1.Significance of oral pancreatin enzyme capsules on life quality of patients with total gastrectomy

Objective To explore the clinical significance of oral pancreatin capsules for patients after total gastrectomy.Methods Ninety cases of total gastrectomy patients in Nanyang City Center Hospital were selected and randomly divided into trypsin group and control group,with 45 patients in each group.The control group were treated by conventional therapy,and the patients of trypsin group were treated by pancreatin capsules on the basis of conventional therapy after 3 weeks postoperation.Scores of EORTC QLQ-C30,korenaga and fecal fat contents were compared between two groups six months postoperation.Results No statistically significant difference was found in the scoring system of EORTC QLQ-C30,korenaga and fecal fat contents in two groups before administered,The score of role function,emotional function and overall health status in the scoring system of EORTC QLQ-C30 of patients in trypsin group were higher than control group after 6 months administration.The differences were statistically significant (P<0.05 ).The score of fatigue,loss of appetite,nausea,vomiting,diarrhea in the scoring system of EORTC QLQ-C30 of patients in trypsin group were lower than control group after 6 months administration.The differences were statistically significant (P<0.05).The loss of appetite,swelling postprandial satiety,insomnia and the extent of weight loss in the scoring system of korenagain trypsin group were lower than control group after 6 months administration.The differences were statistically significant (P<0.05 ).The fecal fat contentsin trypsin group were lower than control group at 24 h and 72 h.The differences were statistically significant (P<0.05).The cases of 24h fecal fat content>7g in trypsin group was lower than control group at 24 h.The differences were statistically significant (P<0.05).Conclusion Oral pancreatin capsules could significantly improve the quality of life of patients after total gastrectomy, which is worthy of promoting.

3.Preparation and assessment of mechanical properties of complex material—hydroxyapatite/silicone rubber

Objective To prepare complex material—hydroxyapatite/silicone rubber (HA/SR) used in filling parenchyma and to measure and analyse its mechanical properties, looking for proper proportion of material. Methods The complex material was mixed based on different proportion (the HA/SR mass ratio of 23%, 33.3% and 41%), and its respective properties were tested according to national standard and performed statistical analysis. Results With the increasing proportion of hydroxyapatite in complex material, the rigidity of complex material enhanced gradually, but the mechanical property decreased. Conclusion If the HA/SR mass ratio was 23% to 33.3%, the mechanical properties of complex material are equal to that of pure SR. If the HA/SR mass ratio was over 41%, the mechanical properties of complex material decrease so that the complex material can not be used in filling.
4.Effects of embryonic neural stem cells on trauma of red nucleus neurons of the rats with spinal cord injury after transplantion

Objective To study the effects of embryonic neural stem cells transplantion on trauma of red nucleus neu-rons of the rats with spinal cord injury.Methods NSCs in logarithmic phage were labeled with BrdU,a Sprague Dawley rat mode of spinal cord injury (SCI) was developed with electrocircuit control spinal cord injuring device.Thirty SD rats were randomly divided into three groups: sham group,SCI group and NSC group.The NSCs were trans-planted into injured site three days after SCI.Then NSCs labeled with Brdu were detected by immunohistochemisty,rubrospinal tract (RST) neurons were labeled by retrograde transport of the horseradish peroxidase (HRP) from the lesion site,which were taken by damaged axons and remained in the neurons,then the labeled red nucleus (RN) neurons were counted.Hind limb function of experimental rats was evaluated by a blinder observer using BBB open field locomotion rating score.Results BrdU positive NSCs were detected in the spinal cord after transplantation,the number of RST neurons labeled by HRP in NSC group was more than that in SCI group (P <0.01),the BBB score of NSC group was higher than SCI group (P <0.01).Conclusion The transplanted NSCs can survive in the injured site of spinal cord and protect RN,then promote more remarkably functional recovery after SCI.
5.Clinical effect of surgical reconstruction of Ⅱ and Ⅲ degree cicatricial ectropion in 46 patients

