1.Practice and experience in the implementation of ISO9001

The Hospital of Xinle Sector of Xuhui District in Shanghai started implementing the 2000 version of ISO9001 from September 2000. In doing so it highlighted respectively the following aspects: preparations for the implementation, intensified training, compilation of documents on the quality system, operation of the quality management system, examination and verification of the internal quality system, and examination and verification of attestation. As a result of conscientious implementation, the hospital was issued a certificate for its quality management system in May 2001 by the Quality Attestation Center of the Square & Round Mark Attestation Commission. The hospitals chief experience is that implementation of ISO9001 is a new means of constructing a model of modern medical quality management; it is a prerequisite for the sustained development of a hospital; and it guarantees organizationally the effective operation of medical quality management.
2.A long-term follow up of surgical treatment for a child with melorheostosis: a case report and literature review

Melorheostosis is a rare disorder of osteopathia. The clinical characteristics of melorheostosis in children is totally different from that in adults. The radiographic features of melorheostosis include the hyperostosis in endosteal on the one side of the cavum medullare and formed streakiness. The soft tissue contracture of the limb and joint deformities are the symptoms of melorheostosis. Here, the authors reported a melorheostosis case of 6 years old girl who suffered from severe valgus deformity of the right knee with permanent patellar dislocation. A surgical stabilization was applied with lateral soft tissue release, medial soft tissue stabilization and transferred the vastus medialis laterally (kinetic stability). After 19 years follow up postoperatively, the limb developed well in satisfied alignment with good function of knee joints, even participated in some sports activities. Other authors reported a similar case of melorheostosis with surgical treatment and achieved good outcomes in limb realignment and reduction. The further suggested that the surgical treatment should be produced before epiphyseal closure.
3.The design of plantar pressure distribution monitoring system and preliminary clinical application.

Plantar pressure distribution can reflect the force of several key points on foot while standing and walking. A comprehensive understanding of the plantar pressure distribution makes great sense in the following aspects: the understanding of the normal foot biomechanics and function, clinical diagnosis, measurement of disease extent, postoperative efficacy evaluation, and rehabilitation research. A simple plantar pressure measurement device was designed in this study. This paper uses FlexiForce flexible sensor to pickup plantar pressure signal and USB A/D board to do data acquisition. The data are transferred into a laptop and processed by a VB-based software which can display, remember and replay the data. We chose patients with hallux valgus and normal people to measure the pressure distribution and make contrast analysis of plantar pressure with this device. It can be concluded that people with hallux valgus have higher pressure on the second metatarsophalangeal joint and the distribution move outward. The plantar pressure of patients postoperative could be greatly improved compared to the preoperative. The function of this device has been confirmed.

4.Development of Cerebral Palsy-Gross Motor Function Questionnaire

Objective To work out the Cerebral Palsy-Gross Motor Function Questionnaire(CP-GMFQ) and evaluate the reliability,validity and responsiveness of CP-GMFQ.Methods 160 children with cerebral palsy(CP)(97 males and 63 females;average age: 47.7±20.1 months,range: 18~90 months) were involved.The first 27 children received second assessment to assess test-retest reliability with a interval of 7 to 18 days.Criterion-related validity was evaluated by comparing CP-GMFQ scores with Gross Motor Function Classification System(GMFCS) levels and scores of Gross Motor Function Measure(GMFM).21 children were randomly selected for responsiveness analysis by effect size.After a interval of 3 to 7 months the re-assessment GMFQ and GMFM were tested.30 children were randomly selected for comparative testing time of GMFQ and GMFM.Results GMFQ had excellent test-retest reliability(ICC=0.9940);criterion-related validity was excellent between GMFQ scores and GMFCS levels and GMFM scores(r=0.77~0.92).Effect size of GMFQ(0.34) between GMFM88 and GMFM66,testing time of GMFQ(average 4.7±1.6 min) were significantly less than testing time of GMFM(average 17.7±4.6 min)(P<0.001).Conclusion The reliability,validity and responsiveness of GMFQ are satisfying.The gross motor functions of children with cerebral palsy could be easy and reasonably measured by GMFQ.
5.Correlation between Gross Motor Function and Activities of Daily Living in Children with Cerebral Palsy

