Flow cytometric immunophenotyping has evolved from two-parameter quantitative measurement of peripheral blood lymphocytes to five-or more parameter qualitative evaluation of bone marrow and lymph node in hematopathology.Leukemia/lymphoma immunophenotyping represents an important addition to histomorphology in the diagnosis,classification and monitoring of hematolymphoid neoplasms.The complexity of five-or more parameter analyses and the interpretation of the data rely on standardization and validation of the instrument,the reagent and the procedure.In addition,clinical flow cytometry laboratories in U.S.A are required to document proficiency testing,sample preparation as well as accuracy,specificity,sensitivity and precision of methodology.CLSI and UCCC recommend that each laboratory should validate its own qualitative and quantitative procedures.This paper introduces the procedures for validation and quality control in a clinical flow cytometry laboratory in the United States.

B7H1 is a recently described B7 family member.Ligation of PD-1 receptors with B7hl on the surface of activated T cell inhibits T cell proliferation and cytokine production. B7H1 plays a negative regulatory role in peripheral tolerance, autoimmune diseases and chronic infections.B7H1 also has been found to be largely expressed on a broad range of cancers and is thought to contribute to immune evasion by cancers. Blockade of B7H1/PD-1 pathway may contribute to the treatment of autoimmune diseases and maligant tumors.

Objective To evaluate the diagnostic value of myocardial enzymes on the cardiac muscle injury for the children with acute mycoplasma pneumonia(MP).Method The levels of phosphocreatine kinase-MB(CK-MB),lactate dehydrogenase(LDH),aspartate aminotransferase(AST)in 48children with MP(MP group)were measured,48 normal children served as control group.Results The levels of AST,LDH,CK-MB were(37.18±6.15),(294.10±46.20),(84.50±13.70)U/L in acute stage in MP group,they were higher than those in control group[(25.14±10.50),(140.50±76.21),(19.50±4.10)U/L](P＜0.01).All the values above was descended in recovery phase in MP group,but the values of CK-MB[(27.80±8.40)U/L]and LDH[(189.50±69.73)U/L]were still higher than those in control group(P＜0.01 or＜0.05).Conclusions Children with MP may combine myocardial injuries and CK-MB isa parameter to diagnose the damage.Protection therapy for cardiac muscle should be performed on the children with MP.

Objective To investigate effect of hydroxypropyl methyl cellulose (HPMC) to in vitro release characteristics of cefaclor sustained-release tablets.Methods Collected 10 batches of HPMC from different manufacturers.Determined the viscosity, molecular weight and the distribution of molecular weight of HPMC.HPMC from different manufacturers was used as blocking agent for the preparation of cefaclor sustained-release tablets according to the same prescription.Results The molecular weight was positively related to the viscosity of HPMC.The more molecular weight was, the slower the drugs release rate was,the better controlled released.Conclusion HPMC from different manufacturers show different quality stability and effect in vitro release of cefacor sustained-release tablets.

Objective To explore the relationship between the type of TORCH infection and pregnancy outcome through the de-tection of TORCH infection in pregnant women with IgG avidity index (AI) .Methods Collected each type of TORCH pathogens IgM positive maternal serum ,and detect the IgG avidity index and track the outcome of pregnancy .Results The rate of TORCH pathogens recent infection (AI<30% )in pregnant women ,HSV ,RUB ,CMV ,TOXO respectively were 0 ,3 .8% ,9 .3% ,7 .9% ;In the group of pregnant women with TORCH-IgG AI<30% ,there were 2 cases of pregnancy outcome of abnormal (missed abortion) and one case of fetal malformations (hydrocephalus) .Conclusion In the recent infection of TORCH pathogens in pregnant women , mainly are non-primary infection ;The damage of cytomegalovirus and Toxoplasma gondii primary infection to the fetus is much larger than the non-primary infection .

OBJECTIVE To investigate the resistance rates of Gram negative bacilli and the distribution of ?-lactamases in hospital infections(HI) or community acquired infections(CAI) in Tongling.METHODS Antimicrobial(susceptibility)(test) was done on 356 strains of Gram negative bacilli isolated in Tongling from Oct 2003 to Sep 2004 by Kirby-Bauer method.The detection of ESBLs and AmpC ?-lactamases was performed by three-dimensional test,MBL by the double-disk synergy test.RESULTS Among total 356 strains of Gram negative bacilli,267 were with HI(53.9%) isolated from sputum of patients,89(34.8%) were(isolated) from CAI patients and urine.The antimicrobial susceptibility rates of Gram negative bacilli from CAI patients was significantly higher than that from HI patients,for Acinetobacter baumannii they were 1.5 to 3.0,for(Pseudomonas aeruginosa) were 1.2 to 1.6.No strains of Escherichia coli and Klebsiella pneumoniae were found resistant to imipenem.Among 356 Gram negative bacilli,77 strains hyperproducing ESBLs and AmpC ?-lactamases,the detection rate was 21.6%, 69 strains were isolated from HI patients,and 8 strains were from CAI patients.From strains resistant to imipenem had 13 strains detected MBL,the detection rate was 52.0%.All of them were isolated from HI patients.CONCLUSIONS The resistance of Gram negative(bacilli) is a serious problem in Tongling.The antimicrobial(susceptibility) rates of Gram negative bacilli from CAI patients are significantly higher than those from HI patients.Gram negative bacilli produce all kinds of(?-lactamases,) such as ESBLs,AmpC(?-lactamases) and MBL.

