2.PATHOLOGICAL STUDY ON GUNSHOT WOUND TRACK IN DOG UNDER HOT AND HUMID ENVIRONMENT

To investigate the pathological changes in gunshot wound track in limbs of dogs produced under a hot and humid environment, 8 dogs were randomly divided into two groups: hot and humid environment group (HHE) and normal environment group(NE). Gross examination, optical microscopic examination (OM), electron microscopic examination (EM) were performed, and ATP, ADP, AMP content, energy charge (EC) in the gunshot wounds were observed at 4, 8, 12, 24h after wounding. The results indicated that 6~8 h after the injury, the wound tracks and discolored area of tissue in HHE group were more distinguished and enlarged than those in NE group, and the swelling of wounded limbs was obvious. Some of the wounds began to be odoriferous. The signs of infection began to be obvious. While the wounds of NE group did not give off obvious odor of decay until 12~24h after the injury. Under OM and EM, necrosis and damage of muscle fibers in the wound tracts were more obvious in the HHE group than that in NE group, showing a tendency of deterioration. In contrast the changes in the injured tissues in NE group showed a tendency of improving. ATP content of the injured tissues in the contusion region of HHE group was lower than that of NE group at 8h, ATP content of tissues in the shock region of HHE group was decreased continuously and was lower than that of NE group at 12h. ADP content of tissues in the contusion area of HHE group was lower than that of NE group at 8h and also in shock area at 12h. AMP content of tissues in the contusion area of HHE group was higher than that of NE group at 12h, whereas EC of tissues in the contusion area of HHE group was lower than that of NE group at 8h and 12h. Histopathological changes in tissues in the gunshot wound were serious and deteriorated gradually in hot and humid environment. At the same time the energy storage of muscle tissue in the gunshot wound track was lower and decreased more remarkably in this environment. Therefore early and complete debridement of gunshot wounds inflicted in hot and humid environment should be emphasized.
3.Serotyping and genotyping study of clinical Vibrio parahaemolyticus

Objective To study the distribution and molecular characteristics of Vibrio parahaemolyticus(VP) in patients with a‐cute diarrhea ,and provide a scientific basis for the prevention and control of VP infection .Methods From 2010 to 2014 ,62 VP iso‐lates were collected from patients with acute diarrhea ,for serotyping and virulence gene (tdh and tdh) detection of VP .Molecular characteristics analysis was carried out by using multi‐locus sequence typing (MLST) .Results 7 different serotypes were found from the 62 isolates .O3∶K6 was the most common serotype of VP ,accounting for 74 .19% (46 isolates) ,followed by O4∶K68 (6 isolates) .Tdh gene was the mainly virulence gene ,with a percentage of 95 .16% (59 isolates) ,only three isolates were trh positive . 7 STs were found through MLST analysis of 62 VP isolates ,among which ,ST3 was the most important type ,accounted for 85 .50%(53 isolates) .Conclusion O3∶ K6 serotype VP was the most prevalent type .Tdh gene is the most important virulence gene of WP .ST3 was the the dominant epidemic type .

5.The image features of optical coherence tomography in traumatic maculopathy

Objective To investigate the optical coherence tomography (OCT) characteristics of traumatic maculopathy. Design Retrospective case series. Participants 477 patients (486 eyes) with traumatic maculopathy, who aged from 4 years to 76 years. Method The clinical and OCT data of patients from September 2002 to June 2009 in Beijing Tongren Hospital were reviewed. Main outcome measures Features of the OCT images. Results The major findings by OCT in traumatic maculopathy included: macular hole, sensory retinal detachment, macular hemorrhage, epimacular membrane, choroidal rupture, sensory retinal atrophy, retinal pigment ep-ithelium (RPE) and choroid atrophy. In the early stage after trauma, the common findings with OCT are atrophy of RPE(49.0%), macular hole(24.7%), sensory retinal detachment(26.3%),macular hemorrhage(24.2%) and macular edema(19.2%); in the middle-late stage, atro-phy of RPE (63.0%)and atrophy of sensory retina (36.5%) are the most common changes revealed with OCT. Conclusions OCT is a useful diagnostic modality for imaging traumatic maculopathy. Diverse changes of retina and choroid are usually coexisting by OCT. At-rophy of RPE is the most common change throughout the course. In the early stage, macular hole, sensory retinal detachment, macular hemorrhage and edema are the common changes. In the middle-late stage, atrophy of sensory retina and/or RPE is the dominating change. (Ophthalmol CHN, 2009, 18: 236-238)
6.The anti-apoptotic effect of c-fos in drug-resistant breast cancer cells

