1.Polymorphism of the 5R-5-hydroxytriptolide.

5R-5-hydroxytriptolide (LLDT-8) is a new drug candidate which is in clinical trial treating rheumatoid arthritis. Polymorph screening of the compound was carried out in this study. Polymorph of LLDT-8 was prepared by evaporative crystallization and antisolvent crystallization methods and was characterized by powder X-ray diffraction (p-XRD), infrared spectrometry (IR), differential scanning calorimetry (DSC) and thermogravimetric analysis (TG). It was found that p-XRD patterns, DSC curves, TG curves and IR spectra of the LLDT-8 samples prepared by the above recrystallization methods were all consistent. The 20 of main peaks in the p-XRD patterns appeared at 7.58 degrees, 8.14 degrees, 8.66 degrees, 15.46 degrees, 16.46 degrees, 29.54 degrees, 31.16 degrees and 38.26 degrees, while the infrared absorption peaks appeared at 3 471.3, 2 962.2, 2 887.0, 1 762.6, 1 677.8, 1 432.9, 1 365.4, 1 247.7, 1 080.0, 1 031.7 and 877.5 cm(-1). LLDT-8 was decomposed at 271.2 degrees C based on the determination from DSC and TG. It was showed in single crystal X-ray diffraction study that LLDT-8 crystal was monoclinic with the space group being P2 (1). The cell parameters were found to be: a = 11.460 1 (11), b = 6.320 5 (6), c = 13.028 1 (12), alpha = 90.00, beta = 115.557 (2) and gamma = 90.00. The crystal was a hydrogen-bonded dimmer. The slurry experiments, which were further conducted in solvents with different polarities, confirmed the stability of solid state of LLDT-8 based on the p-XRD determination. The polymorph of LLDT-8 made assurance of its efficacy consistence during its clinical trials.
2.Bone morphogenetic protein-1 and its related metalloproteinase

Bone morphogenetic protein-1(BMP-1) and its related molecules are members of metalloendoproteinase astacin family, including BMP-1, mTLD, mTLL-1 and mTLL-2. Even though all of them lack of the ability to induce bone or cartilage formation directly, they play key roles in numerable activities in ECM from embryo to adult, then affect the procedure and the result of osteogenesis and bone remodeling directly or indirectly. They are critical in maturation and deposition of some major collagen types, and in regulating the signaling of some growth factors in TGF-? superfamily by degradation of TGF-? inhibitor such as Chordin. The investigations about tissue distribution of BMP-1 and its related proteinases and also gene knock-out studies strongly indicate that they play key roles in osteogenesis and bone development.
3.The value of echocardiogram in preoperative examination for the diagnosis of silent myocardial ischemia in patients before surgery

Objective To study the value of echocardiogram in preoperative examination for the diagnosis of silent myocardial is-chemia in patients before surgery .Methods 172 patients with surgical preoperative suspected diagnosis of coronary heart disease , were divided into heart valve surgery group (72 cases) and non heart valve surgery group (100 cases) .All of the patients had echo-cardiography and coronary angiography before 2 weeks of operation .We drew a series of analysis to assess the sensitivity、specialty and accuracy of diagnosis of silent myocardial ischemia with regional wall motion abnormality (RWMA) and wall motion score in-dex (WMSI) .Results Coronary angiography as the gold standard of coronary heart disease diagnosis .The sensitivity of RWMA for the diagnosis of silent myocardial ischemia in patients undergoing heart valve surgery group and non heart valve surgery group was 85 .7% and 100 .0% ,while the specificity of RWMA was 100 .0% and 80 .0% ,respectively ,the differences between the two groups had no statistical significance (P>0 .05) .We also drew the ROC curve of WMSI for the diagnosis of silent myocardial ische-mia ,maximum area under the ROC curve of WMSI were 2 .20 and 2 .05 .In patients undergoing cardiac surgery and non-cardiac sur-gery ,the curves indicated that the sensitivity were 79 .7% and 75 .9% ,the specificity were 83 .0% and 67 .7% ,respectively .Area under the ROC curve was 0 .832 and 0 .702 ,but the difference was not statistically significant (P>0 .05) .Conclusion Using RW-MA and WMSI as reference indicators ,surgical preoperative echocardiography in the diagnosis of patients with silent myocardial is-chemia has good sensitivity ,specific ,and accurate ,there is no statistically significant difference between heart valve surgery and non heart valve surgery .
4.Protective effects of Jidesheng Sheyao tablet on endotoxin-induced acute lung injury in rats

