1.Clinical value of LEEP for the diagnosis and treatment of cervical intraepithelial neoplasia

Objective To investigate the clinical values of loop electrosurgical excision procedure (LEEP) in diagnosis and treatment of cervical intraepithelial neoplasia(CIN). Methods Fifty-two patients with cervical intraepithelial neoplasia underwent LEEP following cytology, colposcopy and multiple biopsies, were treated by LEEP. The cure rate, the operative time, bleeding volume and patients reaction of LEEP to the cervical intraepithelial neoplasia were investigated during the follow-up. Results The effective rate was 98.07 % in the patients treated by LEEP. The effective rate was 100 % in the patients in grade CIN Ⅰ or CIN Ⅱ by LEEP. No patients in grade CIN Ⅰ and Ⅱ had recurrence during the follow-up 1 year. The mean operative time was 7.8 minutes. The mean bleeding volume was 10 ml. No secondary bleeding and post-operative infection occurred. All these surgical specimens were checked successfully for pathology diagnosis. Conclusion The advantages to use the LEEP to manage cervical intraepithelial neoplasia include its simpleness to handle, short operative time,less bleeding, less vaginal discharge, safety, and high cure rate. LEEP can offer intact sample for pathological diagnosis. LEEP electrotome is a very ideal therapy for the cervical intraepithelial neoplasia, and can block the development of precancerosis to infiltrating carcinoma effectively.

Rheumatoid arthritis (RA) is an autoimmune disease characterized by the accumulation and proliferation of inflammatory cells in the synovial (joint) lining.By investigating RA pathologic processes and also through experimental models where immune complexes (inflammatory sediments) play a fundamental role.Many other autoantibodies have then come to our knowledge to be associated with the disease.Though it remains unknown the autoimmune pathology of B cells and why the clones of autoreactive B cells survive and proliferate in RA patients,but no doubt these autoantibodies represent a very useful tool in both diagnostic and prognostic terms.In joint synovial fulid,B cells also secrete cytokines,which can be interacted with other cells.B cells express IL-13 receptor a1 (IL-13Ra1),IL-13 can induce CD23 expression on B cells and promote proliferation of naive B cells. In addition,IL-13 is a cytokine which is produced mainly by activated T helper cell 2 (Th2 cells),it can inhibit the production of pro-inflammatory factor and chemokines,and has a certain relationship with the differentiation of B cells. Therefore,it is necessary to summarize the mechanims of RA pathogenesis related with B cells and IL-13,which has great significance in the diagnosis,treatment and basic immunology research of autoimmune diseases such as RA.
3.Effect of He's Santong Method of Acupuncture for Treatment of Cervical Spondylosis of Nerve-root Type

Objective To observe the effect of He's Santong method (Three ways to remove obstruction) of acupuncture for treatment of cervical spondylosis of nerve-root type.Methods The 52 patients with nerve-root type cervical spondylosis were randomly divided into two groups, 26 in each. The treatment group was treated by He's Santong method of acupuncture while the control group was administered Gentongping Granule, 2 weeks as one course. Background Choroidal neovascularization (CNV) is one of the primary causes leading to visual damage in many fundus diseases.Many evidences indicate that macrophage activation and monocyte chemoattractant protein-1 (MCP-1) play important roles in CNV.However, the dynamic expression of macrophage and MCP-1 in the initial stage of CNV is not clear.Objective This study was to investigate the dynamic changes of F4/80 and MCP-1 expressions in retina-choroid tissue with experimental CNV.Methods Laser-induced CNV models were monocularly established in 105 SPF 8-week-old male wild type C57BL/6 mice.The mice were sacrificed at 6,12,24, 48 and 72 hours after photocoagulation, respectively, and the retina-choroid tissue sections and choroidal flatmounts were prepared.The histopathological examination was carried out to observe the changes of morphology and structure as well as inflammatory response in CNV.The expression and distribution of F4/80 and MCP-1 protein in retinachoroid were detected by double immunofluorescence technique.The expression and distribution of F4/80 in choroid were examined by immunofluorescence.The relative expression levels of F4/80 mRNA and the content of MCP-1 protein in RPE-choroid complex were assayed using real-time quantitative PCR and ELISA,respectively.The use and care of the mice complied with the Regulation for the Administration of Affair Concerning Experimental Animals by Ethic Committee of Experimental Animals of Nanjing Medical University.Results The rupture of Bruch membrane, RPE, outer nuclear layer and choroid was exhibited under the optical microscope 6 hours after photocoagulation.Infiltration of inflammatory cells and tissue edema were seen as the lapse of photocoagulation time, and proliferation of vascular endothelial cells was found 72 hours after photocoagulation.F4/80 was expressed in photocoagulation area 6 hours later, and MCP-1 was expressed around the area.With the lapse of photocoagulation time,the expression intensity of MCP-1 weakened and that of F4/80 enhanced.The contents of MCP-1 protein in RPE-choroid complex were (31.25±4.73), (276.31 ±4.20), (331.95 ±5.86), (221.24±4.42), (179.89 ± 4.10) and (130.80 ± 5.90) pg/mg in the normal control group, photocoagulation 6-, 12-, 24-, 48-and 72-hour groups, respectively,with a significant difference among the groups (F=1 416.46 ,P<0.01).The contents of MCP-1 protein peaked at 12 hours after photocoagulation and then gradually declined.The expression levels of MCP-1 protein in different time groups were higher than those in the normal control group (all at P<0.01).A significant difference in F4/80 mRNA expression in RPE-choroid complex was also found among the groups (F =762.72, P<0.01, and a gradually raising tendency was seen over time, showing evidently increase in comparison with the normal control group (all at P<0.01).Conclusions Inflammatory response occurs in the early stage of experimental CNV.MCP-1 responds to the CNV at early stage,and the accumulation and activation of macrophage play an important role in the development of CNV.
6.Anolysis of related factors about prognosis of patients with Severey Traumatic Brain Injury

