Objective:To investigate the changes of Th1/Th2 ratio in the PBMC and hepatocellular carcinoma tissues of patients with heptocellular carcinoma and its clinical significance.Methods:Th1/Th2 subsets and cellular factor were detected by ELISPOT and double-sandwich ELISA.Results:Th1/Th2 ratio in the PBMC and hepatocellular carcinoma tissues of patients with heptocellular carcinoma,Th1 sebset expressing cellular factor IFN-?,IL-2,IL-12 were significantly lower,and Th2 sebset expressing cellular factor IL-4,IL-10 were significantly higher than those of the normal control group.PMBC in patients with heptocellular carcinoma producing Th1 sebset cytokins deduced by PHA and IL-12 were significantly higher and producing Th2 sebset cytokins were significantly lower than those in the three month after operation.Conclusion:Th1/Th2 ratio in the PBMC and hepatocellular carcinoma tissues were of imbalance and immune functional status of patients with hepatocellular carcinoma was inhibitory.Therefore,it was nessassory to enhance immune functional status of patients during treatment.
3.Effect of helicobacter pylori infection on hyperdynamic circulatory state and portal hypertensive gastrropathy in liver cirrhotic patients

Objective:To evaluate the effect of Helicobacter pylori infection on hyperdynamic circulatory state and portal hypertensive gastrropathy in liver cirrhotic patients.Methods:Helicobacter pylori infection,IL-1,IL-8,TNF-?,Endotoxin,NO,ET,anti-VacA and anti-CagA levels were determined in cirrhotic patients by using rapid urease test,()~(14)C-urea expiratory test,immunoimpression test,nitrite colorimetric test and ELISA.The parameters of portal venous system flow were determined by doppler ultrasonography.Results:HP prevalence in patients with liver cirrhosis was 77.1%,anti-VacA and antiCagA positive rate was 60.4% in common.IL-1,IL-8,TNF-?,Endotoxin,NO and ET levels in anti-VacA and anti-CagA positive group were significantly higher than in anti-VacA and anti-CagA negtive group.The amount of splenic venous flow(SVF) and superior mesenteric venous flow(SMVF)in anti-VacA and anti-CagA positive group was greater and the amount of portal venous flow(PVF) was smaller than those in negtive group.Conclusion:There was the effect of Helicobacter pylori infection on hyperdynamic circulatory state and portal hypertensive gastrropathy in liver cirrhotic patients.
4.Analysis of the clinical effect of she-saiko-to combined with silymarin on liver fibrosis due to chronic hepafitis B

Objective To evaluate the therapeutic effect of sho-saiko-to silymarin on liver fibrosis due to chronic hepatitis B.Methods 90 patients with liver fibrosis due to chronic hepatitis B virus(HBV) were randomly divided into therapy group and control group.50 patients in therapy group were given sho-saoko-to and silymarin;While 40 patients in control group were given silymarin only,they were all treated for 6 months.The changes were observed including liver function,serum liver fibrosis and virology indexes before and after treatment in two groups.Resuits Indexes of liver function and serum liver fibrosis in therapy group were improved obviously(P<0.05 or P<0.01),while better clearance to HBV is comparable to control group(P<0.05).Conclusion The treatment with sho-saiko-to combined silymarin on liver fibrosis due to chronic hepatitis B had explicit clinical effect.
5.Association of human leucocyte antigen-DRB1 alleles with chronic hepatitis B and hepatic cirrhosis in Han population of Jilin area

