1.The use of early enteral nutrition with different routes after gastrointestinal operation

Objectives:To compare the feasibility, complications and advantages of the nose intestine tube and the jejunostic tube in early enteral nutrition after gastrointestinal operation. Methods:55 postoprative patients were divided into three groups. The nose intestine tube group(A group, n =20) and the jejunostic tube group(B group, n =19)received the enteral nutrition(Nutrison Fibre) enterally for 7 days and the conrtrol group(C group, n =16)received intravenous isotonic glucose solution and oral liquid diet after the bowel movement recovery,The blood glucose, the function of liver and kidney, electrolytes and nutritional status were observed. The recovery of bowel movement and other digestive symptoms such as abdominal pain, distention,diarrhea,nausea and vomiting were observed during the period of study. Some complications such as acute intestinal obstruction,pnumonia and rhinolaryngitis were observed during the period of study. 2.Intense pulse light in treatment of different parts of facial pore widening

ObjectiveTo evaluate the effects of intense pulse light (OPT) on the facial pore widening of different parts.MethodsThirty six women with widened pores were enrolled,and treated by OPT for 6 times.All the cases were taken photos before and after treatment.The pores of cheek,nasal ala and nasal tip were graded by using the photographic scale,and the improvement after treatment compared with that before.ResultsThe cheek,nasal ala and nasal tip pores evidently diminished (P<0.01).The improvement rate of cheek pores was higher than that of nasal ala and nasal tip (P<0.01),but that between the latter two was not different (P>0.05)).ConclusionsThe cheek pore widening diminishes more significantly than nasal ala and nasal tip treated with OPT.
3.The diagnostic value of dyslipidemia for avascular necrosis of femoral head after femoral neck fractures operation

Objective To explore the relationship between disorders of lipid metabolism and avascular necrosis of femoral head after operation of femoral neck fracture .Methods 160 patients with a diagnosis of fresh femoral neck fractures treated with AO cannulated compressed screws in orthopedic department of our hospital from January 2007 to January 2012 were involved .All the patients were divided into two groups :ANFH or non ANFH group according to the features of femoral head .The levels of blood lipid were examined .Then ,Logistic regression was used to screening risk factors and evaluate the influence of blood lipid factors ac-cording to βrisk factor .Results The levels of TC ,TG ,LDL and Apo B in ANFH group were (5 .99 ± 1 .05)mmol/L ,(2 .63 ± 0 .85)mmol/L ,(4 .16 ± 1 .21)mmol/L ,(0 .99 ± 0 .28)g/L respectively ,which increased remarkably compared with the control group ;while HDL ,Apo A1 levels in the ANFH group were (0 .88 ± 0 .19)mmol/L ,(1 .20 ± 0 .17)g/L ,which was remarkably lower compared with the control group ,and there were statistically significant difference (P=0 .000) .Serum TC and LDL levels were risk factors impacted on postoperative ANFH .Conclusion Postoperative ANFH was related to disorders of lipid metabolism .Serum TC and LDL levels could be diagnostic values on postoperative ANFH .Early control of blood lipid levels may prevent the development of ANFH in patients treated with AO cannulated compressed screws .
4.Efficacy of tacrolimus combined with methotrezate in the treatment of patients with refractory rheumatoid arthritis

Objective To assess the efficacy and safety of tacrolimus (TAC) combined with methotrexate (MTX) for the treatment of refractory rheumatoid arthritis (RA),and to compare it with cyclophosphamide (CTX) added to MTX for the treatment of refractory RA.Methods Thirty-six cases of refractory RA patients were divided into the observation group and the control group.TAC+MTX were used in the observation group,and CTX+MTX were used in the control group.We used repeated measures to analyze the variance and Fisher exact probability method to analyze the efficacy at 8 weeks and 24 weeks.Results The effective rate of the observation group in 8 weeks,24 weeks were 77.8%(14 cases) and 100%(18 cases) respectively,while those of the control group were 11.1% (2 cases) and 44.4%(8 cases),it showed that both TAC+MTX and CTX+MTX in the treatment of refractory RA were effective,but the efficacy of TAC+MTX was better than CTX+MTX,the difference of C reactive protein (CRP) and disease activity score (DAS)28 was statistically significant (P<0.05),and it could significantly improve the clinical symptoms and laboratory indexes.Conclusion TAC+MTX is effective and safe in treating refractory RA,and is worth of spreading.
5.The expression of proline-rich tyrosine kinase and its role in systemic lupus erythematosus