Objective To improve the detail of the surgery for cicatricial ectropion,and the the effect was investigated. Methods Based on 46 patients withⅡ,Ⅲdegree cicatricial ectropion admitted to our department,choose the junction of normal skin and scar tissue as the incision line according to the degree of ectropion and local situation,and to reserve and recover the orbicularis oculi muscle as possible. The released areas exceeded separately the external canthus and reached the level of eye fissure,take advantage of full-thickness skin graft to correct the parallel lines and make fixation to the eyelid ectropion fusion skin graft on external canthal ligament. Results After followed up for 6 months to 2 years,46 cases of eyelid skin grafts survived completely and eyelids were in good reduction,with none case of incomplete eyelid closure. Conclusion Choose the surgical incision and the released areas in a proper way to reserve the structural integrity of orbicu-laris muscle and make skin graft fixation to inner and external canthal skin can prevent effectively the recurrence of ectropion and improve surgical progress.
6.Simultaneous Determination of Glycyrrhizic Acid and Glycyrrhetinic Acid in Yaotongning Capsule by HPLC

OBJECTIVE:To establish a method for simultaneously determination of glycyrrhizic acid and glycyrrhetinic acid in Yaotongning capsule. METHODS:HPLC was performed on the column of Agilent TC-C18 with mobile phase of methanol-0.2 mol/L ammonium acetate(gradient elution) at a flow rate of 1.0 ml/min;detection wavelength was 250 nm and column temperature was 25 ℃ and injection volume was 10 μl. RESULTS:The linear range was 0.007 1-0.178 0 mg/ml(r=0.999 8)for glycyrrhizin acid and 0.354 8-8.720 0 μg/ml of glycyrrhetinic acid(r=0.999 8);RSDs of precision,stability and reproducibility tests were no more than 1.74%,average recoveries were 95.49%-100.62%(RSD=1.98%,n=9)and 96.80%-102.26%(RSD=1.83%,n=9),respec-tively. CONCLUSIONS:The method is simple,reproducible,accurate and reliable,and can be used for the simultaneous determi-nation of glycyrrhizic acid and glycyrrhetinic acid in Yaotongning capsule.
7.Clinical significance of microrna-625 expression in esophageal squamous cell carcino-ma

Objective: To analyze the correlation of miR-625 expression with clinicopathological characteristics in esophageal squa-mous cell carcinoma (ESCC) and to explore the effect of miR-625 on the migration and proliferation of ESCC cells. Methods:The expres-sion level of miR-625 was determined through real-time PCR in 86 paired human ESCC tissue specimens and tumor-adjacent normal esophageal tissue specimens, ESCC cell lines, and esophageal epithelial cell line. The associations of miR-625 expression with clinico-pathological characteristics and survival in ESCC patients were analyzed. Transwell and CCK-8 assays were performed to examine the effect of miR-625 expression on migration and proliferation of ESCC cells. Results:Compared with tumor-adjacent normal specimens, miR-625 was significantly downregulated in ESCC tissue specimens (P<0.05). MiR-625 expression was decreased in ESCC cell lines com-pared with human esophageal epithelial cell lines (P<0.05). Lower miR-625 expression was associated with poorer prognosis and sur-vival. The migration and proliferation abilities of ESCC cells were inhibited by miR-625 overexpression (P<0.05). Conclusion:MiR-625 acts as a tumor suppressor gene in the development and progression of ESCC, suggesting that miR-625 may serve as an efficient prog-nosis biomarker and a potential therapeutic target for ESCC.
8.Satoh typing for tumor thrombi in hepatocellular carcinoma with tumor thrombi in the bile duct

Objective To evaluate Satoh typing for tumor thrombi in the bile duct in patients of hepatocellular carcinoma.Methods A retrospective analysis was made on clinical data of 51 hepatocellular carcinoma patients with tumor thrombi in the bile duct undergoing hepatectomy in Eastern Hepatobiliary Surgery Hospital from Jan 2004 to Dec 2005.The cumulative disease-free survival by the three Satoh types were compared by Kaplain-Meire analysis.and log-rank test.Results There was no operationrelated motality and the postoperative complication rate was 22%(11/5 1).The overall survival rates at 1-and 2-years were respectively 76%(39/51)and 55%(28/51).The overall disease-free survival rates at 1-and 2-years were respectively 53%(27/51)and 22%(11/51).The disease-free survival rates at 1-and 2-years in type Ⅰ patients were respectively 61%(19/31)and 29%(19/31).The disease-free survival rates at 1-and 2-years in type Ⅱ and Ⅲ patients were respectively 35%(7/20)and 10%(2/20).The cumulative disease-free survival rates at 2-years was statistically different between type Ⅰ,type Ⅱ and Ⅲ (v=7.489,P=0.006)).Conclusions Satoh typing of tumor thrombi helps to determine the surgery plan and assess the prognosis of hepatocellular carcinoma with tumor thrombi in the bile duct.
9. Novel nano-hydroxyapatite/polyurethane composite scaffold in the treatment of chronic osteomyelitis

