4.Unilateral Pulmonary Vein Atresia with Absent Right Superior Vena Cava.

A case of unilateral pulmonary vein atresia with absent right superior vena cava in 9 years-old boy is described with a discussion. Clinical findings were similar to other reports and consisted of hemoptysis, dyspnea and anemia. A chest X-rary revealed irregular vascular marking on left lung fiedld and increased interstitial marking on right upper lung field. The diagnosis could be confirmed with radionuclide perfusion study, MRI scan of heart and great vessels and angiography which demonstrated a small right pulmonary artery, pruned its peipheral branches, stasis of contrast material and nonvisualization of draining righ pulmonary veins. Pneumonectomy, patch angioplasty of diaphargm excision will be necessary.

5.A Case of Kasabach-Merritt syndrome with generalized hemorrhagic diathesis.

Kasabach-Merritt syndrome is the association of thrombocytopenia, disorder of coagulation, spontaneous bleeding and enlargement of a hemangima or extensive hemangiomatosis, which can be often life threatening. We experienced a three years-old girl with suddenly enlarging hemangioma of right cheek, complicated by severe thrombocytopenia, consumptive coagulopathy and hemorrhagic cystitis. Generalized petechia and ecchymosis were noted on the overlying skin of hemangioma, trunk and extremities. But the study of MRI and angiography, hypevascular was located within the right masseter muscle extending to temporalis muscle and anterior to parotid gland. So selective embolization with PVA (polyvinyl alcohol) was performed through the distal branch of internal maxillary artery and facial artery. She was medicated with oral corticosterioid for 3 weeks. After embolizationn, blood flow to hemangioma and facial swelling were markedly decreased. Patient was followed up for 7 years and she showed normal contour of face and no bleeding diathesis.

7.Morphologic change of PCL of MRI in the tear of ACL

MRI has proved to be very reliable in evaluating the menisci and cruciate ligaments. On MRI, several diagnostic criteria of ruptured ACL were reported. Boeree and Ackyroyd reported that when the ACL is ruptured the PCL may appear to be curled up or sigmoid. But these morphologic changes may be shown in the normal ACL, so the quantitative analysis of these morphologic changes in considered as a way to increase the diagnostic sensitivity. We have used 1.0 tesla MRI scanner(SIMENS W. Germany) with a surface coil. We compared two groups of patients; a ruptured ACL group(16 patients) in which had indicated and arthroscopy confirmed rupture of the ACL and control group(46 patients), in which had shown the ACL to be entirely normal. At first, we made a line(basal line) between the femoral attachment and tibial attachment of the PCL and decided the point(apex) which was located far distant from the line. And we made a line(A line) between the femoral attachment and apex of the PCL, another line(B line) between the tibial attachment and apex of the PCL. We divided the basal line into the four areas. We measured the each angle between basal line and A line(angle a), between basal line and B line(angle b). And we measured the entire length of basal line, each height of the PCL previously divided point of the basal line(H1, H2, H3) and the apex of the PCL on the basal line. We compared the control group and ruptured ACL group by t-test from the measured factors angle a, angle b, H1, H2, H3, H4, and length of basal line. We studied factors which were able to decide whether the ACL was ruptured or not in MRI finding by logistic regression. 1. H1, the distance from the basal line to the PCL at 1/4 point on the basal line, were 5.7±1.6 mm in ruptured ACL group, 4.7±1.3 mm in control group, so there was statistically significant increase in ruptured ACL group. 2. The angle a were 56.0±14.4° in ruptured ACL group, 39.7±10.1° in control group, so there was statistically significant increase in ruptured ACL group. 3. From the measured factors angle a was able to decide whether the ACL was ruptured or not in MRI and the slope of angle a in logistic regression was 0.1. In conclusion, when the apex of the PCL is located at proximal 1/4 of the PCL and PCL and greater curve, above signs will be considered to be a sign of ruptured ACL in MRI.

8.Frequency of Meniscal Tear according to Ligament Injury

To know wheter the injury of the ligament may induce the meniscal tear, we statistically analyzed the incidence of meniscal tear with χ2 in cases of fresh and old(more than 3 months since injury) ligament injury in 419 joints of 407 adults who had been treated during the time between early 1984 and the end of 1992 in St. Mary's Hospital. The results were as follows: 1. The meniscal tear occurred more frequently in old ligament tear(44/70, 62.9%) than in fresh ligament tear(7/26, 26.9%) in cases of isolated ACL tear(P=0.062). 2. There was no significant difference between the old(10/34, 29.4%) and the fresh tear(24/96, 25.0%) in the combined ACL tear(P=0.615). 3. Overall ACL tear patients showed higher incidence of meniscal tear in the old ligament tear(54/1-4, 51.9%) than in the fresh ligament tear(31/122, 25.4%)(P=0.000). 4. The frequency of meniscal tear was very low in the isolated PCL tear(fresh tear: 0/14, old tear:1/17, 5.9%). In cases of combined PCL tear the incidence was rather higher in the fresh tear(11/33, 33.3%) than in the old tear(2/30, 6.7%)(P=0.015). 5. Overall PCL tear patients showed higher incidence of meniscal tear in the fresh ligament tear(11/47, 23.4%) than in the old ligament tear(3/47, 6.4%)(P=0.020). 6. The meniscal tear occurred more frequently in the ACL tear than in the PCL tear in both isolated(ACL : 85/96, 53.1%, PCL : 1/31, 3.2%) and overall ACL or PLC tear group(ACL : 85/226, 37.6%, PCL : 14/16.7%)(P=0.000). 7. The meniscal tear occurred less frequently in the isolated clootgeral ligament tear(9/94, 9.6%) than in the isolated cruciate ligament tear(52/127, 40.9%)(P=0.000). 8. The incidence of tear between the medial and the lateral meniscus showed no statistically significantly difference(P=0.215). From these results it would be suggested that the ACL tear, especially isolated tear may induce meniscal tear. On the other hand there was no statistically significant relationship between the incidence of meniscal tear and those of either PCL tear or collateral ligament tear.

10.A Clinical Study on Pseudomembranous Colitis.

The purpose of this study is to review the clinical and pathological features of pseudomembranous colitis in Korea. Ten cases of psedomembranous colitis, which were diagnosed by sigmoidoscopy and biopsy at the Seoul National University Hospital from May 1982 to occtober 1985, were analyzed in terms of the age and sex distribution, underlying diseases, presumptive causative antimicrobials, symptomatology, involved site and tratment. 1) The disease was more prevalent above sixth decade with a slight preponderance on the female. 2) The underlying diseases of the patients were the uterine cervix cancer in 3 cases and one case each of the nasal polyp, cataract, degenerative arthritis, tuberculous gradmuloma, SLE with menngitis, CVA and gingival abscess. 3) As for the presumptive causative drugs, cephalosporin were assaciated in 6 cases, aminoglycoside m 7 cases, penlcillin in 3 cases, Bactrim in 3 cases, antituberculosis drugs in 1 case and anticancer drugs in 1 case. 4) As for the anatotmical distribution of the lesion, the rectum was involved in 5 cases, the rectum and sigmoid colon in 4 cases and up to the descending colon in 1 case. 5) All the cases were cured by medical treatment. In 5 cases, discontinuation of the antimicrobial drugs and supportive care only were effective and in other 5 cases, vancamycin and/or metronidazole together vrith cholestyramine were added to the regimen.
