1.An estimate on three markers in the monitoring of blood metastasis of lung cancer

Objective To understand the clinical value on the detection of carcinoembryonic antigen mRNA (CEA mRNA), cytokeratin 19 mRNA (CK19 mRNA) and telomerase in blood for the monitoring of blood metastasis of lung cancer. Methods CEA mRNA and CK19 mRNA were detected by reverse transcriptase-nested primers-polymerase chain reaction, telomerase was detected by telomeric repeat amplification protocol-hybridism-enzyme-linked immunosorbent assay in peripheral blood. Results The positive rates of the three tumor markers in lung cancer group were much higher than the non-tumor group and the health group (P<0.001). The sensibility of CEA mRNA and telomerase were much higher than CK19 mRNA (P<0.01). The positive rates of the markers in TNM stages Ⅲ and Ⅳ were much higher than in stages Ⅰ (P<0.05 to P<0.01). Conclusion It had high value that detecting CEA mRNA, CK19 mRNA and telomerase in peripheral blood on the discovery of blood metastasis of lung cancer. Among them, the clinical value of CEA mRNA and telomerase are higher than CK19 mRNA, and combined assay of the markers can improve the sensibility.
2.Analysis on the situation of angiotensin-converting enzyme gene polymorphism in military service officers of Jinan theater

Objective To investigate and analysis the situation of angiotensin-converting enzyme (ACE) gene polymorphism in military service officers of Jinan theater .Methods Roche Cycler480II fluorescence quantitative PCR analyzer was used to detected ACE genotype .Meanwhile ,some samples were randomly analyzed by agarose gel electrophoresis .Results ACE DD genotype ac-counted for 23 .3% in the military service officers of Jinan theater cadres ,ID type accounted for 43 .2% ,II type accounted for 33 .5% ,D and I allele frequencies were 0 .44 and 0 .56 ,respectively ;type II ACE gene frequency was highest in female cadres ,while frequency of ID type was highest in male cadres ,the difference was statistically significant between two groups (P< 0 .05);the difference of allele frequency between men and women was not statistically significant (P>0 .05) .Conclusion ACE gene polymor-phism with type II and Iallele were dominant in the military service officers of Jinan theater cadres ;ACE gene polymorphism survey is important for early detection and timely prevention of certain related diseases .
3.Reducing the incidence of postoperative headache, nausea and vomiting in conventional thyroidectomy by using ultrasound-guided stellate ganglion block

Objective To explore the effect of ultrasound-guided stellate ganglion block (SGB) in the incidence of postoperative headache, nausea and vomiting in conventional thyroidectomy. Methods Ninety patients undergoing conventional thyroidectomy were randomly allocated to three groups with 30 patients in each group. In group L, SGB was performed with 5 ml 0.5% lidocaine; in group N, SGB was performed with 5 ml 0.9% sodium chloride; and in group C, no prior block was performed. Postoperatively, during the 48 h after surgery, every episode of postoperative headache and postoperative nausea and vomiting was recorded and a safety assessment was performed. In group L and group N, the hemodynamic status of the vertebral artery and carotid artery was recorded before and after the SGB was performed. In group C, the hemodynamic status of the vertebral artery and carotid artery was recorded before and after the neck was in the full extension position. Results One patient of group N and one patient of group C was discharged. During the 48 h after surgery, headache occured in 5 patients (16.7%) of group L, 8 patients (27.6%) of group N, and 17 patients (58.6%) of group C, and the headache rate in group C was significantly higher than that in group L (P=0.0007). The headache mostly occurred at 2 h and 4 h after operation. During the 48 h after surgery, nausea and vomiting occured in 8 patients (26.7%) of group L, 11 patients (37.9%) of group N, and 20 patients (60.9%) of group C, and the nausea and vomiting rate in group C was significantly higher than that in group L (P=0.0017) and group N (P=0.0343). The nausea and vomiting mostly occurred at 2 h and 4 h after operation.In group L and group N, and the inner diameters of the vertebral and carotid arteries after SGB were wider than those before SGB (P<0.05). In group C, the inner diameters of the vertebral and carotid arteries after the neck extension position were narrower than prostration position (P<0.05). No side effects were observed during or after SGB. Conclusions Preoperative SGB performed with 5 ml 0.5%lidocaine is an effective technique for reducing postoperative headache and nausea and vomiting after thyroidectomy.
4.The recent research advance in sphingosine 1-phosphate and its receptors affecting atherosclerosis

