1.Expression and significance of Glut1 and HIF-1? in human hepatocellular carcinoma

Background and purpose:Glut functioning as a vehicle for transferring glycose into cell,plays an important role in strengthening the glycometabolism process of malignant tumor.HIF-1?,a crucial transcriptional control gene in signaling cell to adapt to anaerobic microenvironment,can activate the expression of many anaerobic-responding genes which includes the expression of Glut1 gene,in order to supply energy for tumor under hypoxia.Our aim was to investigate the expression and clinical significance of Glut1 protein and HIF-1? protein as well as the correlation between them in human hepatocellular carcinoma(HCC). Methods :Immunohistochemical staining: EnVision method was used to detect the expression of Glut1 and HIF-1? in 45 cases with human hepatocellular carcinoma,and in 11 cases with normal hepatic tissue. 2.A case report of Dyke-Davidoff-Masson syndrome

Dyke-Davidoff-Masson syndrome ( DDMS) is a rare epilepsy syndrome which is characterized by cerebral hemiatrophy with ipsilateral compensatory skull changes and contralateral hemiplegia recurrent .Here we reported a case of which the curative effect turned out to be unsatisfied after conservative treatment , physical examination re-vealed mental retardation , facial asymmetry , mild right hemiparesis , MRI scan of the head showed left cerebral hemiatrophy , calvaria thickening , PET-CT showed less functional left cerebral hemisphere .The patient finally un-derwent multi-lobe disconnection to relieve recurrent seizure , whose seizure was well controlled in the following up of one year .
3.Proximal femoral anti-rotation nail Ⅱ fixation repairs intertrochanteric fracture:the importance of unarmed guided needle placement

BACKGROUND:The effect of proximal femoral anti-rotation nail fixation in repair of intertrochanteric fracture is better than the other fixation materials. Unarmed inserting guided needle plays an important role for the quality of intertrochanteric fracture fixation. At present, there have been short of fast and accurate methods for inserting femoral canal guided needle from trochanter. OBJECTIVE:To investigate the operating skil s and application value of unarmed inserting guided needle in the process of proximal femoral anti-rotation nail Ⅱ fixation in repair of intertrochanteric fracture. METHODS:Standard operating procedures of guided needle inserting were established. The location of the guided needle was correct or was not judged through discriminating the feeling of inserting into the tip of femoral canal guided needle from trochanter. Slight resistance prompted that guided needle was in correct position. The guided needle closed to the medial femoral cortex and moved downward in the medul ary cavity. The guided needle was confirmed in the correct position by the perspective of C-arm in positive side position. This process was just one lateral perspective. Smoothly without resistance prompted that if the guided needle was out of the medul ary cavity, the direction of the inserting needle needed to be changed, without perspective. The resistance would be large if the needle tip piercing the medial femoral cortical bone, at this moment, the direction of the needle needed to be adjusted;or was blocked by malreduction of fracture or the free fracture block off the medul ary cavity;at this moment, fracture ends needed to be further restored. The surgery time, the time required to complete the guided needle inserting, the amount of bleeding, weight-bearing ambulation time and complications of 19 patients with intertrochanteric fracture treated by proximal femoral anti-rotation nail Ⅱ fixation were statistical y analyzed. RESULTS AND CONCLUSION:The average completed time for internal fixation was 45 minutes, the average desired time for guided needle inserting was 80 seconds, the average blood loss was 100 mL. There was no wound infection, or pressure sore formation. The shimoji time was an average of six weeks after treatment. Al patients were fol owed up for 4-9 months. Three patients needed crutches to walk before treatment, and after three months of treatment, they returned to the pre-exercise levels of injury. These results suggest that the procedure of unarmed inserting the guided needle provides guidance for the quick and correct completion of needle inserting, so as to enhance the internal fixation quality of proximal femoral anti-rotation nail Ⅱ in repair of intertrochanteric fracture, with a high reliability. It can significantly reduce the number of perspective, shorten surgery time, reduce the amount of bleeding and reduce complications.
4.Pancreatic tuberculosis: MDCT features and differential diagnosis