Objective To investigate the protective role of Jidesheng Sheyao tablet in lipopolysaccharide (LPS)-induced acute lung injury(ALI) in rats. Methods Fifty male SD rats were randomly divided into 6 groups:normal control group,dexamethasone positive group,LPS model group, Sheyao groups of low, middle and high dosages successively.All rats, except normal control group, were administrated with LPS by intravenous injection to induce acute lung injury. The rats in positive group and three different dosage groups were treated by dexamethasone(3 mg?kg -1, iv), low dosage(0.8 g?kg -1), middle dosage(1.6 g?kg -1) and high dosage(3.2 g?kg -1) of Jidesheng Sheyao tablet(oral) respectively before LPS-induced ALI. Each rat in normal control group received injection of 1ml normal saline. The animals were killed after injection of LPS for 2 hours, and then the lung index was calculated, the histopathology of the lung injury was observed by light microscope, the expression of matrix metalloproteinase-2 (MMP-2), matrix metalloproteinase-9 (MMP-9) proteins in the lung tissues and their activity in the bronchoalveolar lavage fluid (BALF) and the lung homogenates were detected by immunohistochemistry and zymography separately. 5.Blood Test Patterns for Blood Donors after Nucleic Acid Detection in the Blood Center.

OBJECTIVETo investigate the blood test patterns for blood donors after nucleic acid detection in blood center.
METHODSThe collected blood samples after voluntary blood donors first were detected by conventional ELISA, then 31981 negative samples were detected via HBV/HCV/HIV combined nucleic acid test of 6 mixed samples(22716 cases) or single samples(9265 cases) by means of Roche cobas s201 instrument. The combined detection method as follows: the blood samples were assayed by conventional nucleic acid test of 6 mixed samples, at same time, 6 mixed samples were treated with polyethylene glycol precipitation method to concentrate the virus, then the nucleic acid test of blood samples was performed; the single detection method as follows: firstly the conventional nucleic acid test of single sample was performed, then the positive reactive samples after re-examination were 6-fold diluted to simulate the nucleic acid test of 6-mixed samples. The positive rate of positive samples detected by combined nucleic acid test, positive samples detected by nucleic acid test of mixed virus concentration and positive samples detected by single nucleic acid test was statistically analyzed. In addition, for HBVpersons the serological test yet should be performed.
RESULTSIn 22 716 samples detected by nucleic acid test of 6 mixed samples (MP-6-NAT) , 9 cases were HBV(0.40‰, 9/22716); at same time, the detection of same samples by nucleic acid test of mixed sample virus concentration showed 29 cases of HBV(1.28‰, 29/22716). In 9265 samples detected by single nucleic acid test(ID-NAT) 12 cases showed HBV(1.30‰, 12/9265), meanwhile the detection of these 12 samples with HBVby 6-fold dilution for virus concentration found only 4 samples with HBV. In serological qualified samples, ID-NAT unqualified rate was 1.28‰, which was higher than that of MP-6-NAT(0.4‰) (χ=8.11, P<0.05); but there was no statistical difference between unqualified rate of ID-NAT and MP-6-NAT(1.3‰ vs 1.28‰)(χ=0.00, P>0.05). In 41 samples with HBsAgHBV DNAdetected by ELISA, 36 samples were confirmed to be occult HBV infective(OBI) by HBsAb, HBcAb test of ELISA; out of these 41 samples, 33 samples showed HBcAb(91.66% of OBI), 5 might be HBV "window period" infective, moreover the HCV RNA and HIV RNA positive samples were not found.
CONCLUSIONTo avoid the missdiagnosis of donors with low level of virus, the nucleic acid test must be carried out after virus concentration of mixed samples when the blood test pattern of donors is nucleic acid test of mixed samples, otherwise the single nucleic acid test must be performed to obtain more high detected rate of virus nucleic acid. The HBcAb serologic test and physical examination of donors before blood donation must be enhanced on basis of serological test of HBsAg; for high risk people, the persuading no blood donation is simplest pattern.
6.The ratio of platelet count/spleen diameter and albumin as non-invasive parameters to predict the presence of esophageal varices in patients with liver cirrhosis

