1.Investigation on the reference range for 25-hydroxyvitamin D in second trimester pregnant women in Shanghai

Objective·To investigate the reference range for serum 25-hydroxyvitamin D in pregnant women in Shanghai. Methods·A total of 4969 healthy pregnant women in second trimester were enrolled, in whom there were 554 women aged over or equal to 35 years old and 4415 women aged under 35 years old. Meanwhile, 1048 non-pregnant women in child-bearing period were enrolled. The new born children had no complications of vitamin D deficiency such as rickets. The levels of 25-hydroxyvitamin D of all participants were detected by ELISA. The reference interval was defined as the central 95% range between the 2.5th and the 97.5th percentiles. Results·The 95% reference range for 25-hydroxyvitamin D levels showed a positive skewness distribution in second trimester pregnant women. The median of 25-hydroxyvitamin D levels was 36.8 nmol/L. The maximum was 108.6 nmol/L whereas the minimum was 11.8 nmol/L. The 95% reference range for 25-hydroxyvitamin D levels was from 15.9 nmol/L to 70.3 nmol/L. The median 25-hydroxyvitamin D was 36.6 nmol/L in women less than 35 years old, and 35.5 nmol/L in women aged over or equal to 35 years old. A significant difference was found between women aged under 35 years old and women aged over or equal to 35 years old (Z=-2.75, P<0.05). The median, maximum and minimum of 25-hydroxyvitamin D in non-pregnant women was 38.5 nmol/L, 110.5 nmol/L, 12.1 nmol/L, respectively, which were all higher than those in pregnant women (P<0.05). Conclusion·There was a lack of vitamin D in pregnant women in Shanghai, especially in pregnant women aged over or equal to 35 years old, despite the multivitamin supplements. However, no complications of vitamin D deficiency such as rickets were found in new born children. To determine the reference range of vitamin D in Shanghai is helpful for pregnancy women in taking vitamin supplement, especially important in preventing maternal and fetal complications caused by vitamin D deficiency.
2.Mental disorder and its risk factors in first degree relatives of patients with type 2 diabetes mellitus

Objective To investigate the incidence and the risk factors of anxiety or depression and in the first degree relatives (FDRs) of patients with type 2 diabetes mellitus ( T2DM ). Methods A total of 102 T2DM patients who visited our hospital from May to July 2010 and their FDRs completed SAS,SDS,and life satisfaction evaluation. The medical history of T2DM patients was collected and compared with their FDRs with or without mental disorders. The correlations of risk factors with anxiety or depression were analyzed, t test or X2 test were used for statistical analysis and a P value less than 0. 05 was considered statistically significant. Results The incidence of anxiety or depression of FDRs was 31.4% and 17.6%,respectively. The life satisfaction score of FDRs with mental disorders was significantly decreased ( t = 4. 26 and 4. 09, P<0. 05 ). Fasting glucose level of T2DM patients with anxious or depressive FDRs was significantly increased ( t = -2. 48 and -2. 15, P<0. 05 ). Higher medical cost and multiple treatment strategies were positively correlated with SAS and SDS score of the FDRs. Conclusion Higher incidence of mental disorders and lower satisfaction scores could be found in the FDRs of T2DM patients. Better control of blood glucose and reduced treatment cost and strategies may be useful in improving metal condition and life satisfaction of the FDRs of T2DM patients.
3.Efficacy Observation of Polyene Phosphatidyl Choline Combined with Hugan Tablets in the Treatment of Drug-induced Liver Injury

