1.Comparative proteomics studies in gastric cancer from serum by SELDI TOF mass spectrometry

Objective To search for the biomarker used to determine gastric cancer by the application of protein mass spectrometry analysis. Methods The relative contents of serum proteins were detected of 38 patients with gastric cancer and 82 healthy people by IMAC3 (CipherGen Inc.) chip and proteinchip. Results At the M/Z values range from 1 723Da to 14 048Da, 18 kind of protein contents are obviously different between the two groups. In the learning mode, all the 120 testers were correctly distinguished, both the sensitivity and specificity reached to 100%. While in the test mode, 31 patients and 81 control people were correctly distinguished, the accuracy were 81 6%(31/38) and 98 8%(81/82), respectively. Conclusion Gastric cancer can be quickly and exactly diagnosed by this method with high sensitivity and specificity. That will be widely used in clinical application
2.Prevalence study of tumor radiotherapy in the Xinjiang Uygur Autonomous Region

Objective To investigate the development status of tumor radiotherapy in the Xinjiang Uygur Autonomous Region.Methods A questionnaire survey we used last time was conducted by letter,Email,telephone or fax to investigate all the medical institutions in the Xinjiang Uygur Autonomous Region for researching the current status of radiotherapy academic settings,human resources,allocation of radiotherapy facilities and standardization of medical practices.Results Up to the end of January 2015,there were 30 medical electron linear accelerators,21 simulated locators,10 breach-loading brachytherapy machines,31 treatment planning systems and 5γ or X knives in Xinjiang.Radiotherapy was carried out in 20 medical institutions,including 14 three-dimensional conformal radiotherapy and 9 intensity modulated radiation therapy,8 breach-loading brachytherapy,3 stereotactic radiotherapy,and 2 volumetric modulated arc therapy.There were 280 radiation personnel (except nurses),including 159 radiation oncologists,40 radiotherapy physicists and 81 radiotherapy technologists.There were 1167 hospital beds and 855 cancer patients received radiotherapy on average every day.Conclusions Although radiation personnel has increased,radiotherapy facilities have been improved,hospital beds and patients received radiotherapy also have become more,the improvements are needed for cause of imbalance in precise radiation therapy development.

4.Investigation of the current state of pension security for rural doctors in several provinces

Objectives: To investigate the current state of pension security for rural doctors in 12 provinces those have not introduced related policies. Methods:Two counties were chosen in each province. Data was analyzed in terms of coverage, types and intention of pension security for rural doctors in these areas. Results:The study found that the proportion of rural doctors with pension security was 81. 4% in these areas. 57. 1% of rural doctors with pen-sion security had urban and rural pension insurance. Differences exist among these areas. The safeguard level is too low, and the willingness of rural doctors to attend to pension security is not strong enough. Results:The mechanism of pension security for rural doctors must be improved. These areas should implement classified guidance and syn-chronize various models to solve the problem of pension security for rural doctors.
5.A serum proteomics study on early cancerization of post-hepatitic cirrhosis by SELDI protein chip technique

Objective To explore the serum proteomics characteristics of early carcerization of post-hepatitic cirrhosis.MethodsThe serum protein profiles were detected of 62 patients with post-hepatitic cirrhosis and 100 patients with hepatic carcinoma in Ⅰ-Ⅱ stage (T1-2N0M0) and cirrhosis by surface-enhanced laser desorption/ionization-time of flight-mass spectrometry (SELDI-TOF-MS) IMAC30-Cu2+ ProteinChip array (Ciphergen Biosystems Inc.,USA),the distinct proteins were analyzed and a classification tree model was established by using Biomarker Patterns software.The diagnostic efficacy of the model was blindly tested using the serum protein from both groups.ResultsThe serum protein profiles of post-hepatitic cirrhosis and hepatic carcinoma complicating with cirrhosis were analyzed and a classification tree model was established including 4 distinct proteins with different M/Z.In the training mode,the accuracy was 99.4% (161/162) on differential diagnosis to differentiate post-hepatitic cirrhosis and hepatic carcinoma complicating with cirrhosis,the sensitivity and specificity were 99.0% (99/100) and 100.0% (62/62),respectively,on the diagnosis of hepatic carcinoma complicating with cirrhosis.In the testing mode,the accuracy of differential diagnosis was 94.4% (153/162),the sensitivity and specificity were 95.0% (95/100) and 93.5% (58/62) on the diagnosis of hepatic carcinoma complicating with cirrhosis.ConclusionSELDI-TOF-MS ProteinChip technique is of convenient and rapid,and with high sensitivity and specificity on the diagnosis of early carcerization of post-hepatitic cirrhosis.
6.Construction of the classification tree model of colorectal cancer with lymphatic metastasis by surface-enhanced laser desorption/ionization time-of-flight mass spectrometry

