1.Effect of edaravone on inflammatory factors 6-keto-prostaglandin F1α, thromboxane B2, endothelial function, copeptin and N-terminal brain natriuretic peptide in patients with acute cerebral infarction

Objective To investigate effect of edaravone on serum inflammatory factors, 6-keto-prostaglandin F1α(6-keto-PGF1α), thromboxane B2, endothelial function and serum copeptin and N-terminal brain natriuretic peptide (NT-proBNP) in patients with acute cerebral infarction.Methods From March 2013 to September 2014, 213 cases of acute cerebral infarction were selected in the hospital and randomly divided into control group (n=101) and observation group (n =112).Control group were given conventional symptomatic treatment, and observation group were given edaravone injection on the basis of control group.The serum inflammatory cytokines, 6-keto-PGF1α, thromboxane B2, endothelial function, serum copeptin, NT-proBNP and nerve function score and activities of daily living ( ADL) score were compared between two groups.Results Serum CRP, IL-8, IL-10 in observation group after treatment were significantly lower than control group (P<0.05).Plasma thromboxane B2 in observation group after treatment was significantly lower than control group (P<0.05).The levels of 6-keto-PGF1αwas significantly higher than control group (P<0.05).Serum copeptin and NT-proBNP levels in observation group after treatment were significantly lower than control group (P<0.05).Plasma ET-1 in observation group after treatment was significantly lower than control group ( P<0.05 ) , and plasma NO was significantly higher than the control ( P<0.05 ).Neurological function in observation group after treatment was significantly lower than control group (P<0.05), and ADL score was significantly higher (P<0.05). Conclusion The preliminary study shows that edaravone in treatment of acute cerebral infarction may be associated with decreasing serum inflammatory cytokines, increasing 6-keto-PGF1αand reducing thromboxane B2, improving endothelial function and reducing serum copeptin and N-terminal natriuretic peptide.
2.Impact of rosuvastatin on cardiac function, lipid levels, blood rheology level and endothelial function in patients with coronary artery disease

Objective To investigate impact of rosuvastatin on cardiac function, lipid levels, blood rheology level and endothelial function in patients with coronary heart disease.Methods 149 cases were randomly divided into two groups, the treatment group (n=76) and control group(n=73).Control group with conventional symptomatic treatment, treatment group on the basis of control group combined rosuvastatin treatment.Cardiac function, blood lipid levels, blood rheology changes and endothelial function levels before and after treatment were comparative analysis.Results LVEF significantly increased after treatment than before treatment between two groups (P<0.05), while LVESV, LVEDV after treatment than before treatment were significantly lower ( P<0.05 ) , LVEF in treatment group was significantly higher than control group ( P<0.05 ) , LVESV, LVEDV was significantly lower than that of control group (P<0.05).The two groups of LDL-C, TC, TG after treatment was significantly lower than before treatment (P<0.05), but HDL-C was higher than before treatment(P<0.05).LDL-C, TC, TG in treatment group after treatment was significantly lower than control group (P<0.05), whereas HDL-C after treatment was significantly higher than control group (P<0.05).Fibrinogen, plasma viscosity, whole blood low shear viscosity, whole blood viscosity was lower than that before treatment (P<0.05), which in treatment group was significantly lower than control group (P<0.05).NO, FMD was significantly higher than before treatment (P<0.05), and ET was significantly lower than before treatment (P<0.05).NO, FMD in treatment group was significantly higher than control group (P<0.05), and ET was significantly lower than control group (P<0.05).Conclusion Rosuvastatin can significantly improve cardiac function, improve blood lipid levels, lower levels of blood rheology, improve endothelial function.
3.Effects of Different Magnitudes of Mechanical Stretch on Human Intervertebral Disc Cells