Objective:To observe the efficacy of polyene phosphatidyl choline combined with Hugan tablets in the treatment of drug-induced liver injury. Methods:Totally 80 cases of drug-induced liver injury were randomly divided into the observation group (40 cases )and the control group(40 cases). The observation group was given polyene phosphatidyl choline injections 697. 5mg+5%glucose injections 250ml,ivd,qd,combined with Hugan tablets 1. 44g,po,tid,the control group was given polyene phosphatidyl choline injections 697. 5mg+5%glucose injections 250ml,ivd,qd,and the symptomatic treatment in the two groups was the same. The two groups were treated for 20d. Alanine aminotransferase ( ALT), aspartate aminotransferase ( AST), total bilirubin ( TBIL), glutamyl transpeptidase(GGT) and alkaline phosphatase(ALP)were detected respectively before and after the treatment, and the therapeutic effects were compared between the two groups. Results: After the treatment,the total effective rate in the observation group was 97. 5%,which was significantly higher than that in the control group(P <0. 01),Symptoms,signs and biochemical indices in the two groups were improved obviously. The decrease of ALT,AST and GGT in the observation group after the treatment was obviously lower than that in the control group(P<0. 05). Conclusion: Polyene phosphatidylcholine injections combined with Hugan tablets in the treatment of liver injury is better than polyene phosphatidyl choline, which is worthy of clinical application.
4.Effects of Shenmai injection on I?B and TLR4 mRNA expression in lungs of rats exposed to hemorrhagic and LPS shock

AIM: To explore the molecular mechanism of anti-shock effect of Shenmai (SM) injection and to observe its influence on TLR4 and I?B mRNA expression in hemorrhagic shock (HS) plus LPS rat lung. METHODS: Rats were divided into HS, HS+LPS, HS+LPS+ dexamethasone (DEX), HS+LPS+SM group, respectively. HS+LPS injury model of rats were induced by hemorrhagic shock (MAP 40 mmHg for 10 min) followed by intravenous administration of lipopolysaccharide (LPS, 1.5 mg/kg). 5.Experimental research on the electromagnetic radiation immunity of a kind of portable monitor.

The paper is focused on a kind of portable monitor that is widely used in military hospitals. In order to study the electromagnetic radiation immunity of the monitor, the experiments of electromagnetic radiation caused by radio frequency continuous wave in reverberation chamber and by ultra wide band (UWB) electromagnetic pulse have been done. The study results show that UWB electromagnetic pulse interferes observably the operating state of the monitor. It should be paid high attention to take protective measures. The monitor tested has some electromagnetic immunity ability for radio frequency continuous wave radiation. The frequent abnormal phenomena are baseline drift and waveform distortion. The electromagnetic sensitivity of the monitor is related to the frequency of interference source. The monitor tested is most sensitive to the frequency of 390 MHz.

6.Effect of weight loss on metabolic parameters in over-weight or obesity patients combined with impaired glucose tolerance

Objective To evaluate the effective of weight loss on blood glucose,lipid profile,and blood pressure in over-weight or obesity patients combined with impaired glucose tolerance(IGT).Methods Body weight of 91 over-weight or obesity IGT patients were managed by doctors and nurses.Body weight,waist circumference,blood glucose,lipid profile,blood pressure and liver function of the patients were compared at 3 months with baseline.Results After 3 months' intervention,the levels of body weight,waist circumference,fasting plasma glucose,postprandial glucose,systolic blood pressure,diastolic blood pressure,and glutamie oxalacetie transaminase (COT),instead of triglyeeride and hish-density lipoprotein cholesterol (HDL-C),were significantly reduced.Over weight or obesity IGT patients saw a 1.8 and 2.3 kg body weight loss,respectively.Decreased plasma glucose and blood pressure were found both in the over-weight group and in the obesity groups,but decreased waist circumference and GOT were found only in the over-weight patients.Conclusions Body-weight control may improve the blood glucose,blood pressure,central obesity,or fatty liver in over weight or obesity IGT patients.For those obesity patients,longer and stronger body weight control should be needed.
7.Dynamic analysis of heart rate variability based on wavelet transform.

