2.Erythema ANnulare Centrifugm: Report of one Case.

Erythema Annulare Centrifugum is characterized by migratory annular, arcuate or polycyclic erythemas with central clearing. Its etiology is not conclusive but there are many reports about the etiology, i.e." due to dermatophytid"," the drugs",' intemal diseases", or" unknown etiology" etc. A case of Erythema Annulare Centrifugum with "unkown etiology" was reported. The patient revealed slightly elevated, migratory annular erythemas with central clearing on the both posterior portions of thighs. Histopathologically, there was no abnormality in the epidermis. 4.Hemodynamics and Left Ventricular Cineangiographic Findings in Idiopathic Dilated Cardiomyopathy.

The authors analyzed data from 15 patients with idiopathic dilated cardiomyopathy to evaluate the hemodynamic changes and left ventricular cineangiogram as compared with normal control. Mean right atrial pressure, right ventricular systolic pressure, mean pulmonary artery pressure and mean pulmonary wedge pressure were signigicantly elevated in patients with dilated cardiomyopathy. Left ventricular enddiastolic volume was increased in idiopathic dilated cardiomyopathy(139.9+/-58.73 ml/m2). Cardiac index, left ventricular ejection fraction and circumferential fiber shortening were significantly reduced in patients with dilated cardiomyopathy as compared with normal control(p<0.001). Hypokinetic, diffuse wall motion abnormalities of left ventricle were common in idiopathic dilated cardiomyopathy. A few cases of akinetic or dyskinetic segmental wall motion abnormalities were present. Left ventricular configurations in patients with idiopathic dilated cardiomyopathy were globe shape(53.4%) as compared with pear core shape(90%) of normal control. Associated mitral regurgitations in patients with idiopathic dilated cardiomyopathy confirmed by left ventricular cineangiogram were 53.3 percent. Mild to moderate mitral regurgitations were often present(46.6%).

6.Poikilodermatomyositis.

The authors observed 2 cases of poikilcdermatomyositis who were 16 and l0 years old female. The former visited our dept. due to rashes on the anterior portion of right thigh with 3 months duration. The skin lesion revealed mottled skin pigmentation, telangiectasia, atrophy, and cigarrette paper like wrinklings. On the palpation, hardness and tenderness in the lesion were felt. The latter visited our dept. due to weakness of the right lower extremity for 3years. The skin lesion revealed the same feature as the former but more prominent. Histophthologically, P.V.A. patterns are seen in the both skin lesions. The epidermis shows moderate atrophy of St. malphigi and flattening of reteridges. The dermis is edematous, vascular dilatations, and cellular infiltrations, most of lymphocytes, around the blood vessels. The muscle biopsies for the Quardriceps Femoris sbow Dermatomyositis patterns which the muscle bundles are degenerative changes and many cellular infiltrations, chiefly of lymphocytes, are seen between the muscle bundles. Diagnosis was confirmed by the clinical and histological pictures. These patients were markedly improved by the prednisolone therapy.

8.Latissimus Dorsi Flap for Breast Reconstruction.

Breast reconstruction following mastectomy has, become increasingly popular in recent years. Reconstruction surgeons have responded to patients' needs for effective and versatile procedures that will restore the normal shape and symmetry of breasts. Among the various techniques for these purpose, transverse rectus abdominis myocutaneous(TRAM) flap is now cosidered as the most acceptable technique. However, there are some limitations to use the TRAM flap, such as heavy smoker, obesity, and previous abdominal surgery. Also TRAM flaps are not good candidates for the patients with absent anterior axillary fold. In these respects, latissimus dorsi flap can be used as an alternative method for successful breast reconstuction. Latissiomus dorsi flaps have several advantages as compared to TRAM flaps, such as reliable blood supply, versatility of skin paddle orientation, and low donor site morbidity. It is also available for patients without anterior axillary fold, such as radical mastectomy defect or Poland syndrome. Latissimus dorsi flaps, however, and implants to obtain projection and symmetry of the reconstructed breast. Main disadvantages of latissimus dorsi flap method are difference in color and texture between skins of flap and chest, and capsular contracture resulted in high riding implant or distortion of breast contour. In this study, we performed latissimus dorsi flap for reconstruction fo breast and anterior axillary fold in 10 cases. To minimize the above problems more fullness and natural shape, implants were completely covered with muscle and overexpanded for more than 3-months and then deflated to the desired volume. These maneuvers produced aesthetically acceptable results during the latissimus dorsi breast reconstruction.
