1.Application of Penumbra retrograde semi-retrieval Solitaire stents technique in mechanical thrombectomy in patients with acute basilar artery occlusion

Objective:To explore the curative effect of Penumbra retrograde semi-retrieval Solitaire stents technique in the treatment of acute basilar artery occlusion.Methods:The clinical data of 15 patients with acute basilar artery occlusion in Shengjing Hospital of China Medical University from January 2017 to December 2019 were analyzed retrospectively. All patients were treated with Penumbra retrograde semi-retrieval Solitaire stents technique. The modified thrombolysis in cerebral infarction (mTICI) was used to evaluate the therapeutic effect.Results:Fifteen patients achieved vascular recanalization of occluded basilar artery, and mTICI 2b grade was in 3 cases, 3 grade in 12 cases. One operation completed vascular recanalization in 11 cases, 2 times in 3 cases,3 times in 1 case. CT images 24 to 48 h after operative showed no intracranial hemorrhage. The National Institutes of Health stroke scale (NIHSS) score at discharge was significantly lower than that before surgery: (4.8 ± 2.1) scores vs. (16.1 ± 5.7) scores, and there was statistical difference ( P<0.01). Conclusions:Penumbra retrograde semi-retrieval Solitaire stents technique can improve the efficiency of thrombus grasping and significantly improve patient symptoms.
2.Influence of Astragalus membranceus and mung bean on arsenolite-induced rats

AIM: To investigate the effects of oral administration of Astragalus membranceus, mung bean and arsenolite on the toxic of the arsenolite induced rats and the possible mechanisms with metallothionein (MT). METHODS: All the rats were oral administration with arsenolite. The Astragalus membranceus and mung bean were compared with the cadmium chloride which induced MT synthesis. The contents of MT were determined by cadmium saturation method, the liver mRNA levels for MT 1, MT 2 were detected by RT PCR. The protective effects of renal and liver were observed by testing alanine aminotransferase (ALT), blood urea nitrogen (BUN) and serum creatinine (SCR). RESULTS: The contents of MT were accorded with the mRNA expression of MT 1, MT 2. Arsenolite, Astragalus membranceus and mung bean could induce the synthesis of MT, but the contents of MT which arsenolite induced were trace. 3.Expression of COX-2 in acute spinal cord injury of rats

Objective:To investigate the changes in the expression of COX-2 in acute spinal cord injury of rats. Methods:Sixty adult SD rats were randomly divided into two groups: Trauma group (n=48) subjected to laminectomy and bounce of spinal cord; control group (n=12) only received laminectomy. The activity of COX-2 was detected using immunohistochemistry and Western blot analysis in cytoplasm of spinal cord. Results:The expression of COX-2 increased at 2-hour after injury and reached the top level at 24-hour. The expression of COX-2 dropped at 48-hour after injury and returned to basal level at 72-hour. Conclusion: The expression of COX-2 could be regarded as an index of inflammation on acute trauma spinal injury. Inhibition of COX-2 might be an new therapeutic method for spinal cord injury.
4.Protective Effect of PARP Inhibitor on Cortical Neurons in Streptozotocin-induced Diabetic Rats

Objective To investigate the effect of PARP inhibitor 3?aminobenzamide(3?AB)on cortical neurons in streptozotocin?induced diabe?tes mellitus(DM)rats and the mechanism. Methods A total of 60 rats were divided into 3 groups randomly:sham?operated control group,DM group and DM+3?AB group,20 rats in each group. Morris maze was used to detect learning and memory abilities in each group. Spectrophotometer assay was used to detect the levels of superoxide dismutase(SOD),glutathione peroxidase(GSH?Px)and malondialdehyde(MDA)in cortex. West?ern blot was used to determine the expression of poly(ADP?ribose)polymerase 1(PARP1)in cortex. Immunohistochemical staining was used to de?termine the expression of caspase?3?immunoreactive neurons in cortex. Results DM rats showed significantly declined learning and memory abili?ties. Compared with the control group,the levels of SOD and GSH?Px were significantly reduced(P<0.01)and the level of MDA was significantly up?regulated(P<0.01)in the DM group. Compared with the DM group,the levels of SOD and GSH?Px were significantly higher(P<0.05)and the level of MDA was significantly lower in the DM+3?AB group(P<0.05). The expression level of PARP1 was significantly up?regulated in the DM group and was significantly decreased in the DM+3?AB group(P<0.01). The level of caspase?3 was significantly higher in the DM group than in the control group(P<0.01),and was significantly decreased in the DM+3?AB group(P<0.01). Conclusion 3?AB protected cortical neurons from apoptosis in DM rats by inhibition of PARP1 and alleviation of oxidative stress.