The analysis of heart rate variability (HRV) has become a tool for noninvasively detecting the cardiovascular modulation of autonomic nervous system. Traditional analysis in frequency domain mainly includes calculating the power and detecting the peak frequency of each physiological frequency band. Whether employing the classical method or AR model to estimate the spectrum, the approximate stationarity of HRV is presupposed. Only in short term analysis can data meet this condition, while in long term the nonstationarity of HRV notably appears. A dynamic analysis method based on wavelet transform was proposed in this paper, which not only can obtain the traditional indices in frequency domain, but can compute their dynamic values varying with time, called short-time power and short-time LF/HF ratio. The latter can dynamically evaluate the activity of autonomic nervous. Finally the method was applied to trace the balance of autonomic nervous in Atropin drag experiment.

8.A new method based on down-sampling for QRS complexes detection.

This paper presented a new method based on down-sampling for QRS complexes detection for the reason that ECG's frequency components mostly centralize 0.05-35 Hz. Firstly, the original ECG signals were down-sampled. Secondly, self adapting and self study theory were used to select the amplitude threshold and slope threshold for R-wave detection. At the same time, the refractory idiosyncrasy of heart was taken into account to reduce the misdetection. Thirdly, back-trace technique was used to reduce undetected error. Finally, the corresponding R-wave position was up-dated in the original ECG signals. The performance of the method was tested by MIT/BIH ECG database and the clinical data were collected from the affiliated hospital of Chongqing Medical University. The detection rate of 98% was achieved. Experimental results indicated that the method was simple, effective, accurate and unaffected on sample rate.

9.Prenatal sonographic diagnosis of urorectal septum malformation sequence in one of twins: a case report

Urorectal septum malformation sequence (URSMS) is a rare congenital complex malformation characterized by severe abnormalities in the urinary, reproductive and digestive systems. It is difficult to diagnose URSMS by prenatal ultrasound due to its complex and variable manifestations. This paper reported a twin with partial URSMS. Prenatal ultrasound findings included pelvic "trilobe" cystic masses, sacrococcygeal hemivertebral malformations, imperforate anus, and transient ascites. Postnatal examination confirmed the diagnosis of URSMS, as the baby girl was born with anal atresia. Her colon, urethra, and vagina converged and formed a common tract with a single perineal opening. The baby died after her parents' refusal to surgical treatment.
10. Effect of iron dextran dispersible tablets on heart failure patients with iron deficiency

AIM: To evaluate the clinical effect of Iron Dextran Dispersible Tablets on patients with chronic heart failure who reduced ejection fraction after 24 weeks. METHODS: From January 2020 to June 2022, forty-five patients with heart failure complicated with iron deficiency and reduced ejection fraction were selected as the research objects. According to the random number table, they were randomly divided into control group and observation group.The control group was given routine anti-heart failure treatment such as Sacubitril Calsartan sodium tablets, while the observation group was given iron dextran dispersible tablets 50 mg three times a day on the basis of the anti-heart failure treatment of the control group for 8 weeks. The 6-minute walking distance, Hemoglobin, Serum Ferritin, N-terminal B-type natriuretic peptide precursor, Left Ventricular Ejection Fraction, Left Ventricular end Diastolic Diameter and 12-item Kansas City Cardiomyopathy Questionnaire (KCCQ-12) overall summary score and clinical summary score were compared between the two groups. RESULTS: There was no significant difference in baseline data between the two groups (P > 0.05). After treatment, the 6-minute walking distance in the observation group was longer than that in the control group, while the serum ferritin level in the observation group was higher than that in the control group. The N-terminal pro-B-type natriuretic peptide level in the two groups was lower than that before treatment, and the left ventricular end diastolic diameter was shorter than that before treatment, and the left ventricular ejection fraction, clinical comprehensive score and symptom score were higher than that before treatment. The difference was statistically significant (P < 0.05). There was no significant difference in the total incidence of adverse reactions between the two groups (P > 0.05). CONCLUSION: Iron Dextran Dispersible Tablets can improve the exercise endurance and quality of life of patients with chronic heart failure who reduced ejection fraction after 24 weeks.