Objective To explore the use of magnetic resonance imaging (MRI) in the display of detailed anatomical structures at the pancreaticobiliary junction. Methods 112 patients who received enhanced MRI of upper abdomen were included in the study. Patients with pancreatic and/or biliary diseases diagnosed clinically and with laboratory tests were excluded. The types of junction between the terminal common bile duct and the pancreatic duct, and the location of the major duodenal papilla were studied on MRI. We measured the angle between the duodenum and the common pancreaticobiliary duct or the common bile duct. Results Of the 112 patients, the duodenal papillas were located at the upper, middle, and lower segment of the duodenum in 17. 0%, 66. 0% and 17. 0%, respectively.The angle between the common pancreaticobiliary duct or the distal common bile duct and the descending duodenum was 44. 4°±17. 3°. The pancreatic duct and the common bile duct opened separately in 9 patients (8. 0%). The confluence of the two ducts was present inside and outside of the duodenum wall in 13 (11. 6%) and 90 patients (80. 4%), respectively. The angle between the distal common bile duct and the pancreatic duct was 37. 8°±15.1°. Conclusion MRI was able to display detailed anatomical structures of the pancreaticobiliary junction, including the angle of the junction between the two ducts and the location of the duodenal papilla. It has the ability to provide meticulous anatomical data for the diagnosis and treatment of diseases at the pancreaticobiliary junction and to help surgeons formulate operative plans.

Objective To compare dexmedetomidine versus lidocaine for suppression of fentanyl-induced coughing during induction of general anesthesia in patients.Methods Two hundred ASA physical status Ⅰ or Ⅱ patients,aged 36-50 yr,undergoing elective gynecological operations under total intravenous anesthesia,were randomized into 2 groups (n =100 each):dexmedetomidine group (group D) and lidocaine group (group L).Dexmedetomidine 0.5 μg/kg was intravenously infused over 10 min at a rate of 0.05 μg· kg-1· min-1 starting from 20 min before induction of anesthesia in group D.Lidocaine 0.5 mg/kg was injected intravenously at 1 min before induction of anesthesia in group L.For induction of anesthesia,fentanyl was given first,and 2 min later the other drugs were given.The development and degree of coughing were recorded within 1 min after fentanyl injection.The occurrence of adverse events was recorded.Results The incidence of coughing was significantly higher and the degree of coughing was severer in group L than in group D (P ＜ 0.05).There was no signihicant difference in the incidences of hypotension and severe sinus bradycardia between groups D and L (P ＞ 0.05).Conclusion Dexmedetomidine 0.5 μg/kg injected before induction of anesthesia has better suppressive effect on fentanyl-induced coughing during induction of general anesthesia than lidocaine 0.5 mg/kg in patients.

Objective To evaluate the value of intraduetal ultrasonography (IDUS) in diagnosing biliary and pancreatic disorders. Methods The findings by endoscopic retrograde cholangiopancreatography (ERCP) and IDUS from 19 patients with suspected biliary and pancreatic disorders from July 2006 to August 2007 in our hospital were analyzed retrospectively. Results Of the 19 patients, 17 had obstructive jaundice (including 6 eases of cholangiocarcinoma, 2 pancreatic adenocareinoma, 2 gallbladder carcinoma, 2 chole-docholithiasis with bile duct stricture, 2 autoimmune pancreatitis, 1 papillary adenocarcinoma, 1 papillary adenoma, and 1 sclerosing cholangitis) and 2 intraduetal papillary mueinous tumor (IPMT). The diagnosis was confirmed by surgery and pathological findings in 11 patients. The diagnostic accuracy of ERCP and IDUS was 73. 7% (14/19) and 84. 2% (16/19), respectively, and that of ERCP combined with IDUS was 89. 5% (17/19). The sensitivity and specificity of ERCP to differentiate benign bile duct strictures from ma-lignant ones were 100. 0% (11/11) and 83.3% (5/6), respectively; and those of IDUS were 100. 0%(11/11) and 100. 0% (6/6), respectively. The sensitivity and specificity of ERCP in diagnosing cholan-gioeareinoma were 83.3% (5/6) and 60% (3/5), respectively; and those of IDUS were 100. 0% (6/6) and 40. 0% (2/5), respectively. Conclusion Combination of ERCP with IDUS can improve the diagnostic accuracy of pancreaticobiliary disorders. Additionally, IDUS shows higher sensitivity and specificity in differ-entiation between benign and malignant bile duct strictures, but it is still difficult to identify the etiologic factors of malignant bile duet strictures by IDUS.