Objective To investigate whether the spleen diameter,serum albumin and periphefial blood cells might be as non-invasive predictive indicators for the presence of esophageal varices(EV)in patients with liver cirrhosis.The predictive values of these parameters to the large esophageal varices were evaluated.Methods OBe hundred and sixty-seven patients with liver cirrhosis underwent endoscopic examination.Among them,127 patients(mild in 41,medium in 38,severe in 48)were found with EV and 40 patients without(NEV).The diameters of portal vein and spleen vein,the sizes of spleen and the ratio of platelet count/spleen size were examined by Doppler ultrasound.The platelet count and the level of albumin were calculated.Results The average of age,diameter of portal vein and spleen vein,and sizes of spleen were higher in EV group than those in NEV group,while the platelet count,the level of albumin and the ratio of platelet count/spleen size in EV groups were lower than those in NEV group.Multifactor analysis revealed that the index related to serious EV were the blood platelet count(<70×109/L),the ratio of platelet count/spleen size(<1.0)and albumin level (<35 g/L).Conclusions The degree of EV in patients with liver cirrhosis were paralleled with the degree of portal hypertension.The patients who present with platelet count<70×109/L,or platelet count/spleen size<1.0 or albumin<35 g/L should be considered as EV,and endoscopic examination is needed.
7.Living condition and healthcare demand of elderly adults in Hangzhou

Objective To find out the living condition of the elderly adults and their needs for community bealthcare services so as to provide evidence-based guidance for our govemment. Methods A total of 925 participants aged > 60 years were personally interviewed for questionnaire and ADL scale evaluation. Chi-square test and Kmskal-Wallis H test were used for data analysis. Results The subjects showed relatively lower education level and the illiteracy rate was high. Their marriage was stable and a decreased divorce rate was found. The economic situation was satisfactory. Their life satisfaction was farbeyond the health satisfaction. Most of the elderly did not live with their offspring,although 21.8% could not take care of themselves. The prevalence rate of chronic diseases was 81.5% ,which resulted in more demands for social activities, health consultation, rehabilitation nursing, and home healtheare services in the community. Conclusion They need to pay more attention to physical and mental health of the elderly. A community healthcare system that could provide services in an-all-round way,including medical care, should be established and improved.
8.The comparison of clinical characteristics and HBeAg status in HBV liver cirrhosis patients with different HBV DNA levels

