1.Exploration on thyroid hormones changes of different gestation periods

Objective To investigate the state of thyroid hormones and thyrotropin level in different gestation periods. Methods FT3, FT4, TSH and hCG were measured in 623 patients by ACS-180 automatic chemiluninescence analyzer. The urinary iodine was random measured by the use of iodine catalytic reaction mechanism of Arsenic-Cerium, rapid urinary iodine was detected with cold digestion method of quantitative detection. Result Serum FT3,FT4 ,and hCG levels in the first and second trimester were higher than in the third trimester ( F =53. 19,78.00,58.77, P <0. 05) ,and TSH levels were lower than in the third trimester ( F =5.68, P <0.05). The incidence of thyrotropin level disorder was 7. 54%, there were no significant differences in the gestation periods ( χ2 = 3.92, P > 0. 05 ). The incidence of subclinical hypothyroidism,subclinical hyperthyroidism, hypothyroidism and hyperthyroidism were 3.53%, 2. 57%, 1.12% and 0. 32%, respectively. The incidence of subclinical hypothyroidism in the first and third trimester was high, while the incidence of subclinical hyperthyroidism in the second trimester was high. Conclusions Thyrotropin level in the third trimester was lower than that of the first and second trimester. HCG changes were not the main reason that caused the thyrotropin level disorder during the pregnancy.
2.Detection and antimicrobial resistance of hypermutable Pseudomonas aeruginosa strains

Objective To investigate the isolation and antimicrobial resistance of hypermutable Pseudomonas aeruginosa strains.Methods The cultured bacteria were identified by API 20NE system.The susceptibilities of bacteria were detected by disk diffusion method and the results were confirmed with the rules of NCCLS/CLSI.The hypermutable strains were identified byhigh concentration of rifampin-resistant growth assay.Results Among analyzed 100 strains,37 strains(37.0%) were hypermutable.The resistant rates of hypermutable strains to imipenem,meropenem,cefoperazone/sulbactam,piperacillin/tazobactam,ceftazidime,cephfime,aztreonam,amikacin and ciprofloxacin were significantly higher than those of non-hypermutable strains.The multidrug resistance rates of hypermutable strains were also higher than those of non-hypermutable strains.Conclusion The hypermutable strains of Pseudomonas aeruginosa sourced from chronic respiratory tract infections dominantly.The hypermutable strains were hyper-resistant and multidrug resistant to many antibiotics,so strengthened monitoring should be encouraged to control the prevalence of hypermutable Pseudomonas aeruginosa strains.
3.Influence of nursing intervention on cognition level for pressure sores and psychological state among accompany family members of patients with cerebral injury

Objective To survey the influence of nursing intervention on cognition level for pressure sores and psychological state among accompany family members of patients with cerebral injury. Methods 123 accompany family members of patients with cerebral injury were selected and took part in the investigation about cognition level for pressure sores, besides, SDS and SAS were also adopted to evaluate their psychological state. Later nursing intervention was given to increase their cognition level for pressure sores, then another evaluation was carried out. Results before and after the intervention were compared. Results The accompany family members lacked knowledge of pressure sore, showed serious anxiety and depression for nursing pressure sore before intervention. But their cognition level for pressure sore improved and anxiety and depression lightened after intervention. Conclusions Nursing intervention is effective to alleviate cognition level for pressure sore and psychological state among accompany family members of patients with cerebral injury, which is worthy of clinical application.
4.Observing the Inlfuence of Dexmedetomidine on Femoral Artery Hemodynamics During the Induction of General Anesthesia Using Color Doppler Ultrasound