Objective: To evaluate the bone repair efficacy of the new nano-hydroxyapatite (n-HA)/polyurethane (PU) composite scaffold in the treatment of chronic osteomyelitis in tibia. Methods: A novel levofloxacin@mesoporous silica microspheres (Lev@MSNs)/n-HA/PU was successfully synthesized. Its surface structure was observed by scanning electron microscopy (SEM). Fifty adult female New Zealand rabbits were randomly selected, and osteomyelitis was induced in the right tibia of the rabbit by injecting bacterial suspension ( Staphylococcus aureus; 3×10 7 CFU/mL), which of the method was described by Norden. A total of 45 animals with the evidence of osteomyelitis were randomly divided into 4 groups, and the right medullary cavity of each animal was exposed. Animals in the blank control group (group A, n=9) were treated with exhaustive debridement only. The remaining animals were first treated by exhaustive debridement, and received implantations of 5 mg Lev@PMMA (group B, n=12), 1 mg Lev@MSNs/n-HA/PU (group C, n=12), and 5 mg Lev@MSNs/n-HA/PU (group D, n=12), respectively. At 12 weeks postoperatively, the right tibia of rabbits were observed by X-ray film, and then gross observation, methylene blue/acid fuchsin staining, and SEM observation of implant-bone interface, as well as biomechanical test (measuring the maximal compression force) were performed. Results: X-ray films showed that the infection were severer than those of preoperation in group A, while the control of inflammation and bone healing of rabbits in group D were obviously better than those at preoperation. The gross observation showed extensive bone destruction in group A, a significant gap between bone tissue and the material in groups B and C, and close combination between bone tissue and the material in group D. The histology of the resected specimens showed that there was no obvious new bone formation around the materials in groups B and C, and there was abundant new bone formation around the periphery and along the voids of the materials and active bone remodeling in group D. The SEM observation of the bone-implant interface demonstrated that no new bone formation was observed at the bone-implant interface in groups B and C. However, bony connections and blurred boundaries were observed between the material and host bone tissue in group D. The biomechanical test showed the maximal compression force of groups B and D were significantly higher than that of groups A and C ( P<0.05), but there was no significant difference between groups B and D ( P>0.05). Conclusion: The novel synthetic composite Lev@MSNs/n-HA/PU exhibit good antibacterial activities, osteoconductivity, and biomechanical properties, and show great potential in the treatment of chronic osteomyelitis of rabbits.
10.Effect of alanyl-glutamine on immune function of patients with gastric cancer

ObjectiveTo investigate the effect of alanyl-glutamine supplemented total parenteral nutrition (TPN) on immune function of patients with gastric cancer.MethodsFifty gastric cancer patients with radical gastrectomy were randomly divided into the control group (n=25) and experiment group (n=25). Patients in the control group received conventional TPN support, and patients in the experiment group received TPN support and alanyl-glutamine (0.34 g/kg). All patients were observed for 7 days. The levels of IgG, IgA, IgM, CD3, CD4, CD8 and CD4/CD8 were measured before operation and on first and eighth day after operation.ResultsAll the levels of immune function were decreased on first day after operation in two groups. But the levels of IgG, IgA, IgM, CD3, CD4 and CD4/CD8 were significantly different between two groups on eighth day after operation (P<0.05). No serious infectious complication occurred in both groups.ConclusionTPN supplemented with alanyl-glutamine can improve the immune function of patients with gastric cancer radical gastrectomy.