Objective To analyze the correlation between gross motor function and activities of daily living(ADL)in children with cerebral palsy.Methods 42 children with CP(26 males,16 females,average age 49.5±21.5 months,11 children with spastic quadriplegia,22 with spastic diplegia,5 with spastic hemiplegia,2 with athetosis and 2 with dystonia)were assessed with Gross Motor Function Classification System(GMFCS)and Gross Motor Function Measure(GMFM)for the assessment and classification of gross motor function.Pediatric Evaluation of Disability Inventory(PEDI)was used to assess the ADL.The ADL of children with different levels of GMFCS and the correlation between gross motor function and ADL were analyzed.Results The more severe of GMFCS,the less standard score of PEDI.There was the highest correlation between the GMFM66 scores and scaled scores of PEDI in mobility and social function(r=0.84),whereas a weaker correlation appeared between GMFM66 scores and scaled scores of PEDI in social function(r=0.72).Conclusion Since the gross motor function of children with CP greatly affects the ADL,the gross motor function training should be the main item for CP children,the training goal of ADL should be made according to the levels of GMFCS.
6.Application of subtalar joint distractor in operative treatment for comminuted calcaneal fracture

Objective To evaluate the method and clinical effect of subtalar joint distractor assisted open reduction and internal fixation of comminuted intra-articular calcaneal fracture.Methods Twenty-two patients with fresh closed calcaneal fracture treated with open reduction and internal fixation from May 2011 to July 2013 were included in the study.According to the modes of operation,the patients were randomly divided into two groups (n =11 in each):in group A patients underwent fracture reduction assisted by subtalar joint distractor and in group B patients underwent Steinmann pin traction and poking reduction.All were fixed using the lateral calcaneal plate.Operation time,B(o)hler angle,Gissanes angle,calcaneus height,incision healing time and complications of the two groups were investigated for evaluating the effect of operation.Clinical effect was evaluated using the American Orthopedic Foot & Ankle Society (AOFAS) score before the removal of internal fixation.Results Period of follow-up was (14.3±0.8)months (range,12 to 18 months).Two patients in group B had delayed wound healing,and were treated with regular wound care.At the final follow-up,no infection,nonunion,malunion and internal fixation failure were observed.B(o)hler angle,operation time and calcaneus height observed in group A were superior to those in group B (P <0.01).There was no significant difference in Gissanes angle between the two groups (P > 0.05).Compared to the preoperative data,B(o)hler angle,Gissanes angle and calcaneus height were significantly improved in both groups (P <0.01).AOFAS score was (83.6 ± 1.4) points in group A and (81.7 ± 1.5) points in group B.Conclusion Subtalar joint distractor assisted open reduction and internal fixation is effective to shorten operation time,improve fracture reduction,reduce wound complications and increase the operative effect for comminuted intraarticular calcaneal fracture.
7.Unilateral percutaneous vertebroplasty with bone cement plus hyperextension position reset for Kümmell disease in the elderly

BACKGROUND:Percutaneous vertebroplasty with bone cement plus hyperextension position reset for acute thoracolumbar vertebral osteoporotic compression fractures has been reported to achieve good clinical efficacy.OBJECTIVE:To evaluate the efficacy of unilateral percutaneous vertebroplasty with bone cement plus hyperextension position reset for Kümmell disease in the elderly.METHODS:Twenty-two old patients with Kümmell disease were included,involving 8 males and 14 females,aged 55-84 years old,and the injuried vertebrae included T11 (n=2),T12 (n=8),L1 (n=7),L2 (n=4),L3 (n=2),L4 (n=2),and L5 (n=1).All patients underwent the unilateral percutaneous vertebroplasty with bone cement plus hyperextension position reset.The visual analogue scale and the Oswestry Disability Index scores,vertebral body height as well as vertebral kyphosis angle were determined before and after treatment.RESULTS AND CONCLUSION:(1) All patients were successfully operated,pain relieved or disappeared within 24 hours postoperatively.No spinal cord injury,pulmonary embolism and other complications were found.One patient presented with cement leakage without obvious clinical symptoms;two patients had non-adjacent vertebral fractures during follow-up.(2) The visual analogue scale and Oswestry Disability Index scores at 1 day postoperatively were significantly lower than those at baseline (P < 0.05).(3) The anterior,medial and posterior height of involved vertebral body,kyphotic angle of involved vertebral body at 1 day postoperatively were significantly higher than those at baseline (P < 0.05).(4) These results manifest that unilateral percutaneous vertebropiasty with bone cement plus hyperextension position reset to treat Kümmell disease in the elderly can significantly relieve back pain,restore partial vertebral height,correct local kyphosis and improve functional recovery of the injured vertebrae.
8.Electroencephalogram and CT Analysis of 224 Children with Cerebral Palsy

