The abnormal activation of phosphatidylinositol-3-kinase (PI3K)-proiein kinase B (Akt) pathway has already been confirmed to be closely related to the origin and development of various tumors.PI3KAkt pathway plays an important role in development of colon cancer,it can regulate cell survival,proliferation,anti-apoptosis,cell cycle progression,etc.Furthermore,inhibition of this pathway may play a potential roll in oncotherapy.
2.Clinical study on thirty-eight cases of percutaneous microwave ablation in the treatment of hypersplenism

Objective To investigate the efficacy and safety of percutaneous microwave ablation in the treatment of hypersplenism.Methods From March 2007 to May 2011,38 patients with hepatitis B virus caused liver cirrhosis and complicated with hypersplenism received percutaneous microwave ablation treatment for several times.Before percutaneous microwave ablation treatment,the volume of spleen was calculated according to 3D computed tomography (CT) scan.Ultrasound,blood routine,urine routine,liver function test,kidney function test,serum amylase and lipase were also tested.For the first time,1/3 volume of spleen was ablated.If no complication were observed in one week after ablation,then another 1/3 volume of spleen was ablated.Blood routine and liver function test were checked on the 1st,3rd and 5th day after microwave ablation.Blood routine,liver function test and ultrasound were examined on 7th and 14th day after microwave ablation.On the 30th day CT examination was conducted.Ultrasound,blood routine,urine routine,liver function test,kidney function test,serum amylase and lipase were detected at one month,three month and every three month after microwave ablation.The follow up duration was over two years.The t-test was performed for clinical data comparison.Results The mean ratio of ablated spleen was (47 ±5)% (range 41% to 57%).Preoperative white blood cell count was (2.46±0.78)× 109/L,which gradually increased after operation and peaked on the 3rd day after operation ((5.34 ± 2.10) × 109/L).Then gradually decreased,which was (3.16 ± 1.02) × 109/L at 24 month and the difference was statistically significant compared with that of preoperation (t=-3.349,P<0.01).Preoperative platelet count was (46.58 ± 17.30) × 109/L,which gradually decreased after operation and was lowest on the 3rd day after operation.Then gradually increased,which peaked at 30 days after operation ((101.79 ± 25.80) × 109/L) and then gradually decreased,which was (61.97 ± 15.09) × 109/L at 24 month and the difference was statistically significant compared with that of preoperation (t=-4.135,P<0.01).The inner diameter of portal vein was (14.66±0.88) mm preoperation,which was (13.22±0.64) mm at three month after operation and the difference was statistically significant compared with that of preoperation (t=8.145,P<0.01).It was (14.64±0.81) mm at six month after operation and the difference was not statistically significant compared with that of preoperation (P> 0.05).The major adverse effects were fever,left upper abdominal pain,left shoulder pain,pleural effusion,intraperitoneal hemorrhage and temporary hemoglobinuria which all recovered after symptomatic treatment.No severe complication such as uncontrollable bleeding,splenic abscess,spleen rupture and the surrounding organ injury and treatment related death were observed.Conclusion Percutaneous microwave ablation for several times could safely destroy suitable volume of spleen,increase platelet and white blood cell count,improve portal hypertension and with rare complications,which might be a minimally invasive techniques with clinical application value in the treatment of the hypersplenism.
3.The learning curve of laparoscopic totally extraperitoneal (TEP) inguinal hernia repair