Objective To explore the possible role of proline-rich tyrosine kinase (Pyk2) in the pathogenesis of systemic lupus erythematosus (SLE).Methods The expression of Pyk2 in the peripheral blood mononuclear cells (PBMCs) of 50 patients with SLE and 36 healthy controls were tested with RT-PCR assay.The activation of Pyk2 was inhibited using specific inhibitor Pyk2 (TyrA9).Semi-quantitative PCR methodwas used to detect the Blys expression of PBMCs.One-way ANOVA and Pearson's correlation analysis were used for statistical analysis.Results The Pyk2 expression level (28.31 ±0.91) of SLE patients was significantly higher than that in healthy controls (33.69±0.04),the difference was statistically significant (P<0.05).The activation of Pyk2 was stimulated and the expression levels of Blys in the PBMCs of patients with SLE was elevated.By inhibiting the activation of Pyk2,the BLyS expression levels decreased significantly.Conclusion Pyk2 may be involved in the abnormal activation of lymphocytes which lead to the pathogenesis of SLE.Pyk2 expression is associated with SLE disease activity,disease aggravation,and the Pyk2 expression levels is also increased significantly.In addition,the expression level of Pyk2 is higher in patients with renal involvement than those patients with other organ involvement.
6.Expressions of Toll-like receptor 2 and 4 in peripheral blood monouclear cells of patients with leprosy

Objective To study the mRNA expressions of Toll-like receptor (TLR) 2 and 4 in peripheral blood mononuclear cells (PBMCs) of patients with leprosy. Methods SYBR Green fluorescence quantitative RT-PCR was conducted to detect the mRNA expressions of TLR2 and 4 in PBMCs from 30 patients with cured leprosy, 30 patients with active leprosy and 30 normal human controls. Results The expression of TLR2 mRNA in patients with active leprosy was significantly higher than that in those with cured leprosy and in controls (both P < 0.01 ), but there was no significant difference between the latter two groups (P > 0.05). The expression of TLR4 mRNA was of no significant difference between patients with active leprosy and those with cured leprosy orthe controls (both P> 0.05). Conclusion The expression of TLR2 mRNA is decreased in PBMCs from leprosy patients, suggesting that TLR2 may be a specific recognition receptor in patients with leprosy.
7.Comparison of improvement of peri-orbital rhytides and patient's satisfaction treated by intense pulse light, Elos and activeFXTM fractional CO2 laser

Objective To compare the improvement of peri-orbita rhytides and the patients' satisfaction treated by intense pulse light, Elos and activeFXTM fractional CO2 laser. Methods Two groups of the study were conducted. In first group, 42 volunteers in the hospital were enrolled. All were took photos to evaluate the peri-orbita rhytides by the physician before and one month after activeFX? treatment. Doctors and volunteers evaluated the improvement of peri-orbita rhytides and satisfaction together. In second group, a retrospective study was conducted, in which 40 patients were treated with intense pulse light, and other 40 patients treated with Elos in 2006-2007. Their improvements were analyzed through comparison of their photos before and one month after treatment, and the satisfaction was also evaluated. All data were analysed by SPSS10. 0. Results ActiveFXTM fractional CO2 laser is better than other two techniques for improving peri-orbita rhytides, and patients were more satisfied with this technology. Conclusion The improvement of peri-orbita rhytides and satisfaction treated by activeFXTM is higher than intense pulse light and Elos techniques.
8.Study on Foxp3 promoter region methylation of mononuclear cells in patients with rheumatoid arthritis

Objective By detecting the expression levels of Foxp3 in CD4+CD25+ regulatory T cells of peripheral blood mononuclear cells (PBMCs) in rheumatoid arthritis (RA),and the Foxp3 gene promoter region methylation to explore its role in the pathogenesis of RA.Methods Twenty-five RA patients and 10 healthy controls were selected,and the PBMCs were extracted by density gradient centrifugation.Foxp3 expression levels of CD4+CD25+ regulatory T cells were detected by flow cytometry.The real-time fluorescence quantitative PCR assay was used to detect the Foxp3 mRNA expression in PBMCs; and bisulfate processing gene sequencing was used to determinethe differences in Foxp3 gene promoter sequence methylation level of PBMCs.The comparison between groups was analyzed using one-way ANOVA; two sets of qualitative data were compared using Fisher's exact test.Results The expression levels of Foxp3 mRNA in the CD4+CD25+regulatory T cells of active RA patients (2.31±0.25) was significantly lower than inactive RA group (3.68±0.26) and healthy controls (5.67±0.34),the difference was statistically significant (P<0.05).The Foxp3 mRNA expression level in inactive RA group was lower than that of the healthy controls (P<0.05).Foxp3 promoter region-67,-74 sites of methylation level in PBMCs of RA patients (46%) was significantly higher than that of the healthy controls (6%).Conclusion Reduction in the number of CD4+CD25+ regulatory T cells may be involved in the pathogenesis of RA and Foxp3 gene promoter methylation levels plays a key role in this process.
9.Clinical Observation of Kang'ai Injection Adjuvant Chemotherapy for Advanced Lung Cancer

