1.Analysis of Three Treatment Regimens and Pharmaceutical Care in One Patient with Lipoprotein Glomeru-lopathy

Objective:To provide reference for the treatment of lipoprotein glomerulopathy and to investigate the participation of clinical pharmacists in the whole treatment course. Methods: Three different treatment regimens, including immunosuppressive thera-py, dual plasma filtration therapy and lipid-lowering combined with reducing urinary protein therapy was respectively adopted for one patient with lipoprotein glomerulopathy, and the efficacy was evaluated. Clinical pharmacists assisted physicians in deciding treatment regimen, performed pharmaceutical care, adjusted medication and analyzed the prognosis of the patient during the follow-up. Results:1. Immunosuppressive therapy was ineffective for the patient;2. The dual filtration acted quickly, while the expense was high and the disease was easy to relapse;3. The third therapy was relatively safer and more economical with long-term effect. Conclusion:Combi-nation therapy of lowering lipid and reducing urinary protein is the most suitable treatment regimen for the patient with lipoprotein glo-merulopathy. Clinical pharmacists play an important role in the whole course of treatment to assist physicians in obtaining the maximum benefit of patients.
2.Paclitaxel combined with 5- Fluorouracil continuous infusion for treatment of advanced gastric cancer

Objective To observe the efficacy and toxicity of paclitaxel combined with leucovorin/5- fluorouracil continuous infusion in patients with advanced gastric cancer. Methods 26 patients with a histologic diagnosis of advanced gastric cancer were observed. The chemotherapy regimen comprised of PTX 135 mg/m2 infusion for 3 hours, LV 200 mg/m2 infusion for 2 hours followed by 10 minutes intravenous infusion of 5- Fu 375 mg/m2, then 120 hours infusion of 5- Fu 3.0 g/m2 with an ambulatory pump, repeated every 3 weeks. All patients were received at least two cycles of treatment. Results All the cases could be evaluated. The total response rate was 46.2 %. Two obtained complete remission (CR) (7.7 %). The major toxic side effects were neutropenia, nausea, vomiting, mucositis, alopecia, arthralgia, myalgia and cardiac toxic reaction, but were tolerable. Conclusions The combination of paclitaxel and 5- Fu continuous infusion is an effective and tolerable regimen in the treatment of gastric cancer.
3.Dose-response relationship of 0.2% ropivacaine for ultrasound-guided stellate ganglion block

Objective To determine the dose-response relationship of 0.2% ropivacaine for ultrasoundguided stellate ganglion block (SGB).Methods Seventy-five ASA physical status [or Ⅱ patients with migraine,aged 23-55 yr,with body mass index of 22-28 kg/m2,scheduled for elective ultrasound-guided SGB,were randomly divided into R1-5 groups (n =15 each) using a random number table.In R1,R2,R3,R4 and R5 groups,the patients underwent ultrasound-guided SGB with 0.2% ropivacaine 1,2,3,4 and 5 ml,respectively.A successful SGB block was confirmed by the onset of ptosis (Horner syndrome) on the injected side.Probit analysis was used to calculate the effective dose of 0.2 % ropivacaine in 50 % and 95 % of the patients (ED50 and ED95) and 95% confidence interval (95% CI).Results The ED50 of 0.2% ropivacaine for ultrasound-guided SGB was 2.2 ml (95%CI 1.9-2.5 ml) and ED95 was 3.2 ml (95%CI 2.8-4.1 ml).Conclusion The ED50 and ED95 of 0.2% ropivacaine for ultrasound-guided SGB are 2.2 and 3.2 ml,respectively.
4.Distribution and drug resistance analysis of CR-AB and MRSA from 2011 to 2014