Objective:To investigate the association of human leucocyte antigen-DRB1 alleles with chronic hepatitis B and hepatic cirrhosis in Han population of Jilin area.Methods:HLA-DRB1 alleles in 61 patients with chronic hepatitis B,44 patients with hepatic cirrhosis,32 spontaneous rehabilitees after HBV infectin,and 50 healthy controls were analyzed by using polymerase chain reaction/sequence specific primer technique.Results:The frequency of HLA-DRB1*1201-3 allele in chronic hepatitis B group was higher than that in healthy control group(17.21% vs.8.00%,P=0.0427,RR=2.391).The frequency of HLA-DRB1*0701 allele in chronic hepatitis B group and hepatic cirrhosis group was markedly higher than that in healthy controls group(11.48 % vs.2.00%,P=0.006 6,RR=6.35;12.50% vs.2.00%,P=0.004 6,RR=7.00)and in spontaneous rehabilitee group (11.48% vs.1.56%,P=0.0183,RR=8.17;12.50% vs.1.56%,P=0.0136,RR=9.00).No significant differences frequencies of the other HLA-DRB1 alleles were observed among the four groups.Conclusion:Alleles of HLA-DRB1*1201-3 and HLA-DRB1*0701 are closely associated with chronic hepatitis B and maybe susceptible genes or linkage genes of chronic hepatitis B.HLA-DRB1*0701 is also closely associated with hepatic cirrhosis and maybe a susceptible gene or linkage gene-posthepatitis B-cirrhosis in Han population of Jilin area.
6.The clinical significance and detection of p53、c-erbB-2、p21、nm23 oncoprotein expression with immunohistochemical method in gastric cancer tissue

Objective:To evaluate p53,c erbB 2,p21 and nm23 oncoprotein expression in diagnosis and treatment of gastric cancer.Methods:The expression of p53,c erbB 2,p21 and nm23 oncoproteins was detected with immunohistochemical method in 63 surgically resected specimens and its endoscopic biopsy specimens of gastric cancer.Results:The positive rates of p53,c erbB 2,p21 and nm23 oncoprotein expression were 37 6%~46 2%,34 6%~56 8%,37 8%~61 5%,70 3%~30 8% respectively.Oncoprotein expression was not observed in non tumor endoscopic biopsy specimens.The expression of c erbB 2,p21 was correlated with grade of tumor differentiation and the expression of p21,nm23 oncoproteins was correlated with the depth of tumor invasion and clinical different stage,namely tumor metastasis.Positive rates of p53,c erbB 2,p21 and nm23 oncoproteins between biopsy and resected specimens was of coincidence.Conclusion:Determination of p53,c erbB 2,p21 and nm23 oncoprotein expression was might be useful in diagnosis and treatment of gastric cancer,differentiating benign and malignant tumor and clinical stages,of gastric cancer. 7.Detections and relevance of Ghrelin and inflammatory cytokines in peripheral blood of patients with chronic hepatic diseases

Objective:The purpose of this study is to determine levels of Ghrelin,TNF-α,IL-1 and IL-6 in peripheral blood of patients with chronic hepatitis B,liver cirrhosis,and non-alcoholic fatty liver disease which diagnosed at different extent of damage,and based on it to have a further study on the relevance between Ghrelin and inflammatory cytokines in peripheral blood in above diseases.Methods:Ghrelin,TNF-α,IL-1 and IL-6 in peripheral blood were determined by ELISA method.Results:In patients with chronic hepatitis B andhepatic cirrhosis,Ghrelin level increased significantly as compared with that in normal control group (P<0.05,P<0.01).Ghrelin levels in decompensated liver cirrhosis (degree B and C)groups were much higher than in chronic hepatitis B group (P<0.05). Ghrelin level in group of non-alcoholic fatty liver disease decreased significantly as compared with that in normal control group (P<0.01). Ghrelin and inflammatory cytokines,including TNF-α,IL-1 and IL-6 showed a positive relevances in groups of chronic hepatitis B and liver cirrhosis,but not in NAFLD group which showed a negative correlations.Conclusion:Ghrelin has a higher level in peripheral blood of patients with chronic hepatitis B and liver cirrhosis,but it has a lower level in patients with non-alcoholic fatty liver disease,suggesting correlation of Ghrelin level with the occurrence and the procession of chronic liver diseass.
