5.Fluoride Release and Recharge Properties of Several Fluoride-Containing Restorative Materials

The aim of the study is to compare the fluoride release and recharge properties of glass ionomer cements and ‘alkasite’.
Specimens of two glass ionomer cements (Fuji Ⅸ GP and Riva Self Cure), ‘alkasite’ restorative material (Cention N) and composite resin (FiltekTM Z350XT) were prepared. The fluoride release of each specimen was measured for 28 days. Thereafter, 1.23% acidulated phosphate fluoride (APF) gel was applied to experimental groups. No treatment was performed on control groups. The fluoride release was measured for additional 7 days to evaluate the fluoride recharge properties of each materials.
The fluoride release was highest in Riva Self Cure, followed by Fuji Ⅸ GP, Cention N (p < 0.05). Fluoride release of Cention N was measured to be approximately 49% of Fuji Ⅸ GP’s. After the application of 1.23% APF gel, increases in fluoride release were observed in Riva Self Cure, Fuji Ⅸ GP and Cention N (p < 0.05). Fluoride recharge was observed in Cention N as well as in glass ionomer cements. Further studies are required to evaluate the anti-cariogenic properties of Cention N at clinical conditions.
6.Resilience and Characteristics of Sleep and Defense among Shift Work Nurses.

OBJECTIVES: Shift work is a stressful situation. It is important to know the factors associated with the ability to adapt to a shift work schedule. The aim of the present study was to investigate the association between sleep, as well as personality variables, and the resilience of shift work nurses. METHOD: Self-report questionnaires were administered to 95 nurses who worked in one national university hospital. Connor-Davidson resilience scale, hospital anxiety and depression scale, morningness-eveningness scale, Pittsburgh sleep quality index, other sleep-related questionnaires, and Korean defense style questionnaires were used. RESULTS: Age, shift work duration, off-day oversleep, depression, anxiety, adaptive defense style, and self-suppressive defense style were significantly associated with resilience (p < 0.05). Multiple regression analysis showed that age (beta = 0.34, p < 0.05), depression (beta = -0.25, p < 0.05), adaptive defense style (beta = 0.45, p < 0.001), and self-suppressive defense style (beta = -0.19, p < 0.05) significantly predicted the resilience of shift work nurses. Concerning individual defense mechanisms, resignation (beta = -0.20, p < 0.05), sublimation (beta = 0.19, p < 0.05), omnipotence (beta = 0.19, p < 0.05), and humor (beta = 0.20, p < 0.05) significantly predicted the resiliency. CONCLUSION: The findings indicate that a specific defense style and other mechanisms were associated with the resilience of shift work nurses. A future prospective study with more participants could further clarify the relationship between sleep-related variables, as well as personality factors, and resilience of shift work nurses.

7.A New Neolignan Derivative, Balanophonin Isolated from Firmiana simplex Delays the Progress of Neuronal Cell Death by Inhibiting Microglial Activation.

Excessive activation of microglia causes the continuous production of neurotoxic mediators, which further causes neuron degeneration. Therefore, inhibition of microglial activation is a possible target for the treatment of neurodegenerative disorders. Balanophonin, a natural neolignoid from Firmiana simplex, has been reported to have anti-inflammatory and anti-cancer effects. In this study, we aimed to evaluate the anti-neuroinflammatory effects and mechanism of balanophonin in lipopolysaccharide (LPS)-stimulated BV2 microglia cells. BV2 microglia cells were stimulated with LPS in the presence or absence of balanophonin. The results indicated that balanophonin reduced not only the LPS-mediated TLR4 activation but also the production of inflammatory mediators, such as nitric oxide (NO), prostaglandin E2 (PGE2), Interleukin-1β (IL-1β), and tumor necrosis factor-α (TNF-α), in BV2 cells. Balanophonin also inhibited LPS-induced inducible nitric oxide synthase (iNOS), and cyclooxygenase-2 (COX2) protein expression and mitogen activated protein kinases (MAPKs), including extracellular signal-regulated kinase (ERK1/2), c-Jun N-terminal kinase (JNK), and p38 MAPK. Interestingly, it also inhibited neuronal cell death resulting from LPS-activated microglia by regulating cleaved caspase-3 and poly ADP ribose polymerase (PARP) cleavage in N2a cells. In conclusion, our data indicated that balanophonin may delay the progression of neuronal cell death by inhibiting microglial activation.

Good sleepers tend to more resilient than poor sleepers. As sleep and resilience may have a bidirectional relationship, it is important to explore the relationship between healthy sleep and resilience. Objectively and subjectively measured sleep quality showed positive association with resiliency. In one study, more resilient adolescents had higher sleep efficiency, less light sleep, more slow-wave sleep, and a smaller number of awakenings after sleep onset. Circadian typology, jetlag and circadian misalignment may be associated with the capacity to deal with adversity. Eveningness, exposure to chronic jetlag and circadian misalignment might be risk factors for development of psychological problems and mental disorders. Additional studies are needed to evaluate whether promoting healthy sleep behaviors results in increased resilience.

9.Analgesia-based Sedation Using Remifentanil during Percutaneous Endoscopic Lumbar Discectomy.

BACKGROUND: The aim of this study was to examine the safety and efficacy of sedation and analgesia using remifentanil during percutaneous endoscopic lumbar discectomy (PELD). METHODS: Eighty ASA patients with physical status 1 or 2 who underwent a PELD were enrolled in this study. They were randomized to receive one of two treatments: a fentanyl bolus of 0.7microgram/kg 5 min before the procedure and of 0.7microgram/kg during the procedure (n = 40, group F), or a remifentanil titration at an infusion rate of 0.1-0.3microgram/kg/min available throughout the procedure according to the appeal of pain, level of sedation and side-effects (n = 40, group R). The observer's assessment of alertness/sedation (OAA/S) scale, blood pressure, heart rate, respiratory rate, SpO2, and end tidal CO2 were assessed and measured during and/or after the procedures. The visual analogue scale of pain (VAS), and the patient and endoscopist satisfaction scale were assessed after the procedures. RESULTS: There were no significant differences between the two groups in terms of the recovery characteristics, incidence of complications and satisfaction score of patients. In 92.5% of the cases among the remifentanil group, the spine surgeon made uniform judgements that remifentanil worked better than the usually used fentanyl procedure, whereas in 7.5% of the cases the effects were indifferent. The VAS score of the R group was significantly lower than that of the F group. CONCLUSIONS: We concluded that sedation and analgesia with remifentanil is very useful for painful local procedures such as PELDs.
