Objective To study the analysis of insulin glargine combined with acarbose in type 2 diabetes compared with pre mixed insulin treatment efficacy and safety. Methods 100 patients with type 2 diabetes mellitus treated in our hospital from March 2015 to August 2016 were selected and randomly divided into the control group and the experimental group, with 50 patients in each group. The control group were treated with premixed insulin treatment, the experimental group were treated with basal insulin combined with acarbose. The clinical indexes of the experimental group and the control group were compared and analyzed. Results After the corresponding treatment, the fasting blood glucose level of the control group was (7.56 ± 1.13) nmol / L, and the fasting blood glucose level of the experimental group was (7.01 ± 0.92) nmol / L. The level of fasting blood glucose in the experimental group was significantly lower than that in the control group, with statistical difference (P<0.05). The level of HbA1c in the experimental group was (7.01 ± 0.82)％, significantly lower than that in the control group, and the level of HbA1c (7.45 ± 0.91)％, with statistical difference (P<0.05). Among the 50 patients in the control group, hypoglycemia occurred in 20 patients, the incidence of hypoglycemia was 40％, significantly higher than that in the experimental group, and the incidence of hypoglycemia was 14％, which was statistically significant (P<0.05). Conclusion Basal insulin combined with acarbose compared with premixed insulin treatment of type 2 diabetes treatment can significantly hypoglycemic effect, high safety, with further clinical promotion and application significance.

Objective To study the rationality of hypoglycemic drugs in patients with gastroparesis. Methods The mechanism of gastroparesis was analyzed, and the mechanism, indications, adverse reactions and contraindications of common hypoglycemic agents were studied. Results There were significant differences in the mechanism of gastric hypoglycemia and hypoglycemic therapy. The mechanism of action and the application of the patients were obvious. According to the characteristics of the patients, the selection of the appropriate drugs could improve the clinical curative effect and ensure the safety of the drug. Conclusion The pathogenesis of gastroparesis is more complicated. The clinical practice should be based on the pathogenesis of the patients and the hypoglycemic drug regimen.

Objective To investigate the application value of Tangniaole capsule combined with metformin in patients with newly diagnosed type 2 diabetes mellitus (T2DM).Methods 120 cases of patients with newly diagnosed T2DM were divided into the control group and the observation group.The control group was treated with metformin while the observation group was additionally treated with Tangniaole capsule.Symptoms and signs, changes in blood glucose, urine glucose, insulin resistance index (HOMA-IR) and insulin secretion (HOMA-β) were compared between the two groups.The incidence of adverse reactions was statistically analyzed.Results The curative effect in the observation group was better than control group (P<0.05).After treatment, scores of symptoms and signs, blood glucose, glycosylated hemoglobin (HbA1c), 24 h urine glucose quantitation and HOMA-IR were lower, and HOMA-β was higher in the observation group than in the control group (P<0.05).Conclusion Tangniaole capsule combined with metformin is effective in the treatment of newly diagnosed T2DM, and it can reduce blood glucose, urine glucose and insulin resistance.

Objective To explore the effect of gliclazide and Sig Leo Dean on insulin treatment for poor glycemic control in obese patients with type 2 diabetes mellitus. Methods 80 cases of insulin treatment for poor glycemic control in obese patients with type 2 diabetes mellitus treated in our hospital from March 2015 to January 2017 were randomly divided into two groups,with 40 cases in each group.The control group was treated with gliclazide The observation group was treated with Sig Leo Dean.The therapeutic effects of the two groups were observed and compared. Results 2 hours postprandial blood glucose levels improved in the observation group was better than that of the control group, the body weight, blood pressure, fasting blood glucose of two groups were improved, and there were no significant difference between the two groups; The incidence rate of adverse reactions was 2.5% in the observation group, 10% in the control group, the difference was not statistically significant (P<0.05). Conclusion Analysis of gliclazide and Sig Leo Dean on insulin treatment for poor glycemic control in obese patients with type 2 diabetes mellitus curative effect, Sig Leo Dean for the glycaemic improvement effect is higher, reduce the incidence of adverse reactions, so more medication safety, it is worthy of clinical promotion Application.

Purpose To explore the effect of speckle tracking imaging (STI) in early detection of coronary artery disease (CAD) in patients with hypertension (HT),and to intervene early to alleviate or delay the progression of CAD.This would provide theoretical evidence in controlling the development and improving the prognosis of CAD.Materials and Methods From January 2013 to April 2015,a total of 166 cases of hospitalized patients from the First Affiliated Hospital of Sun Yat-sen and Jiangmen Central Hospital were retrospectively analyzed,including HT group (n=42),CAD group (n=35),HT+CAD group (n=49) and control group (n=40).Conventional measurement and STI were used to analyze the conventional values and the 17 section peak strain value of the left ventricle.Results The total mean strain,average peak strain of lesion segment and minimum peak strain in CAD group and HT+CAD group were significantly lower (P＜0.05) compared to the control group.ROC analyses showed that the AUC for total mean strain,average peak strain of lesion segment and minimum peak strain value were 0.71,0.76,and 0.70,respectively.Using a combined cut-off of ≥-18％ of average peak strain of lesion segment,≥-19％ of total mean strain and ≥-6％ of minimum peak strain value,the performance of CHD diagnosis had a sensitivity of 91.3％ and a specificity of 83.5％.Conclusion Speckle tracking automatic function imaging can be used to quantitatively analyze abnormal left ventricular segmental wall movement.It improves the detection in hypertension patients with coronary artery disease.

