5.Morphologic change of PCL of MRI in the tear of ACL

MRI has proved to be very reliable in evaluating the menisci and cruciate ligaments. On MRI, several diagnostic criteria of ruptured ACL were reported. Boeree and Ackyroyd reported that when the ACL is ruptured the PCL may appear to be curled up or sigmoid. But these morphologic changes may be shown in the normal ACL, so the quantitative analysis of these morphologic changes in considered as a way to increase the diagnostic sensitivity. We have used 1.0 tesla MRI scanner(SIMENS W. Germany) with a surface coil. We compared two groups of patients; a ruptured ACL group(16 patients) in which had indicated and arthroscopy confirmed rupture of the ACL and control group(46 patients), in which had shown the ACL to be entirely normal. At first, we made a line(basal line) between the femoral attachment and tibial attachment of the PCL and decided the point(apex) which was located far distant from the line. And we made a line(A line) between the femoral attachment and apex of the PCL, another line(B line) between the tibial attachment and apex of the PCL. We divided the basal line into the four areas. We measured the each angle between basal line and A line(angle a), between basal line and B line(angle b). And we measured the entire length of basal line, each height of the PCL previously divided point of the basal line(H1, H2, H3) and the apex of the PCL on the basal line. We compared the control group and ruptured ACL group by t-test from the measured factors angle a, angle b, H1, H2, H3, H4, and length of basal line. We studied factors which were able to decide whether the ACL was ruptured or not in MRI finding by logistic regression. 1. H1, the distance from the basal line to the PCL at 1/4 point on the basal line, were 5.7±1.6 mm in ruptured ACL group, 4.7±1.3 mm in control group, so there was statistically significant increase in ruptured ACL group. 2. The angle a were 56.0±14.4° in ruptured ACL group, 39.7±10.1° in control group, so there was statistically significant increase in ruptured ACL group. 3. From the measured factors angle a was able to decide whether the ACL was ruptured or not in MRI and the slope of angle a in logistic regression was 0.1. In conclusion, when the apex of the PCL is located at proximal 1/4 of the PCL and PCL and greater curve, above signs will be considered to be a sign of ruptured ACL in MRI.

6.Establishment for Reference Range of Serum HER-2/neu in Korean Healthy Women.

PURPOSE: HER-2/neu oncogene is known to play a part in the process of carcinogenesis, while the biological characteristics of HER-2/neu oncoprotein include regulating cell growth and increasing the reproductionability of a tumor. The extracelluar domain (ECD), whose molecular weight is between 95 and 105 kD among the HER-2/neu oncoprotein structures, is proteolyzed and separated from the cell surface by metalloproteases and goes into the blood stream where it starts circulation. Since monoclonal antibody was developed for the serum HER-2/neu ECD, it's now possible to measure HER-2/neu ECD in the serum with the immunoassay method. The measurement of serum HER-2/neu ECD is used for prognosis of metastatic breast cancer and for testing the treating effect of trastuzumab (Herceptin(R)), a target agent for the patients positive to the HER-2/neu receptor. In Korea there is no report on the accurate reference range of serum HER-2/neu for healthy women. The purposes of this study were to measure the serum HER-2/neu ECD in healthy Korean women, analyze the reference range. METHODS: The subjects of the study include 200 healthy Korean women with 50 from each in their twenties, thirties, forties, and fifties. As for methodology, the HER-2/neu in the serum separated from their blood was measured. The serum HER-2/neu level was measured quantitatively with the recently developed ADVIA Centaur(R) automated immunoassay analyzer and ADVIA Centaur(R) HER-2/neu assay reagent. With the measurement, you can use the sandwich immunoassay and direct chemiluminescence technique for two monoclonal antibodies for the epitopes located in the serum HER-2/neu ECD. The reference ranges were calculated based on the mean +/- 2 SD. RESULTS: One of the 200 healthy subjects was excluded from analysis for having the highest value of serum HER-2/neu (23.1 ng/ml), and the data of total 199 were used for analysis. The analysis results indicated that the minimum value was 3.5 ng/mL, the maximum value 14.5 ng/mL, the mean 8.6 ng/mL, average 8.77 ng/mL, and SD 1.61 ng/mL. The reference range of the 199 subjects's serum HER-2/neu measurements was calculated by the mean +/- 2 SD. Since the mean +/- SD of their HER-2/neu measurements was 8.8 +/- 1.6 ng/mL, the reference range was 5.6~12.0 ng/mL. The reference ranges for the age groups were 6.1~10.9 ng/mL, 5.3~11.4 ng/mL, 5.0~12.6 ng/mL and 6.3~12.6 ng/mL for the twenties, thirties, forties and fifties, respectively. The reference ranges for the age groups were analyzed statistically and there was statistical difference (p= 0.002) between fifties and twenties or thirties. The upper limit level of the reference range of serum HER-2/neu in healthy Korean women was 12.0 ng/mL. CONCLUSION: The results suggest that the reference range of serum HER-2/neu in healthy Korean women is 6.1~10.9 ng/mL, 5.3~11.4 ng/mL, 5.0~12.6 ng/mL and 6.3~12.6 ng/mL for the twenties, thirties, forties, and fifties, respectively. There is no significant difference between the twenties, thirties, forties, each other. According to analyzed statistically, there is difference between fifties and twenties or thirties (p= 0.002), but there is no statistically significant difference between forties and fifties.

7.Clinical Evaluation of Recurrent Intussusception.

To evaluate the clinical findings of the recurrent intussusception. 351 patients with 445 intussusceptions were reviewed. Recurrence rate, pattern of recurrence, reducibility, pathologic lead points (PLP), and operative findings and long term follow up of the multiple recurrences were analyzed. Of 351 patients, 303 had no recurrence, 26 had one recurrence, and 22 had multiple recurrences. Over all recurrence rate was 16.4% ; 18.5% were managed by air reduction, 16.2% by barium reduction and 5.9% by operation. Elven PLPs were proved operatively operatively and an additional 6 suspected PLPs were depicted radiologically. The most frequent PLP was ileal lymphoid hyperplasia. Intervals between reduction and recurrence were less than 2 weeks in 31 cases, between 2 weeks and 1 year in 55, and more than 1 year in 8. The longest interval was 2 years and 4 months.

10.Five-Layered Reinforcing Amniotic Membrane Transplantation for Treatment of Deep Corneal Ulcer or Perforation.

PURPOSE: To evaluate the efficacy of five-layered reinforced amniotic membrane (AM) transplantation for treating deep corneal ulcer or perforation. CASE SUMMARY: We performed a five-layered reinforced AM transplant in three cases of corneal ulceration or perforation using mechanical compressed lyophilized AMs. In all cases, the perforated cornea healed with re-epithelization within 2 weeks and the ocular surface was stabilized for more than 1 year. CONCLUSIONS: Five-layered reinforced AM transplantation may be an alternative method for treating deep corneal ulcers or perforations.