Objective To investigate the learning curve of laparoscopic totally extraperitoneal (TEP) inguinal hernia repair.Methods The clinical data of 450 cases of patients who received laparoscopic inguinal hernia repair by the same group of surgeons from December 2012 to August 2016 in our hospital were sequentially divided into three groups,with 150 cases in each group.The cases of each group who received TEP inguinal hernia repair were extracted and enrolled into group A,B and C.The preoperative,intraoperative and postoperative indicators were compared among the three groups.Results Before operation,no statistically significant difference was found in age,gender,incidence rates of scrotum hernia,recurrent hernia and bilateral hernia,and types of hernia among the three groups (P>0.05).And there was no statistically significant difference in incidence rates of postoperative urinary retention,analgesia and seromas,and hospitalization expenses among the three groups (P>0.05).During operation,differences in conversion rate,operative time and volume of blood loss among the three groups were statistically significant (P<0.05).Moreover,statistically significant differences in conversion rate,operative time and volume of blood loss were found between group A and group B (P<0.05),while there was no statistically significant difference between group B and group C (P>0.05).The regression curves were drawn based on operation time,volume of blood loss and the number of cases,which estimated the inflection point was 150.Conclusion The learning curve of TEP inguinal hernia repair include approximately 150 cases of laparoscopic inguinal hernia repair (including 16 cases of patients received TEP).The TEP inguinal hernia repair operation during the learning curve can be safety on the basis of preoperative evaluation and appropriate case selection.
4.Influencing factor analysis of the number of lymph nodes harvest after radical resection of colorectal cancer

Objective To investigate influencing factors of the number of lymph node harvest after radical resection of colorectal cancer.Methods The retrospective case-control study was conducted.The clinicopathological data of 227 patients with colorectal cancer who underwent radical resection in People's Hospital of Changshou Chongqing from June 2010 to June 2016 were collected.The surgical method and resection extention were determined depending on the tumor location showed on imaging examinations,and all patients underwent radical resection and sufficient lymph nodes dissection.Observation indicators:(1) intra-and post-operative situations;(2) influencing factors analysis of the number of lymph nodes harvest after radical resection of colorectal cancer;(3) follow-up and survival situations.Follow-up using outpatient examination and telephone interview was performed to detect patients' survival up to October 2016.Measurement data with normal distribution were represented as (x)±s.Univariate analysis was done using the chi-square test or Fisher exact probability method.Multivariate analysis was performed using the binomial Logistic regression analysis.Results (1) Intra-and postoperative situation:all the 227 patients underwent successful radical resection of colorectal cancer,including 67 with radical resection of right colon cancer,16 with radical resection of left colon cancer,26 with radical resection of sigmoid colon cancer and 118 with radical resection of rectal cancer.Of 227 patients,118 received laparoscopic surgery,109 received open surgery including 8 converted to open surgery from laparoscopic surgery.Tumor located in right hernicolon,left hemicolon,sigmoid colon and rectum were respectively detected in 67,16,26 and 118 patients,same as results of imaging examintions.Operation time,volume of intraoperative blood loss and number of lymph nodes harvest in 227 patients were (192 ± 72) minutes,(94± 84) mL and 14 ± 4.Of 8 patients in 227patients with postoperative complications,2 received secondary suture due to wound infection,2 received reoperation due to intestinal obstruction,1 received transverse colostomy due to anastomotic leakage,and 3 received stoma reconstruction due to stoma retraction.Duration of postoperative hospital stay of 227 patients was (22±9) days.Postoperative pathological examininations:35 and 192 patients were respectively diagnosed with rnucinous adenocarcinoma and non-mucinous adenocarcinoma.Moderate-and low-differentiated carcinoma and high-differentiated carcinoma were respectively detected in 47 and 180 patients.(2) The influencing factors analysis of the number of lymph nodes harvest after radical resection of colorectal cancer:univariate analysis showed that tumor location and tumor pathological T stage were related factors affecting the number of lymph node harvest after radical resection of colorectal cancer (x2=10.066,P<0.05).Multivariate analysis showed the tumor location and tumor pathological T stage were independent factors affecting the number of lymph nodes harvest after radical resection of colorectal caucer (OR=1.283,6.075,95% confidence interval:1.031-1.597,1.215-30.385,P<0.05).(3) Follow-up and survival situations:190 of the 227 patients were followed up for 4-72 months,with a median time of 32 months.During the follow-up,21 patients died,23 patients survived with tumor,and 146 patients survived without disease.Conclusion Tumor location and tumor pathological T stage are independent factors affecting the number of lymph node harvest after radical resection of colorectal cancer.
5.Research on performance evaluation framework of public health in rural areas