Sphingosine 1-phosphate (S1P) is one of the crucial signal molecules, which can regulate many biological functions inside and outside cells. It plays an important role in regulating numerous physiological and pathological processes after being combined with S1P receptors (S1PRs). S1P/S1PRs signaling pathways have become a hot spot in the current research on endothelial inflammation and atherosclerosis. This review described the current development of the role of S 1P and its receptors in atherosclerosis.
5.Thoracotomy and endovascular repair for traumatic aortic rupture

Objective To summarize experiences in treatment of traumatic aortic rupture. Methods Between July 2001 and December 2008, 17 patients with acute traumatic aortic rupture were treated in our department. One patient died of hemorrhagic shock one hour after admission before opera-tion. Nine patients underwent thoracotomy under general anesthesia with double lumen endotracheal tube and normothermic femoral-femoral partial cardiopulmonary bypass, with bypass time for 35-139 minutes and aortic clamping time for 25-87 minutes. Successful operation was performed in seven patients inclu-ding one treated with simple repair and the other six with partial replacement of thoracic aorta with artifi-cial vascular graft. The other seven patients underwent endovascular repair and received stent grafts at the site of thoracic injury via right lilac-femoral artery under general or local anesthesia. Results One pa-tient free from operation was died of hemorrhagic shock. Of nine patients treated with thoracotomy, two patients died of hemorrhagic shock during operation and the other seven survived, with operation time ran-ging from 100 to 180 minutes. Seven survivors were followed-up for 2-6 years, with no death during fol-low-up period. Seven patients in endovascular repair group recovered, wiht operation time ranging from 50 to 70 minutes. All these seven patients were followed up for 3-14 months, which showed no death. Reex-amined CT in six patients showed no mediastinal hematoma or leakage of contrast medium from the aorta isthmus at 2-5 months after operation. Conclusions Endovascular repair is simple, safe and effective for traumatic aortic rupture. The selection of thoracotomy and endovascular repair is based on following conditions: the combined injuries of patients, the equipments of hospital and the skills of operators.
6.The demand for discharge preparation services for patients readmitted to hospital with chronic heart failure-a qualitative research

Objective:The aim was to explore the needs of patients readmitted with chronic heart failure (CHF) for pre-discharge preparation services and to provide reliable evidence to support the development of a discharge preparation service plan.Methods:The research was based on the phenomenology research method. Using a purposive sampling method, 12 readmitted patients with CHF from the First Hospital of Heilongjiang University of Traditional Chinese Medicine were selected and interviewed in a semi-structured manner between February and April 2023, and the data were analyzed using the Colaizzi seven-step analysis.Results:Among 12 patients, 6 males, 6 females, aged 53 to 85 years old. The discharge preparation service needs of readmitted patients with CHF can be categorised into 3 major themes of physiological-psychological-social, containing 6 sub-themes: the need to improve self-management skills, the need for rehabilitation and exercise, the need for improvement of negative emotions, the need for improvement of health education, the need for improvement of home care, and the need for health resource services.Conclusions:Patients readmitted with CHF have greater needs for self-management knowledge and skills, family care needs, professional guidance needs, and information on referral services from primary health resources, and nursing staff should focus on assessing such needs of patients in order to develop individualized plans for discharge preparation services for patients with CHF.
7.Effects of bushen tongmai recipe on expression of phosphatidylinositol-3-kinase in PCOS rats accompanying with insulin resistance.

OBJECTIVETo observe the effects of bushen tongmai recipe (BSTMR) on mRNA and protein expressions of phosphatidylinositol-3-kinase (PI-3K) p85alpha in hepatic, adipose, muscular and ovarian tissues in PCOS rats with insulin resistance (IR).
METHODTwenty-three-day-old female SD rats were injected subcutaneously with sodium prasterone sulfate (90 mg x kg(-1) x d(-1)) for 20 days, and fed with high-fat forage for 80 days to induce PCOS rats with IR Then the rats were randomly divided into the model group and the treated group. Meanwhile, a group of fifteen rats of the same age was considered as the normal control group. The treated group were administered with BSTMR. The ovulation condition was examined by hematoxylin-eosin (HE) staining. Fasting blood glucose (FBG) was determined using glucose oxidase method. Serum fasting insulin (Fins) was determined by radioimmunoassay (RIA). The mRNA level of PI-3K p85alpha was measured by reverse transcription polymerase chain reaction(RT-PCR). Immunohistochemistry staining was wtilised to detect protein expression of PI-3K p85alpha in ovary.
RESULTCompared with the model group, the mean number of corpus luteum and the rate of ovulation in the treated group increased significantly (P <0. 01). The level of Fins in the treated group was much lower than that in the model group (P < 0.01). Both mRNA and protein expressions of PI-3K p85alpha in target tissues were up-regulated significantly in the treated group (P < 0.01 or P < 0.05).
CONCLUSIONBSTMR could improve IR and ovulation dysfunction in PCOS rats accompanying with IR and its molecular mechanisms might be closely related with the elevation of mRNA and protein levels of PI-3K p85alpha in target tissues of the model rats.

