) and transforming growth factor-β1 (TGF-β1) expresssion in focal segmental glomerulosclerosis (FSGS) model rats. Methods Forty-eight healthy male SD rats were randomly divided into control group (n=12) and modeling group (n=36). Rats of modeling group were injected by doxorubicin hydrochoride for FSGS model. Rats of modeling group were sub-divided into model group, benazepril group and TCM group randomly. In 12 weeks, TCM group was given by intragastric administration of ShensuⅡ(3.5 g/100 g), benazepril group was given by intragastric administration of benazepril suspension (0.33 mg/100 g), control group and model group were given by intragastric administration of same volume of saline. HE/Masson staining was used to observe changes of tubulointerstitial pathomorphology. The degree of injury and fibrosis was measured. The expressions of fibronectin (FN) and TGF-β1 were detected by immunohistochemical SP method. Results The process of renal interstitial fibrosis was slower in FSGS rats of TCM group. Renal interstitial pathological index was 1.51 ± 0.80 in TCM group, which was lower than that of model group (2.18 ± 0.38) and benazepril group (1.79 ± 0.24). The index of renal interstitial fibrosis was 2.39 ± 0.13 in TCM group, which was lower than that of model group (3.11 ± 0.25) and benazepril group (2.80 ± 0.41). The relative expression of FN in renal interstitial was 14.19 ± 3.06 in TCM group, which was lower than that of model group (21.25 ± 3.31) and benazepril group (18.51±2.29). The relative expression of TGF-β1 in renal interstitial was 2.64±0.21 in TCM group, which was lower than that of model group (6.02 ± 0.12) and benazepril group (3.79 ± 0.46). All the differences were statistically significant (P<0.05). Conclusion Xintong Changluo complex prescription ShensuⅡcan reduce the process of renal interstitial fibrosis in FSGS model rats, which may be related with the inhibiting expression of TGF-β1.
2.Effect of Danzhi Jiangtang Capsules on Serum Interleukin-18, Cystatin C and Relative Biochemical Indexes in Patients with Early Diabetic Kidney Disease

Objective To observe the effect of Danzhi Jiangtang Capsules(DJC) on serum interleukin-18 (IL-18),cystatin C (CysC) and relative biochemical indexes in patients with early diabetic kidney disease(DKD),and to explore its protective action on the kidney.Methods Sixty-two hospitalized patients with early diabetic nephropathy (period 11-Ⅲ) in Endocrinology Department of the First Affiliated Hospital of Anhui University of Traditional Chinese Medicine were divided into western medicine group and DJC group,31 cases in each group.The patients in western medicine group received conventional hypoglycemic drugs,and DJC group received DJC based on the treatment for the western medicine group.Before and after treatment,we observed the levels of serum IL-18,CysC,fasting plasma glucose (FPG),2-hour postprandial glucose (2hPG),glycosylated hemoglobin (HbA1c),serum creatinine (SCr),blood urea nitrogen (BUN),microalbumin (mAlb),urinary albumin /creatinine ratio (UACR),and estimated glomerular filtration rate (eGFR).Results The total effective rate was 90.0% in DJC group and 76.7% in the western medicine group,there being significant difference between the two groups(P < 0.05).After treatment,the main traditional Chinese medical syndrome scores of DJC group were significantly lower than those before treatment,and the improvement was better than that of the western medicine group (P < 0.05 or P < 0.01).After treatment,the serum levels of IL-18,CysC,2hPG,SCr,BUN,mAlb,UACR in the two groups were significantly lower than those before treatment (P < 0.05 or P < 0.01),while eGFR level was higher than that before treatment (P < 0.05 or P < 0.01),and the improvement effect (except for BUN) in the DJC group was superior to that in the western medicine group(P < 0.01).DJC had an effect on lowering the FPG and HbA1c (P < 0.05 or P < 0.01 compared with those before treatment),and the western medicine had no obvious effect on the two indexes (P > 0.05).The results of Spearman correlation analysis showed that IL-18 was correlated with CysC,IL-18 and CysC were correlated with HbA1c,mALB and uACR.Conclusion DJC can protect renal function of patients with early diabetic nephropathy.
3.The validity and responsiveness of the Chinese version of the mini-Balance Evaluation Systems Test

