1.Analysis of blood pressure, heart rate, NIHSS score, GCS score of cerebral infarction patients with cerebral hemorrhage after recombinant human tissue-type plasminogen activator (rt-PA) therapy

Objective To analyze the blood pressure,heart rate,the US national institutes of health stroke scale(NIHSS) score,Glasgow coma scale(GCS) score for cerebral infarction patients with cerebral hemorrhage after recombinant human tissue-type plasminogen activator (rt-PA) therapy.Methods From April 2011 to April 2013,48 cases with cerebral infarction after intravenous rt-PA thrombolysis were collected.These cases were divided into two groups:the first group (32 cases):without cerebral hemorrhage after thrombolysis; the second group (16 cases):with cerebral hemorrhage after thrombolysis.The systolic blood pressure,diastolic blood pressure,heart rote,NIHSS score and GCS score were compared between the two groups before and after thrombolysis.Results After thrombolysis,the systolic and diastolic blood pressure were significantly increased in cerebral hemorrhage cases compared with before thrombolysis and patients without cerebral hemorrhage.After thrombolysis,the NIHSS score was significantly reduced in the cases without cerebral hemorrhage compared with that before thrombolysis,while the NIHSS score was significantly higher in the cases with cerebral hemorrhage compared with that before thrombolysis.After thrombolysis,the heart rate and GCS score showed no significant difference between the patients with and without cerebral hemorrhage.Conclusions The rising of systolic blood pressure,diastolic blood pressure and NIHSS score helps us determine the early cerebral hemorrhage after thrombolysis in cerebral infarction patients.
2.Reevaluation of the Systematic Review of the Traditional Chinese Medicine Combined with Transarterial Che-moembolization in the Treatment of Primary Liver Cancer

OBJECTIVE:To evaluate the methodological bias of systematic review and the reliability of counclusions of tradi-tional Chinese medicine (TCM) combined with transarterial chemoembolization in the treatment of primary liver cancer,and pro-vide evidence-based reference. METHODS:Retrieved from Cochrane Library,PubMed,Guideline,CBM,CJFD and Wanfang Da-tabase,high quality systematic review about TCM combined with transarterial chemoembolization in the treatment of primary liver cancer was collected for reevaluation. RESULTS:Totally 12 systematic review were involved. Results showed TCM combined with transarterial chemoembolization can improve tumor’s objective response,prolong the survival time of patients,improve life quali-ty,reduce postoperative adverse reactions with no serious adverse reactions reported related to TCM in the treatment of primary liv-er cancer. CONCLUSIONS:The clinical efficacy of TCM combined with transarterial chemoembolization is superior to transarterial chemoembolization alone in the treatment of primary liver cancer,with better safety.
3.Methylprednisolone's influence on the plasma plasminogen activator inhibitor-1 in lupus nephritis patients

Objective:To investigate the effect of methylprednisolone that decrease plasminogen activator inhibitor-1(PAI-1) in plasma through mediating the PA/plasmin system.Methods:Employed immunohistochemical technique-ELISA to determine PAI-1 level in plasma.Results:①PAI-1 in LN patients is significantly higher than that in the control group (P0.05).Conclusion:It is thus proved that methylprednisolone is capable of decreasing PAI-1 in plasma through mediating the PA/plasmin system and two shocks is the minimum for effective cure. [
4.Fetal external ear observation on standard sections and the best time of check-up

Objective To detect the fetal external ear observation on standard sections and the best time with ultrasonography. Methods Total 2212 normal fetuses 17-40 weeks of gestation were divided into six groups. Fetal ears were obtained on transverse-incline view at cervical vertebra level (section A) and mandible level (section B), on parasagittal view (section C) and coronal view (section D) at external ear level. Fetal ears were evaluated and analyzed statistically. Results Among 2212 normal fetuses, 3454 ears (78.07%), 1694 ears (38.29%), 3454 ears (78.07%) and 2304 ears (52.08%) were obtained on section A, B, C and D, respectively. The obtained-rate of fetal ears on section A was the same as section C at the same week of gestation, but was significant higher than that of section B and section D (P<0.05). The obtained-rate of fetal ears was statistical different between section B and section D at the groups of 25-40 weeks (P<0.01). The obtained-rate of fetal ears was negatively correlated with gestational age (P<0.01). Conclusion It is useful and easy to observe the fetal external ear with ultrasonography to detect fetal ear abnormalities. Section A and section C are recommended to be standard observation sections for fetal external ear. Section B and section D can be used as supplementary for section A and section C. The best time for fetal external ear observation with ultrasonography is 17-24+6 weeks gestation.

6.A primary study of transvaginal real-time three-dimensional hysterosalpingo-contrast sonography in the assessment of tube patency

Objective To evaluate the feasibility and accuracy of the transvaginal real-time three-dimensional hysterosalpingo-contrast sonography(RT 3D-HyCoSy) with SonoVue in the assessment of tubal patency.Methods Tubal patency was investigated by RT 3D-HyCoSy in 96 unselected infertile patients (a total of 191 tubes).The image quality of RT 3D-HyCoSy was evaluated.Laparoscopy was performed in 35 cases (a total of 70 tubes) of them.The efficacy of the procedure was compared with laparoscopy.Results The mean score of image quality of RT 3D-HyCoSy in ninety-six cases was 2.94 ± 0.26.The accordance between RT 3D-HyCoSy and laparoscopy was good,with the Kappa value was 0.802.Sensitivity,specificity,positive predictive value,negative predictive value and accuracy in diagnosing tubal patency by using RT 3D-HyCoSywas 87.5% (14/16),94.4% (51/54),82.3% (14/17),96.2% (51/53),92.9% (65/70),respectively.Conclusions Transvaginal RT 3D-HyCoSy is a good imaging technique which can display the morphological character of fallopian tube and assess the patency.
7.Fasting plasma obestatin levels in patients with different glucose tolerance and its association with lipid metabolism

