1.Analysis of clinicopathological characteristics and prognosis for triple negative breast cancer

Background and purpose: Triple negative breast cancer (TNBC) is a high risk breast cancer characterized by the negative expression of estrogen receptor(ER), progesterone receptor (PR) and Her-2 that have no specific therapy. This study was to analyze clinical pathological characteristics, survival, and prognostic factors of patients with TNBC. Methods: Clinical and pathological as well as follow-up data of TNBC, treated at the Cancer Centre of Sun Yat-sen University from Jan. 2000 to Dec. 2003, were collected and analyzed. Results: A total number of 128 women were identified as having triple negative breast cancer. The median age of these patients was 46 years, and 60.9% of them had stage Ⅰ or Ⅱ disease. The majority of pathological types were invasive ductal carcinomas, and 78.1% of tumors were staged T1 or T2. And 48.4% of these patients were involved in lymph node. Event-free survival, local replase-free survival, distant metastasis-free survival and overall survival at five years were 71.1%, 84.3%, 75.8% and 83.6% respectively. Though lymph node metastasis, tumor masses, stage and lymph-vascular invasion were all found to be related to overall survival, however, only lymph node metastasis and tumor masses affected the overall survival as revealed by the Cox proportional hazard model analysis. Conclusion: Triple negative breast cancer has distinct clinical and pathological characteristics. The patients are usually young, with large masses, lymph node metastasis, family history of breast cancer and poor prognosis; lymph node metastasis and tumor mass are important prognostic factors.
2.The dynamic changes of plasma yon willebrand factor and P-selectin in the finger replanted patients and their clinical significance

Objective To explore the dynamic changes of von willebrand factor(VWF)and Pseleetin in the finger-replanted patients,and the relationship between the prognosis of the surgery and hypercoagulability.Methods From December 2004 to December 2006,eishty finger-replanted patients were recruited to our study.with 40 healthy volunteers as controls.Plasma VWF and P-selectin were detected by enzyme-linked immunosorbent assay(EUSA)in both controls and patients before or after replantation.Results The VWF and P-selectin levels had significant differences between the replantations and controls(F=14.76,11.76,P<0.01).The VWF levels in the patients of 1,4,8,16 hours after replantation were(1 715±493),(1 396±549),(1 266±504),(1 163±436)U/L respectively,all markedly higher than the controls(P<0.01).The P-selectin levels in patients of 1,4,8,16,24 hours after operation were(14.7±2.6),(12.5±3.0),(11.8±3.2),(11.1±3.0)、(10.5±2.6)μg/L,significanfly higher than the controls(P<0.01).The VWF levels in patients of pre-replantion and the 1,4,8,16,24,48,72 hours after replantation were(854±209),(1 535±389),(1 177±407),(1 040±283),(958±216),(829±193),(777±151),(713±137)U/L in successful group,and were(1 202±164),(2 333±243),(2 146±161),(2 039±244),(1 865±170),(1 645±283),(1 427±331),(1 188±262)U/L in unsuccessful groups.They were all significantly different at the same test-time points between two groups(t=4.44,5.12,6.10,8.43,10.17,8.85,5.10.4.61,P<0.05).The P-selectin levels in patients of 1,4,8,16,24,48,72 hours after replantation were(13.9±2.5),(11.2±2.0),(10.2±1.6),(9.6±1.2),(9.2±0.9),(9.5±0.6),(9.3±0.4)μg/L in successful group,and(17.2±1.0),(16.9±1.0),(17.0±1.3),(16.1±1.1),(14.9±1.5),(13.8±1.4),(12.8±1.2)μg/L in unsuccessful group.Significant difference existed at the same testtime points between two groups again(t=5.22.9.91,10.35,12.79,9.46.9.45,9.33,P<0.01).After replantation,both VWF and P-selectin were rapidly elevated and went to the summit 4 hours later,then declined to pre-replantation level about 24 to 48 hours later after replantation.Conclusions VWF and P-selectin were associated with the hypercoagulability.Dynamic monitoring VWF and p-selectin may be useful in determining the existence of hypercoagulability and the therapy of anti-coagulability.
3.Experimental study on treatment of glioma by embryonic neural stem cell transplantation in rats.