Objective To evaluate the validity and responsiveness of the Chinese version of the miniBalance Evaluation Systems Test(mini-BESTest).Methods A total of 208 stroke patients were evaluated by 2 professionals using both mini-BESTest and Berg balance scale (BBS).All data were analyzed using Pearson correlation,t-test and descriptive statistical analysis.Results Significant correlation was observed between each entry of the mini-BESTest and its domain,with correlation coefficients ranging from 0.78 to 0.93 (P < 0.05).The scores of whole assessment and the three related domains were significantly correlated between mini-BESTest and BBS results,the correlation coefficients ranging from 0.68 to 0.87 (P < 0.05).After 2 weeks of treatment,there was significant improvement in each domain and in the total assessment score (P < 0.05).No floor or ceiling effects were observed in the total mini-BESTest scores,but there was a slight ceiling effect in the anticipatory and sensory orientation domains,and a slight floor effect in the reactive postural control domain.Conclusion The mini-BESTest has good validity and responsiveness,and can be applied to evaluate patients after stroke.
4.Clinical research on Angongniuhuang pill adjuvant to haloperidol for the treatment of intensive care unit patients with postoperative delirium

Objective To observe the clinical effects and safety of Angongniuhuang Pill (a traditional Chinese medical preparation)adjuvant to haloperidol for the treatment of Intensive Care Unit (ICU)patients in postoperative delirium.Methods A prospective study of 243 postoperative patients with delirium adimitted in Intensive Care Unit of Xinjiang Medical University First Affiliated Hospital (January 2013-December 2014),by using confusion assessment method of Intensive Care Unit (CAMICU).All of 243 patients with delirium were randomly divided into two groups by computer randomization software (SAS 9.1):group A in which patients were treated with Angongniuhuang Pill adjuvant to haloperidol and group Fin which patiets were treated with haloperidol alone.The end of two intervention methods was determined by two consecutive CAMICU showing negative or the treatment were ended up at seven days treatment.The data of general information,and the results of treatment were recorded and analized by statistical software SPSS 19.0.The measurement data was expressed as mean ±standard deviation;the comparision between two groups was tested by t test;and repeated measurement was analised by variance.Count data was described as number and constitution ratio;and intergroup comparision was tested by Chi-square test. Results There was no significant difference in general information between two groups.The delirium duration and ICU treatment time in group A were shorter than those in group F [(3.0 ±0.9)d vs.(3.8 ±1.0) d,P =0.010; (6.2 ± 1.4) d vs.(6.7 ± 1.5) d,P = 0.008].When delirium was diagnosed (T0),all patients had EEG abnormalities.As treatment went on,EEG abnormal rate gradually declined.At day one (T1),there was no statistically significant difference in EEG findings between group A and group F (95.7% vs.97.5%,P = 0.045 ).At day two (T2 )and day three (T3 ),there was statistically significant difference in EEG findings between group A and group F (65.5% vs.77.7%,P =0.038;42.2% vs.56.2%,P = 0.032).The levels of S100βprotein,interleukin-6 (IL-6),tumor necrosis factor α(TNF-α)in two groups at T0 were all elevated without noticeable difference.These markers were all deceased at T1,T2,T3,but the speed of descend in group A was relatively faster than that in group F.The differences were statistically significant (S100β:time effect P <0.01,inter-group effectp =0.002,interaction effectp =0.686;IL-6:time effect P <0.01,inter-group effectp =0.034,interaction effectp =0.01;TNF-α:time effect P <0.01,inter-group effectp =0.003,interaction effectp =0.516). The discharge rate in group A after improvement of general condition was higher than that in group F (89.7% vs.79.3%,P =0.029).The incidence of postoperative complications and mortality 28 days after operation in group A were lower than those in group F (45.7% vs.58.7%P =0.045;12.9% vs. 24.0%,P =0.028).Conclusion During the treatment of delirium,Angongniuhuang pill adjuvant to haloperidol was a better method compared with using haloperidol alone in respect of effectiveness and safety perspectives.It may be a novel approach to the treatment for delirium by the combination of Chinese and Western medicine.
5.Inhibitory effect of recombinant adenovirus-mediated mouse endostatin on pulmonary metastasis of osteosarcoma cell line MG-63 in nude mice