Objective To search for the specific biomarkers associated with local lymphatic metastasis of colorectal cancer in serum.Methods The serum protein profile of colorectal cancer patients was determined by surface-enhanced laser desorption/ionization time-of-flight mass spectrometry (SELDI-TOF-MS),and the peak values of proteins were identified with the matched software,and then clustered to construct the classification tree model.Seventy colorectal cancer patients with local lymphatic metastasis and 75 patients in matched age and gender without local lymphatic metastasis were assigned as a training set to construct the classification tree model,and 35 colorectal cancer patients with local lymphatic metastasis and 30 patients in matched age and gender without local lymphatic metastasis were assigned as test set to make the independent sample double-blind test.Results Forty-six distinct proteins were identified from the two groups,and the classification tree model formed by 5 proteins (M/Z:3104,3781,5867,7970 and 9290) could be used to identify the two groups with a sensitivity of 94.3% (66/70) and a specificity of 100.0% (75/75).The double-blind test challenged the model with a sensitivity of 91.4% (32/35),a specificity of 96.7% (29/30),and a positive predictive value of 97.0% (32/33),respectively.ConclusionThe constructed classification tree model may distinguish colorectal cancer patients with or without local lymphatic metastasis correctly,and show a great potential for preoperatively screening the colorectal cancer patients with or without local lymphatic metastasis.
7.Serum proteomic analysis in patients with lung cancer

Objective To explore and determine the biologic markers for the diagnosis of lung cancer by comparison of proteomics among patients with lung cancer,those with benign lung tumor and healthy people.Methods The serum proteomics patterns of 89 cases of lung cancer,64 cases of benign lung tumor and 68 healthy subjects were read by surface enhanced laser desorption/ionization-time of flight-mass spectrometry (SELDI-TOF-MS) to screen significant differential proteins,and to develop a classification tree model for the diagnosis of lung cancer.Thirty cases of lung cancer,30 cases of benign lung tumor and 30 healthy subjects were randomly selected at the same period and assigned as test groups for double-blind verification of the model.Results Thirty-nine differential proteins were identified from the three groups,and the classification tree model formed by 17 proteins (M/Z:4485,5252,5807,5908,5969,6113,6625,8946,8998,9137,9183,9298,9498,13878,15128,15867 and 16081) could be used to identify lung cancer,benign lung tumor and healthy subjects with an accuracy of 98.2% (217/221),sensitivity of 98.9% (88/89) and specificity of 97.7% (129/132),respectively.The double-blind test challenged the model with a sensitivity of 90.0% (27/30) and specificity of 93.3% (56/60).Conclusion The classification tree model constructed by SELDI-TOF-MS possesses high sensitivity and specificity,and it may be used for rapid diagnosis of lung cancer.
8.Effects of Cilostazol on microvascular complications of diabetic patients

Objecfive To evaluate the effects of cilostazol on the prevention of microvascular complications in diabetic patients.Methods 60 diabetic patients with microvascular complications were orally given cilostazol for 1 month.Changes of Mean platelet volume (MPV),plateletcrit (PCT),platelet distribution width (PDW) and platelet count (PLT) were studied.Results With administration of cilostazol,MPV and PDW decreased significantly. Conclusion Cilostazol improves platelet parameters,suggesting that it could prevent the progression of microvascular diseases.
9.Changes of free radicals and antioxidative system of prostate tissue in the male rats with short-term diabetes

