1.Analysis of inhaled and food allergen spectrum of patient with allergic rhinitis in north of Zhejiang.

OBJECTIVE:
To explore the inhaled and food allergen distribution of patient with allergic rhinitis in north of Zhejiang and to analyze the difference of allergen distribution among different age groups.
METHOD:
One thousand and forty eight patients in north of Zhejiang diagnosed with allergic rhinitis in our outpatient department were tested with skin prick test (SPT). The positive rate of inhaled and food allergens were calculated. To analysis the difference of positive rate between children and adult.
RESULT:
Nine hundred and eighty-eight cases (94.3%) had the positive reaction. Dermato phagoides farinae and Dermatophagodies pteronyssinus had the highest positive ratio (72.1%, 71.8%) in inhalation group, followed cockroach (14.1%). In food group, Shrimp and peanuts had the highest positive ratio (18.3%,14.2%). Between children group and adults group, positive rate of food allergen was significantly different (P < 0.05), but of inhaled allergen was not significantly different.
CONCLUSION
The distribution of some allergens in children group and adults group was variable. The study shows that Dust mite was the mostly responsible common allergen in north of Zhejiang.

2.Application of miR-9 and miR-195-3p in the diagnosis of primary hepatic carcinoma and their changes before and after interventional therapy

Objective:To investigate the diagnostic efficacy of miR-9 and miR-195-3p for primary hepatic carcinoma (PHC), and the changes in miR-9 and miR-195-3p levels after interventional therapy.Methods:A total of 123 cases of PHC patients and 30 cases of liver cirrhosis patients attending Tangshan People's Hospital from May 2019 to May 2020 were selected as the PHC group and the liver cirrhosis group, respectively, and 50 people who were physically healthy during the same period were selected as the healthy group. Serum miR-9 and miR-195-3p levels were detected by real-time quantitative PCR. The relationship between serum miR-9 and miR-195-3p levels and clinical-pathological characteristics of PHC patients was analyzed. Receiver operator characteristic (ROC) curve was applied to analyze the diagnostic efficacy of miR-9 and miR-195-3p for PHC. The changes in serum miR-9 and miR-195-3p levels in PHC patients before and after transcatheter arterial chemoembolization (TACE) were compared.Results:There were statistically significant differences in serum miR-9 (0.99±0.10, 1.31±0.28, 1.68±0.43) and miR-195-3p (0.97±0.10, 0.83±0.22, 0.63±0.18) levels among the healthy group, liver cirrhosis group, and PHC group ( F=69.78, P<0.001; F=74.82, P<0.001), with serum miR-9 levels increased successively and miR-195-3p levels decreased successively among the three groups (all P<0.05). There were statistically significant differences in serum miR-9 ( t=7.45, P<0.001; t=5.32, P<0.001; t=4.96, P<0.001) and miR-195-3p ( t=16.17, P<0.001; t=4.21, P<0.001; t=7.53, P<0.001) levels in PHC patients with different maximum diameters of tumor, clinical stages and degrees of differentiation. ROC curve analysis showed that the area under the curve (AUC) for the combined differential diagnosis of liver cirrhosis and PHC by serum miR-9 and miR-195-3p testing was 0.919, which was higher than the AUC for the differential diagnosis of serum miR-9 (AUC: 0.712, Z=4.38, P<0.001) and miR-195-3p (AUC: 0.844, Z=2.04, P=0.042) alone. After TACE treatment, serum miR-9 levels decreased (1.39±0.21 vs. 1.68±0.43, t=14.22, P<0.001) and miR-195-3p levels increased (0.78±0.22 vs. 0.63±0.18, t=14.84, P<0.001) in patients compared to pre-treatment levels. Conclusion:Serum miR-9 level is increased and miR-195-3p level is decreased in patients with PHC compared with patients with liver cirrhosis and healthy subjects, and the combination of the two has high differential diagnostic efficacy for liver cirrhosis and PHC. After TACE treatment, serum miR-9 level is decreased and miR-195-3p level is increased in PHC patients.
3.Low concentration contrast medium and low radiation dose for infants and young children abdominal CT scanning