Objective:To construct an adenovirus-mediated mouse endostatin vector (Ad-mEndo) and to observe its inhibitory effect on the pulmonary metastasis of osteosarcoma in nude mice, so as to discuss the relationship between ES expression and the pulmonary metastasis of osteosarcoma. Methods: Recombinant adenovirus plasmid pDC315-mEndo was constructed and used to prepare recombinant Ad-mEndo. Osteosarcoma MG-63 cells were subcutaneously injected into the right fore limbs to establish nude mouse model of osteosarcoma; and the models were randomly divided into 3 groups: Ad-mEndo group, Ad-EGFP group and PBS group; animals receiving no transplantation served as blank control. The corresponding agents were injected (20 ?l per time) for a consecutive of 5 times on a weekly basis. The tumor volumes, histopathological characteristics were observed; ELISA was employed to examine the serum ES level. Animals were sacrificed 7 weeks later and the pulmonary metastasis was observed. Results: Sixteen days later,the tumor volume was (1.53?0.05)cm3 in Ad-EGFP group, (1.56?0.07)cm3 in the PBS group, and (0.91?0.03)cm3 in the Ad-mEndo group, with the tumor inhibitory rate being 40.7% in the last group. 6.Biology and Future Clinical Perspectives of Adipose-tissue Derived Mesenchymal Stem Cells

Adipose-tissue mesenchymal stem cells, is one kind of multipotent stem cells, can differentiate into adipogenic, osteogenic, chondrogenic, myogenic cells and so on in vitro and in vivo. Human adipose tissue is plentiful, easily harvested in large quantity with little patient discomfort. Adipose-tissue derived mesenchymal stem cells may be an alternative stem cell source for mesenchymal tissue regeneration and engineering without ethical consideration of embryonic stem cells and severe pain resulted by bone marrow procurement. The research work on adipose-tissue mesenchymal stem cells and future clinical perspectives were reviewed.
7.Study on Effect of Regulation to Th1/Th2 in AUC Patients with Chinese Medicine Combined with SASP Enema under Guidance of Synchronous Treatment of Lung and Intestine

This study was aimed to prove the scientific method theory through revealing the effect of synchronous treatment of lung and intestine in the treatment of active ulcerative colitis (AUC) patients. A total of 90 cases with traditional Chinese medicine (TCM) diagnosis of heat-toxin syndrome and western medical diagnosis of AUC were randomly divided into the control group, treatment of intestine group, and synchronous treatment of lung and intestine group, with 30 cases in each group. SASP enema (control group), SASP with Bai-Tou-Weng (BTW) decoction and Tiao-Wei Cheng-Qi (TWCQ) decoction enema (treatment of intestine group), SASP with BTW decoction and Liang-Ge-San (LGS) enema (synchronous treatment of lung and intestine group) were given, respectively. The treatment was given once a day and continued for 60 days. Then, clinical therapeutic evaluation was given before and after the treatment with enteroscopy, Th1/Th2 of ulcerative colon tissues was detected. The results showed that the sequence of clinical effect from top to bottom was synchronous treatment of lung and intestine group, treatment of intestine group, control group, with statistical significance (P< 0.05 or P< 0.01). Comparison of Th1/Th2 before and after treatment among each group showed that Th1 (INF-γ, IL-2) was decreased, Th2 (IL-4, IL-5) was increased (P< 0.05), Th1/Th2 was decreased, with statistical significance (P< 0.01). Comparison among each group after treatment showed that the sequence of Th1 from top to bottom was control group, treatment of intestine group, synchronous treatment of lung and intestine group; that of Th2 was synchronous treatment of lung and intestine group, treatment of intestine group, control group; that of Th1/Th2 was control group, treatment of intestine group, synchronous treatment of lung and in-testine group, with statistical significance (P< 0.01). It was concluded that synchronous treatment of lung and intes-tine had guiding significance on anti-inflammatory effect of enema therapy to AUC.
8.Function and interaction of dendritic cells and CD4+CD25+regulatory T cells in immune tolerance