Objective To study the application value of electroencephalogram and CT in children with cerebral palsy.Methods Electroencephalogram and CT examinations were carried out in 224 children with cerebral palsy.Results Abnormal prevalence of electroencephalogram was 47.77%, 58.88% of which were found as epileptic discharge. Abnormal prevalence of CT was 79.46%. The abnormal rate of CT was significantly higher than that of electroencephalogram ( P<0.001). The abnormal rates of electroencephalogram and CT among different types of cerebral palsy were not significantly different ( P>0.05).Conclusion Combination of CT and electroencephalogram will be helpful for the diagnosis and treatment in children with cerebral palsy.
9.Radial locking compression plate for treatment of distal radius fracture with dorsal instability

Objective To investigate the clinical efficacy of fixation of distal radius fracture with dorsal instability with locking compression plate (LCP) via radial approach.Methods From September 2009 to October 2012, 21 cases underwent LCP fixation of the distal radius fracture with dorsal instability via radial approach.The study included 14 males and 7 females (mean age, 54.5 years;range, 38-81 years).Twelve cases were injured in traffic accidents, 7 in falls and 2 in high falls.Fracture AO classification was type A3 in 10 cases, type C1 in 8 cases and type C2 in 3 cases.Two cases (one type A3 and one type C1) had malunion.When the lateral column, intermediate column, radial edge and dorsal plane were exposed by radial incision, LCP fixation with bone grafting was performed to restore the height of radial styloid process, ulnar deviation and palmar tilt.According to the GartlandWerley score, wrist joint function was evaluated.Results Mean duration of follow-up was 13.5 months (range, 12 to 24 months).Stage Ⅰ bone union was shown on X-rays, with the healing time of 7.5 weeks.Based on the X-rays at postoperative 6 months, 1 year and 2 years, the shortening of radial styloid process was ≤ 2 mm, mean volar tilt was 12.5°(range, 8°-17°) , mean ulnar tilt was 20.5° (range, 15°-26°), and step-off or gap of the articular surface was ≤ 2 mm.In postoperative wrist motion assay, mean volar flexion was 60°(range, 30°-70°), mean dorsal flexion was 55° (range, 30°-65°), mean radial deviation was 17°(range, 10°-20°), mean ulnar deviation was 25.5° (range, 20°-30°),pronation was 65.5° (range, 60°-70°) , and mean supination was 75.5° (range, 60°-80°).Mean grip strength was 75% (60%-95%)of the contralateral side.Two cases suffered from pain (one complained of moderate pain and one minor pain occasionally).According to the Gartland-Werley score, 16 cases were rated excellent, 2 good, 2 fair and 1 poor, with the excellent rate of 86%.No complication was found as injury of the superficial branch of radial nerve, infection, nonunion, nail loosening, medium neuritis and tendon injury.Conclusions Lateral column, intermediate column, radial edge and dorsal plane can be shown via the radial approach.LCP fixation combined with bone grafting contributes to fracture anatomical reduction.
10.Therapeutic Observation on Treatment of Infantile Cerebral Palsy with Integrative Chinese-Western Medicine: A Clinical Report of 104 Cases

Objective: To study the therapeutic effects of combined treatment with Chinese and western medicine in children with cerebral palsy (CP). Methods: 104 CP children were treated mainly with physiotherapy and acupuncture, together with occupational therapy, sensory integration and electrical stimulation. Gross Motor Function Measure (GMFM) was used to assess the effects of treatment. Results: After treatment for 3.29±0.80 months, the majority of CP children improved their IQ and limb symptoms, with decreased abnormal posture, increased muscle strength and improved motor developmental function. GMFM88 and GMFM66 scores had significantly increased after treatment. Conclusions: Combined treatment with Chinese and Western Medicine has confirmed effects on CP children.