Background and purpose:Breast cancer is one of the most common carcinoma among female patients with high mortalities. Drug-resistance is the major reason that leads to chemotherapy failure in clinical practice. MCF-7/ADR is a multi-drug resistant cell line that was established on the basis of breast cancer cell line MCF-7. This research aimed to investigate the anti-apoptotic effect of c-fos in resistant breast cancer cell MCF-7/ADR, and to compare with its sensitive counterpart MCF-7.Methods:Doxorubicin with various concentrations was used to treat MCF-7 as well as its MDR- counterpart MCF-7/ADR. The growth inhibitory rate of MCF-7 and MCF-7/ADR wasdetermined by MTT assay. Additionally, RT-PCR was used to test the expression of P-gp and c-fos mRNA in MCF-7 and MCF-7/ADR; The expression of c-fos mRNA was detected by RT-PCR after 3 μmol doxorubicin treatment;We further established cell lines that stably interfered with c-fos, named MCF-7/ADR/si-fos-8B, MCF-7/ADR/si-fos-3D. Flow cytometry and MTT assay were used to investigate the apoptosis rate and inhibitory rate in these above cells under the treatment of 5-FU, CDDP or γ-radiation. At last, RT-PCR and Western blot analysis were used to detect the expression of bax, bcl-2, puma, p53.Results:The expression of c-fos and P-gp (MDR-1) was up-regulated in MCF-7/ADR, compared with its sensitive counterpart MCF-7. Additionally, the resistant fold of MCF-7/ADR to doxorubicin was nearly 40; The expression of c-fos was gradually up-regulated after 3 μmol doxorubicin treatment; The sensitivity to drugs (5-FU and CDDP) was increased after c-fos interference while the apoptosis rate was also increased after 5-FU, CDDP and γ-radiation treatment. RT-PCR and Western blot analysis indicated that up-regulation ofbax,puma,p53 after c-fos interference while the expression of bcl-2 was down-regulated.Conclusion:c-fos may act as an anti-apoptotic protein in resistant breast cancer cell line MCF-7/ADR by regulating the expression of apoptosis related proteins, and may play a vital role in the formation of multi-drug resistance phenotype.
7.Three-dimensional visualizational reconstruction of pedicled island skin flap with arterial arch at the superior border of abductor hallucis muscle

BACKGROUND:At present, design of pedicled island skin flap with arterial arch at the superior border of abductor hal ucis muscle mainly depends on clinician’s abstract thinking, which makes it difficult for quantitative analysis and exchange learning, and makes it limited in clinical application. OBJECTIVE:To perform the three-dimensional reconstruction of pedicled island skin flap with arterial arch at the superior border of abductor hal ucis muscle, and to establish visible models of the flap. METHODS:One fresh adult cadaver specimen was col ected and perfused with the mixture of lead oxide-gelatine through anterior tibial artery and posterior tibial artery, and then received spiral CT scanning. The branch of anterior medial mal eolus artery, medial tarsal artery and the superficial branch of medial plantar artery as wel as its anastomotic branch were reconstructed with mimics10.0 software, in order to observe the constitute, track-way and distribution of arterial arch at the superior border of abductor hal ucis muscle. According to the feature of anatomical structures, reconstruction was conducted in skin and vessel to obtain three-dimensional arterial skin flap structure. RESULTS AND CONCLUSION:The layers of three-dimensional reconstruction were distinctively displayed, and the arterial arch at the superior border of abductor hal ucis muscle was displayed wel with strong physical sense and clear relationship between skin and arteries. The morphology of various anatomical structures could be observed clearly on the three-dimensional reconstruction image, especial y the formation of arterial arch at the superior border of abductor hal ucis muscle and its surface projection. Three-dimensional reconstruction of pedicled island skin flap with arterial arch at the superior border of abductor hal ucis muscle may provide great value for basic research, clinical experiments and surgical planning, and provide visual reference for the individual operation design and can be able to enhance the successful rate of surgery.
8.Accuracy of X-ray fluoroscopy versus CT three-dimensional image navigation in cervical pedicle screw insertion

