1.Decompression Only by Unilateral Biportal Endoscopic Surgery for Adjacent Segment Degeneration: A Multi-Institution Retrospective Study

Objective:
Endoscopic surgery is gaining popularity as a minimally invasive option for adjacent segment degeneration (ASD). However, most of the previous literature has focused on transforaminal endoscopic techniques. This study aimed to assess the outcomes of decompressive laminectomy (unilateral laminectomy and bilateral decompression) with unilateral biportal endoscopic (UBE) in patients with lumbar ASD presenting with spinal stenosis.
Methods:
Thirty-nine ASD patients presenting with spinal stenosis who underwent UBE decompressive surgery between June 2018 and November 2022 at 4 different institutions were enrolled. The postoperative decompression amount and spinal instability were assessed using magnetic resonance images and dynamic radiographs. All patients were followed for at least 1 year, and clinical outcomes were assessed.
Results:
In the 39 patients, the cross-sectional area of the dural sac (0.55±0.21 cm2 to 1.11±0.41 cm2, p<0.001) indicated significant spinal decompression. Dynamic radiographs did not demonstrate significant occurrence of instability in operated segment. Three patients experienced complications, with one case each of hematoma, dural tear, and neurologic deficit. Visual analogue scale scores of the back (6.4±2.4 to 2.1±1.2, p<0.001) and leg (7.5±1.5 to 1.6±1.6, p<0.001), and Oswestry Disability Index scores (47.5±16.3 to 19.8±9.7, p<0.001) indicated significant clinical improvement that was sustained over 1 year. During the average follow-up period of 2 years, 2 patients underwent revision surgery due to failure and relapse.
Conclusion
UBE decompressive surgery had satisfactory outcomes in ASD patients, without significant failure or relapse.

A total of 968 male firefighters were analyzed. Smoking history and cumulative pack-years were obtained using self-reported questionnaires. Alcohol use disorder was defined using the Alcohol Use Disorder Identification Test.PAH exposure was assessed by urinary metabolites. Regression analyses were performed between exposure (smoking), outcome (alcohol use disorder), and mediator (PAH metabolites) variables. A mediation analysis was performed to test the indirect effect of PAH metabolites on the association between smoking and alcohol use disorder. All analyses were repeated for 770 participants who were followed up after 2 years, while alcohol use disorder was redefined from follow-up data ensuring the temporal sequence of the variables.
Results:
Both 2-naphthol [β=0.78, 95% confidence interval (CI): 0.59–0.98] and 2-hydroxyfluorene (β=0.69, 95% CI: 0.56–0.82) were associated with smoking history. Furthermore, 2-naphthol and 2-hydroxyfluorene mediated the associations of smoking history (proportion mediated: 14.2%, 23.6% respectively) or cumulative pack-years (proportion mediated: 14.4%, 25.4% respectively) with alcohol use disorder. The results were consistent in longitudinal settings.
Conclusion
Exposure to PAHs mediated the association between tobacco smoking and alcohol use disorder. PAH exposure from tobacco may increase the risk of addictive disorders.

7.Posterior Mediastinal Paraganglioma With SDHB Gene Mutation:Case Report

Paragangliomas, a term used for tumors of extra-adrenal origin, are chromaffin cell tumors that secrete catecholamines. Approximately one in three patients with paraganglioma has a gene mutation associated with familial paraganglioma syndromes (FPGLs), resulting from mutations in one of the subunits of the succinate dehydrogenase (SDH) gene. Most extraadrenal paragangliomas involve the head and neck, and only 2% of paragangliomas are found in the mediastinum. We report the case of a 37-year-old woman with a posterior mediastinal paraganglioma attached to the heart and recurrent glomus jugulare who underwent glomus tumor resection at the age of 17 years. Genetic testing revealed a mutation in the SDH subunit B gene associated with FPGL type 4 (FPGL4). This case report describes the features of multimodal imaging of a posterior mediastinal paraganglioma and explain how a multidisciplinary approach led to the diagnosis of FPGL4.
8.Interlaminar Approach in the Lumbar Spine