OBJECTIVE:To investigate clinical efficacy and safety of Kang'ai injection adjuvant chemotherapy for advanced lung cancer. METHODS:A total of 146 patients with advanced lung cancer during May 2013-Apr. 2014 were divided into observa-tion group and control group according to odd-even admission number,with 73 cases in each group. Control group was given GP (gemcitabine+cisplatin)chemotherapy. Observation group was additionally given Kang'ai injection 60 mL added into normal saline 250 mL,qd,ivgtt,for consecutive 14 d each chemotherapy cycle,on the basis of control group. Both groups were given tropise-tron 5 mg for preventing gastrointestinal reaction during chemotherapy. A treatment course lasted for 3 weeks,and both groups were treated for 3 courses. Clinical efficacies of 2 groups were compared;the levels of serum inflammatory factors (hs-CRP, TNF-α,IFN-γ,IL-10),immune function indexes(CD3+,CD4+,CD8+,CD4+/CD8+)and tumor markers(SCC-Ag,CEA,CA50, CA72-4) were compared before and after treatment. The occurrence of ADR were recorded in 2 groups. RESULTS:The response rate and disease control rate of observation group were 39.73% and 84.93%,which were significantly higher than 23.29% and 71.23% of control group,with statistical significance(P<0.05). Before treatment,there was no statistical significance in the lev-els of serum inflammatory factors,immune function indexes or tumor markers between 2 groups (P>0.05). After treatment,the levels of hs-CRP,TNF-αand IFN-γin 2 groups were significantly lower than before treatment,while the level of IL-10 was signifi-cantly higher than before treatment;above indexes of observation group were significantly better than control group,with statistical significance(P<0.05). The levels of CD4+ and CD4+/CD8+ in control group were decreased significantly and lower than observa-tion group,with statistical significance(P<0.05). Serum levels of SCC-Ag,CEA,CA50 and CA72-4 in 2 groups were decreased significantly,and observation group was significantly lower than control group,with statistical significance(P<0.05). The inci-dence of thrombocytopenia,neutropenia,abnormal liver function,nausea and vomiting,anemia in observation group were signifi-cantly lower than control group,with statistical significance(P<0.05). CONCLUSIONS:Kang'ai injection adjuvant chemothera-py can effectively relieve inflammation symptoms of patients with advanced lung cancer,reduce toxic reactions,enhance immune function, and improve short-term therapeutic efficacy with good safety.
10.Comparison of neuropsychological characteristics inpatients with amnestic and vascular mild cognitive impairment

Objective To identify the differences inneuropsychological characteristics between amnestic(AMCI)and vascular mild cognitive impairment(VMCI).Methods Totally 297 old community residents with mild cognitive impairment(MCI)were divided into amnestic MCI(AMCI)and vascular MCI(VMCI)subgroup from Guangzhou MCI prevalence survey.The elderly with MCI were interviewed and tested with the Chinese version of Montreal Cognitive Assessment(MoCA),the Mini-Mental state examination(MMSE),Auditory Verbal Learning Test(AVLT),the Clinical Dementia Rating scale(CDR),Functional Activity Questionnaire(FAQ),the Modified Hachinski Ischemic Scale(M-HIS),Center for Epidemiologic Studies(CES-DC)to evaluate neuropsychological characteristics.Results AMCI versus VMCI group showed that the total scores of MoCA were(9.63±5.17 vs.9.98±6.02),total scores of MMSE were(16.90±4.84 vs.16.90±6.19),AVLT immediate memory was(2.35±1.39 vs.2.91±1.84),AVLT delayed recall was(2.23±2.09 vs.2.47±2.20),AVLT delayed recognition was(7.33±3.98 vs.6.85±4.02)and total scores of CDR(0.5 vs.0.5),with no differences between the 2 groups(all P>0.05).Based on MoCA survey,AMCI versus VMCI group showed statistically significant differences(all P<0.05)in parameters of visual space and execution(0.71±1.02 vs.0.92±1.26),language function(0.34±0.56 vs.0.50±0.80)and abstract thinking(0.25±0.49 vs.0.15±0.43),but based on MMSE survey,no difference was found in the various cognitive domains between the two groups.The AMCI versus VMCI group showed statistically significant differences(all P<0.05)in parameters of CES-DC scale(1.75±4.27 vs.2.76±6.72),FAQ scale(4.42±4.66 vs.8.71±7.03),M-HIS scale(0.40±0.64 vs.7.59±3.53).Conclusions There is no significant difference in general cognitive impairment between AMCI and VMCI,but the visual space and execution,language function are more impaired in AMCI than VMCI,and the abstract thinking,social function are more impaired with more depressive symptoms in VMCI than in AMCI.
