Anti-thrombin III deficiency is known as a disease of autosomal dominant trait and relatively common, but in Korea, exact incidence and mortality is not known, In general, Anti-thrombin III deficiency is expressed to venous thromboembolism like deep vein thrombosis or pulmonary embolism. But, arterial embolism is very rare. We experienced a case of Antithrombin III deficiency expressed as myocardial infarction of inferior wall by huge thrombosis in the mid and distal right coronary artery.

The coronary arteriovenous fistula (CAVF) is a rare congenital anomaly but constitutes the most common hemodynamically significant coronary artery anomaly. Transcatheter embolization is as an effective alternative to surgery even though procedure may be complicated by migration of the coil into peripheral vessels or pulmonary arteries. To our knowledge, the association of CAVF with Turner syndrome was not reported. We report a case of successful coil embolization of CAVF using a complex, helical-fibered platinum coil in patient with Turner phenotype with 46,XX.

5.Screening of Genetic Polymorphisms of CYP3A4 and CYP3A5 Genes.

Given the CYP3A4 and CYP3A5's impact on the efficacy of drugs, the genetic backgrounds of individuals and populations are regarded as an important factor to be considered in the prescription of personalized medicine. However, genetic studies with Korean population are relatively scarce compared to those with other populations. In this study, we aimed to identify CYP3A4/5 polymorphisms and compare the genotype distributions among five ethnicities. To identify CYP3A4/5 SNPs, we first performed direct sequencing with 288 DNA samples which consisted of 96 Koreans, 48 European-Americans, 48 African-Americans, 48 Han Chinese, and 48 Japanese. The direct sequencing identified 15 novel SNPs, as well as 42 known polymorphisms. We defined the genotype distributions, and compared the allele frequencies among five ethnicities. The results showed that minor allele frequencies of Korean population were similar with those of the Japanese and Han Chinese populations, whereas there were distinct differences from European-Americans or African-Americans. Among the pharmacogenetic markers, frequencies of CYP3A4*1B (rs2740574) and CYP3A5*3C (rs776742) in Asian groups were different from those in other populations. In addition, minor allele frequency of CYP3A4*18 (rs28371759) was the highest in Korean population. Additional in silico analysis predicted that two novel non-synonymous SNPs in CYP3A5 (+27256C>T, P389S and +31546T>G, I488S) could alter protein structure. The frequency distributions of the identified polymorphisms in the present study may contribute to the expansion of pharmacogenetic knowledge.

6.According to Extent of Sympathectomy, Compensatory Hyperhidrosis in Essential Hyperhidrosis.

BACKGROUND: Since 1992, we developed the technique for video endoscopic sympathectomy to treat palmar hyperhidrosis. It was soon proven to be a simple and effective therapy for essential hyperhidrosis. Compensatory hyperhidrosis, however, is the main cause of patient dissatisfaction after video-assisted thoracoscopic sympathectomy. According to many authors, initial satisfaction rate was high(94-98%), but it was declined with time (66%) due to mainly to embarrassing side effects. MATERIAL AND METHOD: From January 1992 to February 1998, the thoracoscopic T2 sympathicotomy, T2 sympathectomy and T2-4 sympathectomy were performed in 315 patients suffering from Essential hyperhidrosis in the Department of Thoracic and Cardiovascular Surgery in the Respiratory Center of Yongdong Severance Hospital Seoul, Korea. Eighty-nine patients underwent T2 sympathicotomy, and Eighty-eight patients underwent division T2 sympathectomy. RESULT: All of the treated patients obtained satisfactory alleviation of essential hyperhidrosis. The global rate of compensatory sweating were ; 64.0% in T2 sympathicotomy, 73.8% in T2 sympathectomy and 87.8% in T2-4 sympathectomy. The rate of embarrassing or disabling compensatory sweating was significantly higher in T2 sympathicotomy 15.7%(14/89) and in T2 sympathectomy 32.8%(28/88) than in T2-4 sympathectomy 58.0%(80/138) with significancy in statistic analysis(p<0.05). Video- assisted thoracoscopic sympathectomy is an effective minimally invasive and effective procedure. CONCLUSION: We suggest that the incidence and degree of compensatory hyperhidrosis was closely related to the extent of thoracic sympathectomy.

7.Pedicle and Skin Territory of the Vastus Lateralis Muscle for the Anterolateral Thigh Free Flap.