Objective To study the clinical characteristics and HBeAg status in HBV liver cirrhosis patients with different HBV DNA levels, Method Three hundred and thirty-seven patients with liver cirrhosis caused by chronic HBV infection were investigated. HBV DNA levels were detected by PCR, and HBV markers were detected by MEIA. The ratio of patients with HBeAg positive or negative in groups with different HBV DNA levels was compared, and the clinical characteristics in patients with different HBV DNA levels and HBeAg status were evaluated. Results The positive ratio of HBV DNA and HBeAg were 80.4% (271/337) and 31.5% (106/337). The negative ratio of HBeAg was 68.5% (231/337). The proportion of patients with Child-Pugh grade A, B or C and hepatocellular carcinoma (HCC) in different groups of HBV DNA levels and in different HBeAg status showed no significant difference, but the ratio of HCC in patients with HBV DNA 3-4 lg copies/ml was higher than that in patients with HBV DNA <3 lg copies/ml (P=0.014) and ≥7 lg eopies/ml (P =0.009). No significant difference of HBV DNA levels was found in different age groups, but the negative ratio of HBeAg increased with the increasing of the age. Conclusions More than 80% of patients with liver cirrhosis caused by chronic HBV infection axe HBV DNA positive, and 2/3 of them are HBeAg negative. Suppressing HBV replication may improve the prognosis of HBV related cirrhosis and HCC.
9.Study on Hydrolysis Reaction of Novel Camptothecin Derivative(L-P) Using Capillary Zone Electrophoresis

Capillary zone electrophoresis (CZE) was developed to investigate the structure stability of novel camptothecin derivative (L-P) at different pH,the kinetics and thermodynamics of hydrolysis reaction from lactone form to carboxylate form direction at near physiological conditions (pH 7.4,310 K). Uncoated fused-silica capillaries(35 cm×50 μm i. d,with effective length of 26.5 cm) were used. The background electro-lyte( BGE) was 0.025 mol/L sodium phosphate buffer with pH varied at 2.5,4.0,5.0,6.0,7.0,7.4 and 9. 0. The electrophoresis voltage was maintained at 14 kV when the pH of BGE ranged between 2.5 and 5.0,otherwise,the voltage was maintained at 10 kV. The UV detector was set at 260 nm. All samples were introduced using hydrodynamic injection at 5 kPa for 4 s. L-P was found to be lactone form as the solution pH was below 4. 0. As pH increased,the lactone form of L-P would undergo hydrolysis reaction to be carboxylate form. As pH was 9.0,L-P existed almost completely as carboxylate form. The rate constant of the hydrolysis increased as temperature raise. The energy of activation ( Ea) ,the enthalpy ( ΔH) and entropy (ΔS) of the hydrolysis reaction were determined as 72. 6 kJ/mol,10. 5 kJ/mol and 50. 9 J/( mol K) ,respectively. The proposed capillary zone electrophoresis could efficiently separate two pH-dependent structural forms of the novel camptothecin derivative( L-P). The positive enthalpy and entropy values of the L-P hydrolysis indicated that the reaction was endothermic and entropically driven and higher temperature favored.
10.Changes and significance of serum glial fibrillary acidic protein (GFAP) in patients with Parkinson disease

Objective To investigate changes in serum level of glial fibrillary acidic protein (GFAP) in patients with Parkinson disease (PD) and its clinical significance. Methods Serum GFAP was determined with sandwich ELISA for 82 patients with PD and acute cerebral infarction (ACI), as well as healthy normal controls. Patients with PD were then divided into two sub-groups in terms of their course duration, one with less than five years and the other with more than or equal to five years. Effects of course duration and age of the patients on their serum GFAP were analyzed. Results Serum level of GFAP was significantly higher in patients with PD [(1.628±0.104) μg/L] and ACI [(1. 637±0. 063 )μg/L] than that in healthy normal controls [ (0. 025±0. 003)μg/L, t = 82. 7, 142. 2, all P <0. 05 ]. But, there was no significant difference in serum GFAP between patients with PD and ACI ( t =0. 214, P > 0. 05 ). Serum level of GFAP in PD patients had no significant correlation with their age. There was no significant difference in serum GFAP between two sub-groups of patients with PD (P > 0.05 ). Conclusions Serum level of GFAP increased significantly in PD patients, as compared to healthy normal controls, but it is not specific. Serum GFAP in patients with PD maintained at certain high level with progression of the disease, indicating effect of astrocytes may persistently exist during the disease course.
