1.HPLC Fingerprints of Freeze-dried Xueshuantong for Injection

OBJECTIVE: To establish the HPLC fingerprints of the freeze-dried Xueshuantong for injection.METHODS: The HPLC chromatographic column was Zorbax SB-Aq-C18(250 mm?4.6 mm,5 ?m) and the mobile phase consisted of acetonitrile-water for gradient elution.The UV detection wavelength was set at 203 nm.RESULTS: There were 13 mutual peaks in the HPLC fingerprints of the freeze-dried Xueshuantong for injection.The relative retention time and relative peak area ratio of the fingerprints in which ginsenosides Rg1 served as internal standard were obtained.The area of non-mutual peaks represented no more than 5% of total peak area.CONCLUSION: The method is simple and reliable and it is applicable for the quality control of the freeze-dried Xueshuantong for injection.
2.Application of vacuum sealing drainage in the treatment of severe necrotizing fasciitis in extremities of patients.

OBJECTIVETo study the application of VSD in the treatment of severe necrotizing fasciitis in extremities of patients.
METHODSEight patients, suffering from severe necrotizing fasciitis, who had been traditionally treated with iodophor-soaked gauze for 21 to 365 days in other hospitals, were transferred to our institute because of the nonhealing wounds and systemic toxic symptoms induced by infection, from January 2011 to August 2013. After admission, surgical debridement was performed timely, and the necrotic tissue was collected during the operation for pathological observation after HE staining. After the operation, VSD was started with negative pressure ranging from -100 to -80 kPa, and the furacilin solution (0.2 g/L) and oxygen (2 L/min) were continuously infused into the wound during the treatment. Surgical debridement was performed repeatedly according to the wound condition followed by change of VSD dressings to continue VSD treatment. The wounds were closed by suturing or with autologous skin grafts after being covered by fresh granulation tissue. The times of surgical debridement, times of change of VSD materials, wound healing status, and length of stay in our institute were recorded. All patients were followed up for a long time. Results HE staining showed that there were diffuse necrotic adipose and fibrous connective tissues in the necrotic tissue, and the normal tissue structure disappeared accompanied by significant infiltration of inflammatory cells. The number of surgical debridement was 2 to 10 (3.9 +/- 2.8) times. The number of VSD materials change was 2 to 10 (4.0 +/- 2.9) times. Wounds were closed by suturing and healed in two patients; wounds in the other six patients were partially sutured, their residual wounds were healed by autologous skin grafting. The length of stay in our institute was 20 to 49 (33 +/- 10) days. All patients were discharged after recovery. Patients were followed up for 2 to 24 months, and their wounds were found to be in good condition without ulceration or recurrence.
CONCLUSIONSVSD can effectively remove the necrotic tissues and exudates from the fascial spaces and promote proliferation of granulation tissue. Therefore it serves as an effective approach to the treatment of severe necrotizing fasciitis in extremities.

3.Clinic research of video-assisted thoracoscopic solitary pulmonary nodule resection after CT-guided hardening agent localization

Objective The aim of this study is to evaluate the efficacy and safety of preoperative CT-guided hardening agent localization.Methods From December 2010 to January 2012,27 patients with 29 solitary pulmonary nodules who had undergone CT-guided hardening agent localization and video-assisted thoracoscopic surgery (VATS) were studied.Results All cases were underwent CT-guided hardening agent localization successfully,and no patient had serious complication that required any intervention.The diameter of nodules ranged from 3 to 21 mm as measured by CT[mean (11.27 ± 6.32) mm].The distance between the center of nodule and visceral pleural ranged from 4 to 38 mm[mean (14.45 ± 4.32) mm].Conversion from VATS to thoracotomies was not necessary during the diagnostic resection procedure nodules.29 solitary pulmonary nodules underwent thoracoscopic wedge resection,and no intra-or postoperative mortality or morbidity was recorded.Conclusion CT-guided hardening agent localization before video-assisted thoracoscopic solitary pulmonary nodule resection is a safe and effective procedure for accurate diagnosis and resection of indeterminate solitary pulmonary nodules.
4.Reversion of hypoxta and reoxygenation injury of alveolar type Ⅱ cells by simvastatin

Objective To investigate the protective effects of simvastatin on cobalt choride ( CoCl2 ) -induced hypoxia and reoxygenation injury on alveolar type Ⅱ cells and the underlying mechanisms.Methods CoCl2 was used to establish the hypoxia and reoxygenation injury model on AT Ⅱ cells.Blank,control and variant doses simvastatin-treated groups ( 5,10,20,30,50,100 μ mol/L) were designed in the present study.The proliferation of AT Ⅱ cells was evaluated by Cell Counting Kit-8 ( CCK-8 ) assay.The percentage of apoptotic cells was assessed by flow cytometry AV/PI double-staining.The protein levels of surfactant protein-C (SP-C) and proliferating cell nuclear antigen (PCNA) in AT Ⅱ cells was determined by Western blot.Results As compared with the control group,pretreatment with low dose (5 - 20 μmol/L),but not high dose simvastatin (50 - 100 μmol/L) markedly reduced A549 cells apoptosis,and increased their proliferation and the protein levels of SPC and PCNAin vitro.The protective effect could be reversed in vitro by L-mevalonate,a simvastatin competitive inhibitor,which indicated that the inhibition of mevalorate pathway was involved in the simvastatin induced AT Ⅱ cells function restoration.Condusion Low doses simvastatin reversed CoCl2-induced hypoxia and reoxygenation injury of AT Ⅱ cells.The inhibition of mevalonate pathway contributed to simvastatin induced AT Ⅱ cells function restoration.
5.Cross-sectional Survey on Therapeutic Drug Monitoring of Vancomycin in Our Hospital