Objective Optimizing the preparation technology of Red buttock ointment,by exploring the best extraction conditions of the alkannin,radix arnebiae seu lithospermi polysaccharides and so on.Methods Selecting radix arnebiae seu lithospermi 1kg and using orthogonal experiment to explore the best extraction conditions.Naphtho-quinone pigment chemicals were extracted by reflux extraction method;ethanol and paraffin wax were chosen as the composite solvent.Radix arnebiae seu lithospermi polysaccharide was extracted by water decocting,which to observe the rationality of process.Results The best extraction condition is A3 B2 C1 D1 , including section A:ethanol was 20 times the quality of medicinal materials (select radix arnebiae seu lithospermi 1kg and ethanol 20kg),section B:the amount of paraffin was 5% of ethanol(select radix arnebiae seu lithospermi 1kg and paraffin 1 kg),section C:Boiling water was 20 times the quality of medicinal materials,section D:the boiling time was 2h.Conclusion The more practical production process conditions:Ethanol was 10 times the quality of medicinal materials,and the amount of paraffin was 5%of ethanol.Boiling water was 10 times the quality of medicinal materials,and the boiling time was 1 h.This method is economic, simple, reasonable, stable, reproducible and easy to operation.Red buttock ointment obtained by this method possesses of excellent quality,good performance and superior effect.
6.A single surgeon's experience of pancreaticoduodenectomy on 169 patients

Objective To investigate the complications and the risk factors for pancreatic leakage after pancreaticoduodenectomy.Methods One hundred and sixty-nine patients who received pancreaticoduodenectomy in our hospital between January 2000 and December 2009 were reviewed.Chisquare and logistic statistic analysis were performed to determine the risk factors for pancreatic leakage.The difference in complication rates between different periods were analyzed.Results The mortality was 2.4%.The morbidity was 34.9%,and the pancreatic leakage rate was 7.7%.Logistic analysis revealed significant risk factors for pancreatic leakage included intraoperative bleeding of more than 400 ml(OR=2.87; 95% confidence interval:1.17-8.19; P=0.048),soft texture of remnant pancreas(OR =1.95 ; 95 % confidence interval:0.87-6.19 ; P =0.032)and pancreatic duct diameter smaller than 3 mm(OR=3.78 ; 95 % confidence interval:1.01-10.63 ; P =0.019).There was no significant difference in mortality,morbidity,pancreatic leakage,and upper gastric bleeding between the periods 2000-2004 and 2005-2009.However,re-operation rate and postoperative hospital stay were significantly higher in the period of 2005-2009.Conclusions Intraoperative bleeding,soft texture of remnant pancreas and pancreatic duct diameter smaller than 3mm were significant risk factors for postoperative pancreatic leakage.A pancreaticojejunostomy anastomotic technique familiar to the surgeon might reduce postoperative pancreatic leakage.
7.Study on the influencing factors of pain in newborn infants

Objective To investigate the influencing factors of pain and the changes of vital signs in newborn infants. Methods Forty two newborn infants were rated by the behavioral scale of acute pain in newborn infants. The scores of pain were compared among infants with different gender,gestational age,birth weight,birth age,type of puncture and whether by vaginal birth or not. At the same time,the respiration rate,heart rate,blood pressure and oxygen saturation (SO2)were dynami-cally recorded by the multi-function monitor in the process of puncture. Results The average score of pain was 7.6. There was no significant difference among newborn infants with different gender,gestational age,birth weight and type of puncture(P> 0.05),while significant differences among infants with different birth age and whether by vaginal birth or not (P=0.015 and 0.043 respectively). In the process of puncture,the SO2 was significantly decreased,while the respiration rate,heart rate,sys-tolic and diastolic blood pressure were significantly increased. Conclusions Pain is prevalent in newborn infants and accom-panied by obvious changes of vital signs. The means of childbirth and birth age have significant influence on the neonatal pain. It is suggested to pay close attention to the neonatal pain and take effective interventions.
8.Early postoperative enteral nutrition compared with parenteral nutrition after hepatectomy:A prospective randomized study