Purpose To investigate the influence of single slow intravenous infusion of dexmedetomidine (Dex) on femoral artery color Doppler hemodynamics during the induction of general anesthesia. Materials and Methods Forty patients of elective abdominal surgery under general anesthesia were elected and randomly divided into Dex group and the control group, with 20 cases in each group. Dex (0.2 μg/kg) 20 ml was injected with intravenous infusion in Dex group, and 20 ml saline was infused in control group, the infusion time was 10 min;then intravenous injection of Sufentanil, Propofol and Rocuronium were performed in turn for the induction of general anesthesia and endotracheal intubation. Femoral artery peak systolic velocity (Vs), early diastolic reverse peak velocity (Vd), systolic diameter (Ds), diastolic diameter (Dd), systolic blood pressure (SBP), diastolic blood pressure (DBP), mean arterial pressure (MAP) and heart rate (HR) changes at each time point were observed and recorded. Results ① Compared with baseline, Vs increased and Vd decreased (P<0.05) 10 min after the infusion of Dex in Dex group;after induction, Vs and Vd in both of the two groups decreased, and the difference between the two groups was not statistically significant (P>0.05);Vs reduced significantly in both of the two groups at the intubation moment and 1, 3, 5 min after intubation, but the Dex group changed more smoothly (P<0.05), and Vd raised in both groups with the Dex group changing more smoothly (P>0.05);Ds and Dd of both groups did not change significantly at each time point (P>0.05). ② Compared with baseline, MAP and HR decreased (P<0.05) after the infusion of Dex in Dex group;after induction, MAP and HR of both groups decreased, and the difference between the two groups was not statistically significant (P>0.05); MAP and HR increased in both groups at intubation moment, but the Dex group changed more smoothly than the control group (P<0.05).③SBP, DBP and HR were negatively correlated with Vs (r=-0.507,-0.619,-0.750, P<0.05) in both groups; SBP, DBP and HR were positively correlated with Vd (r=0.821, 0.881, 0.883, P<0.05) in both groups;there was no significant correlation (r=0.419, P>0.05) between Vs and Vd. Conclusion Single slow intravenous infusion of Dex (0.2μg/kg) can accelerate the femoral artery Vs and slow down the Vd, resulting in more stable hemodynamics during the induction of general anesthesia.
5.Pathologic quantitative analysis of atherosclerrosis in rabits to verify introvensional ultrasonographic results

To verify the role of introvascular ultrasound(IVUS) in diagnosis of atherosclerosis. MethodsINUS wasused to measure the position, quality and the thickness of the plaque in the elastic artery and biopsy materials. After using HE andHart stains, a micrometer was used to measuse the thickness of plaques and arterious walls. And electron microscopy was used to ob serve the pathological changs of the plaque. ResultsBy histological observation compared with IVUS, the conformable rate of plaque anmounted to 94%. The plaque could be seen in 12 sites of control group. Among 33 sites, 32 plaques were found pathologic exmination while IVUS showed 30 plaques in the experimental group. The two methods had a gcod correlation on the thick measurement of plaque and the media( r = 0.835, P<0.05 and r = 0.814, P< 0.05). But they had a poor correlation on the measurement of the intirna( r = 0.055). ConclusionsBy pathological examination and the quantitative analysis, IVUS can obtain the common purpose in diagnosing atherosclerosis and measuring the media thickness. But it can not be used in measurement of the intima.
6.Effect of Guanxinshutong on cerebral ischemia reperfusion in rats learning and memory ability and brain Bcl-2,Bax protein expression

Objective:Guanxinshutong on cerebral ischemia reperfusion ( I/R) in rats learning and memory ability and brain Bcl-2,Bax protein expression to explore the protection mechanisms of Guanxinshutong on cerebral I /R injury in brain tissue.Methods:Healthy Wistar rats for the study were randomly divided into sham operation group ,I/R model group,high dose Guanxinshutong (1 g/kg) and low dose Guanxinshutong (0.5 g/kg) group,n=15.cerebral I/R model was estabhished by using the left commom cartid artery ligation method,modeling each group were fed daily before saline ,saline,Guanxinshutong (1 g/kg) and Guanxinshutong (0.5 g/kg), continuous 7 days;3 d after water maze training while continuing administration ,continuous 7 d,the eight day ,Morris water maze test;after the test,the rats were sacrificed blood and brain tissue ,serum by ELISA of Bcl-2 and Bax protein levels change;using HE staining of rat brain pathology;using immunohistochemistry assay in rat brain organization of Bcl-2 and Bax protein levels changes.Results:Compared with the sham group ,Guanxinshutong high-dose group test latency slightly longer timers after platform and platform quadrant dwell decreased slightly ,serum and brain tissue slightly elevated levels of Bax protein ,Bcl-2 protein levels decreased slightly but not significantly different ( P>0.05 ) ,low-dose group Guanxinshutong through latency test in rats ,after the number of platforms and platform quadrant dwell time decreased , serum and brain tissue elevated levels of Bax protein , Bcl-2 protein content decreased significantly different(P<0.01),cerebral I/R model group was significantly prolonged latency test ,after the number of platforms and platform quadrant dwell time significantly reduced serum and brain tissue was significantly increased Bax protein ,Bcl-2 protein content decreased with significant differences ( P<0.01 ).Conclusion: Guanxinshutong can significantly improve spatial memory , and by reducing Bax protein content and increased Bcl-2 protein,inhibition of cerebral I/R injury of apoptosis,and thus the brain tissue I/R has a protective effect.
7.In vitro differentiation of mouse fetal liver mesenchymal stem cells into islet B-like cells