Objective To provide a scientific reference for the choice of antimicrobial drugs by analyzing the distributions and the antimicrobial resistances of Carbapenem‐resistant Acinetobacter baumannii (CR‐AB) and methicillin‐resistant Staphylococcus au‐reus (MRSA) in the Northern Jiangsu People′s Hospital .Methods All the CR‐AB and the MRSA isolated from hospitalized pa‐tients from January 2011 to March 2014 were collected and analyzed .Use the disk diffusion method for antimicrobial resistance tes‐ting .Results A total of 170 CR‐AB strains and 168 MRSA strains were isolated .CR‐AB mainly distributed in the ICU ,which ac‐counting for 61 .17% (104/170) .MRSA mainly isolated in neurosurgery ,about 27 .98% (47/168) .67 .06% of CR‐AB strains and 54 .17% of MRSA strains were from Sputum specimens .The antimicrobial resistance rate of CR‐AB to most antimicrobial drugs were reached 50 .00% ,the resistance rates to cefoperazone/sulbactam (27 .65% ) ,minocycline (21 .18% ) ,amikacin (19 .41% ) and tobramycin (12 .35% ) were all less than 30 .00% ,the strain that resistant to polymyxin was not found .The sensitivity rates of MR‐SA to linezolid ,vancomycin ,and tigecycline were 100 .00% ,and the resistance rates to nitrofurantoin and kuinuputing/ dalfopristin were low (less than 2 .00% ) .Conclusion The antimicrobial resistances of CR‐AB and MRSA are serious .In order to control effec‐tively the spreading of CR‐AB ,MRSA and other multi‐drug resistant bacteria ,it′s necessary for us to strengthen the management of key departments ,and select antimicrobial drugs based on the results of drug susceptibility testing .
5.Dosimetric comparison of stereotactic body radiation therapy plans for spine metastasis using flattening filter free and flattening filter modes

Objective To compare the dosimetric difference of stereotactic body radiation therapy (SBRT) plans for spine metastasis using 6 MV X-ray flattening filter free (FFF) and flattening filter (FF) modes.Methods A total of nine previously treated patients with a total of twelve spine metastasis were retrospectively included and replanned using FFF-SBRT and FF-SBRT, respectively.The dose-volume histograms (DVH), target conformity index (CI), monitor unites (MUs) and treatment time of target volume, organs at risk and normal tissues were compared between the plans.Results Both FFF-SBRT and FF-SBRT met the clinical objectives.Dose distribution of target volume, organs at risk and normal tissues were similar.MUs of the FFF-SBRT were significantly higher than that of FF-SBRT (t =-5.20, P <0.0l), while the treatment time was almost half of the latter (t =17.27, P < 0.01).Conclusions Two plan modes are both clinically acceptable.FFF-SBRT plans are delivered in less time and with better efficiency.Trial registration Chinese clinical trial registry, ChiCTR-TRC-14004281.
6.Down-regulation of leucine-rich repeats and immunoglobulin-like domain proteins (LRIG1-3) in HP75 pituitary adenoma cell line.

Three human leucine-rich repeats and immunoglobulin-like domains (LRIG) genes and proteins, named LRIG1-3, has been previously characterized and it was proposed that they may act as suppressors of tumor growth. The LRIG1 protein can inhibit the growth of tumors of glial cells and the down-regulation of the LRIG1 gene may be involved in the development and progression of the tumor. Real-time reverse transcription-polymerase chain reaction (RT-PCR) is a recently developed technique for quantitative assessment of specific RNA levels. In the current study, it was demonstrated that LRIG1-3 and EGFR mRNA was detected in human pituitary adenoma cell lines and a normal pituitary sample, with differences in the expression levels. Compared to the normal pituitary samples, the expression of LRIG1-3 in HP75 cell line was lower, but the expression of EGFR in HP75 cell line was higher. The results are consistent with LRIG1-3 being tumour suppressor genes, and LRIG genes decreasing the expression of EGFR. The ratio of EGFR/LRIG1 was increased at least 13-fold in HP75 cells compared with the normal pituitary cells, which was also the case for the ratio of EGFR/LRIG2 (14-fold increase in HP75) and EGFR/LRIG3 (11-fold increase in HP75). Further studies were needed to elucidate the explicit role of LRIG genes as negative regulators of oncogenesis in human pituitary adenoma.
7.Isometry of grafts in PCL reconstruction under femoral tunnel shifting condition

Objective To explore the isometry of grafts in PCL(posterior cruciate ligament)double-bundle re-construction under femoral tunnel shifting condition.Method Knee specimens from ten fresh frozen cadavers were used.PCL were divided into anterolateral bundles(ALB)and posteromedial bundles(PMB)to the inser-tion footorint.The anterior,postedor,proximal,distal and central points of the two bundles'femoral attachment site were respectivelyanchored to the middle of the PCL's tibial attachment site by the trial wires.Changes in length of the intra-articular part of the wires were recorded while the knee was flexed from 0°to 120°.Result The length changes in every point were compared.All of the maximal length changes of ALB's proximal,pos-todor points and PMB's proximal points were not greater than 2mm.No significant difference between the length changes of ALB's proximal point and posterior(P=0.864>0.05)was found.Conclusions The femo-ral tunnel for the PCL double-bundle reconstruction should be located as follows:ALB should be at the middle point of upper edge of femoral attachment site(proximal point),while PIVIB at the middle point of femoral attachment site(proximal point).
8.Regulation of swimming motility by H-NS in Vibrio parahaemolyticus