Induction of immune tolerance is characterized by high efficiency,low toxicity and economic advantages,and immune tolerance has very important significance in organ transplantation.In the establishment of immune tolerance,dendritic cells (DCs) play the key roles such as clonal deletion or clonal anerge,expressing t-cell inhibiting factors,activating helper T cells electively and inducing regulatory T cells,especially CD4 + CD25 + regulatory T cells,etc.Similarly,CD4 + CD25 + regulatory T cells whose primary target point is dendritic cell also have various functions such as affecting dendritic cells'maturing,restraining dendritic cells'antigen presentation to T cells,inhibiting the activation of T cells in non-specific ways and inducing immune tolerance.Current research shows,dendritic cells and CD4 + CD25 + regulatory T cells regard each other as target point and work synergetically.Here we mainly discuss the function and interaction of dendritic cells and CD4 + CD25 + regulatory T cells in the immune tolerance.
9.Chest imaging characteristics of hand-foot-mouth disease in children

Objective To study radiological characteristics of hand-foot-mouth disease(HFMD) in children. Methods The chest X-ray films of 1295 children patients of HFMD were analyzed,for the general X-ray manifestations and the evolution. Results A total of 1427 films was obtained from all patients, in which 1203 cases were normal and 224 cases were abnormal. The interstitial changes characterized the abnormal group, mainly as increased and vague lung markings, increased hilar shadows (137 cases).The parenchyma changes appeared as patchy exudative shadows(49 cases). Short-term dynamic observation was applied in 62 cases, 38 cases pulmonary disease progression manifested as normal and the interstitial type changing into the parenchyma type and the mixed type, the localized type changing into the diffuse type. Conclusions Most children patients of HFMD showed normal chest films, while the abnormal patients were characterized by interstitial and parenchyma pulmonary edema. Serial chest X-ray examination and short-term dynamic observation were important to identify the severe cases and assess patients' condition.
10.A study on the diagnostic reliability of classification of intertochanteric fractures

Objective To compare the diagnostic reliability of AO classification and Evans-Jensen classifications in X ray film and three-dimensional CT reconstruction images intertochanteric fractures, and explore advantage of the three-di?mensional CT. Methods A retrospective study was performed to evaluate 54 patients with intertochanteric fractures. Three orthopaedic surgeons were asked to make assessment of fracture classifications using X ray film and the three-dimen?sional CT images. Agreement test was performed to evaluate interobserver and intraobserver reliability for fracture classifica?tion. Results When X ray film was used, mean Kappa values of interobserver reliability for AO and Evans-Jensen classifi?cations between three surgeons were 0.597 and 0.571, that was medium consistency. While using three-dimensional CT to AO classification, lower Kappa value was 0.411, medium consistency. Evans-Jensen classification, Kappa value was 0.704, highly consistency. Each surgeon respectively using X ray film and three-dimensional CT, the mean Kappa values of AO and Evans-Jensen classification were 0.464, medium consistency and 0.191 (0.160-0.233), weak consistency. Conclusion Both classifications based on X ray film show moderate consistency, using three-dimensional CT does not improve the consis?tency of AO classification. The three-dimensional CT is not applicable in Evans-Jensen classification.
