The species composition, population density, and seasonal prevalence of vector mosquitoes in an inland area of Kyungpook province were studied, based on light trap and human bait trap collections, and the recent patterns of infestation for infective larvae of Brugia malayi in these vector hosts were investigated from May to November in 1985. Nine species in four genera of mosquitoes were collected by light trap, human bait trap, and/or by nets. Anopheles sinensis Wiedemann was the most abundant species collected by light traps during this year. Culex tritaeniorhychus Giles was the second abundant species, and Aedes vexans nipponii(Theobald) ranked third in total abundance. The earlist time when A. sinensis were found was the middle of May. At that time the temperature ranged from 14.3 C to 22.8 C and the humidity 53-90 per cent. The month of highest average nightly catch was July, when the temperature was betweeen 21.5 C and 30.6 C and the humidity 72-91 per cent. The peak time of biting activity of mosquitoes was different in each month, i.e. between 22:00-23:00 in July, and 20:00-21:00 hours in September, when the temperature was between 24.3 C and 26.5 C and the humidity 73-88 per cent in the field. While infective larvae of B. malayi were reported to have been found in one species of mosquito in 1975, no larvae were found in any species collected in this survey.

2.Changes in Glucose and Cholesterol Level in Schizophrenic Patients Treated with Atypical Antipsychotics or with Valproate Combination.

OBJECTIVE: The association of obesity, diabetes mellitus, drug compliance, and body image with use of antipsychotic medication has been documented in many studies. We aimed to assess the effects of antipsychotics and sodium valproate combination therapy on glucose, cholesterol level and weight in schizophrenic patients. METHODS: Chronic schizophrenic patients were grouped according to drug combination: risperidone, olanzapine, haloperidol, risperidone+valproic acid (VP), olanzapine+VP, haloperidol+VP. Planned assessment of included glucose and cholesterol, which were collected at baseline and at the end of the 12 weeks later. RESULTS: There were significantly increased glucose and cholesterol level, in olanzapine+VP group, but not in risperidone group. Cholesterol levels were not increased at the end of 12 weeks for haloperidol and risperidone group. Glucose and cholesterol levels had no significant association with weight gain. CONCLUSION: Changes in glucose and cholesterol level, weight, and body mass index associated with antipsychotics including sodium valproate combination were reported in our report. In the future, we need follow up study and various studies about other combination drug and metabolic symptoms.

4.The Reasonability of Using Radical Surgery for Improving Survival Rate in Elderly Gastric Cancer Patient.

Three hundred ninety potentially curative resections for an adenocarcinoma of the stomach were performed in the Surgical Department of Chonbuk National University Hospital between 1991 and 1995. Eighty-nine patients were over 65 years of age, and three hundred-one patients were under 65 years of age. Pre-operative risk factors were statistically common in the over 65-years-old group(p<0.0044). Among the risk factors, pulmonary dysfunction was the most common pre-operative risk factor in both age groups, but diabetes mellitus was statistically significant factor in the under 65-years-old group. The incidence of post-operative complications revealed no statistical difference between two groups, and diarrhea was the most common complication in both groups. The incidence of complications increased when the disease was in an advanced stage and we did an extended operation, including a gastrectomy and a lymphadenectomy, but there was no statistical difference between the two age groups. The 3-year survival rate was higher when we did a curative resection with a subtotal or a total gastrectomy with a D2 lymphadenectomy than when we did non-curative resection, but there was no statistical difference between the two age groups. Hence, a curative resection including a radical gastrectomy with a D2 lymphadenectomy, if it is indicated, is reasonable approach for improving the survival rate in patients with an adenocarcinoma of the stomach, even elderly patients over 65 years of age.

5.Analysis of predictive factors for difficult ProSeal laryngeal mask airway insertion and suboptimal positioning.

BACKGROUND: There has been controversy about predicting difficult LMA insertion and suboptimal position. Our aim was to evaluate bedside predictors for difficult LMA ProSeal(TM) (PLMA) insertion and suboptimal position. METHODS: As the potential predictive factors for difficult PLMA insertion and suboptimal position, we considered male gender, increased body mass index (BMI), seven individual items suggesting difficult airway [modified Mallampati classification > or = III, inter-incisor distance < or = 5 cm, thyromental distance < or = 6.5 cm, head/neck movement < or = 90degrees, history of difficult intubation, buck of teeth > or = moderate, upper lip bite test (ULBT) > or = II] and > or = 3 of total airway score which is the sum of scores assessed by a score of 0, 1, 2 in seven individual items. The PLMA position was assessed by fiberoptic bronchoscopy to determine whether these predictors predict suboptimal position of PLMA (fiberoptic score < 3, as graded on a standard fiberopitc scale). We also investigated the effect of predictive factors on the failure of the first insertion of PLMA and time required for successful Proseal LMA insertion on the first attempt. RESULTS: 154 patients were enrolled in the study. The total airway score did have a significant relationship with the fiberoptic findings. The male gender and ULBT I of investigated predictors did significantly correlate with failure on the first insertion of PLMA. We did not find any significant relationship between the predictive factors and PLMA insertion time on the first attempt. CONCLUSIONS: The male gender and ULBT I indicate difficult PLMA insertion, and the total airway score > or = 3 indicates suboptimal position of PLMA.