Objective To provide reference for improving the performance of public health by establishing performance evaluation framework of public health in rural areas of China.Methods The performance evaluation framework of public health in rural areas is established by literature research,theoretical derivation and Delphi method.The results of expert consultation are tested by W test of Kendall coordination coefficient and chi-square test.Results The performance evaluation framework of public health in rural areas contains four conceptual modules,ten performance goals and twenty-eight performance evaluation indexes.The positive coefficient of experts is 0.889.The average values of authoritative grade of experts are between 0.681~0.715.The Kendall W uniformity coefficients of the necessity,sensitivity and maneuverability of the indexes are respectively 0.167,0.209 and 0.185.The P values are less than 0.01.Conclusion The performance evaluation framework of public health in rural areas has certain application value.It is necessary to pay attention to scientific nature and applicability in developing performance evaluation of public health in rural areas.
6.Expression of COX-2 and ERCC1 in gastric cancer and their clinical significance

Objective To investigate DNA excision repair cross-complementing gene-1 (ERCC1) and cyclooxygenase-2 (COX-2) expression levels in gastric cancer (GC) and evaluate the correlation between two proteins. Methods Immunohistochemistry was performed to detect the expression of ERCC1 and COX-2 in 239 GC cases, and analyzed the association between ERCC1 and COX-2 expression levels and clinicopathologic parameters. Results ERCC1 and COX-2 expression levels were significantly correlated with lymph node metastasis (P <0.05), while not associated with tumor size, location, depth of invasion and tumor differentiation (P>0.05). The positive expression rate of COX-2 in GC patients with HP infection was 86.5%, higher than those without HP infection. No correlation was found between two proteins. Conclusion Both ERCC1 and COX-2 were related with tumor progression, which may serve as valuable markers for determining biological behavior in GC. However , they did not show positive interaction.
7.Analysis of DMD gene mutations in 59 families in northern China

Objective To detect the DMD gene mutation sites and the regions of breakpoints in Duchenne/Becker muscular dystrophy (DMD/BMD) patients in northern China. Methods Multiplex amplifiable probe hybridization (MLPA) was used to detect the mutation in 59 cases (51 cases with DMD and 8 with BMD) from northern China and dystrophin gene mutations in their parents. Results From northern China and dystrophin gene mutations 59 families found gene deletions in 33 cases of 59 DMD/BMD patients (55.9%), duplications in 6 cases (10. 2%) and point mutation in one case (1.7%). Intron 44 was most frequently affected (n = 13, 33.3%), followed by intron 50 (n = 11, 28.2%) and intron 45 (n=8, 20.5%). The novel mutations were identified, in two patients including two independent duplications carried by patient D1 149 and a point mutation [5208del(A)] carried by patient D1 65, which were not included in Leiden database. In addition, an exon 22 deletion was found in one patient, which was the first reported case in Chinese patients. Conclusions Deletions are mostly located in the hotspot between exon 45 and 50. Duplications mostly occurred in the 5' end of the gene. Intron 44 is the most frequently affected breakpoint in northern Chinese population.
8.Relationship between TDAG8 and endogenous neuron-protective mechanism against oxygen-glucose deprivation and restoration-induced apoptosis in rat neurons