Herein, we describe the surgical techniques of endoscopic interlaminar approach in the lumbar spine through a video presentation of cases involving 3 patients. In every procedure using the interlaminar approach, the lower margin of the lamina should be first identified and used as a surgical landmark. Laminectomy with drilling begins from the lower margin of the lamina and continues upward along the ligamentum flavum. The laminectomy ends when the drill meets the midline cleft of the ligamentum flavum. Once the midline cleft has been detected, an additional bony work-up is performed to detach the lateral and lower margins of the ligamentum flavum. Then, the ligamentum flavum is removed and the thecal sac is exposed. Full exposure of the lateral margin of the thecal sac and ipsilateral traversing root is required. For cases that require contralateral decompression, an additional contralateral bony work-up and decompression of the contralateral traversing root are performed. Decompression of the contralateral exiting root can also be performed with the advantage of a wide endoscopic view angle. Retraction of the thecal sac and traversing root exposes the surface of the disc, and discectomy can be performed. Before the termination of surgery, surgeons should always confirm sufficient decompression of the nerve roots and hemostasis.
9.Mesenchymal Stem-Like Cells Derived from the Ventricle More Effectively Enhance Invasiveness of Glioblastoma Than Those Derived from the Tumor

Purpose:
Glioblastoma (GBM) is one of the most lethal human tumors with a highly infiltrative phenotype. Our previous studies showed that GBM originates in the subventricular zone, and that tumor-derived mesenchymal stem-like cells (tMSLCs) promote the invasiveness of GBM tumorspheres (TSs). Here, we extend these studies in terms of ventricles using several types of GBM patient-derived cells.
Materials and Methods:
The invasiveness of GBM TSs and ventricle spheres (VSs) were quantified via collagen-based 3D invasion assays. Gene expression profiles were obtained from microarray data. A mouse orthotopic xenograft model was used for in vivo experiments.
Results:
After molecular and functional characterization of ventricle-derived mesenchymal stem-like cells (vMSLCs), we investigated the effects of these cells on the invasiveness of GBM TSs. We found that vMSLC-conditioned media (CM) significantly accelerated the invasiveness of GBM TSs and VSs, compared to the control and even tMSLC-CM. Transcriptome analyses revealed that vMSLC secreted significantly higher levels of several invasiveness-associated cytokines. Moreover, differentially expressed genes between vMSLCs and tMSLCs were enriched for migration, adhesion, and chemotaxis-related gene sets, providing a mechanistic basis for vMSLC-induced invasion of GBM TSs. In vivo experiments using a mouse orthotopic xenograft model confirmed vMSLCinduced increases in the invasiveness of GBM TSs.
Conclusion
Although vMSLCs are non-tumorigenic, this study adds to our understanding of how GBM cells acquire infiltrative features by vMSLCs, which are present in the region where GBM genesis originates.
10.Y-stent-assisted coiling with Neuroform Atlas stents for wide-necked intracranial bifurcation aneurysms: A preliminary report

Objective:
To report our experience on Neuroform Atlas Y-stenting for coiling of unruptured wide-neck bifurcation aneurysms.
Methods:
From March 2018 to January 2021, we treated 473 aneurysms in 436 patients with coil embolization, of which 15 cases with wide-necked bifurcation aneurysms were treated by Y-stent-assisted coiling with two Neuroform Atlas stents. We retrospectively reviewed the characteristics of patients and aneurysms, procedure-related complications, radiographic results, and clinical outcomes.
Results:
All 15 cases using Neuroform Atlas Y-stenting were successful. Patients included 6 men and 9 women with a mean age ± standard deviation of 56.4±6.6 years. The mean dome and neck sizes were 6.4±3.1 mm and 4.7±1.8 mm, respectively. Immediate post-procedural angiograms showed complete occlusion in 46.7%, neck remnant in 13.3%, and incomplete occlusion in 40% of cases. No treatment-related morbidity or mortality occurred in any patients. All patients had good clinical outcomes (Glasgow Outcome Score 5) at both discharge and during a mean 12.3-month (range 1-28 months) follow-up. All aneurysms showed improved or stable occlusion on follow-up imaging. Further, the latest follow-up angiography showed complete occlusion in 73.3%, neck remnant in 6.7%, and incomplete occlusion in 20%.
Conclusions
Y-stent-assisted coiling with Neuroform Atlas stents might be a feasible and safe option for wide-necked bifurcation aneurysms.