6.Insecure Attachment and Impulsivity-Inattention Problem in Adolescent with a High Risk of Substance or Internet Addiction.

OBJECTIVES: This study was done in adolescents with a high risk of substance or internet addiction in order to confirm the assumption that insecure attachment formation and impulsivity-inattention problems are major risk factors in adolescent addictive behavior. METHODS: 2,188 middle and high school students including a nicotine dependent treatment group were assessed using self-reporting scales : Adolescent Drinking Index (ADI), Fagerstrom Tolerance Questionnaire, Young Internet Addiction Scale, Revised Adult Attachment Scale (RAAS), and the Conner and Well's Self-reporting Scale for ADHD (CASS) were used. Subjects were classified into risk groups including substance addiction, internet addiction, as well as a combined group. RESULTS: Significant correlations were found between attachment formation and internet addiction tendencies, with respect to dependence, anxiety, and closeness (r=-0.185, r=0.248, r=-0.147, p<0.01, respectively). Impulsivity-inattention problems had positive correlations with internet addiction, alcohol addiction and nicotine addiction groups (r=0.345, r=0.211, r=0.187, p<0.01). With regard to attachment formation, the four groups showed significant differences regarding dependence (F=19.427, p<0.01), anxiety (F=28.926, p<0.01), and closeness (F=12.853, p<0.01). In addition, the four groups showed significant difference with respect to impulsivity-inattention problems (F=83.857, p<0.01), of which the combined risk group showed the highest scores, and the non-addicted group had the lowest scores. CONCLUSION: Insecure attachment formation and impulsivity-inattention problems were major risk factors for adolescent addictive behavior including internet addiction.

OBJECTIVES : We investigated the relationship between periventricular and deep white matter hyperintensity and cognitive function in Alzheimer's disease (AD) and mild cognitive impairment (MCI). METHODS : T2-weighted MRI scans were performed in 41 subjects with AD 38 subjects with mild cognitive impairment and 38 control subjects. Periventricular and deep white matter hyperintensities were rated on a Fazekas 0-3 scale by a medical specialist of the department of radiology blind to clinical diagnosis. Cognitive function was assessed by using Cognitive Assessment and Reference Diagnoses System. RESULTS : No significant differences between demographic characteristics and vascular risk factors were revealed comparing AD, MCI and controls. The frequencies of AD were significantly higher than those of MCI and normal control in Grade 2 and 3 of periventricular hyperintensity and Grade 3 of deep white matter hyperintensity. The scores of amnesia, executive function and attention were significantly lower in Grade 2 and 3 of periventricular hyperintensity than in Grade 0 and 1. The scores of attention were significantly lower in Grade 3 of deep white matter hyperintensity than in Grade 0, 1 and 2. CONCLUSION : Periventricualr hyperintensities are associated with cognitive decline in amnesia, executive function and attention, while deep white matter hyperintensities are associated with cognitive decline in attention.

The purpose of this extension study was to assess the long-term efficacy and safety of gemigliptin 50 mg in patients with type 2 diabetes mellitus (T2DM). Patients with T2DM who had completed the initial 24-week study comparing gemigliptin monotherapy with placebo were eligible to enrol. In the open-label, 28-week extension study, all enrolled patients received gemigliptin, regardless of the treatment received during the initial 24-week study period. The mean reduction±standard deviation (SD) in glycosylated hemoglobin (HbA1c) observed after 24 weeks of treatment (–0.6%±1.1%) was further decreased for the gemi-gemi group and the mean change in HbA1c at week 52 from baseline was –0.9%±1.2% (P<0.0001). For the pbo-gemi group, HbA1c decreased after they were switched to gemigliptin, and the mean change in HbA1c at week 52 from baseline was –0.7%±1.2% (P<0.0001). Furthermore, the overall incidence of adverse events demonstrated that gemigliptin was safe and well tolerated up to 52 weeks.