The pattern of location, perforator artery and skin territory of the descending branch of the lateral circumflex femoral artery are very important to the anterolateral thigh free flap surgery. The author studied the pattern of vascularization of the anterolateral thigh in 110 Korean cadavers using macroscopic dissection and radiographic methods. The location of the points in this study were expressed as XY coordination in reference to the line between ASIS and center of the lateral margin of the patella. The location of pedicle of the vastus lateralis muscle at XY coordination valued X: 20.8+/-6.0 cm, Y: -1.2+/-1.0 cm. The length and external diameter of the pedicle were 91.7+/-54.9 mm, and 2.7+/-0.8 mm respectively. The average number of perforator artery to the vastus lateralis muscle were 3.2+/-1.5. The skin territory of the perforator of the vastus lateralis muscle was 6 x 18 cm2, its location was 20.8 cm on the line AB. The pattern of distribution and territories of the anterolateral thigh obtained in this study will provide useful anatomical backgrounds for the flap surgery of anterior and anterolateral thigh.

8.Detectability of the Mediastinal Lines: Comparison of Conventional Film-Screen Radiography and DigitalRadiography.

PURPOSE: Using dynamic range compression (DRC) processing, this study compared the detectability ofmediastinal lines by conventional film screen rediography (FS) and by storage phosphor digital radiography(DR). MATERIALS AND METHODS: We selected 200 normal consecutive chest radiographs (100 FS, 100 DR) ; dynamic rangecompression was applied to DR processing and moving grids were used in both systems. Seven mediastinal lines (leftfaraspinal, right paraspinal, azygoesophageal, left para-aortic, posterior junctional, anterior junctional andright paratracheal) were scored from 0 point to 3 point(0:not visible, 1:suspiciously visible, 2:visible, but notclear, 3: clearly visible) according to visibility and sharpness, as agreed by a radiologist and a resident. Thedifferences between the two modalities were compared and analyzed by chi-square test. RESULTS: Among the 1400mediastinal lines analyzed, 419 lines by DR(59.9%) and 232 lines by FS(33.1%) were scored more than 2 points. Inall mediastinal lines except the left para-aortic, DR was more detectable and clearer than FS, with statisticalsignificance(P<.01). CONCLUSION: DR processed with DRC visualizes mediastinal lines more frequently and clearlythan conventional FS, and is therefore thought to be useful for the evaluation of mediastinal diseases.

9.A Case of Complete Remission in Ruptured Hepatocellular Carcinoma after One -time Transcatheter Arterial Chemoembolization.

There are many kinds of treatments for hepatocellular carcinoma (HCC) such as surgical resection, liver transplantation, chemotherapy, interventional therapy [TACE, ethanol embolization, Immuno -chemoembolization, I131 -lipiodol embolization], thermal therapy, cryotherapy, and radiation therapy. Generally spontaneous remission is not common in HCC, however underlying mechanism of spontaneous remission is uncertain. We report a case of complete remission after one time TACE in ruptured HCC with review of literature about the effect of TACE and spontaneous remission. We conclude that arterial embolization is an effective alternative to surgery for hepatic hemostasis in patients with spontaneous rupture of hepatocellular carcinoma.

10.Morphometrics of Arterial Supply for External Pudendal Artery Flap in Koreans.

This study was performed to measure the distance of perforating point for superficial and deep external pudendal arteries from pubic tubercle and anterior superior iliac spine, patterns of arterial distribution, and external diameter. Fifty three thighs from 27 Korean cadavers (13 males / 14 females), clinically normal and without deformity, were dissected and standard points were determined as follows: point of pubic tubercle (A) and point of anterior superior iliac spine (B). The obtained results were as follows: 1. The external pudendal artery branched from femoral artery was distributed in the medial thigh. Superficial external pudendal artery is located above the saphenous opening and Deep external pudendal artery is located below the saphenous opening. 2. The saphenous opening were 4.7+/-1.2 cm apart from point A and 10.1+/-1.3 cm from B. 3. The superficial external pudendal artery were 5.2+/-1.2 cm apart from A and 8.9+/-2.1 cm from B. 4. The deep external pudendal artery were 4.7+/-1.1 cm apart from A and 10.8+/-1.3 cm from B. Consequently, the patterns of distribution of superficial and deep external pudendal arteries, obtained in this study, will provide useful anatomical backgrounds for the external pudendal flap surgery in korean.