Objective To investigate the relationship between T?cell death?associated gene 8 ( TD?AG8) and endogenous neuron?protective mechanism against oxygen?glucose deprivation and restoration ( OGD∕R)?induced apoptosis in rat neurons. Methods The primary cortical neurons obtained from fetal rats were seeded in 6?well plates at a density of 1×105 cells∕ml and divided into 5 groups using a random number table: control group ( group C, n=24 ) , group OGD∕R ( n=48 ) , TDAG8 agonist BTB09089 group (group BTB, n=24), TDAG8?siRNA group ( group siRNA, n=24), and blank vehicle group ( group V, n=24) . The medium was replaced with glucose?and serum?free Locke′s buffer, and the neu?rons were exposed to 95% N2?5% CO2 in an air?tight incubator at 37℃ for 60 min followed by routine cul?ture to establish the model of OGD∕R. In BTB, siRNA and V groups, 20 μmol∕L TDAG8 agonist
BTB09089, 200 pmol∕L TDAG8?siRNA, and 6 μl∕200 μl transfection reagent were added, respectively, at 24 h before oxygen?glucose restoration. At 6 h of oxygen?glucose restoration, the neuronal viability and a?mount of lactic dehydrogenase ( LDH) released were measured, and the expression of TDAG8 and caspase?3 mRNA in neurons was detected by fluorescent quantitative real?time polymerase chain reaction. In group OGD∕R, the expression of TDAG8 and caspase?3 was measured by Western blot at 0, 3, 6, 12 and 24 h of oxygen?glucose restoration. In C, OGD∕R, BTB, siRNA and V groups, the expression of TDAG8, caspase?3 and p?Akt was detected at 6 h of oxygen?glucose restoration. Results In group OGD∕R, the ex?pression of TDAG8 was gradually up?regulated after oxygen?glucose restoration, and the expression of caspase?3 peaked at 6 h of oxygen?glucose restoration. Compared with group C, the neuronal viability was significantly decreased, the amount of LDH released was significantly increased, and the expression of TD?AG8 and caspase?3 protein and mRNA and p?Akt was significantly up?regulated in OGD∕R, V and siRNA groups ( P<0?05) . Compared with group OGD∕R, the expression of TDAG8 protein and mRNA and p?Akt was significantly up?regulated, the expression of caspase?3 protein and mRNA was significantly down?regu?lated, the neuronal viability was significantly increased, and the amount of LDH released was significantly decreased in group BTB, the expression of TDAG8 protein and mRNA and p?Akt was significantly down?regulated, the expression of caspase?3 protein and mRNA was significantly up?regulated, the neuronal via?bility was significantly decreased, and the amount of LDH released was significantly increased in group siR?NA ( P<0?05) , and no significant change was found in the parameters mentioned above in group V ( P>0?05) . Conclusion TDAG8 is partially involved in the endogenous neuron?protective mechanism against OGD∕R?induced apoptosis in rat neurons, which may be related to activation of Akt signaling pathway.
9.Self-management and its influencing factors of elderly patients with chronic heart failure

Objective To investigate the status of self-management of elderly patients with chronic heart failure (CHF) in Xinjiang Region,and to explore the effects of different demographic factors on self-management.Methods A total of 704 patients with chronic heart failure were randomly selected from Urumqi,Kashgar,Khotan,Changji,Shihezi,Yili Hazakh in Xinjiang Uygur Autonomous Region via convenience sampling method and were investigated using heart failure self-management scale.Results The total score of self-management of chronic heart failure elderly patients was(47.6±11.1) and scoring rate was 59.5%.There were statistically significant differences(P<0.01) in self-management score among patients with different ethnicity,religion,self-care ability,per capita income.The influencing factors of self-management were self-care ability,NYHA class and per capita monthly income(P<0.05).Conclusion The overall level of self-management of elderly patients with chronic heart failure in Xinjiang Region is low,and self-management is affected by self-care ability,NYHA class and per capita monthly income.Tailored culture-based education should be provided to improve patients' self-management.
10.Detection of epileptic spike wave in EEG signals based on morphological component analysis.

This paper proposed a morphological component analysis (MCA) method, which is based on sparse representation, to detect the spike wave in electroencephalogram (EEG) signals. It takes the advantage of MCA being able to extract the background waves and the spike waves from the EEG signals, respectively,as the dictionaries and chooses the discrete cosine transform (DCT) and the daubechies order 4 wavelet (db4) transformation as the dictionaries of MCA to detect the spike waves from the epileptic EEG. The experiment results showed that the MCA could detect epileptic spike waves in EEG signals very effectively, and it yielded high selectivity of 89.01% and sensitivity of 90.71%. As a feature extraction/decomposition algorithm, MCA can be used to extract the spike waves from EEG signals.