The neural stem cells in Wistar rats were cultured in vitro, purified, and transplanted into C6 glioma model in order to observe their biological characters and provide a basic foundation for treatment of neurological diseases by neural stem cell transplantation. The cells at hippocampal area from gestation 15-day rats were cultured in vitro, and frozen and preserved in liquid nitrogen. C6 tumor-bearing models (n=25) and neural stem cells transplantation models (n=35) were established. When the tumor grew to 3 to 4 weeks, 5 rats in each group were randomly selected for MRI examination. At different intervals, the rats were perfused and sampled for HE staining, GFAP and BrdU immunohistochemical staining. The results showed that after resuscitation of neural stem cells at 1-4 passages, the cell viability was 40%-63% with the difference being not significant. The cells could proliferate, passage, and most cells transplanted into glioma model survived. The mean survival time in neural stem cell transplantation group and control was 4.28 and 3.88 weeks respectively, and the average tumor size in the former was smaller than in the latter. It was concluded that embryonic neural stem cells in rats could proliferate and differentiate, and after resuscitation the biological characteristic and viability of the cells were not influenced. Neural stem cells had inhibitory effects on the growth of glioma cells and could prolong the survival of rat model.

4.Effects of peri-operative enteral nutrition and antibiotics on intestinal flora in patients with esophageal carcinoma

Objective To investigate the effects of peri-operative enteral nutrition (EN) and antibiotics on intestinal flora balance in patients with esophageal carcinoma.Methods Sixty patients were randomly divided into six groups:Group B,treated with antibiotics for 3 days and supported with EN before and after operation; Group C,treated with antibiotics for 3 days and supported with parenteral nutrition (PN) before operation and EN after operation ; Group D,treated with antibiotics for 3 days and supported with PN before and after operation ; Group E,treated with antibiotics for 7 days and supported with EN before and after operation; Group F,treated with antibiotics for 7 days and supported with PN before operation and EN after operation ; and Group G,treated with antibiotics for 7 days and supported with PN before and after operation.The first stool after surgery was obtained and tested for Bacteroides,Bifidobacterium,Lactobacillus,Escherichia coli,and Enterococcus.Ten healthy adults were enrolled as the blank control group.The numbers of the flora and ratio of Bifidobacteria to Enterococci (B/E) were compared.Complications such as incision infections,lung infections,and anastomotic fistula were recorded.Results The numbers of Bifidobacterium [(10.59 ± 0.39) vs.(10.88 ± 0.10) lg10n/ml,P =0.186),Lactobacillus [(8.59±0.31) vs.(8.72 ±0.22) lg10n/ml,P=0.534],Escherichia coli [(8.43 ±0.50) vs.(8.67 ±0.24) lg10n/ml,P=0.266],Enterococcus [(7.40 ±0.61) vs.(7.78 ±0.16) lg10n/ml,P =0.111],and B/E value [(1.2589 ± 0.0644) vs.(1.2560 ± 0.0330),P =0.825] in the Group B were not significantly different from the blank control group.The numbers of Bacteroids [(11.08 ± 0.48),P =0.139 ; (9.23 ± 0.42),P =0.000; (10.80±0.26),P=0.004; (10.24±0.45),P=0.000; (8.05±0.53),P=0.000vs.(11.36±0.48) lg10n/ml],Bifidobacterium [(10.19 ±0.49),P=0.062; (9.00 ±0.52),P=0.000; (9.31 ±0.45),P=0.000; (8.47±0.56),P=0.000; (6.99 ±0.56),P =0.000 vs.(10.59±0.39) lg10n/ml],Lactobacillus [(7.99 ± 0.58),P =0.006 ; (6.84 ± 0.47),P =0.000 ; (7.72 ± 0.35),P =0.000 ; (6.93 ±0.43),P =0.000; (5.93 ±0.76),P=0.000 vs.(8.59 ±0.31) lg10n/ml],Escherichia coli [(8.19 ±0.43),P=0.258; (7.93±0.60),P=0.020; (7.47±0.43),P=0.000; (6.90±0.42),P=0.000; (6.58±0.57),P =0.000 vs.(8.43 ± 0.50) lg10n/ml],and Enterococcus [(6.90 ± 0.54),P =0.037 ; (5.89 ± 0.68),P =0.000; (6.20±0.52),P=0.000; (5.91 ±0.39),P=0.000; (5.14±0.58),P=0.000 vs.(7.40±0.61) lg10n/ml] of groups C,D,E,F,and G decreased compared with those of the Group B.The values of B/Ein the D and G groups decreased significantly when compared to the blank control group (1.1433 ±0.1350,P =0.025 ; 1.0706 ± 0.1413,P =0.000 vs.1.2560 ± 0.0330).The incidences of pulmonary infections (x2 =3.647,P =0.601) and anastomotic leak (x2 =5.000,P =0.416) among all groups were not significantly different.Conclusions EN applied 3 days before surgery and after surgery is beneficial for maintaining the balance of intestinal flora.Long-term administration of antibiotics may cause dysbacteriosis and even increase complications.
5.CT measured parameters of upper airway in OSAHS patients