8.Analysis of serum neutralizing antibody response in patients with primary dengue virus type 1 infection.

OBJECTIVETo investigate the characteristics and dynamic changes of serum neutralizing antibody response in patients with primary infection of dengue virus type 1 (DENV-1).
METHODSSerum samples were obtained from the same patients with primary infection of DENV-1 within 2 weeks after symptom onset in 2006 and in 2010. A group-specific DENV NS1 capture ELISA-based micro-neutralizing test (ELISA-MNT) capable of detecting neutralizing antibodies against all the 4 serotypes of DENV was used to test the neutralizing antibody titers against DENV in the serum samples. The neutralizing antibody titers against a standard strain and 2 clinically isolated strains of DENV-1 were detected in serum samples collected in 2010.
RESULTSCross-reactive neutralizing antibody response against all the 4 serotypes of DENV was found in both of the serum samples collected in 2006 and 2010, but the samples collected in 2006 showed stronger cross-reactive neutralizing antibody responses. The neutralizing antibody against DENV-2, rather than the anticipated DENV-1 antibody, had the highest titer in the samples collected in 2006, whereas the antibody against homologous DENV-1 had the highest titer in the samples obtained in 2010. The neutralizing antibody titers against the homologous DENV-1 was significantly higher in samples collected in 2010 (U=86.500, P=0.000), which also demonstrated significantly different neutralizing antibody titers against the 3 different strains of DENV-1 (Χ(2)=12.123, P=0.002).
CONCLUSIONThe production of cross-reactive neutralizing antibodies between the 4 serotypes of DENV is a characteristic of DENV infection, particularly during early infection, but only the homologous neutralizing antibody increases obviously over time. The titers of the neutralizing antibodies against different strains, even of the same serotype, may differ distinctly.

9.Comparative analysis of nursing needs and actual service status of inpatients

Objective To probe into the gaps between the inpatient care needs at public tertiary hospitals and existing nursing services,for measures to improve the quality of nursing service.Methods The simple clinical score(SCS),Barthel index scale,patient care needs questionnaire,and actual service status questionnaire were used in a multi-centered stratified random sampling survey, covering the adult inpatient wards of 72 departments at six tertiary hospitals.The results were subject to a stratified analysis of patient care needs, and a comparative analysis between patient care needs and the actual status of the service.Results Statistically significant differences were found between the nursing needs of patients from among different departments,self-care abilities and those with different conditions(P<0.05); the patient care needs and actual service status in cleaning, eating, excretion, activities, health education, basic treatment and nursing of traditional Chinese medicine in terms of the differences were statistically significant (P<0.05); the comparative analysis found differences between patient care needs and actual service status.The top five patient care needs which were greater than the actual service status were namely temperature measuring,rehabilitation guidance,routine inspection,observation and psychological care.The top five areas with actual service status greater than patient care needs were namely turnover /back flapping, toilet assistance,temperature measuring,hands washing before and after meals,and herbal fumigation.The top five areas with minimal gap between needs and service were namely oral care, moxibustion, perineal care,assistance in using toilet in bed and manicure.Conclusions Departments, self-care abilities and conditions of patients vary with their needs of nursing.Based on the existing problems between patient care needs and the actual service status,nurses should increase their care in such soft indicators as rehabilitation guidance,routine inspection and disease observation, and fully meet the needs of patients.On the other hand,they are expected to enhance patients′health education and health literacy in such hard indicators as temperature measuring,turnover and back flapping.
10.Clinical study of emerging sternal rigid plate:a randomized controlled double-center trial

Objective To evaluate the security and effectiveness of emerging sternal rigid plate by comparing with wires closure. Methods 60 patients who undergoing median sternotomy from two centers were enrolled in this study, 30 received wire cerclage( control group) and the other 30 received rigid plate fixation( experimental group) . Patients' sternal were closed with wires or rigid plates, and the painness, sternal union and status of wires or rigid plates were followed up and assessed one week, three months and six months after procedures. Results All patients survived and were followed up. 117 rigid plates were implanted. There was no significant difference in pre-operative data, post-operative painness, sternal union and status of wires and plates between control and experimental group. No patient reached the standard of clinical sternal union one week post-operation, but all patients reached the standard of clinical sternal six months post-operation. One patient in control group suffered sternal nonunion, movement and wound infection after sternotomy. Conclusion Compared to wire closure, sternal closure with rigid plates is the same safe and effective.