Objective To evaluate prognostic indicators in severe traumatic brain injury and intracania hamatomas with hernia and analyze which is the most important indicator.Methods Data of 84 cases with severe traumatic brain injury were retrospectively analyze .Age,GCS,pupil reflex,midline shift,compression of the cisteme,decompression time and complex injury were considered as possible prognostic indicators.SPSS13.0 was employed to analyze the data,Logistic regression(Forward conditional)analysis was done to confirm which are the most important prognostic indicators for severe traumatic brain injury and to evaluate the practical value in predicting prognosis(X2= 22.92,P< 0.01).Results 48 patients died and 36 survived.56(67%)patients had a bad prognosis(GCS≤3)and 28(33%)had a good prognosis(GCS >3).Those who had a shorter decompression time(≤3h)had a better prognosis than those who had a longer decompression time(>3h),with the mortality rate of 11% and 67% respectively.Lesser cisteme compression predicts better outcome.GCS,pupil reflex,midline shift,and complex injury didn' t enter the logistic regression equation.According to compression time,90.5% of the cases' prognosis was accurately predicted,according to compression time and cisterne compression,95.2% was accurately predicted.Conclusion The mortality rate of severe traumatic brain injury with hernia was high and surgical intervention was effective.Decompression time and cisteme compression were the most important factors affecting prognosis in severe traumatic brain injury and they could predict prognosis of most cases correctly.
8.Effects of Uygur Medicine Branchlets roses extracts on blood glucose and glucose tolerance in diabetes mice induced by alloxan

Objective To observe the effect of extract from Branchlets roses on blood glucose and glucose tolerance in diabetes mice induced by alloxan.Methods Diabetes animal model was established by alloxan.Dividing the model mice into eight groups:model group,water extract high,middle,and low dose (3.70,1.85,and 0.93 g/kg) group,and ethanol extract high,middle,and low dose (2.75,1.37,and 0.70 g/kg) group,and metformin (positive drug,200 mg/kg) group,and normal mice were taken as control group.Drug was ig administered to mice 3 d after molding once daily.Blood glucose test paper was used to determine fasting blood glucose 0,10,20,and 28 d after modeling,and the glucose tolerance test was performed 30 d after modeling.Results The extract of Branchlets roses from all the groups could decrease the blood glucose and improve the glucose tolerance,and showed a certain dose-effect relationship.In all the extracts,the alcohol extract had the best effect,but the effect was not as good as the positive control drug metformin hydrochloride group.Conclusion The extract of Branchlets roses can reduce the blood sugar content of diabetic mice,and improve the glucose tolerance.
9.Application of real-time fluorescence isothermal RNA amplification assay in the determination of Mycobacterium tuberculosis in pleural effusion

Objective To evaluate clinical value of utilization of real‐time fluorescence isothermal RNA amplification assay (SAT)for detecting Mycobacterium tuberculosis(MTB) in pleural effusion samples of patients with tuberculous pleurisy .Methods 90 hydrothorax specimens of patients with tuberculous pleurisy and 52 hydrothorax specimens of patients with non‐tuberculous pleurisy were selected ,and MTB was detected by using Roche culturing method ,SAT and TB DNA fluorescence quantitative expan‐sion method respectively .The sensitivity ,specificity and clinical value of SAT for detecting MTB were analysed .Results Taking Roche culturing method as the standard ,the sensitivity ,specificity and Youden index of SAT for detecting MTB were 90 .9% , 72 .1% and 0 .630 respectively .In patients clinically diagnosed with tuberculous pleurisy ,the positive detection rate of hydrothorax specimens detected by using Roche culturing method and SAT were 24 .4% and 43 .3% respectively ,and had statistically significant differences(χ2 =7 .166 3 ,P=0 .001 4) .Conclusion SAT is a rapid ,sensitive and specific method for the detection of MTB in pleu‐ral effusion samples ,which could improve the detection rate of MTB and decrease incidence of missed diagnosis and misdiagnosis .
10.Study of immunoregulatory activity of rhizoma menipermi extracts on mouse and human lymphocytes and macrophages in vitro

Objective:To study the immunoregulatory activity and mechanisms of Chinese medicinal herb Rhizoma Menipermi extracts.Compare the biological activity of Rhizoma Mednispermi extract with water (RMW),ethanol (RME) and Rhizoma Menispermi Polysaccharide (RMP).Methods:The chemical composition of Rhizoma Menipermi extract was separated with water,ethanol extraction and distillation methods.The effect of Rhizoma Menipermi extracts on lymphocytes and the effect on the metabolism and phagocytosis of mouse macrophages were studied by MTT colorimetric method and neutrol red method.Results:RMP can stimulate the proliferation of mouse spleen cells and human lymphocytes and enhance the metabolism and phagocytosis of mouse macrophages.RMW and RME have inhibitory effect on mouse spleen cells and human lymphocytes and mouse in vitro,and inhibited the metabolism and phagocytosis of mouse macrophages.Conclusion:Diffrent Rhizoma Menipermi composition showed diffrent effects on lymphocytes,RMW and RMP can be used for the supplementary treatment of cancer.