OBJECTIVE:To explore the necessity of developing therapeutic drug monitoring of vancomycin in our hospital and its existing problems,and provide a reasonable basis for the clinical rational use of vancomycin. METHODS:The cross-sectional survey was designed to collect the clinical data of 92 patients with therapeutic drug monitoring of vancomycin and statistically ana-lyze 192 cases of plasma concentration monitoring data. RESULTS:The average plasma trough concentration was (15.96 ± 8.06) mg/L;with the increase of age,the plasma trough concentration was increasing,there was no significant difference in the plasma trough concentration among different age groups (P=0.000);there were only 13 cases (6.77%) that obtained the plasma trough concentration within 30 min before the fourth dose;after using wancomycin,clearance rates of Cr and the endogenous creatinine were slightly higher than before,but there was no significant difference(P=0.722);36 cases(39.13%)showed vancomycin sus-ceptible gram positive cocci;after using wancomycin,the body temperature,white blood cell count and neutrophil percentage were lower than before,the differences were statistically significant (P=0.006,P=0.000,P=0.000);48 cases (52.17%) in treatment received initial loading dose,and only 15 cases (16.30%) did not use in combination with other anti infective drugs. CONCLU-SIONS:The results showed there are still a lot of problems in the treatment of vancomycin in our hospital,for example,the stan-dard rate of the plasma trough concentration is about 50%;most of the time of blood sampling is not reasonable;the detection rate of the pathogen is low;only about half of the cases are given the loading dose,etc. Therefore clinical pharmacists’intervention for blood sampling is an important part to promote rational drug therapy monitoring. Meanwhile,data interpretation of the monitoring results of serum drug concentration of vancomycin is a basic method for clinical pharmacists in clinical monitoring to correct the un-reasonable operations,and also the necessary measures for preventing the drug renal toxicity,it is a very important significance for the medication safety and effectiveness especially in severe infection patients,the elderly,the children and the people with renal function insufficiency.
6.Initial experience of previously blocking both pulmonary artery and veins in true video assisted thoracic surgery for the treatment of NSCLC

ObjectiveWe evaluate our experience of previously blocking both pulmonary artery and veins in true video assisted thoracic surgery (VATS) for the treatment of non-small-cell lung carcinoma ( NSCLC ).MethodsSix patients undergoing lobectomy with previously blocking both pulmonary artery and veins in the VATS in our institution between December 2007 and June 2011 were reviewed.ResultsIn our series,six patients required previously blocking both pulmonary artery and veins in the VATS,The mean occlusion time of PA and PV was (42.0 ± 8.6 ) min and ( 39.3 ± 8.2 ) min,respectively.Four patients required partial PA reconstruction.The mean repair time of the PA was (21.25 ±9.91 ) min.No complications attributable to the technique or mortality were seen.There were no local recurrences on the pulmonary artery.ConclusionIt is feasible and safe that the technology of previously blocking of the pulmonary artery and veins was applied in VATS.This technique can reduce the risk of intraoperative bleeding and the chance of converting to open thoracotomy,and extend the inclusion criteria of VATS lobectomy.
7.The expression and its significance of detecting carbonic anhydrase Ⅸ(CAⅨ), paxillin and somatostatin receptors (SSTR) in patients with duodenal carcinoma

Objective To investigate the expression and significance of detecting carbonic anhydrase Ⅸ(CAⅨ), pax-illin (paxillin) and somatostatin receptors (SSTR) in patients with duodenal carcinoma. Methods The immunohisto-chemistry was used to detect 87 cases of duodenal carcinoma carbonic anhydrase Ⅸ (CA Ⅸ), paxillin (paxillin) and somatostatin receptors (SSTR) expression. Results Among 87 cases of duodenal cancer, CA Ⅸ positive rate of 71.3%(62/87), paxillin positive rate of 74.7% (65/87), was significantly higher than other groups (P<0.05). The rate of so-matostatin receptor-positive rate of 48.2%(42/87),was significantly lower than than other groups (P<0.05). And the positive rate of duodenal CA Ⅸ poorly differentiated cancer group, the positive rate of protein pile, SSTR-positive rate compared with the well-differentiated group (P<0.05). The positive rate of M1 CA Ⅸ of duodenal papilla cancer, the positive rate of paxillin, SSTR-positive rate compared with M0 stage(P<0.05). Conclusion Carbonic anhydrase Ⅸ(CAⅨ), occurrence, development and metastasis of somatostatin receptor (SSTR) and paxillin and three closely related duodenal cancer, can serve as an important indicator of early diagnosis of cancer, can also be used to assess tumor development and prognosis.
8.Therapeutic effect of salidroside in a mouse model of Sjogren's syndrome and its possible mechanism