Objective To evaluate the impact of early enteral nutrition (EEN) compared with parenteral nutrition (PN) on patients after hepatectomy. Methods Seventy-eight patients undergoing liver resection were randomized prospectively into two groups: EEN group receiving early enteral nutrition (n=35) and PN group receiving parenteral nutrition (n=43). The patients in both groups received isocaloric and isonitrogenous nutritional formulas 24 h after operation and the formulas were stopped on postoperative day 7. The general conditions, liver function tests, clinical complications,and clinical nutritional variables at three time points that included preoperative phase, postoperative day 1 (POD 1) and postoperative day 8 (POD 8) were observed. Results No significant differences were found in length of hospital stay, liver function and clinical nutritional variables between the 2 groups. In the EEN group, the serum prealbumin level almost returned to the preoperative level on POD 8. The nutritional complication rate of the EEN group was increased significantly but it was milder than that of the PN group. The time of gut function recovery in the EEN group was shorter than that of the PN group. The costs of nutritional drugs showed a significant decrease in the EEN group.Conclusion Early enteral nutrition is safe, rational and effective in patients who have undergone hepatectomy. Early enteral nutrition is better than parenteral nutrition in promoting liver function recovery, liver protein synthesis, postoperative recovery of gut function and decreasing costs of nutritional drugs.
9.Research in influence of pain on neonatal early neurobehaviour

Objective To investigate the influences of pain on early neonatal neurobehavioral development Methods 65 newborn infants admitted to the Neonatal Intensive Care Unit( NICU )of our hospital from October,2009 to March,2010 were randomly chosen as the objects of this study.In light of Neonatal Behavioral Neurological Assessment( NBNA) revised by Professor Bao Xiulan,examinations were carried out before and after pain stimulation,and a statistical analysis of the results of the examinations was conducted.Results The total scores of NBNA before and after the pain stimulation were (36.49±1.73) vs.(34.80±1.79) respectively,demonstrating a significant difference.Specifically,after the pain stimulation,the scores of behavioral ability and active muscle tension decreased,with a very significant difference.However,there was no significant difference in terms of the scores of passive muscle tone,primitive reflexes and common reactions.Conclusions Neonatal pain exerts influences on early neurobehavioral development,particularly on behavioral ability and active muscle tension.The training of neonatal health care professionals in the management of and the intervention in neonatal pain should be strengthened in order to decrease the adverse effects of pain on neonates.
10.Analysis of complications after pancreaticoduodenectomy

Objective To investigate the complications and the risk factors of pancreatic fistula after pancreaticoduodenectomy.Methods The clinical data of 339 patients who underwent pancreaticoduodenectomy at the First Affiliated Hospital of Sun Yat-Sen University from January 2000 to Decembcr 2009 were retrospectively analyzed.The risk factors of pancreatic fistula were analyzed.The incidences of complications accured from 2000 to 2004 and from 2005 to 2009 were compared.All data were analyzed by the t test,chi-square test,Fisher exact probability or Logistic regression model.Results The incidence of complications of all patients was 33.0% ( 112/339),and the incidence of pancreatic fistula was 8.6% (29/339).Of the 29 patients complicated with pancreatic fistula,6 patients were in grade A,8 in grade B and 15 in grade C.Soft texture of remnant pancreas and the diameter of pancreatic duct smaller than 3 mm were the independent risk factors of pancreatic fistula( OR =1.75,3.75,P < 0.05 ).The number of hospital death was 12,including 1 patient died during the first period (2000-2004) and 11 patients died during the second period (2005-2009).Three patients died of pancreatic fistula and abdominal hemorrhage,3 died of postoperative upper gastrointesitnal bleeding,2 died of cardiac insufficiency,1 died of respiratory failure,1 died of pancreatic fistula,abdominal infection and necrotic pancreatitis,1 died of abdominal hemorrhage and hepatic and renal failure,1 died of bililary fistula,abdominal infection and multiple organ dysfunction syndrome.Conclusions Soft texture of remnant pancreas and the diameter of the pancreatic duct smaller than 3 mm are important risk factors of postoperative pancreatic fistula.Pancreatic fistula is the main factor causing death after pancreaticoduodenectomy.