OBJECTIVE To explore the value of spiral CT in detecting narrow level of obstructive sleep apnea hypopnea syndrome(OSAHS) through investigating measured parameters of upper airway. METHODS The upper airway levels from the roof of nasopharynx to the inferior border of cricoid cartilage were scanned with spiral CT in 127 OSAHS patients and 52 controls.The coronal and sagittal diameter and the cross sectional areas of palate,uvula,tongue, epiglottis level were measured using image handling workstation.The measured values of each level were divided by the measured values of the cricoid cartilage level,and get relative values.The relative values of the OSAHS patients and controls were compared. RESULTS The relative values of posterior soft palate and uvula region,lingual region and epiglottis regions were statistically different.The difference also existed between genders.The relative values of 46 controls and 17 OSAHS patients were in the normal range. Among 127 OSAHS,the relative values of 110 cases with OSAHS(86.6%) were below the normal range in every region,while in control group,the relative values were found below the normal range in only 6 persons (11.2%).CONCLUSION The relative values of each regions is useful to judge the narrow level of upper airway in OSAHS patients.
6.HypnoPTT in the diagnosis of the sleep respiratory disease in children

OBJECTIVE To explore a more effective method of detecting sleep respiratory events in children. METHODS Thirty-eight children were tested with HypnoPTT and 31 with polysomnography (control). The test parameters and operative methods were compared. RESULTS In addition to the parameters comm.on to both HypnoPTT and polysomnography, inspiratory flow limitation and spontaneous microarousal can be measured using HypnoPTT and fewer electrodes were needed. CONCLUSION HypnoPTT is a convenient method characterized by less sleep disturbance and credible results, rendering it is especially suitable for pediatric cases. Pulse transit time is a useful parameter for diagnosing the sleep respiratory disease.
7.Effects of Rhodiolae Crenulatae Radix ET Rhizoma extracts on the aphrodisiac qualities of male mice

Objective To study the effects of Rhodiolae Crenulatae Radix ET Rhizoma extracts on sexual behavior of male mice.Methods 50 healthy male mice were randomly divided into the low dose, middle dose and the high dose Rhodiola group, theNanbao capsules group and the normal control group, 10 mice per group. The low dose, middle dose and high dose group were drenched with 0.05, 0.20 and 0.80 g/kg Rhodiola diluent respectively. TheNanbao capsules group mice were drenched with 2.00 g/kg turbid liquid. The normal control group were drenched with saline in the same volume. Liquid is drenched two times each day for 21 days. After 21 days, 50 female mice were matched with to the ratio of 1:1. The number of free movement and swimming test were observed before execution. After the execution, the organ indexes were calculated, and then the contents of SOD and MDA in the testis and liver were measured.Results Compared with the normal control group, capturing latency period of low dose group and middle dose group (20.88 ± 19.94 s, 35.40 ± 22.02 svs.78.11 ± 43.33 s) significantly decreased (P<0.05 orP<0.01). Testicular coefficient of the middle dose group (0.72% ± 0.10 %vs. 0.64% ± 0.08%) was significantly increased (P<0.05); the content of SOD in testicular of the middle dose group, the high dose groups and theNanbao capsules group (152.71 ± 38.10 U/mg, 122.32 ± 52.76 U/mg, 94.38 ± 22.20 U/mgvs. 25.30 ± 14.21 U/mg) increased (P<0.01); the content of SOD in liver of the middle dose group and theNanbaocapsules group (77.71 ± 26.35 U/mg, 74.10 ± 26.04 U/mgvs. 57.92 ± 17.17 U/mg) significantly increased (P<0.05).Conclusion Rhodiola extract can improve the ability of sexual behavior of male mice, and improve the antioxidant capacity of testis and liver.
8.A clinic analysis on 30 cases of mucoepidemoid carcinoma in nasal cavity and nasal sinuses