This study was designed to investigate the effect of salidroside on Sjogren's syndrome in mice and its mechanism.Mice in negative control group and model group were given normal saline intragastric administration,mice in 3 salidroside groups were given salidroside intragastric administration(doses of 20,40 and 80 mg/kg),and mice in positive control group were given hydroxychloroquine sulfate(100 mg/kg)intragastric administration once a day.After continuous intragastric administration for 8 weeks,water intake and saliva flow rate were detected,infiltrated submandibular gland lymphocytes were evaluated,the levels of IL-17,IL-10,NF-κB P65 and IκBα and the ratio of Th17/Treg cells were detected.In the model group,the acinus atrophied with unclear margin and decreasing in number,and the lymphocyte infiltration were observed and lymphocyte focus was formed.After the intervention with salidroside and hydroxychloroquine sulfate,the degree of acinus lesions was relieved to a certain extent,and the lymphocyte infiltration was reduced.Compared with negative control group,water intake,salivary flow rate,submandibular gland index,IL-10 and IκBα levels were decreased in other groups,while lymphocyte infiltration,the levels of IL-17,IL-17/IL-10,NF-κB P65 and NF-κB P65/IκBα were increased(P<0.05).Compared with model group,water intake,salivary flow rate,submandibular gland index,IL-10 and IκBα levels were increased in each salidroside dose group,while submandibular gland lymphocyte infiltration,the levels of IL-17,IL-17/IL-10,NF-κB P65 and NF-κB P65/IκBα decreased in a dose-dependent manner(P<0.05).Compared with the negative control group,the proportion of Th17 cells in the serum of model group was increased,and the proportion of Treg cells was decreased,while salidroside in all doses could reverse these changes(P<0.05).Taken together,salidroside alleviates submandibular gland inflammatory responses by mediating Th17/Treg immune balance and inhibiting NF-κB P65/IκBα,thus playing a therapeutic role in SS treatment.
9.The mechanism of metformin-mediated osteogenic differentiation of rat jaw BMSCs under high-glucose and high-fat environment

Objective:To explore the mechanism of metformin on osteogenic differentiation of rat jaw bone marrow mesenchymal cells(BMSCs)under high sugar and high fat environment.Methods:BMSCs of male SD rats were isolated and cultured in vitro and divided into 4 groups:Control group(C group),high sugar and high fat group(H group),control cells and H group cells were respectively stimulated with metformin(CM group and HM group).EDU staining was used to detect the cell proliferation;after osteogenesis and adi-pogenesis induction of BMSCs,alkaline phosphatase staining,alizarin red(O)staining and oil red O staining were respectively used to detect the osteogenic and adipogenic differentiation of the cells.The protein expression of Runx2 and OCN was detected by Western blot and immunofluorescence staining respectively.Results:Compared with the H group,in the HM group the cell proliferation ability was enhanced,the expression levels of Runx2 and OCN were increased(P<0.05),the osteogenic differentiation ability was significantly in-creased,and the adipogenic differentiation ability was significantly decreased.Conclusion:In vitro,metformin promotes the prolifera-tion and osteogenic differentiation of rat jaw BMSCs cultured with a high sugar and high fat environment by increase of the expression of Runx2 and OCN.
10.Expression and significance of miR-125a and Mcl-1 in intestinal tissue after massive small bowel resection in rat.

OBJECTIVETo investigate the expression and significance of miR-125a and anti-apoptotic protein Mcl-1 in intestinal tissue after massive small bowel resection in intestinal adaptation.
METHODSSprague-Dawley rats (54 male rats, 8-week old) were divided into 3 groups randomly, including two control groups. Rats in the experiment group were subjected to 70% massive small bowel resection. Rats in the resection group underwent simple intestinal resection and anastomosis. Rats in the control group underwent laparotomy alone. A 5 cm intestine approximately 1 cm distal to the anastomosis was harvested a week after operation. Expression of Mcl-1 was assessed by immunohistochemistry and real-time PCR was used to detect the expression of miR-125a in intestinal tissue.
RESULTSThe positive expression of Mcl-1 in the experiment group was 18.8%(3/16), significantly lower than that in the control group(76.5%, 13/17) and the resection group (83.33%, 15/18)(both P<0.01). The expression of miR-125a in the experiment group was 1.92, significantly higher than that in the control group (1.01) and the resection group (1.05)(both P<0.01).
CONCLUSIONmiR-125a and anti-apoptotic protein Mcl-1 may play an important role in intestinal adaptation process and they may regulate each other through a certain pathway.
