Bioethics evolved from traditional medical ethics impinges upon the sanctity of life together with its intrinsic logical fallacy and shows it in stark contrast with the concept of the quality of life,which fallacy has made end-of-life and other critical decisions hard to be materialized.The continuous development and progress of life sciences out of the constraints of old-fashioned and outdated faiths and theological considerations need to further the utilitarian appeals in order to promote both individual and social net gains in beneficence and benefits.Aiming at the Pareto Optimum,the social and individual value systems should be regarded separately,with the latter considered in a closed way,allowing for autonomy or self-determination in the evaluation of intrinsic values.In this manner,related morality and legal systems as necessary and unavoidable vices are expected to be improved and to serve and guide our medical practices.
2.Clinical observation on short-term efficacy of MINE regimen for patients with recurrent and refractory NHL

Objective To investigate the efficacy and side effects of MINE regimen, as a salvage regimen, in treating patients with recurrent and refractory intermediate grade non-Hodgkin lymphoma(NHL).Methods 45 patients failing to CHOP or CHOP-like regimen were treated with MINE regimen and every patient had received 2-6 cycles of treatment. IFO 15 g/m2, ivgtt on d1-3, combined with mesna 400 mg iv for3 times on Oh, 4h, 8h after IFO ivgtt; MIT 10 mg/m2 ivgtt on dl; VP1665 mg/m2 ivgtt on d1-3. 21-28 daysconsisted of one cycle. The therapeutic effects and side effects were evaluated according to the WHO criteria.Results In all 45 patients, 53.3 % of overall response rate were achieved CR for 9 cases, 15 cases PR, 13 cases SD and 8 cases PD. Main side effects were myelosuppression, nausea, vomit, alopecia, etc. Conclusion MINE regimen is an effective and safe salvage regimen in treating patients with recurrent and refractory intermediate grade NHL. Further clinical study on the application of MINE regimen is recommended.

4.Meta-analysis of association between peptidylarginine deiminase Ⅳ gene and rheumatoid arthritis

Objective To systematically analyze and evaluate the association between the peptidylarginine deiminaseⅣ(PADI4) gene and rheumatoid arthritis (RA) based on the published data,and to provide evidence for the pathogenesis of RA.Methods By selecting five SNPs in PADI4 (rs11203366,rs11203367,rs874881,rs2240340,rs1748033) which had been extensively examined.Meta-analysis on each SNP was performed step by step according to Hugenet manual to investigate the association of the polymorphisms of the PADI4 gene with RA.Results This Meta-analysis enrolled 15 659 RA patients and 22 019 healthy controls from 21 studies worldwide.It demonstrated that rs11203366,rs11203367,rs2240340 and rs 1748033 confered susceptibility to RA in Asian ethnicity (P<0.01,0.03,<0.01,<0.01),while rsl1203366,rsl1203367 and rs874881 confered susceptibility to RA in Caucasian of European ancestry (P=0.0002,0.004,0.03).It also shown that no significant association between rs874881 and RA in the Asian ethnicity populations (P=0.2),or rs2240340,rs1748033 and RA in Caucasian of European ancestry (P=0.18,0.1 ).A linkage disequilibrium study was also performed.The LD study showed that rs11203366,rs11203367,rs874881,rs2240340 and rs1748033 were in linkage disequilibrium both in the Asian ethnicity and Caucasian,which was basically inconsistent with the results of Meta-analysis.The conflicting results should be explained by many aspects such as bias in sample selection,genotyping,and the stratification.Conclusion The PADI4 genotype is partially associated with RA,and the underling mechanisms need further study.Haplotype based research and Metaanalysis would be valuable.
5.Effects of BNIP3 on the migration of human dermal microendothelial cell and its possible molecular mechanism