Objective To study the multi-detector spiral CT (MDCT) features of pancreatic tuberculosis to improve the awareness and correct diagnosis. Methods Imaging features of MDCT were retrospectively reviewed on 9 cases with pancreatic tuberculosis that were diagnosed from 2003 to 2009, among them two patients underwent MDCT angiography, the features were compared with those of operative and pathological findings. Results One case of pancreatic cancer, 2 cases of cystadenocarcinoma and 1 case of pseudocyst, 2 cases of pancreatic cancer with retroperitoneal lymph node metastasis, 2 cases of lymphoma were diagnosed by MDCT, and all the lesions were diagnosed as pancreatic tuberculosis pathologically, and the misdiagnosis rate was 100%. Pancreatic tuberculosis often presented as pancreatic head mass, sometimes pancreatic body and tail were involved. MDCT showed isopycnic or low density solid-cyst mass, occasionally calcification was present, with slightly or moderate ring-like enhancement after contrast injection. There was no enlargement of the pancreatic duct. The retroperitoneal and adjacent lymph nodes were always enlarged with ring-like enhancement. Sometimes there was evidence of tuberculosis of other abdominal organs. Conclusions Ring-like enhanced lesions with ring-like enhanced lymph nodes without enlargement of pancreatic duct are characteristic MDCT features of pancreatic tuberculosis.
5.Study of the community-based indexing system for prevention and control technologies of chronic non-communicable diseases

An indexing system is constructed,based on the implementation of prevention and control technology for chronic non-communicable diseases in community health service institutions,using both literature review and Delphi method,and using the Analytic Hierarchy Process (AHP) to calculate the weight of the indexing system.The system comprises three level-1 indexes,8 level-2 indexes and 33 level-3 indexes.The research proposed a transition from emphasis of therapeutic means to prevention,and emphasis on prevention and control technologies to the high-risk population in the prevention and control technology of chronic non-communicable diseases,along with development of techniques and measures to encourage behavioral changes of the population with chronic diseases.

Clavicular crook plate is designed based on anatomic features of acromioclavicular joint,which matches clavicle appearance,and well attaches the shaft.Not passing through the joint,the internal fixation exhibits many advantages,such as convenient usage,reliable fixation and micromovement of fixed acromioclavicular joint.So the patients could move the joint early and avoid osteoarthritis.Allogenous tendon has abundant sources,simple sampling process,and short repairing duration and can reduce injury to patients.Cryopreserved allogenous tendon has low antigenicity and immunological refection,good histocompatibility.Reconstructed coracoclavicular ligament by allogenous tendon has good material intensity,and can meet the requirements of local stress,better substitute integrity and function of coracoclavicular ligament.Moreover,it has precise effect and simple operation.It acts as long-term fixator following plate removal,thus is important supplement of clavicular crook plate fixation.Clavicular crook plate fixation in combination with allogenous tendon transplantation reconstructs acromioclavicular joint stability,effectively provides early mechanical stability and subsequent biological stability,and maximizes restoration of acromioclavicular joint structure and function.
7.Study of proximal femoral locking plate internal fixation after dynamic hip fixation intertrochanteric fracture

Objective To study the mechanics situation of proximal femoral locking plate internal fixation after dynamic hip fixation in-tertrochanteric fracture. Methods Totally 10 couple of elderly proximal femur specimens were collected and intertrochanteric fracture model were prepared. Fixation material was removed after dynamic hip screw fixation. The left sides were collected as control group and given anti-rotation intramedullary nail internal fixation, while the right side were collected as observation group and given proximal femoral locking plate internal fixation. Then vertical displacement, axial stiffness and rotational stiffness under different loads were compared. Results Under dif-ferent loads, femoral bone vertical displacement and femur tuberosity vertical displacement in the observation group were both significantly shorter than those in the control group (P<0. 05), and femoral bone and femur tuberosity axial stiffness and rotational stiffness in the observation group were significantly higher than those of the control group (P<0. 05). Conclusion Proximal femoral locking plate internal fixation can improve stress load and enhance axial stiffness and rotational stiffness, and it's an ideal material for refracture fixation model after dynamic hip fixation intertrochanteric fracture.
8.Expression of CXC chemokine receptor 4 in clear cell renal cell carcinoma