Objective To explore the effects of insulin-like growth factor-l (IGF-l),insulin-like growth factor binding protein-1 (IGFBP-1) and growth hormone (GH) on the generation, development and treatment of diabetes chronic complications. Methods 20 healthy controls,10 type 1 diabetes mellitus patients and 62 type 2 diabetes mellitus patients were enrolled into the study. IGF-1, IGFBP-1, GH, HbAlc. serum insulin (INS) and serum C-peptide (C-P) were measured. Results ①Serum level of IGF-1 was obviously lower in the diabetes mellitus group than that in controls (type 1 P＜0. 05,type 2 P＜0. 05). ②Serum level of IGFBP-1 was significantly higher in type I diabetes mellitus than that in controls (P＜0. 05),and it was obviously lower in type 2 diabetes mellitus with obesity and hyperinsulinism than that in contris (P＜0. 05). ③Serum level of GH was higher in type 1 diabetes mellitus than that in controls (P＜0. 05 ) and was not significantly different from that in controls (P＞0. 05). ④The level of IGF-l was higher in diabetes nephropathy (DN) patients and diabetes retinopathy (DR) patients than that in controls (P＜0. 05). ⑤The level of IGF1 was negatively correlated with HbAlc (P＜0. 01, type 1r=- 0.73, type 2r=-0. 62). Objective To explore the effects and possible mechanism of baicalin on renal interstitial fibrosis in unilateral urethral obstruction (UUO) rat.Methods Forty male SD rats were randomly divided into the sham operation group,model group,baicalin group,benazepril group and baicalin+ benazepril group,8 rats in each group.The baicalin group received the gavage of baicalin 40 mg · kg-1 · d 1,the benazepril group received the gavage of benazepril suspension 10 mg · kg-1 · d-1,the rats baicalin+ benazepril group received the gavage of baicalin 40 mg · kg-1 · d 1 and benazepril suspension 10 mg · kg-1 · d-1,the model group and sham operation group were given the same volume of normal saline.Rats were sacrificed in each group on 14,21,28 d after model construction and the kidney sample was obtained.Immunohistochemistry was used to detect the expression of TGF-β1 and α-SMA.Results Compared with the sham operation group,the TGF-β1 and α-SMA expression levels of renal tissue in the model group were significantly increased(P＜0.05),compared with the model group,the TGF-β1 and α-SMA expression levels on 14,21,28 d of model construction in the baicalin group,baicalin+ benazepril group and benazepril group were decreased to different extents (P＜0.05).But the TGF-β1 and α-SMA levels at each time point had no statistical difference among the baicalin group,benazepril group and baicalin+benazepril group(P＞0.05).Conclusion Baicalin may delay the progress of renal interstitial fibrosis in UUO rat by down-regulating the TGF-β1 expression and reducing α-SMA expression.

[Objective]To study the effects of Caspase-1 specific inhibitor AC-YVAD-CMK on intimal hyperplasia after carotid artery balloon injury in rats and its possible mechanism.[Methods]A total of 33 male adult SD rats were randomly divided into sham group,balloon injury group and balloon injury+AC-YVAD-CMK group. Using the method of balloon injury to establish rat carotid ar?tery intimal hyperplasia animal model,rats were sacrificed and blood vessels were harvested 14 days after operation. Fifteen vascular segments embedded in OCT and the intima to media(I/M)area ratio of neointima was measured by hematoxylin-eosin(HE)staining;18 vascular segments were harvested and the expression of NLRP3 inflammasome,cleaved-Caspase-1,interleukin(IL)-1βand IL-18 were measured by Western blot.[Results]HE staining showed that AC-YVAD-CMK significantly inhibited the degree of intimal hyperplasia compared with the balloon injury group[(0.78 ± 0.13)vs(1.52 ± 0.14);P=0.000]. The expression of NLRP3 inflamma?some was increased in balloon injury group while the AC-YVAD-CMK attenuates the expression of NLRP3(P=0.009);The expres?sion of cleaved-Caspase-1 was in line with the expression of NLRP3(P=0.000). The levels of pro-inflammatory cytokines IL-1βand IL-18 in balloon injury+AC-YVAD-CMK group were significantly lower than those in the balloon injury group(P=0.000).[Conclusion]AC-YVAD-CMK can attenuate intimal hyperplasia after balloon injury of carotid artery in rats,which might be related to its effect on inhibiting the activation of Caspase-1,which could affect the release of pro-inflammatory cytokine of IL-1βand IL-18.

Objective To explore the clinical accuracy of urine cellular components of RBC,WBC,etc.detected by the Urit-1500 urine analyzer.Methods 1 085 urine samples in this hospital from June to October 2013 were selected and detected by the Urit-1500 urine analyzer and the microscope detection respectively.The results of urine cellular components detected by the urine analy-zer were analyzed.Results In 1 085 urine samples,the coincidence rate of two detection methods in detecting RBC and WBC was 93.8%(1 018/1 085)and 94.0%(1 020/1 085)respectively.In 295 RBC positive urine samples tested by the urine analyzer,the number of negative results tested by microscopy was 49,accounting for 16.6%.In 790 RBC negative urine samples tested by the u-rine analyzer,the number of negative results tested by microscopy was 18,accounting for 2.3%.In 197 WBC positive urine samples tested by urine analyzer,the number of negative results tested by microscopy was 28,accounting for 14.2%.In 888 WBC negative urine samples by urine analyzer,the number of negative results tested by microscopy was 37,accounting for 4.2%.Conclusion De-tecting urine RBC and WBC by using the Urit-1500 urine analyzer should combined with the microscopy and other test indexes and even clinical data to conduct the comprehensive analysis for reducing the misdiagnosis and missed diagnosis as far as possible.