Objective:To explore the effects and mechanism of B-cell lymphoma-2/E1B 19 000 interacting protein 3 (BNIP3) on the migration of human dermal microvascular endothelial cells (HDMECs) under hypoxia.Methods:The experimental research method was applied. (1) HDMECs were divided into normoxic group received routine culture and hypoxic 6, 12, 24 h groups treated with hypoxia under oxygen volume fraction of 2% for corresponding time. Western blotting was used to detect the protein expressions of BNIP3 and microtubule-associated protein 1 light chain 3Ⅱ (LC3Ⅱ) in HDMECs. (2) HDMECs were divided into normoxia+unloaded group, normoxia+BNIP3 knockdown group, hypoxia+unloaded group, and hypoxia+BNIP3 knockdown group which were transfected with unloaded virus or BNIP3 knockdown virus and were subjected normoxic or hypoxic treatment. The BNIP3 protein expression was detected by Western blotting and immunofluorescence staining. The scratch area at 24 h post scratching was detected by scratch test, and the wound healing rate was calculated. The curve distance of cell movement was measured with the living cell workstation, and the speed of movement was calculated within 3 hours. (3) HDMECs were grouped and treated as experiment (2). Western blotting and immunofluorescence staining were performed to detect the protein expression of LC3Ⅱ. The samples were 3 in the above-mentioned experiments. Data were statistically analyzed with one-way analysis of variance and LSD test.Results:(1) Compared with normoxic group, the protein expressions of BNIP3 and LC3Ⅱ of cells in hypoxic 6, 12, 24 h groups were significantly increased (P<0.01). (2) After 6 hours of culture, compared with hypoxia+unloaded group, the BNIP3 expression of cells in hypoxia+BNIP3 knockdown group was significantly decreased (P<0.05). The red fluorescence of BNIP3 expression of cells in normoxia+unloaded group and normoxia+BNIP3 knockdown group was weak, the red fluorescence of cells in hypoxia+unloaded group was strong, and the red fluorescence of cells in hypoxia+BNIP3 knockdown was significantly decreased compared with that in hypoxia+unloaded group. After scratching for 24 hours, the scratch of cells in hypoxia+unloaded group basically healed, while the remaining scratch area in the other three groups were large. The wound healing rates in normoxia+unloaded group, normoxia+BNIP3 knockdown group, hypoxia+unloaded group, and hypoxia+BNIP3 knockdown group were (61±4)%, (58±4)%, (88±4)% and (57±4)%, respectively. There was significant difference in general comparison among these groups (F=14.57, P<0.01). The wound healing rate in hypoxia+unloaded group was significantly higher than that in normoxia+unloaded group (P<0.01) and hypoxia+BNIP3 knockdown group (P<0.05). Within 3 hours of observation, the range of cell movement in hypoxia+unloaded group was significantly larger than that in normoxia+unloaded group, and the range of cell movement in hypoxia+BNIP3 knockdown group was significantly smaller than that in hypoxia+unloaded group. Within 3 hours of observation, the curve movement velocity of cells in hypoxia+unloaded group was significantly higher than that in normoxia+unloaded group and hypoxia+BNIP3 knocdown group (P<0.01). (3) After 6 hours of culture, compared with hypoxia+unloaded group, the BNIP3 protein expression of cells in hypoxia+BNIP3 knockdown group decreased significantly (P<0.05). After 6 hours of culture, the red fluorescence of LC3 expression of cells was weak in normoxia+unloaded group and normoxia+BNIP3 knockdown group, the red fluorescence of LC3 expression of cells was significantly enhanced in hypoxia+unloaded group, and the red fluorescence of LC3 expression of cells was significantly inhibited in hypoxia+BNIP3 knockdown group.Conclusions:BNIP3 can promote the migration and motility of HDMECs under hypoxia, and autophagy may be involved in the regulation migration and motility of HDMECs by BNIP3.
6.Rules of acupuncture points in the treatment of dysmenorrhea of endometriosis

Objective:To summarize the application rules of acupuncture and moxibustion in the clinical trials for dysmenorrhea and endometriosis.Methods:The relevant articles of clinical research on acupuncture and moxibustion for the treatment of dysmenorrhea in endometriosis were retrieved electronically from CNKI, Wanfang, VIP, PubMed and Cochrane Library (Retrieved from database establishment until 31 October 2021). The clinical literatures that met the inclusion criteria were included into the database (Microsoft Office Excel 2019), and the main table and sub-table were established through Microsoft Office Excel 2019. Descriptive statistic analysis was performed by using SPSS 26.0 software. StataSE15 (64-bit) software was used to analyze the correlation between the combinations of acupoints with single acupoint frequency>5.Results:A total of 40 clinical studies were included. Of them, the top 3 acupuncture methods according to the frequency were acupuncture (11 times), moxibustion (11 times), and electro-acupuncture (8 times), the top 3 acupuncture points used according to the frequency were Guan Yuan (34 times), San Yin Jiao (26 times), and Zhong Ji (25 times), the top 3 meridians of the body acupuncture points according to the frequency were Ren Meridian (36 times), Spleen Meridian (32 times), and Stomach Meridian (22 times). According to the correlation analysis, body points of Guan Yuan, San Yin Jiao, Uterus, Qi Hai and Zhong Ji were stronge related.Conclusion:Acupuncture can effectively treat EMT dysmenorrhea with less adverse reactions. The methods are mainly acupuncture, moxibustion and electroacupuncture, mostly local acupoints, intersection acupoints and empirical acupoints.
7.Quality evaluation of health education guidelines for discharged recipients with solid organ transplantation