OBJECTIVE To evaluate the clinic feature and therapeutic principles and to improve curative effect of mucoepidemoid carcinoma originated in nasal cavity and nasal sinuses. METHODS A restrospective review of clinic features ,therapeutic methods and consequences was undertaken in thirty cases with nasal cavity and nasal sinuse mucoepidemoid carcinoma and treated at the 81st hospital of CPLA from 1990 to 2001. RESULTS Twenty patients received surgery、radiotherapy and chemotherapy, of which sixteen survived in five to ten years follow-up . Six patients received surgery and radiotherapy, of which four survived in five years follow-up. Only one out of two patients who received radiotherapy and chemotherapy respectively survived in five years follow-up. The total five years survival rate was 73 %(22/30). CONCLUSION CT or MRI scan may be helpful to evaluate the location and scale of mucoepidemoid carcinoma.Final diagnosis rely on histopathology examination. Early radical resection combined with radiotherapy and chemotherapy pre- or postoperatively could improve five-year-survival rate and reduce recurrence rate.
9.The study of calcification of autogenous bone marrow stem cell transplantation on alveolar bone defect in dogs.

OBJECTIVETo evaluate the effect of calcification of autogenous bone marrow stem cell transplantation in periodontal tissue regeneration.
METHODSBone marrow stem cells derived from the same dog were cultured with alpha-MEM. 1 x 10(7) cells of first passage were allowed to attach to the collagen membrane for 24 hours. The membrane-cells were transplanted into periodontal defect in the same dog. Then the defects were covered with e-pTFE membranes. The defects covered only with e-pTFE without membrane-cells were served as control. Eighteen teeth of 6 dogs for every group were studied. The dogs were sacrificed after 6 weeks.
RESULTSThe results showed that new bone formation in test group was significantly higher than that of control group. The calcification of new bone in test group was better than control group.
CONCLUSIONSThe results suggested that autogenous bone marrow stem cell transplantation with guided tissue regeneration technique could enhance periodontal tissue regeneration and could form new bone tissue fast and could shorten times of periodontal tissue regeneration in dogs.

10.Cloning and studying on the expression and function of thyroid hormone-response protein-1 gene, a novel thyroid hormone-response gene from neonatal rat brain

Objective To is ol ate novel thyroid hormone-response genes, to study the characterizations of the ir expressions and to predict their possible functions in neonatal rats. Methods A neonatal rat model with congenital hypothyr oidism was established and cDNA fragments of novel thyroid hormone-response gen es from cerebral cortex of neonatal rats were obtained by fluorescence-labeled DD-PCR analysis, subcloning and sequencing. Complete cDNAs of novel thyroid hor mone-response genes were cloned by the techniques of electronic clone, RT-PCR and sequencing, their expressions regulated by thyroid hormone were confirmed b y Northern blot analysis, their distributions, transcription levels in different tissues and different brain areas were further observed by semiquantitative RT -PCR analysis, and their possible functions were postulated through bioinformat ic techniques. Results A novel complete cDNA of thyroid hormone-response protein-1 (TRP-1) gene is cloned. It is 973 bp in f ull-length (Gene Bank accession no. AF348365), and its transcription was enhanc ed in cerebral cortex in neonatal hypothyroidism rats. The expression of its mRN A was very extensive, but more abundant in brain. Its transcriptional level in d ifferent brain areas was not uniform, much higher in olfactory bulb. Its encodin g protein had some significant domains and motifs. Conclusion TRP-1 gene is a new thyroid hormone-response gene and may play an important role during normal brain development. Its abnormal expression may b e partially responsible for neurological defects in brain arising from thyroid h ormone deficiency during critical period for perinatal rats.
