1.Significance of Stone Mineral Density in Patient with Renal Stone as a Predictive Parameter for Effect of ESWL.

The predictive parameters for effect of ESWL have been considered by stone size, locations, chemical composition and hydronephrosis, etc. However, there are several questions that ""how much shock wave will be needed for completely removal of stone and how can we predict effect of ESWL in some patients with renal stone?"" Therefore we have measured bone mineral density (BMD) and stone mineral density(SMD) as a predictive parameter for the effect of ESWL in patient with renal stone. As results, The storages of ESWL for complete response were correlated with the sizes (p<0.01, r=0.740) and the stone mineral density (p<0.01, r=0.852). Stone mineral density was variable even same size stones and the requirement of shock wave for complete response in 1.00-1.99 cm sized stones was correlated with SMD. (p<0.01, r=0.795 ). From this study, the measurement of SMD may be the objective and predictive parameter for effect of ESWL in patient with renal stone.

2.A Case of Scrotal Cystic Lymphangioma.

Lymphangiomas are benign tumors of congenital origin, occurring mostly in children. They are found rarely in the scrotum. This rare clinical entity should be included in the differential diagnosis of cystic lesions of the external male genitalia, especially hydrocele in children. We present a case of 3 year old male with the scrotal lymphangioma arising from the perineum that have not any connection with the testis or spermatic cord.

3.Usefulness of Rapid PSA Kit(R).

PURPOSE: The incidence rate of prostate cancer has increased remarkably in Korea. The serum prostate specific antigen (PSA) value has been used for screening, although its clinical significance in prostate cancer screening is still inconclusive. However, if the measurement time was short and the cost was low, such an assay kit should be sufficient for prostate cancer screening. MATERIALS AND METHODS: We developed pared monoclonal antibodies against PSA which could be used in assay kits for PSA. The Rapid PSA Kit(R) used an immunochromatographic method to qualitatively judge a positive or negative result. Serum specimens from 78 men with benign prostate hyperplasia or prostate cancer were tested using the kit. RESULTS: The sensitivity of the kit was determined to be 4ng/ml. 33 samples had a value of greater than 5ng/ml, so were considered positive. 5 samples had values between 4ng/ml and 5ng/ml, of which 3 were positive. The other 40 samples had values less than 4ng/ml, and 11 of these were judged positive. These results indicated that the sensitivity and specificity of the Rapid PSA Kit(R) were 94.7 and 72.5%, respectively. CONCLUSIONS: Tests using the Rapid PSA Kit(R) can be easily performed at outpatient clinics or elsewhere. This kit is useful in the initial screening of prostate cancer as the results can be obtained within 15 minutes and the cost is lower than with ordinary serum PSA tests.

5.Gangioneuroma Misdiagnosed as Ovarian Tumor.

A ganglioneuroma is a rare neoplasm in adults, which is most commonly locating in the posterior mediastinum, but can also be located in the retroperitoneum, pelvis, adrenal medulla and neck. They predominantly occur in females, with 60-70% of diseases originating before the age of 20 years. Herein, a case of a 51-year-old woman suffering from a pelvic mass, with a history and physical findings similar to those of an ovarian tumor, is reported.

6.Penicillium ulleungdoense sp. nov. from Ulleung Island in Korea

In a study of the fungal diversity on Ulleung Island in Korea, three novel strains of Penicillium were isolated. Different sites on Ulleung Island were selected for collecting endophytic fungi, and three endophytic fungal strains showed unique morphological characteristics. DNA sequence of the internal transcribed spacer, β-tubulin, calmodulin, and RNA polymerase II second largest subunit regions of the strains were analyzed and they showed unique taxonomic position from the other species of Penicillium section Sclerotiora. The new strains were named Penicillium ulleungdoense sp. nov. As the novel endophytic Penicillium taxa were discovered in a unique environment, the data could be meaningful for understanding the geographical distribution of Ascomycetes on Ulleung Island.

Emery-Dreifuss muscular dystrophy is characterized by 1) early contractures of the elbows, Achilles tendons, and postcervical muscles, 2) slowly progressive muscle wasting and weakness with a humeroperoneal distribution in the early stages, and 3) cardiomyopathy with conduction defects and risk of sudden death. The inheritance is usually X-linked recessive but can be autosomal dominant and recessive. We report a case of 28-year old woman who presented with dizziness, palpitation, and progressive muscular weakness. Her ECG revealed high degree AV block and muscle biopsy demonstrated diffuse degenerative change consistent with Emery-Dreifuss muscular dystrophy. She was diagnosed as autosomal dominant Emery-Dreifuss muscular dystrophy by characteristic clinical features, and findings of ECG, nerve conduction test, electromyography and muscle biopsy findings. A VVI-type permanent pacemaker was implanted.

High signal intensity on diffusion-weighted image (DWI) at the site of venous occlusion has previously been reported in cerebral venous thrombosis (CVT). The frequency and diagnostic value of these signal changes in CVT were unknown. Some authors suggest that the presence of high signal intensity on DWI in occluded veins might help diagnose CVT and suggest low rate of recanalization. We experienced two cases of CVT with high signal intensity at the site of intravascular clot on DWI.

10.A Case of Proliferating Trichilemmal Cyst on the Elbow.

Proliferating trichilemmal cyst is a rare benign tumor of external root sheath origin that usually occurs on the scalp of elderly women. It is rarely found on the face, neck, trunk and back. A 17 year-old man presented with a skin-colored, subcutaneous, tender nodule on the elbow with a history of bee sting. A histological examination revealed a well demarcated multilobular cystic mass characterized by trichilemmal keratinization in the dermis. The skin lesion was totally excised and recurrence has not been found. We present a case of proliferating trichilemmal cyst of unusual predilection site.