Objective:To evaluate the guidelines on discharged health education for solid organ transplant recipients at home and abroad, analyze the characteristics of each guideline, so as to provide reference for the development of health education for organ transplantation in China.Methods:This was a guideline quality evaluation study. The guidelines on health education for discharged recipients with solid organ transplantation were searched on the websites of clinical practice guidelines, the websites of transplantation professional associations, and Chinese and English databases at home and abroad through computers. The search time limit was from January 1, 2011 to January 1, 2022. The quality of guidelines that met the inclusion and exclusion criteria was evaluated using the Appraisal of Guidelines for Research and Evaluation Ⅱ (AGREE Ⅱ) .Results:A total of 8 guidelines were included. The overall quality evaluation of the guideline was Grade A ( n=2) and Grade B ( n=6) . In terms of standardized scores in six areas of quality evaluation, the average values of scope and purpose, stakeholder involvement, rigour of development, clarity and presentation, applicability as well as editorial independence were 87.62% (70.37%-100.00%) , 75.46% (44.44%-97.22%) , 66.54% (43.75%-93.06%) , 90.16% (77.78%-96.30%) , 48.18% (20.83%-68.75%) and 74.13% (8.30%-97.22%) , respectively. The recommendations of the guidelines covered 9 aspects: team composition and support services, drug management, infection prevention, complication management, diet and nutrition, daily life and social reintegration, contraception and reproduction, psychological guidance and follow-up review. Conclusions:The development of guidelines for different types of solid organ transplantation varies. The overall quality of the guidelines needs to be improved and the content needs to be refined. In the future, we can learn from foreign high-quality guidelines to carry out localization practice, so as to build a health education related guideline for discharge of solid organ transplantation that is suitable for China's national conditions as soon as possible.
8.A resting-state functional magnetic resonance imaging study of altered brain function of benign epilepsy in children with central-temporal spikes

Objective:To analyze the brain functional fluctuation of benign epilepsy in children with central-temporal spikes(BECTS) by using ReHo algorithm based on the resting-state brain functional imaging, and to explore the connection of the brain function and changes of the connection pattern, so as to find the damage of the cognitive function of BECTS children in the early stage.Method:s Perspectiveness and simple random selection of 20 BECTS children and 20 healthy control children admitted to Shenzhen Children′s Hospital from January 2015 to December 2017 were conducted for basic information collection and functional magnetic resonance imaging (fMRI) testing in a resting-state.Result:s Significantly lower ReHo value appeared in the default mode network (DMN) area, and the precuneus (voxel=422, t=-5.085 6), cuneus (voxel=85, t=-4.240 3), angular gyrus (voxel=191, t=-4.681 2), cingulate cortex (voxel=313, t=-5.238 2), anterior central gyrus (voxel=12, t=-3.482 7), and supplementary motor area (voxel=1 356, t-6.596 2). The significantly increased ReHo was found in the bilateral cerebellum (voxel=71, t=5.658 2), right superior temporal gyrus (voxel=24, t= 5.184 0), operculum insulae (voxel=337, t=6.814 9), left parietal lobe (voxel=12, t=4.378 7), and inferior parietal lobule (voxel=11, t=3.433 7). Conclusions:Significant impairment of DMN, Wernicke and angular gyrus functions in BECTS children may be one of the mechanisms of cognitive dysfunction.Enhanced sensorimotor area and cortical brain functions near the operculum insulae and central sulcus lead to seizures with typical clinical symptoms.fMRI has a high specificity and sensitivity for evaluating the brain function of children with BECTS, and it can detect the impairment of cognitive function in children with this type of epilepsy at an early stage.