Objective To investigate the effects of different magnitudes of mechanical stress on human interverte-bral disc degeneration. Methods The human intervertebral disc cells were subjected to different magnitudes of mechanical stress (0, 6%, 12%, or 18%elongation) for 24 h using a Flexercell Strain Unit. The mRNA expressions of anabolic genes (col-lagen-1A1, collagen-2A1, aggrecan and versican) and catabolic genes (MMP-3, MMP-13, ADAMTS-4 and ADAMTS-5) were examined by real-time PCR and Western blot methods. Results The expression levels of collagen-1A1 and collagen-2A1 were increased at 12%of mechanical stress, and collagen-2A1 was decreased at 18%of mechanical stress compared with those of control. The mRNA expressions of catabolic genes, MMP-13 and ADAMTS-5, were increased at 12%and 18%of mechanical stress than those of control. The mechanical stretch induced a magnitude-dependent increase in ADAMTS-4 synthesis, which was finely tuned by stretching-triggered activation of distinct mitogen-activated protein kinase cascades. Specifically, an ERK1/2 specific inhibitor, U0126, significantly inhibited the stretching-induced ADAMTS-4 expression, whereas the inhibitors of p38 and JNK, SP6000125 and SB203580, showed only slightly effect on the stretching-induced ADAMTS-4 expression. Conclusion The different magnitudes of mechanical stretch exhibited different effects on the bio-logical behavior of intervertebral disc cells, which profoundly affects the intervertebral disc degeneration.
4.Etiology, Pathology and Prospects of TCM in Osteoporosis Treatment

Osteoporosis is a progressive metabolic disease. In traditional Chinese medicine (TCM), deficiency of the liver, spleen and kidney and blood stasis can induce osteoporosis. The main etiology was kidney deficiency which harmed the liver and spleen and causeqi-blood deficiency and blood stasis. The insufficiency of natural endowment and postnatal malnutrition caused poor nutrition of tendons and vessels, which induced debility of bone. This article discussed the etiology and pathogenesis of TCM understanding on osteoporosis in details. Opportunities and challenges of TCM in osteoporosis treatment were explored. The homology of Chinese medicine and food as well as the long-term of osteoporosis showed that TCM had incomparable superiority in osteoporosis treatment. While, the scientific research methods and reasonable evaluation of TCM safety were important to display its advantages in osteoporosis treatment.
5.Expressions of oxidative stress related factors in idiopathic membranous nephropathy

Objective To investigate the expression of oxidative stress related factors,such as,superoxide dismutase (SOD),heme oxygenase-1 (HO-1),malondialdehyde (MDA) and 8-hydroxy-2'-deoxyguanosine (8-OHdG) in patients with idiopathic membranous nephropathy (IMN) and explore the possible role of oxidative stress in the pathogenesis of IMN.Methods Serum,urine and renal tissue of 40 patients with IMN,20 patients with secondary membranous nephropathy (SMN),and 20 patients with minimal change nephropathy (MCD) were collected from the nephrology department of the First Affiliated Hospital of Zhengzhou University.All of them were diagnosed by renal biopsy from January 2016 to June 2016.Serum,urine and renal tissue of 20 healthy persons were collected as the normal control (NC).The expressions of SOD,HO-1,and 8-OHdG in renal tissues were detected by immunohistochemistry.The levels of SOD,HO-1,MDA and 8-OHdG in serum and urine specimens were detected by ELISA.The relationship between oxidative stress related factors and proteinuria in IMN was investigated as well.Results The expressions of SOD,HO-1 and 8-OHdG were significantly higher than those in MCD group and NC group in renal tissue;the expression of SOD in IMN group was significantly higher than that in MCD group and NC group in serum (all P < 0.01).The expressions of HO-1,MDA and 8-OHdG in the serum and urine of the IMN group were significantly higher than those of the MCD group and the NC group (all P < 0.05).There was a significant positive correlation between the urine expression of 8-OHdG and MDA and proteinuria in the IMN group.Conclusions Oxidative stress-related factors in IMN patients are elevated,which is closely related to clinical manifestations.In that sense,it is confirmed that oxidative stress is involved in the pathogenesis of IMN.
6.The role of aquaporin 4 antibody in the injury of retinal microstructure in neuromyelitis optica spectrum disorders