Objective:To isolate mesenchymal stem cells (MSCs) from mouse fetal liver and induce them differentiate into islet B-like cells. Methods:MSCs were isolated from C57BL/6J mouse fetal liver and were induced with high concentration of glucose, basic fibroblast growth factor(bFGF) ,and nicotamine medium. The gene expressions related to islet B cells such as pancreatic duodenal homeobox-1 (PDX-1), proinsulin-1 (INS-1) ,and glucose transporter-2 (GLUT-2) were detected by RT-PCR. Insulin in the treated cells was examined by immunocytochemistry. The insulin clusters were stained with dithizone (DTZ), a zinc-chelating agent known to selectively stain pancreatic B cells. Results: RT-PCR showed that the treated cells expressed PDX-1, INS-1 and GLUT-2, while the undifferentiated cells did not. After approximately 10 d of treatment, the fetal liver cells formed DTZ-stained cell clusters in flasks (80-120 clusters in a flask 25 cm2 in area). Immunocytochemistry also confirmed that these aggregates were strongly positive for insulin. Conclusion: MSCs derived from fetal liver can be induced into islet B-like cells in vitro.

9.Application of Spinal Finite Element Modeling in Human Body and Scoliosis Rectification

Objective To research a fast method on establishment of simplified model of the spine.Methods The 3D geometry of the spine is reconstructed using a multiview radiography reconstruction technique through the two plane X-rays by extracting the spinal anatomy information combined with the statistical geometrical information.Results The entire spine finite element modeling including the thoracic spine(T1-T12),lumbar spine(L1-L5) and the sacrum was reconstructed,and the analysis proved that lateral corrective force can correct scoliosis to a certain extent.Conclusion Because the method needs less time,and causes less radiation to patients,it can be brought much convenience and support to the clinical application.
10.Comparison of propofol-remifentail and propofol-fentanyl anesthesia for painless artificial abortion

Objective To observe and compare the efficacy and safety between propofol-remifentanil and propofol-fentanyl for anesthesia of artificial abortion.Methods 200 ASA I patients scheduled for painless artificial abortion were randomly divided into two groups.R group were given intravenous remifentanil 1.0μg/kg then propofol 1~1.5mg/kg(n=100);F group were given intravenous fentanyl 0.05mg then propofol 1~1.5mg/kg(n=100).Total propofol dosage,analgesic effect during intro-and post-operative,consciousness loss time,wake-up time and independenly leaving time were recorded.BP,HR,SpO2 and side effects such as nausea and vomiting were observed during intro-and post-operative.Results The dosage of propofol was less in group R than that in group F[(80.4±10.8)mg vs(110.1±13.6)mg,P<0.01)];consciousness loss time,wake-up time and independenly leave hospital time were shorter in group R than that in group F(P<0.05);SpO2 down range and rate were significandy higher in group R(P<0.05);Incidence of nausea and somnolence in group R was obviously lesS than that in group F.Conclusion Artifical abortion can be preformed effectively and safely with propofol-remifentanil or propofol-fentanyl,but propofol-remifentanil especially fits for painless artificial abortion compared with propofol-fantanyl.