AIM: To study the dynamic changes of free radicals and antioxidative system of prostate in rats with short-term diabetes. METHODS: 48 Wistar male rats were divided into 6 groups at random (8 rats in each group). One group were injected(ip) with sodium citrate buffer to be the control group, the other 5 groups were injected (ip) with steptozocin(STZ) 60mg/kg body weight which were called diabetes group. Rats were killed after 1, 3, 5, 7 and 14 days; superoxide dismutase(SOD), malondialdehyde(MDA), glutathione(GST), glutathione s-transferase(GST), glutathione peroxidase(GSH-px), nitric oxide (NO) and nitric oxide synthase(NOS) of prostate were examined. RESULTS: The levels of MDA, GSH-px and NO in prostate homogenate of diabetes group were higher evidently as compared with control levels. The activities of SOD, GST, NOS and the levels of GSH in prostate homogenate were increased obviously after injection of STZ with control levels, then returned near to control values. CONCLUSION: The evidences mentioned above indicate that high-level of free radicals and decrease in antioxidative system rendere the prostate cells to be in oxidative stress.
10.Prognostic factors for patients after curative resection for proximal gastric cancer.

The factors influencing the long-term survival of patients with proximal gastric cancer (PGC) after curative resection were investigated. Data from 171 patients who underwent curative resection for PGC were retrospectively analyzed. The patients were grouped according to the clinicopathological factors and operative procedures. The tumor depth (T stage) and lymph node metastasis (pN stage) were graded according to the fifth edition of TNM Staging System published by UICC in 1997. The metastatic lymph node ratio (MLR) was divided into four levels: 0%, <10%, 10%-30% and >30%. The data of survival rate were analyzed by Kaplan-Meier method (log-rank test) and Cox regression model. The 5-year overall survival rate of 171 patients was 37.32%. The univariate analysis demonstrated that the survival time of the postoperative patients with PGC was related to tumor size (chi (2)=4.57, P=0.0325), gross type (chi (2)=21.38, P<0.001), T stage (chi (2)=27.91, P<0.001), pN stage (chi (2)=44.72, P<0.001), MLR (chi (2)=61.12, P<0.001), TNM stage (chi (2)=44.91, P<0.001), and range of gastrectomy (chi (2)=4.36, P=0.0368). Multivariate analysis showed that MLR (chi (2)=10.972, P=0.001), pN stage (chi (2)=6.640, P=0.010), TNM stage (chi (2)=7.081, P=0.007), T stage (chi (2)=7.687, P=0.006) and gross type (chi (2)=6.252, P=0.012) were the independent prognostic factors. In addition, the prognosis of patients who underwent total gastrectomy (TG) was superior to that of patients who underwent proximal gastrectomy (PG) for the cases of tumor >/=5 cm (chi (2)=6.31, P=0.0120), Borrmann III/IV (chi (2)=7.96, P=0.0050), T4 (chi (2)=4.57, P=0.0325), pN2 (chi (2)=5.52, P=0.0188), MLR 10%-30% (chi (2)=4.46, P=0.0347), MLR >30% (chi (2)=13.34, P=0.0003), TNM III (chi (2)=14.05, P=0.0002) or TNM IV stage (chi (2)=4.37, P=0.0366); and combining splenectomy was beneficial to the cases of T3 (chi (2)=5.68, P=0.0171) or MLR >30% (chi (2)=6.11, P=0.0134). It was concluded that MLR, pN stage, TNM stage, T stage, and gross type had advantages in providing a precise prognostic evaluation for patients undergoing curative resection for PGC, in which MLR was the most valuable index. TG and combining splenectomy were useful to improve the prognosis to patients with PGC of TNM III/IV stage, serosa invasion, or extensive regional lymph node metastasis.