Objective To evaluate the injury of retinal microstructure using optical coherence tomography (OCT) and investigate the role of aquaporin 4 antibody (AQP4 Ab) in this injury process.Methods Forty patients with neuromyelitis optica spectrum disorders (NMOSD) were retrospectively studied,each of whom reported at least one episode of optic neuritis (ON),namely 59 ON eyes involved in all.All patients were divided into two subgroups based on AQP4 Ab tests including 25 patients (37 ON eyes) with AQP4 positive (Ab+/NMOSD group) and 15 patients (22 ON eyes) negative (Ab-/NMOSD group).In addition,10 healthy controls (20 eyes) matched for age and sex (HC group) were analyzed.Spectral domain optical coherence tomography (SD-OCT) was used to quantify peripapillary retinal nerve fiber layer (RNFL).Nonparametric test was used to compare differences between groups.Results Age distribution and gender ratio were comparable in three groups (P>0.05).Visual acuity in ON eyes of Ab+/NMOSD group was worse than that of Ab-/NMOSD group (P=0.02).There were no significant differences between Ab+/NMOSD and Ab-/NMOSD in aspects of disease duration (2.6 vs.1.9 year),ON episodes (2 vs.1),longitudinal extensive transverse myelitis (LETM) ratio (48.0% vs.66.7%),NMOSD specific intracranial lesions ratio (32.0% vs.53.3%),positive autoimmune antibody ratio (52.0% vs.20.0%) (P=0.13,0.08,0.25,0.18,0.06,respectively).The thickness of temporal,superior,nasal,inferior and average RNFL in ON eyes of both Ab+/ NMOSD and Ab-/NMOSD group were thinner than those in eyes of HC group (all P<0.05).The thickness of superior and inferior RNFL in ON eyes of Ab+/NMOSD were 61.0 μm and 62.0 μm,which was thinner than those of Ab-/NMOSD 94.5 μm and 97.0 μm (P=0.03 and 0.01,respectively).Conclusions RNFL reflects the injury of retinal microstructure in NMOSD patients.AQP4 Ab seropositivity is correlated to the severity of RNFL damage,implying the potential role of AQP4 Ab in this pathological process.
7.Laparoscopic radiofrequency ablation for centrally located renal tumors.

OBJECTIVETo study the safety and effectiveness of laparoscopic radiofrequency ablation for centrally located renal tumors.
METHODSFrom January 2009 to April 2013, thirteen patients who diagnosed as centrally located renal tumors were treated with laparoscopic radiofrequency ablation in the Department of Urology of Nanjing Drum Tower Hospital Affiliated to Nanjing University Medical School. All of the cases were T1aN0M0 stage, 9 patients were male, 4 were female, the mean age was 56 years (range, 38-73 years). All tumors were unilateral, eight lesions were in the left kidney and five in the right kidney. Intraoperative real-time ultrasound and temperature probes were used to guide the range of radiofrequency ablation. Ice saline was injected through ureteral catheter for cooling the collecting system. The postoperative serum creatinine and glomerular filtration rate (GFR) data were collected,all patients were followed up with enhanced CT or MRI.The pre- and post-operative date were compared by paired t test.
RESULTSAll patients underwent laparoscopic radiofrequency ablation successfully. The mean operation time was (113±13) minutes and the mean blood loss was (99±23) ml. The mean pre- and post-operative serum creatinine was (71±11) µmol/L and (74±11) µmol/L, the mean pre- and post-operative GFR was (49±8) ml/min and (45±7) ml/min. There was no significant statistic difference between pre-operation and post-operation (t=-1.371 and 1.986, P>0.05). The mean follow-up was 37 months, range 12-63 months. No evidence of local recurrence or distant metastasis was found.
CONCLUSIONSLaparoscopic radiofrequency ablation for T1aN0M0 centrally located renal tumors could be performed safely with good outcomes. Intraoperative real-time ultrasound and temperature probes are helpful to control the range of radiofrequency ablation. Physical cooling of renal collecting system could reduce the occurrence of postoperative hydronephrosis and leakage of urine.

8.Risk factors for axial deviation in treatment of tibial bone defects by bone transport with Orthofix unilateral external fixator

Objective:To investigate the risk factors for axial deviation in the treatment of tibial defect susing Orthofix unilateral external fixator and proximal tibial osteotomy for bone transport.Methods:A retrospective study was performed to analyze the clinical data of 90 patients who had been treated for tibial bone defects by the Orthofix unilateral external fixator at Department of Microrepair and Reconstruction, The First Hospital Affiliated to Xinjiang Medical University from May 2012 to June 2019. There were 77 males and 13 females with a mean age of 41.2 years (from 17 to 63 years).The bone defects ranged from 4 to 13 cm in length. According to the Paley criteria for axial deviation, the 90 patients were divided into 2 groups: a deviation-free group with no axial deviation or an axial deviation ≤5° and a deviation group with an axial deviation>5°. The 2 groups were compared in terms of age, number of prior surgery, defect length, placement angle of Schanz screws, external fixation time, external fixation index and bending degree of Schanz screws at the last follow-up.The factors with P<0.05 were analyzed by multivariate logistic regression to find the risk factors for coronal axial deviation. Results:The 90 patients were followed up for an average of 23 months (from 12 to 40 months). The incidence of axial deviation in this cohort was 36.7% (33/90).The deviation group had a significantly larger number of prior surgery [5 (3, 6) times], a significantly longer defect length [8 (8, 9) cm], a significantly longer external fixation time [400.0 (341.8, 426.3) d], and a significantly greater bending degree of Schanz screws at the last follow-up [1.2° (0.4°, 3.5°)] than the deviation-free group [3 (2, 3) times, 6 (5, 8) cm, 340.8 (226.5, 422.8) d, and 0.8° (0.2°, 3.7°)] (all P<0.05). Multivariate logistic regression analysis showed that the number of prior surgery ( OR=2.581, 95% CI: 1.496-4.450, P=0.001) and the defect length ( OR=5.310, 95% CI: 1.952-14.442, P=0.001) were the risk factors for the axial deviation. Conclusion:In the treatment of tibial defect susing Orthofix unilateral external fixator and proximal tibial osteotomy for bone transport, the more prior surgeries and the longer a bone defect, the higher the risk for axial deviation.
9. Expressions and prognostic significance of PTEN and PD-1 protein in patients with classical Hodgkin’s lymphoma