BACKGROUND: Navigation systems all can improve the accuracy rate in cervical pedicle screw insertion, but cannot achieve desired outcomes because of some shortcomings, such as poor imaging quality, complicated operation, poor real-time performance, and invasive navigation.OBJECTIVE: To study the accuracy of X-ray fluoroscopy and CT three-dimensional image navigation in cervical pedicle screw insertion.METHODS: Totally 90 patients undergoing cervical pedicle screw insertion from the First Affiliated Hospital of Nanyang Medical College were enrolled, and randomly divided into control and experimental groups (n=45 per group). 132 cervical pedicle screws were inserted into the patients in the control group manually under X-ray fluoroscopy, and 128 ones were inserted into the patients in the experimental group assisted with CT three-dimensional image navigation. The operation time and intraoperative blood loss in the two groups were recorded. Postoperative CT three-dimensional reconstruction was performed to compare the placement accuracy between two groups.RESULTS AND CONCLUSION: The excellent and good rate of placement in the experimental group (95.3%) was significantly higher than that in the control group (88.6%, P < 0.05). (2) The operation time and intraoperative blood loss in the experimental group were significantly higher than those in the control group, and the navigation matching time and radiant quantity in the experimental group were significantly less than those in the control group (P < 0.05). (3) The Japanese Orthopedic Association scores showed no significant difference between two groups before surgery (P > 0.05),and were significantly improved in the experimental group compared with the control group at 3 and 6 months postoperatively (P < 0.05). The incidence of complications in the experimental group was significantly lower than that in the control group (7% versus 16%, P < 0.05). (5) These results suggest that compared with X-ray fluoroscopy, CT three-dimensional image navigation can improve the accuracy of cervical pedicle screw insertion, showing higher safety and precision. However, it needs complicated operation skills, and long operation time results in massive intraoperative blood loss; thereafter, choosing which placement method depends on the patient condition.
9.Recurrent pulmonary infection, offline difficulties and airway abnormalities

Objective To discuss the relationship between pulmonary infection, offline difficulties and airway abnormalities and to discuss the type of airway abnormalities. Methods The clinical data of 43 patients of severe pneumonia with inspection of electronic fibro-bronchoscope in PICU from Feb 2008 to May 2010 were retrospectively analyzed. Results In the 43 cases,24 cases existel different degrees of airway abnormalities, most of airway cartilage softening,in 15 cases; laryngeal dysplasia in 3 cases; absence of the right bronchus and right lung in 1 case. Conclusion Many children may exist airway abnormalities,if they have such performance as the clinical manifestations of recurrent wheezing after birth, refractory respiratory infection or difficulties in clinical offline, the increase of PCO2 though the effective suction, paroxysmal cyanosis and excluded with heart disease. Those children should undergo inspection with electronic fibro-bronchoscope to confirm the diagnosis as early as possible.
10.Causes and countermeasures of complications of one-stage surgical management for spinal tuberculosis

Objective To analyze the causes of the complications of one-stage surgical management for spinal tuberculosis and its countermeasures.Methods From May 1997 to December 2008,116 patients with spinal tuberculosis underwent operations.Among them,6 were in cervical vertebra,3 in cervicothoracie vertebra,29 in thoracic vertebra,37 in thoracolumbar vertebra.29 in lumbar vertebra and 12 in hmbrosacrum vertebra.All cases underwent the anterior and/or radical debridement,decompression,autogenous bone grafting or radical debridement,decompression,autogenous bone grafting and internal fixation.The causes of the complications of surgical management and the countermeasures were analyzed.Results One hundred and ehven cases were followed upfor an average of(3.86±2.16)years(range 6 months to 12 years).The rate of complication was 13.5%(15/111).Recurrent spinal tuberculosis occarred in 5 cases,internal fixation loosened in 1 case,rupture of the external iliac vein occurred in 2 cases,lateral ventral syndrome in 1 case,venous embolism of the lower extremity in 1 csse,reactional psychosis in 2 cases,pneumatothorax in 2 cases and femoral nerve irritating syndrome in 1 case.Among the recurrent cages,2cases with anterior debridement and 2 cases with posterior debridement underwent anterior operation again andgot good results, 1 case with anterior debridement gave up the reoperafion.Other cases relieved after conservative therapy.Conclusions Standardized anti-tuberculosis thempy is fundamental for preventing the recurrence of tuberculosis.Correct approach for the operation and thorough radical debddement are the key points for preventing the recurrence of tuberculosis.Good and effective therapy surround the operation is helpful for preventing the recurrence of tuberculosis.
