Chinese Journal of Emergency Medicine 2024;33(4):485-489
Objective:To analyze the clinical characteristics of invasive klebsiella pneumoniae liver abscess syndrome (IKPLAS), and compare it with common pyogenic liver abscess (CPLA). Methods:The social demography and clinical data of inpatients with pyogenic liver abscess from January 2011 to December 2021 in the Peking University People's Hospital were collected. Based on the presence or absence of invasive infections and the results of bacterial etiology, IKPLAS was diagnosed and compared with CPLA. The general information, symptoms, past medical history, auxiliary examinations and prognosis indicators of the two groups of patients were compared. Results:Total of 172 patients with pyogenic liver abscess were collected, including 25 cases of IKPLAS. Compared with CPLA group, the proportion of fever in IKPLAS group was lower, the proportion of diabetes history was higher, the proportion of monocytes was lower, and procalcitonin and urea nitrogen were higher(all P<0.05), the proportion of multiple abscesses is higher, and the positive rate of blood culture and the cultivation of klebsiella pneumoniae are both higher (all P<0.05).A total of 9 cases (5.2%) of patients developed septic shock, of which 2 cases (1.2%) died. The IKPLAS group had a higher proportion of ICU admissions ( P<0.05),but but the difference of mortality between the two groups was not statistically significant ( P>0.05). The most common sites of invasion infection in the IKPLAS group are the lungs(22/25), brain(9/25), and eyes(9/25). Conclusions:There are differences in clinical characteristics between IKPLAS and CPLA, the most common sites of invasion infection are the lungs, brain, and eyes, but there is no difference in mortality in this study. For PLA with pathogenic Klebsiella pneumoniae, it is necessary to carefully evaluate the presence of invasive lesions and provide targeted local treatment to better improve prognosis.

Male lower urinary tract symptoms is a collective term for a group of symptoms associated with lower urinary tract disorders characterized by frequent urination, urgent urination, and difficulty of urination, of which the common causes are benign prostatic hyperplasia or overactive bladder. With the aging of the global population, the incidence of male lower urinary tract symptoms is increasing year by year. Based on the theoretical connotation of " zang-fu extraordinary connection", the relationship between the lungs and the bladder, the spleen and the small intestine, and the kidneys and the sanjiao with the formation of male lower urinary tract symptoms is explained from three perspectives. It is believed that lung qi depression and closure, disturbance of qi transformation in bladder, and insufficient spleen transport occur, the small intestine is dysfunctional, kidney yang is exhausted, and obstruction of the sanjiao waterway are the basic pathogenesis of male lower urinary tract symptoms. It is emphasized that the location of the disease should be identified. If it is related to the lungs and the bladder, then the disease should be treated by lifting the pot to lift the lid and diffusing the lung qi to benefit the bladder with Wuling Powder plus perilla leaf and bitter apricot seed; if it is related to the spleen and the small intestines, then the disease should be treated by raising the clear and directing the turbid downward and improving the spleen qi in order to support the small intestines with Shenling Baizhu Powder; if it is related to the kidneys and the sanjiao, then the disease should be treated by seeking the yang within the yin and warming and tonifying the kidney qi in order to dredge up the sanjiao with Bawei Shenqi Pill. According to the patient's condition, treatment can be combined with acupuncture, exercise, and other therapies. This paper can provide reference for clinical treatment of male lower urinary tract symptoms.

Male infertility,a common condition in andrology, falls under the category of"no son"in traditional Chinese medicine(TCM).Unhealthy eating habits and excessive sexual activity, prevalent due to improved living standards, have contributed to the increasing incidence of male infertility.YE Tianshi proposed the theory of"maintaining with the sweet and restoring the body fluids",which emphasizes the importance of ample body fluid for the nourishment of the male essence chamber.Sufficient body fluid is crucial for normal sperm generation. In TCM,the primary pathogenesis of male infertility involves the loss of body fluids and insufficiency of yin essence.Sweet Chinese herbal medicinal is recommended as it nourishes yin, enriches essence, and replenishes male reproductive essence without producing phlegm and dampness. Therefore, when treating male infertility, attention should be given to the use of sweet Chinese herbal medicinal, adhering to the principle of"abundant body fluids nourish kidney yin,and abundant kidney yin supports semen production".Therapies such as purging fire to preserve body fluids with Zengye Decoction, nourishing yin to enrich essence with Guilu Erxian Decoction, benefiting qi to promote fluid production with Buzhong Yiqi Decoction, and warming yang and transporting fluids with Xianfu Shezi Decoction should be considered.Medication and dosage adjustments should be made based on the specific etiology, pathology, and related symptoms to improve the quality of male sperm and enhance the chances of conception.

Objective:
This study aimed to present our clinical experience of the utilization of remnant ear for the inferior part and lobule reconstruction in microtia patient with the fully pre-expansion ear reconstruction technique.
Methods:
During 2017.01—2018.12, 156 cases of unilateral ear reconstruction were performed. The operation was divided into three stages. Six months after the ear reconstruction, the third stage of the remnant ear utilization was performed. The surgery was planned according to the microtia classification and the remnant morphology.
Results:
All the patients had no complications. Follow-up period ranged from 7—24 months, and ten months on average. All the patients with typeⅠmicrotia had satisfying results of the inferior part and lobule reconstruction. Sixteen cases of types Ⅱand Ⅲ microtia patients had unfortunate outcomes, because the residual ears were too small to reconstruct the earlobe, and the incisure appeared when remnant and reconstructed ear was connected in cases of severe remnant malposition. The rest of patients were satisfied with the reconstruction.
Conclusions
The utilization of the remnant ear based on the types of microtia and the remnant morphology could achieve the satisfying result of reconstruction for most of the microtia patients.