Objective To investigate the expression of CXC chemokine receptor 4 (CXCR4) in clear cell renal cell carcinoma (ccRCC) and its relationship with the clinical pathological parameters of ccRCC.Methods The expression of CXCR4 was detected by immuno-histochemistry method in 63 cases of ccRCCs,20 cases of para-carcinoma tissues and 20 cases of normal renal tissues.The correlation between expression level of CXCR4 and clinical pathological parameters of ccRCC patients were analyzed,and the clinical significance of its expression in ccRCC was evaluated.Results The positive expression rate of CXCR4(49.2%) in ccRCCs was significantly higher than that in para-carcinoma tissues (15%) and normal renal tissue (10%),and the difference was statistically significant (P < 0.05).The expression level of CXCR4 and the clinical stage and pathological grade of ccRCC were correlated (P < 0.05),and was associated with lymph node transfer (P < 0.05).The CXCR4 negative group overall survival rate [55.2% (16/29)] and the average survival time(46 months) was significantly better than the positive group [38.5% (10/26),32 months;P < 0.05].Conclusions The expression level of CXCR4 in ccRCC is correlated with the clinicopathological parameters and prognosis.CXCR4 is expected to be an important marker for diagnosis and prognosis evaluation of renal cell carcinoma.
9.Filtration of active fractions with function of expelling water retention with drastic purgative from Kansui Radix stir-baked with vinegar.

To study the function of expelling water retention with drastic purgative of different polarities of Kansui Radix stir-baked with vinegar on the cancerous ascites model rats, the furosemide was taken as positive control drug, and the cancerous ascites model rats were respectively orally administered with different polarities of Kansui Radix stir-baked with vinegar for 7 d. The amount of urine and ascites, the level of urinary sodium, potassium, chloride ion and pH, and the content of PRL1, AII, ALD in serum were investigated. Compared with model groups, ethyl acetate extract group showed a decreasing trend in ascites; the amount of urine of showed a significant increase (P < 0.05); the level of urinary sodium, potassium, chloride ion (P < 0.05, P < 0.01), pH (P < 0.05), and the content of PRL1, AII, ALD in serum all showed a significant decrease (P < 0.01). The effects of petroleum ether extract and n-butanol extract were weaker than that of ethyl acetate extract. The water exact was the weakest. The results showed that ethyl acetate extract is the active part of Kansui Radix stir-baked with vinegar on the function of expelling water retention with drastic purgative on the cancerous ascites model rats, alleviating the water-electrolyte disorder and body fluid acid-base imbalance, regulating the renin angiotensin aldosterone system.

10.Study on toxicity of vinegar-processed Kansui Radix on basis of symptom-based prescription theory.

OBJECTIVETo study the differences in the toxicity of vinegar-processed Kansui Radix on normal and cancerous ascites model rats.
METHODNormal and cancerous ascites model rats were taken as the research objects and orally administered with different doses of vinegar-processed Kansui Radix for 7 d. Pathological sections were prepared to observe the damages in liver, stomach, intestinal tissues in rats and detect the impacts on serum, liver, stomach and intestinal tissues and the oxidative damage index.
RESULTCompared with the blank group, all of normal administration groups and model groups showed significant damages in liver, stomach and intestinal tissues. Compared with the model groups, all of normal administration groups revealed notable alleviation in damages. Compared with the blank group, the model groups showed significant increases in AST, ALT and MDA in serum and liver (P < 0.01) and a significant decrease in GSH in serum and liver, stomach, intestinal tissues (P < 0.01). Compared with the blank group, the results showed significant decreases in ALT, AST in serum and ALT in liver in model low, medium and high dose groups and AST activity in liver tissues in the normal high dose group (P < 0.05, P < 0.01); significant decreases in GSH in serum and stomach tissues in normal low, medium and high dose groups and GSH content in liver and intestinal tissues in normal medium and high dose groups (P < 0.05, P < 0.01); notable rises in MDA in liver tissues in normal low, medium and high dose groups and MDA content in serum and stomach and intestinal tissues in normal medium and high dose groups (P < 0.05, P < 0.01). Compared with model groups, data revealed significant decreases in ALT, AST in serum in model low, medium and high dose groups, AST in liver tissues of model medium and high dose groups and ALT activity in liver in the model high dose group (P < 0.05, P < 0.01); significant increases in GSH content in serum and stomach tissues of model low, medium and high dose groups, GSH in liver tissues in model medium and high dose groups and GSH in intestinal tissues in the high dose groups (P < 0.05, P < 0.01); and notable declines in MDA content in serum in model low, medium and high dose groups, MDA in liver tissues of model medium and high dose groups and MDA in stomach and intestinal tissues the high dose group (P < 0.05, P < 0.01).
CONCLUSIONAccording to the study, vinegar-processed Kansui Radix showed a significant lower toxicity liver, stomach, and intestines of cancerous ascites model rats, which provided a basis for clinical safe application of vinegar-processed Kansui Radix based on symptom-based prescription theory.
