4.The Distribution of Fibronectin in the Experimental Keratitis of Rabbit.

The distribution of fibronection was investigated in the carrageenan-induced keratitis of rabbitis. Keratitis was induced by the injection of carrageenan into corneas of the rabbits, and specimens were taken at various intervals. Histological examinations were done with H and E, silver impregnation and PTAH stains. The distribution of fibronectin was observed by using avidin-biotin-peroxydase(ABC) technique. The results were follow. 1. The histological changes in early stages showed acute inflammatory exudate in stroma. After two weeks, granulation tissues were formed. And then after the fourth week, the granulation tissues began to mature with the apperance of fascicle of collagen fibers and thickened epithelium. 2. Fibronectin was demonstrated in a fibrillar appearance in the acute inflammatory exudate of the stroma, which was normally absent. With the development of chronic inflammatory lesions and granulation tissues, the amount of fibronectin was increased, relative to the increase and codistribution of reticular fibers, but with the appearance of mature collagen fibers, the amount was markedly reduced. 3. The fibronectin which was absent in the normal epithelium was prominent in the regenerating epithelium.

5.Two Cases of Kayser-Fleischer Rings.

The Kayser-Fleischer ring, a characteristic finding in Wilson's disease, occurs in the corneal periphery and is usually yellow-brown but may by red, blue, green, ormixture of any these colors. The Kayser-Fleischer ring, copper deposition at the level of the posterior position of Descemet's membrane, is about 1 to 3mm in width and extends to the limbus without a lucid interval. In addition to involvement of Descemet's membrane, one may find anterior subcapsular sunflower cataract in Wilson's disease. The authors have recently experienced two cases of Wilson's disease. One case, a 17-year-old boy, has Kayser-Fleischer rings and anterior subcapsular sunflower cataracts in both eyes and the other, a 17-year-old aldolescent male, has dense yellow-green colored Kayser-Fleischer rings in both eyes. Both cases have been treated with penicillamine(1gr/day) and low copper diet. After treatment, clinical manifestations have been improved markedly but the Kayer-Fleischer rings have not disappeared yet.

6.A Case of Acute Uveitis Associated with Ankylosing Spondylitis.

Ankylosing spondylitis is a chronic, progressive, inflammatory arthritis that affects principally the sacroiliac joints and spine. Approximately 25% of cases are associated with an acute recurrent nongranulomatous iridocyclitis. This disease occurs most frequently in the second and third decades of the life. Males are affected more frequently than females. There is a striking association between this disease and presence of histocompatibility antigen HLA-B27, which is found in only 4% to 9% of controls. The antigen is present in 88% to 96% of those with ankylosing spondylitis and in 55% of those with uveitis. The authors experienced lease of acute recurrent uveitis associated with ankylosing spondylitis in a 47 year old male patient. We performed radiologic and laboratory examinations. Radiologic examinations revealed the massive and diffuse calcification of all paravertebral ligaments showing a "bamboo spine" appearance. Laboratory examinations revealed elevated ESR and positive HLA-B27. We treated him with the systemic and topical steroids and topical mydriatics, and had a good result.

8.Clinical Experience and Imitation of the Immediate Breast Reconstruction Using the Permanent Expander Prosthesis.

Tissue expansion in breast reconstruction is a invaluable alternative surgical procedure. the advent of permanent expander prosthesis eliminated a second operative procedure for removal of the expander and replacement with another permanent implant. When we consider the choice of method for breast reconstruction, we have to take account of the extent of mastectomy, past-medical history of the patient, timing of operation, and patient's motivation. It is still accepted that autogenous Breast reconstruction is the best choice of the mothed. As autogenous breast reconstruction increase in popularity, however, it requires longer operation time, more complicated technique, and careful attention to donor site. Autogenous breast reconstruction is also not good candidate for the patients who have diabetes, vascular disease, smoking, obesity, previous history of cesarean section, or further pregnancy planning. We have performed 10 immediate breast reconstructions after modified radical mastectomy using the permanent expander prosthesis, manufactured by Mentor corporation. Among these cases, 8 patients underwent cesarean section and 2 patients pregnancy. Although this method has advantages of simple technique, minimal morbidity, and low cost, it has some limitations for orientals. First, it is inevitable in superior fullness of the reconstructed breast because of round shape of the expander. Second, it is difficult to get ptotic breast because of thick and inelastic characteristics of oriental skin. Third, the ratio of expander base to projection is not appropriate for orientals because of their smaller chest size. We experienced 2 cases of spontaneous deflation for its manufactural defect itself. In our experience, we would like to suggest that new type of permanent expander prosthesis should be innovated.

9.Breast Reconstruction Using the Pedicled Transverse Rectus Abdominis Myocutaneous (Tram) Flap.

In many cases the breast reconstruction surpasses the goal of a normal breast mound appearance in clothing and achieves a result that, in time, may look almost normal in the unclothe state. Breast reconstructions with autologous transverse rectus abdominis myocutaneous (TRAM) flaps are well known to produce the most aesthetically excellent results. the pedicled to TRAM flap cannot be used in high risk patient such as obese or smoking patients due to partial flap necrosis. But, the free TRAM flap have more robust blood supply and less donor site morbidity than the pedicled TRAM flap and is therefore the currently preferred technique in western. However, the free TRAM flaps has a problem related to the anastomotic failure which has been reported as high as 6% to 10%. Moreover many Korean women have smaller sized breast than western women and have no risk factors compromising flap circulation. So the single pedicled TRAM flap can be safely used for breast reconstruction without any flap necrosis. 60% of the elevated flap area has a sufficient volume to mold a new breast that matches with the contralateral breast and the unneccessary distal portion of the flap that has the marginal circulation can be resected. In this study we review our 20 consecutive cases of breast reconstruction using the pedicled TRAM flap and have a conclusion that the pedicled TRAM flap produce an aesthetically acceptable new breast in Korean mastecotmy patients without any flap necrosis or donor site morbidity.