Objective To assess the feasibility of low concentration contrast medium (270 mgI/ml) and low radiation dose (100 kV) for enhanced CT scanning in infants and young children abdominal CT examination.Methods Ninety children with abdomen tumors or abdominal injuries who underwent contrast-enhanced CT examination were selected.The patients were divided into 3 groups (each n= 30):Group A with tube voltage of 120 kV for non-contrast enhanced and parenchymal phase scanning and iodixanol contrast-medium (320 mgI/ml);group B with tube voltage of 100 kV for non-contrast enhanced and parenchyrnal phase scanning and iodixanol contrast-medium (270 mgI/rnl);group C with tube voltage of 100 kV for non-contrast enhanced and parenchymal phase scanning and iodixanol contrast-medium (270 mgI/ml).The 4-point scale was used to evaluate the quality of parenchymal phase imaging.The standard difference (SD) of CT value in subcutaneous fat,SNR and CNR of liver parenchyma,splenic parenchyma,renal cortical,renal vein,and abdominal aorta were measured at parenchymal phase,and CT dose index of volume (CTDI,ol),dose length product (DLP) and effective dose (ED) were recorded.The data were statistically analyzed among 3 groups.Results There was no significant difference of SNR,CNR nor objective scores of liver parenchyma,splenic parenchyma,renal cortical,renal vein and abdominal aorta among 3 groups (all P>0.05).The differences of CTDIvol,DLP and ED among 3 groups were statistically significant (all P<0.01).The CTDIvol had no statistical difference between group B and group C (P = 0.001,0.002),DLP (P = 0.013,0.004) and ED (P = 0.03,<0.001) of group A had statistical difference with those of group B and C.Conclusion CNR of the abdominal image can be guaranteed using low concentration contrast medium (270 mgI/ml) combined with 100 kV tube voltage for CT scanning of infants and young children,therefore satisfying clinical diagnostic requirements.
4.Analysis of the compliance and effectiveness of the allergen specific immunotherapy in patients with allergic rhinitis

Objective:To investigate the effectiveness and compliance of the 2 year specific immunotherapy in patients with allergic rhinitis. Method:Two hundred and sixty-two patients of allergic rhinitis were treated with SLIT for 2 years. The symptom questionnaire about effectiveness and compliance were obtained 2 years after SLIT. The patient satisfaction was also investigated. Result:Sixty patients had complete compliance and 202 patients had poor compliance. Of the 142 children, 40 (28%) cases had complete compliance, and of the 118 adult patients, 20 cases (17%) had complete compliance. There was obvious difference between the two (P<0.05). Comparing of before and after treatment, total medication score, total nasal symptom score and every symptom score decreased obviously (P<0.05). Two years after treatment, the total effective rate was 63%,there was significant difference between the children group and the adult group (P<0.05). Conclusion:There are a lot of factors affecting the treatment compliance of sublingual immunotherapy, such as patients, family, health care workers and social support, etc. In the course of treatment, a series of strategies can be adopted to improve the treatment compliance. SLIT is an effective method for the treatment of allergic rhinitis, and the treatment effect of children is better.
5.Prevalence and molecular characterization of Cryptosporidium in captive-bred Mustela putorius furo in Jiangsu Province

Objective To investigate the prevalence and molecular features of Cryptosporidium in captive-bred Mustela putorius furo in Jiangsu Province.. Methods A total of 290 fresh stool samples were collected from a ferret farm in Jiangsu Province on May 2017, and the small subunit rRNA (SSU rRNA) gene of Cryptosporidium was amplified in stool samples using nested PCR assay. The actin, cowp and gp60 genes were amplified in positive samples and sequenced to characterize Cryptosporidium species/genotypes. Results A total of 18 stool samples were tested positive for Cryptosporidium SSU rRNA gene, with a detection rate of 6.2%. Sequence and phylogenetic analyses of SSU rRNA, actin and cowp genes characterized Cryptosporidium isolated from captive-bred ferrets as Cryptosporidium sp. ferret genotype. In addition, gp60 gene was amplified in 10 out of 18 stool samples tested positive for Cryptosporidium. Conclusions Cryptosporidium is widely prevalent in captive-bred ferrets in Jiangsu Province, and Cryptosporidium sp. ferret genotype is the only Cryptosporidium genotype in ferrets.
