1.The Consideration about the Histology and Its Prognosis According to the Gleason Grade System of Prostatic Adenocarcinoma.

Prognosis of prostatic carcinoma can be cheefly influenced not by method of treatment, but histological differentiation and tumor stage. We reviewed histologic grade and survival of prostatic carcinoma retrospectively using the Gleason grade system based on gland differentiation and relation between gland and stroma in 25 patients from Jan, 1980 to Jan, 1987, Whom we had follow-up data about. The following results were obtained. 1. The average age of patients was 69 years with a range of 56 to 84 years. 2. We identified 1 patients as grade 2, 4 patients as grade 3, 9 patients as grade 4, 11 patients as grade 5 of total 25 patients. In 2 year survival, there were 100% for low combined Gleason(2-4), 90% for intermediate group(5-7), 80% for high grade. 3. There was close relationship between tumor grade and stage such that high grade is high stage, low grade is in low stage. 4. There were high mortality index(0.102) in high grade group, low mortality index(0.039 %) in low grade group in prostatic index combined Gleason grade with clinical tumor staging.

2.A Case of Spontaneous Rupture of the Kidney.

Spontaneous renal rupture is relatively uncommon entity. In Korean literature, 8 cases have been reported. We experienced 1 case of spontaneous renal rupture secondary to hydronephrosis caused by metastatic adenocarcinoma of undetermined origin invading upper ureter and presented it with a brief review of literature.

The metastasis of renal cell carcinoma to ureter, urinary bladder and uterus are not common, only a few cases were reported. A 63 years old female was admitted to department of urology, Korea university hospital with intractable hematuria, vaginal bleeding and severe frank pain on left is side with abdominal distension. Herein we are presenting a case of metastatic renal cell carcinoma with review and literature.

4.The Ectopic Ureteral Orifice.

We describe 6 case of ectopic ureteral orifice was on vaginal wall and the most frequent symptom of ectopic ureteral orifice was incontinence. Vagino-gram and indigo carmine test was very important in diagnosis of ectopic ureteral orifice and identification of ectopic ureteral orifice and then nephrectomy, partial nephrectomy with ureterectomy was rational in treating ectopic ureteral orifice combining loss renal function.

5.Giant Hydronephrosis.

Giant hydronephrosis is a clinical entity in which errors of diagnosis are frequent due to the silent development of this pathologic state. We present a case which has been considered worthy of detailed report for various reasons. First, it was a 22 liter hydronephrosis which is, so far as can be discovered from available Korean literature, the largest that has been removed surgically. Secondly, despite of huge hydronephrosis, the patient experienced delivery.

The size of ectopic kidney was unusually small, chest nut size, as dislocated kidney congenitally which made put more effort then usual diagnostic approach to locate it.

8.Is the GABA System Related to the Social Competence Improvement Effect of Aripiprazole? An 18F-Fluoroflumazenil PET Study.

OBJECTIVE: Patients with schizophrenia who are treated with aripiprazole experience some benefits including an improvement of social competence, but the underlying mechanism of this improvement has not been investigated yet. This study aimed to provide preliminary evidence that the GABA system may be involved in the effect of aripiprazole on social competence. METHODS: Seventeen outpatients with schizophrenia (9 taking aripiprazole and 8 taking risperidone) and 18 healthy controls underwent 18F-fluoroflumazenil PET, and GABAA receptor binding potential was compared between the three groups. RESULTS: Voxelwise one-way ANOVA showed that GABAA receptor binding potentials in the right medial prefrontal cortex (p=0.04) and right dorsolateral prefrontal cortex (p=0.02) were significantly lower in the aripiprazole group than the risperidone group, and those in the left frontopolar cortex (p=0.03) and right premotor cortex (p=0.02) were significantly lower in the aripiprazole group than the risperidone and control groups. CONCLUSION: Our results suggest that aripiprazole administration results in increased GABA transmission in the prefrontal regions, and that these increases may be a neural basis of aripiprazole's clinical benefits on an improvement of social competence.

9.Early Experience of Busan-Ulsan Regional Cardiocerebrovascular Center Project in the Treatment of ST Elevation Myocardial Infarction.

BACKGROUND/AIMS: The purpose of this study was to compare changes in primary percutaneous coronary artery intervention (PCI) outcomes after starting the government-directed Busan-Ulsan Regional Cardiocerebrovascular Center Project. METHODS: Patients with ST segment elevation myocardial infarction (STEMI) who visited the Busan-Ulsan Regional Cardiocerebrovascular Center from 1 June 2009 to 30 May 2011 were selected. Their medical records were retrospectively reviewed. Clinical and survival outcomes before and after starting the project were compared. RESULTS: A total of 122 patients (mean age, 63 +/- 13 years; male, 74%) with STEMI were selected for analysis. There were no significant differences in patients' baseline characteristics. After starting the Busan-Ulsan Regional Cardiocerebrovascular Center Project, the door-to-balloon time decreased from 72 +/- 30 to 59 +/- 22 minutes (p = 0.011). The door-to-balloon time when the PCI team did not stay in the hospital also decreased from 80 +/- 30 to 62 +/- 12 minutes (p = 0.005). However, there was no significant change in the total ischemic time (339 +/- 293 vs. 304 +/- 287 minutes, p = 0.514), survival discharge rate (94% vs. 93%, p = 1.000), or 1-year survival rate (89% vs. 91%, p = 0.996). CONCLUSIONS: After starting the government-directed Busan-Ulsan Regional Cardiocerebrovascular Center Project, the door-to-balloon time was significantly reduced. However, the total ischemic time and short-term survival remained unchanged.
