1.Mesenchymal stem cells modulate immune responses in a rat renal transplantation model

Objective To discuss the effect of mesenchymal stem cells (MSCs) in modulating immune responses in a rat renal transplantation model.Methods An in vivo trial of cytology was performed in one centre from March to December in 2008.Wistar rat donors and Lewis rat recipients in a renal transplantation model were randomly divided into 4 groups:MSCs (low dose,1 × 106 )therapy,MSCs (high dose,1 × 107) therapy,CsA monotherapy,and no therapy.Biochemistry methods were used to detect the levels of creatinine in serum.The survival time,renal grafting function and pathological changes of transplanted renal tissues were observed.Results Animal survival was significantly prolonged by MSCs (high dose) therapy and CsA monotherapy as compared with the no therapy group (both P<0.01).Animal survival in the MSCs (low dose) therapy group was prolonged as compared with no therapy group (P<0.01),but shortened as compared with MSCs (high dose) therapy group (P<0.05) and CsA monotherapy group (P<0.05).The MSCs (high dose) therapy and CsA therapy groups showed no special changes in histology,hut the control group showed acute rejection.Conclusion MSCs down-regulated immune responses,reduced production of some inflammatory mediators,preserved graft function in the initial stage after transplantation,and prolonged animal survival,and these effects were the same as those of CsA therapy with 1 × 107/day.
2.Comparison of lymph node dissection of video-assisted thoracoscopic surgery and thoracotomy in the treatment of clinical stage Ⅰ lung cancer: a meta-analysis and system review

Objective To compare the efficacy of lymph nodes(LNs) dissection between video-assisted thoracic surgery (VATS) and thoracotomy in the treatment of clinical stage Ⅰ lung cancer.Methods A comprehensive search of PubMed,Ovid Medline,EMBASE,Web of Science,ScienceDirect,the Cochrane Library,Scopus database and Google Scholar was performed to identify studies comparing VATS and thoracotomy in LNs dissection.The data was analyzed by RevMan 5.3 software and SPSS 18.0.Results After selection,23 articles met the inclusion criteria.2 316 patients were involved in VATS group and 3 346 patients were involved in Open group.Meta analysis showed that less total LNs(95% CI:-1.64--0.60,P < 0.0001),totalLNsstations(95%CI:-0.61--0.01,P=0.04) andN1 LNs(95%CI:-0.28--0.02,P=0.02)were dissected in VATS group.On the left side,more LNs were dissected in VATS group(95% CI:0.51-3.22,P =0.007).The same number of mediastinal LNs (95% CI:-0.74-0.15,P =0.20),mediastinal LNs stations (95% CI:-0.20-0.14,P =0.76),right side LNs (95 % CI:-1.52-2.23,P =0.71) were harvested in both groups.Conclusion In the surgical treatment of clinical stage Ⅰ lung cancer,less total LNs,total LNs stations and N1 LNs were dissected in VATS group,while more left side LNs were harvested by VATS.The same number of mediastinal LNs,mediastinal LNs stations right side LNs were harvested in the two groups.This conclusion still needs to be further proved by more high-quality and large-scale RCTs.
4. Relationship between living alone of retired employees and depression and anxiety and mediating role of social participation quality

Objective:To explore the related factors of depression and anxiety among the retired employees, and to examine the mediating role of social participation quality in the elderly between living alone and depression and anxiety.Methods:The retired employees who met the admission criteria in the First Hospital of Jilin University were selected as the subjects. The basic situation questionnaire was used in this study, and the social participation quality, depression and anxiety were assessed by social aspect of the Quality of Life Scale, Patient Health Questionnaire Depression Table and Anxiety Screening Questionnaire, respectively. A total of 642 valid questionnaires were obtained. Linear regression analysis was used to analyze the influencing factors, and the mediating effect test was conducted by stepwise regression analysis.Results:The influencing factors of depression in the retired employees were physical exercise (β=-0.948, P<0.01), entertainment (β=-1.423, P<0.01), whether to suffer from chronic diseases (β=2.579, P<0.01), family income (β=-0.557,P<0.05), whether to insist on work (β=-1.673, P<0.05), and whether to live alone (β=2.097, P<0.01).The influencing factors of anxiety were physical exercise (β=-0.711, P<0.05), entertainment (β=-1.380, P<0.01), whether to suffer from chronic diseases (β=1.204, P<0.05), and whether to live alone (β=1.639,P<0.05).In the elderly, living alone status and social participation quality were input in the linear regression as the independent variables,and the results showed that the effects of living alone status on depression and anxiety had no the significant differences (β=0.837,P=0.218; β=0.837,P=0.218), and social participation quality played a full mediating role between living alone status and depression and anxiety.Conclusion:Physical exercise, entertainment, whether to suffer from chronic diseases and whether to live alone are the influencing factors of depression and anxiety in the retired employees; social participation quality plays a full mediating effect between living alone status and depression and anxiety.