Objective:
To elucidate the expression levels of key immune biomarkers, phosphate and tension homology deleted on chromosome ten (PTEN) and programmed cell death protein1(PD-1),of different immune tolerance pathway in classic Hodgkin’s lymphoma (CHL) to further determine their clinical role and prognostic significance.
Methods:
The clinical features and prognostic factors of 56 CHL patients, who were admitted to the TianJin Medical University Cancer Institute from February 2003 to August 2013, were retrospectively analyzed. PTEN and PD-1 protein expression levels were analyzed by immunohistochemistry, Epstein-Barr virus encoded RNA (EBER) was performed by in situ hybridization assay. Correlations between the expression of biomarkers and clinicopathologic parameters were examined and survival analyses were performed.
Results:
This cohort of 56 CHL patients included 34 males and 22 females with a median age of 25 years (ranged from 7 to 71 years). In a univariate analysis, age≥45, IPS score >2, EBER positive, high expression of PTEN protein conferred inferior 5-year OS and 5-year PFS; In a multivariate model, age≥45, IPS score >2, EBER positive, high expression of PTEN protein were identified as the independent adverse prognostic factors for CHL.
Conclusions
This study suggested for the first time that PTEN was independent prognostic immune biomarkers in CHL, which provided the novel therapeutic strategy of immune therapy for CHL.
10.Interventional Effect and Metabolomics Analysis of Linderae Radix on Fibrosis of Rats with Intrauterine Adhesions

ObjectiveTo investigate the pharmacological effect and metabolic mechanism of Linderae Radix on the intrauterine adhesion (IUA) rat model. MethodAn IUA rat model was induced by mechanical injury and infection. Molecular biology and pharmacology techniques were employed to evaluate the inhibitory effect of Linderae Radix extract (LAE) on fibrosis in IUA. Serum metabolomics analysis based on gas chromatography-mass spectrometry (GC-MS) was conducted to explore the metabolic regulation mechanism of LAE. ResultAnimal experiments showed that LAE significantly improved the morphology and structural damage of uterine tissue cells in the IUA rat model, promoted endometrial proliferation, vascular regeneration, and morphological recovery, inhibited the mRNA expression of transforming growth factor-β1 (TGF-β1), Smad2, and Smad3, and increased the expression of Smad7 mRNA to suppress fibrosis. Additionally, LAE significantly suppressed the levels of estrogen (E2), follicle-stimulating hormone (FSH), luteinizing hormone (LH), and tumor necrosis factor-α (TNF-α) expression (P<0.01), thereby improving the uterine microenvironment. Metabolomics analysis revealed significant metabolic abnormalities in the serum of IUA rats compared with the results in the normal group, and nine differential metabolites were identified. LAE effectively ameliorated these metabolic abnormalities, primarily by influencing six differential metabolites, including five shared metabolites among the nine identified markers: L-aspartic acid, L-pyroglutamic acid, L-serine, glucose, and L-norvaline. Pathway enrichment analysis indicated that the aminoacyl-tRNA biosynthesis pathway was the main affecting mechanism. ConclusionIn combination with the pharmacological research results, LAE effectively improved uterine damage and inhibited fibrosis in the IUA rat model. Its mechanism may involve the inhibition of the aminoacyl-tRNA biosynthesis pathway and the improvement of the microenvironment.