Objective To investigate whether fasting obestatin level is different in patients with impaired glucose tolerance or type 2 diabetes, and to explore the association between obestatin and lipid metabolism. Methods Eighty-four subjects without known diabetes were divided into three groups: normal glucose tolerance(NGT), impaired glucose tolerance (IGT) and type 2 diabetes (DM) Plasma obestatin levels were measured with a radioimmunoassay. The relationship between fasting obestatin levels and metabolic parameters was also analyzed. Results Fasting obestatin levels were lower in DM group [(2.82±0.78)ng/ml] and IGT group [(3.25±0.29)ng/ml] than in NGT group[(3.55±0.57) ng/ml, P<0.01]. Triglycerides and low density lipoprotein cholesterol levels gradually increased among the three groups (P<0.05). Multiple linear regression analysis revealed fasting obestatin level was independently associated with waist-to-hip ratio, triglyeride and low density lipoprotein cholesterol. The regression equation was obestatin=6.953-3.412×W/H-0.175×TG-0.123×LDL-C. Conclusions The decreased obestatin may be associated with IGR and T2DM, and obestatin level may be associated with lipid metabolism.
8.Hypertriglyceridemia and insulin resistance in the obese elderly

Objective To study the changes of circulating triglyceride (TG) in the obese elderly, and to investigate the effect of hypertriglyceridemia on the development of insulin resistance. Methods A total of 82 subjects were divided into simple obesity group, obesity with IGT group, obesity with T2DM group and normal control group. The body height, body weight, blood pressure, fasting plasma glucose (FPG) and insulin(FPI), circulating TG and total cholesterol (TC) were measured. The homeostasis model assessment-insulin resistance (HOMA-IR) was calculated. Results The levels of serum TG were significantly higher in the simple obesity group ( 1.3 ± 0. 6) mmol/L, obesity with IGT group (1.9 ± 0.9)mmol/L and obesity with T2DM group (2.1±0.7) mmol/L than in the normal controls [(1.0±0.2)mmol/L, all P<0. 05], Furthermore, the levels of TG, FPG, FPI, HOMA-IR and systolic pressure were significantly higher(all P<0. 05) in obesity with IGT group and obesity with T2DM group than in simple obesity group. There were significantly positive correlations between TG and body mass index (BMI), FPG, HOMA-IR, TC, systolic pressure respectively (all P < 0.05). Multiple linear regression analysis indicated that FPG and HOMA-IR were the independent factors affecting TG (both P < 0. 05 ). Conclusions Hypertriglyceridemia may play an important role in the development of insulin resistance.
9.The correlation study of 1-hour postload plasma glucose level and carotid intima-media thickness in elderly male patients with hypertension

Objective To observe the correlation between 1-hour postload plasma glucose and carotid intima-media thickness (IMT) in elderly male patients with hypertension.Methods One hundred and twenty six elderly male patients with hypertension were enrolled and divided into four groups: normal glucose tolerance with normal 1 h plasma glucose (1 hPG) group (low-NGT, n=42); normal glucose tolerance with high lh plasma glucose group (high-NGT, n=23);impaired glucose tolerance group (IGT, n=27) and diabetic group (n=84). The differences of blood pressure, blood glucose, blood lipid and intimia-media thickness among groups were analyzed.Results OGTT 1 hPG in high-NGT group was similar with IGT group[(12.0±1.1)mmol/L vs. (11.1±1.8)mrnol/L, P>0. 053 ,but significantly higher than that in Iow-NGT group ((12.0±1.1)mmol/L vs. (9. 1±1.4) mmol/L, P<0.01]. The value of IMT was gradually increased in four groups (P<0.05) and it was (1.02±0. 12) mm in high-NGT group. OGTT 1 hPG was the independent risk factor for IMT(P< 0.01).Conclusions OC-TT 1h plasma glucose is associated with atherosclerosis in elderly male hypertensive people.
10.Effect of general intervention on the psychological state alleviation of healthy pregnant women

Objective To explore the effect of general intervention in improving the psychological condition of healthy pregnant women. Methods Totally 96 pregnant women who made prenatal visits in our obstetrics and outpatient department were divided into the control and the intervention group stochasti-cally. The intervention group started from first prenatal visit. The general intervention measures included cognitive intervention, the behavioral intervention, family coping intervention and psychological intervention with self-designed relaxation exercises. While conventional prenatal visit, propaganda and nursing were im-plemented to the control group.Self-made general questionnaire and symptom checklist 90 (SCL-90)were used to survey and analyze the results. Results Post-natal depression and anxiety of the control group was obviously higher than that of the prenatal. The Post-natal obsessive-compulsive symptom, interpersonal sensitivity, depression, hostility, terror and bigotry in the intervention group were greatly alleviated. Com-pared with the control group, obsessive-compulsive symptom, interpersonal sensitivity, depression, hostility and terror of the intervention group were significantly different (P<0.05). The depression improved while the difference was not evidently differenL Conclusions Delivery had great influence on the psychological status of women,especially anxiety and depression. General intervention measures like psychological relax-ation exercises can improve the psychological condition of healthy pregnant women obviously,especially de-pression.