6.Clinical effects of second biopsy and resection in patients with high risk superficial transitional cell carcinoma of the bladder

Objective To explore the effects of second biopsy and resection on tumor recurrence and progression in patients with high risk non-muscle invasive bladder cancer. Methods The second biopsy and resections were performed 4-6 weeks after the first transurethral resection in 52 patients. Routine follow-up was done in another 71 patients. The tumor recurrence and progression rates were compared. Results Residual tumors were found in 54%(28/52) of patients underwent second biop-sy and resection, including muscle-invasive tumors in 5 patients. Two patients underwent radical cys-tectomy due to resection findings. During same period, 71 patients were routinely followed. After a median observation of 27 months, patients underwent second biopsy and resection showed lower recur-rence rate (P<0.05). The progression rate was no difference between the 2 groups(P0.05). Conclusion Second biopsy and resection may reduce recurrence rate in high risk non-muscle invasive bladder cancers, but may not change the tumor progression rate.
7.Application of soluble CD30 level measurement in kidney transplantation

BACKGROUND: Some studies in vitro have reported that there are CD30 positive T cells in immunological response of allogenic transplantation.OBJECTIVE: To detect the relationship between the level of serum CD30 (sCD30) and clinical rejection in the patients with or without kidney transplantation, and analyze the importance of sCD30 in the estimation of immune state, monitor of acute rejection, and judgment of prognosis. DESIGN, TIME AND SETTING: Clinical case analysis study was performed at Jiangsu People's Hospital between April 2004 and March 2007. PARTICIPANTS: 153 kidney transplantation cases comprising 103 males and 50 females, averagely aged 37 years. METHODS: 3 mL peripheral blood was obtained from recipients before transplantation (without immunosuppressive agent) and at 0, 1, 3, 5, 7, 14, 21, and 28 days. Serum was isolated from obtained blood and placed at -20 ℃. Soluble CD30 levels were detected using CD30 cytokine ELISA kit supplied by BenderMedSystems. MAIN OUTCOME MEASURE: The relation between the soluble CD30 levels and rejection prior to and following transplantation.RESULTS: There was a significant relation in the sCD30 level between the patients with (n=17) and without acute rejection (n=136). The CD30 levels were 113.2 U/mL in the rejection group and 83.2 U/mL in the non-injection group (P < 0.01). No significant difference was determined between both groups in 5 days following surgery (P > 0.05). Significant difference were detected between both groups from 5 days following surgery (P < 0.01). There was no relation between the soluble CD30 level and the time of rejection and release after kidney transplantation (P > 0.05). Receiver operating characteristic (ROC) curve demonstrated that soluble CD30 levels on day 5 post-transplantation could predict acute rejection (area under ROC curve: 0.850). Meanwhile, 100 U/mL was the optimal operational cut-off level to predict rejection (specificity: 85.0%; sensitivity: 83.6%). The patients with positive of soluble CD30 level showed a lower survival rate than those with negative CD30 level (P < 0.01). CONCLUSION: The soluble CD30 levels contributed to predictive the acute rejection and prognosis of kidney transplantation.
8.Expressions and clinical significances of TRAIL and its receptors in non-small cell lung cancer