Journal of Medical Biomechanics 2020;35(1):E101-E107
Objective To investigate the influence of gravity levels on lower limb motions during human walking. Methods A suspended microgravity simulation system was designed for the experiment. Kinetic parameters from lower limb joints of twelve volunteers during walking were measured by the motion capture system and 3D force plate under simulated Mars gravity (1/3 G), lunar gravity (1/6 G) and earth gravity (1 G). Results Under simulated Mars and lunar gravity, the ranges of motion (ROMs) of hip and knee joints in sagittal plane significantly decreased (P<0.01) while the ROMs of ankle joints obviously increased (P<0.05). The ROMs of hip, knee and ankle joints in sagittal plane under earth gravity were 45.2°, 67.7°, 32.5°, respectively, while the ROMs of hip, knee and ankle joints under lunar gravity were 25.1°, 50.8°, 42.4°, respectively. In addition, the force and torque of lower limb joints in sagittal plane under lower gravity also decreased significantly (P<0.01). Conclusions The results obtained by this study were almost the same as the research findings obtained by using treadmill. Astronauts can use the treadmill and microgravity simulation system for walking exercises under lunar gravity on the earth.

ObjectiveTo explore the effect of Naoxin'an capsule against mitochondrial dysfunction and oxidative damage in brains of rats with vascular cognitive impairment (VCI) induced by chronic cerebral ischemia. MethodA total of 150 rats were randomized into modeling group (130 rats) and sham-operated group (20 rats) with the random number table method. The two-vessel occlusion (2-VO) was employed to induce VCI in rats, and finally 87 rats developed VCI. The VCI rats were classified into model group, positive drug group (Aricept, 0.5 mg·kg-1), and low-, medium- and high-dose Naoxin'an capsule groups (0.18, 0.36, 0.72 g·kg-1, separately), with 17-18 rats in each group. The administration lasted 8 weeks. The learning and working memory of VCI rats were assayed by novel object recognition test and Y-maze test. The 8-oxoguanine (8-OxoG) level was measured based on immunofluorescence staining. Spectrophotometry was performed to determine the activity of mitochondrial respiratory chain complexes Ⅰ, Ⅱ, Ⅲ, and Ⅳ, mitochondrial swelling, mitochondrial membrane potential (MMP), and activity of pyruvate dehydrogenase (PDH) and α-ketoglutarate dehydrogenase (KGDH). The submicroscopic structure of mitochondria was observed with electron microscope. The phosphorylation of cAMP response element-binding protein (CREB) and the expression of peroxisome proliferator-activated receptor γ coactivator-1α (PGC-1α), nuclear respiratory factor 1 (NRF-1), and mitochondrial transcription factor A (mt-TFA) were determined by Western blot. The mitochondrial status Ⅳ H2O2 generation, activity of cerebral superoxide dismutase (SOD) and glutathione peroxidase (GSH-Px), and malondialdehyde (MDA) content were measured with the photochemical method. ResultCompared with the sham-operated group, model group showed decrease in new object discrimination index (P<0.01) and spontaneous alternation rate in the Y-maze test (P<0.01), increase in ROS production in the brain (P<0.01), reduction in MMP and mitochondrial swelling A value (P<0.01), obvious mitochondrial swelling, vacuoles and cristae fractures in mitochondria, decrease in the level of phosphorylated CREB, expression of PGC-1α, NRF-1 and mt-TFA (P<0.01), and activity of the respiratory chain complexes Ⅰ, Ⅱ, Ⅲ, and Ⅳ (P<0.01), PDH and KGDH in the brain (P<0.01), rise of the production of H2O2 at state IV (P<0.01) and the content of MDA (P<0.01), and reduction in the activity of SOD and GSH-Px (P<0.01). Compared with the model group, each dose of Naoxin'an capsules can improve the new object discrimination index (P<0.05, P<0.01) and the rate of spontaneous alternation (P<0.05, P<0.01), decrease ROS production in the brain (P<0.01), improve the MMP and swelling A value (P<0.05, P<0.01), alleviate mitochondrial swelling and mitochondrial submicroscopic damage, elevate the phosphorylated CREB level, the expression of PGC-1α, NRF-1 and mt-TFA (P<0.05, P<0.01), and the activity of mitochondrial respiratory chain complexes Ⅲ, and Ⅳ, PDH and KGDH (P<0.01), decrease state Ⅳ H2O2 generation (P<0.01) and MDA content (P<0.05, P<0.01), and raise the activity of SOD and GSH-Px (P<0.01). ConclusionBy activating the CREB/PGC-1α pathway, Naoxin'an capsule can protect the structure and function of mitochondria, enhance the antioxidant capacity, and inhibit the mitochondrial dysfunction and oxidative damage induced by chronic cerebral ischemia, thus improving the VCI.