Objective To investigate the regulation of swimming motility by H-NS in Vibrio parahaemolyticus(VP). Methods VP was inoculated into the semi-solid swimming agar plate containing 1% Oxoid tryptone, 2% NaCl, 0.5%Difco Noble Agar, and 0.1% arabinose followed by incubation at 37℃ for 4.5 h before the diameters of bacterial lawns were measured.Total RNAs were extracted from the wild-type (WT) strains and the hns null mutant (Δhns), and the quantitative real-time( RT)-PCR( qRT-PCR) was carried out to calculate the transcriptional variation of flaA between WT andΔhns strains.The entire promoter DNA region of flaA was amplified and cloned into the lacZ fusion vector pHRP309 containing a promoterless lacZ gene. The recombinant lacZ reporter plasmid was transformed into WT and Δhns, respectively, to measure the β-galactosidase activities in cellular extracts using the β-galactosidase enzyme assay system. Results and Conclusion The phenotype results showed that swimming motility of VP was enhanced by H-NS.The qRT-PCR and LacZ fusion results indicated that the transcription of flaA was positively regulated by H-NS.Collectively, H-NS promotes the swimming motility of VP, at least partly, by activating the transcription of flaA.
9.Preliminary study on infection status and gene types of Cryptosporidium among HIV/AIDS patients in Guangxi

Objective To understand the infection status and gene types of Cryptosporidium among HIV/AIDS patients in Guangxi Zhuang Autonomous Region. Methods The fecal samples were collected from 285 HIV/AIDS cases in Nanning,Gui?lin,Qinzhou,Baise,Hechi cities of Guangxi and 150 HIV negative persons in Nanning City. The modified acid?fast staining and nested?PCR based on 18S rRNA were employed to detect the infection status of Cryptosporidium. The nested PCR products were sequenced,and the homology searches and identification for the gene types of Cryptosporidium were done by DNAStar soft?ware. Results The infection rate of Cryptosporidium in HIV/AIDS patients was 0.70%(2/285),and the rate of those with chronic diarrhea was 6.67%(2/30),the latter was significantly higher than that of the HIV negative persons(0,0/150)(P=0.002). Both the two HIV/AIDS patients infected with Cryptosporidium were from Guilin City. By molecular identification,the Cryptosporidium strains which the above 2 patients were infected with were Cryptosporidium andersoni and Cryptosporidium hominis respectively. Conclusions Cryptosporidium co?infection can be found in HIV/AIDS patients in Guangxi. The geno?types of the infection strains include Cryptosporidium andersoni and Cryptosporidium hominis.
10.Comparison of bleeding volume during endoscopic sinus surgery in patients with hypertension under total intravenous anesthesia and combined intravenous with inhaled anesthesia

Objective To compare the bleeding volume during endoscopic sinus surgery in pa-tients with hypertension under total intravenous anesthesia or combination of intravenous with inhaled anesthesia.Methods Forty adult patients with hypertension (ASA Ⅰ or Ⅱ),male 25 and female 1 5,aged 35-54 years,agreed to be on electively endoscopic sinus surgery,were divided randomly into two groups(group A and group B,n =20 each).All patients were injected intravenously with fenta-nyl,propofol and cisatracurium during the induction of general anesthesia.During the maintenance stage of the general anesthesia,patients in group A were injected intravenously with propofol,and patients in group B were inhaled with sevoflurane until 5 minutes before the end of the operation.BIS was 40-60 during the operation in the two groups.After induction of anesthesia and tracheal intubation,ni-troglycerin and esmolol were given to adjust blood pressure and hypervolemic hemodilution was performed.. Bleeding volume and operation duration were recorded at the end of the operation.The arterial blood gas, blood lactate and coagulation function were monitored 30 minutes before the operation (T0 ),30 minutes (T1 )and 90 minutes after the operation beginning (T2 )and 30 minutes after the end of the operation (T3 ). Results Bleeding volume in group A was significantly less than that in group B (P <0.05).Fifteen minutes before the operation,the MAP of all the patients in two groups was controlled slowly to 60 mm Hg.Com-pare to those at T0 ,there was no significant difference in the arterial blood pressure,blood lactate and coag-ulation function at T1 ,T2 and T3 .Conclusion During the endoscopic sinus surgery with intravenous anes-thesia for patients with hypertension,the bleeding volume was less than that with inhalation anesthesia.