Objective To investigation the expressions of tumor necrosis factor-related apoptosis-inducing ligand (TRAIL) and its receptors in non-small cell lung cancer (NSCLC) and their clinical significances.Methods The serum expression levels of TRAIL in 79 cases of NSCLC and 80 cases of normal subjects were detected by enzyme-linked immunosorbent assay (ELISA).The expressions of TRAIL-R2 and TRAIL-R4 in 42 cases of NSCLC and matched normal tissues were detected by immunohistochemistry.The relationships among TRAIL,TRAIL-R2,TRAIL-R4 and clinicopathologic features of NSCLC were analyzed.Results The expression of TRAIL in NSCLC patients was lower than that in normal human [(994.3 ±293.0)ng/ml vs.(1 141.7 ±266.1)ng/ml,t =3.29,P =0.00].The expression of TRAIL was closely correlated with clinical stage (F =2.28,P =0.00) and differentiated degree (t =5.76,P =0.00).The positive expression rate of TRAIL-R2 in NSCLC was 73.8% (31/42),significantly lower than that in the normal tissue 100.0% (42/42) (x2 =3.88,P =0.05).The expression of TRAIL-R2 was closely correlated with clinical stage (x2 =27.89,P=0.00) and differentiated degree (x:=9.50,P =0.00).The positive expression rate of TRAIL-R4 in NSCLC was 81.0% (34/42),significantly higher than that in the normal tissue 50.0% (21/42) (x2 =7.34,P =0.01).The expression of TRAIL-R4 was also closely correlated with clinical stage (x2 =17.82,P =0.00) and differentiated degree (x2 =4.47,P =0.03).There was a negative correlation between the expression of TRAIL-R2 and TRAIL-R4 in NSCLC (r =-0.67,P=0.01).Conclusion The decrease of TRAIL and TRAIL-R2 and increase of TRAIL-R4 expression may promote the occurrence and development of NSCLC,and they may provide targets for clinical treatment of NSCLC.
9.Significance of radiological protection of sensitive organs in non-treated region in body γ-knife treatment

Objective To explore the radiation dose from body γ-knife treatment to the nontargeted region's sensitive organs before and after shielding.Methods 20 patients suffering tumors less than 5 cm were selected.Calibrated thermoluminescence dosimeters (TLD) were placed above the sensitive organs,such as lens,thyroid gland and sexual gland,to measure the radiation dose received before shielding.Different plans were prepared for the patients with lung and renicapsule tumors using calibrators of different size.Radiation dose was measured by the aid of water phantom.For selected lungtumor treatment plan,the radiation doses were measured at the same location on the water phantom shielded with and without 1,2 and 4 cm lead,respectively.Results The maximum doses were 1 023.3 mGy for lens,1 235.7 mGy for thyroid gland and 1 176.8 mGy for sexual glands after treatment,respectively,being higher on the left site than the right.The radiation doses to the sensitive organs were higher for the water phatom with more tartgted points,decreasing by 55%-91% after being covered with 1,2 and 4 cm lead shieldings.There were significant differences in doses received before and after lead shielding (t =14.4,12.9,13.3,P < 0.05).Conclusions In the course of body γ-knife treatment,the additional factors would increase the dose to the sensitive organs.Therefore,it is necessary to provide lead shielding protection to the teenagers and adults with fertility when they undergo body γ-knife treatment.Trial registration Chinese Clinical Trial Registry,ChiCTR-OOC-16008259.
10.Protective effect of hydrogen sulfide on PC12 cells injured by ATP

[ ABSTRACT] AIM:To prove the purinergic signaling mechanism of the neuroprotective action of hydrogen sulfide by observing the effects of sodium hydrosulfide ( NaHS) , a donor of hydrogen sulfide, on the cell viability, intracellular Ca2+concentration ( [ Ca2+] i ) and the change of membrane permeability in the PC12 cells injured by adenosine triphos-phate ( ATP) .METHODS: PC12 cells in logarithmic growth phase were randomly divided into 4 groups.In control group, the cells were cultured without ATP treatment.In ATP group, the cells were treated with ATP after cultured for 24 h.In NaHS+ATP group, the cells were incubated with NaHS for 30 min before treated with ATP, and NaHS always exis-ted in the reaction system.In KN-62+ATP group, the cells were pretreated with KN-62 for 30 min, and the other treat-ments were as the same as those in NaHS+ATP group.The cell viability was assessed by MTT assay.The [ Ca2+] i was detected by Fura-2/AM staining.The membrane permeability was observed by staining with fluorescent dye YO-PRO-1. RESULTS:ATP at concentration of 0.3 mmol/L showed no injury effect on the cells.However, the cell viability was dropped gradually in a dose-dependent manner as the ATP at doses of 1, 3, 5 and 10 mmol/L.The decline of cell viability by ATP was obviously reversed by 200 μmol/L of NaHS in the PC12 cells (P<0.05), but exasperated by 800μmol/L of NaHS (P<0.05).At the same time, ATP evoked the increase in [Ca2+]i in a dose-dependent manner, which was inhib-ited by NaHS ( P<0.05) .Furthermore, the YO-PRO-1 uptake induced by ATP in a dose-dependent and time-dependent manner was also reduced by NaHS ( P<0.05) .CONCLUSION:Hydrogen sulfide has protective effect on the PC12 cells injured by ATP.The mechanism may be related to the reverse of the increased [ Ca2+] i and YO-PRO-1 uptake.
