1.Relationship between aflatoxin and primary liver cancer

This paper summarizes the relationship between aflatoxin and primary liver cancer. The results show that the exposure to aflatoxin is consistent with p53 gene mutation. The variation of AFB1 detoxicate enzyme,AFB1 adduct and p53 gene mutation are conelated with occurrence of primary liver cancer.
2.Comparative study on emergency treatment and selective closed reduction and percutaneous pin fixation for hu-meral supracondylar fracture of Gartland typeⅡand typeⅢin children

Objective To investigate the curative efficacy of fracture with closed reduction and percutaneous Kirschner wire fixation of the simple lateral injury for humeral supracondylar fracture of Gartland type Ⅱ and type Ⅲ in children by the emergency treatment and selective surgery. Methods From March 2010 to March 2012,sixty children with Gartland type Ⅱ and type Ⅲ fresh humeral supracondylar fracture were operated in Xianyang Center Hospital. Six-ty children were randomly divided into two groups:group A by emergency operation 8 hours after injury with closed re-duction method and lateral percutaneous Kirschner wire fixation only,group B initially treated with plast fixation,then undergoing surgery after 3 - 5 days of injury with the closed reduction and percutaneous pin fixation. The operation du-ration,frequency of intraoperative image intensifier,postoperative swelling,pain lasting time after injury and hospitaliza-tion cost were investigated in two groups. Results The average operation duration of group A was(18. 5 ± 12. 3)min, and that in group B was(20. 1 ± 15. 3)min,and there was a statistically significant difference between two groups(P ﹤0. 05). The frequency of intraoperative image intensifier was 6. 2 times on the average in group A,but 7. 3 times in group B,and there was a statistically significant difference between two groups(P ﹤ 0. 05). The postoperative swelling rate on 3,5,7 days after injury in group B was significantly higher than that of group A,and there was a statistically sig-nificant difference(all P ﹤ 0. 05). The duration of pain after injury in group A was(3. 44 ± 1. 23)days,but(5. 26 ± 1. 36)days in group B,which was significantly different;the nerve function recovery time of group A was(36. 5 ± 7. 0) d,and it was(36. 2 ± 7. 0)d in group B,which was not significantly different(P ﹥ 0. 05). The average hospitalization time and cost in group A was less than that in group B,which was significantly different(P ﹤ 0. 05). Finally there was no statistically significant difference in elbow joint function between two groups of children(P ﹥ 0. 05). Conclusions The emergency operation of closed reduction and percutaneous Kirschner wire fixation for the treatment of simple lateral injury in children with Gartland type Ⅱ and type Ⅲ fracture has the advantages over the selective surgery,including shorter operation time,less contact of X - ray intraoperatively,light postoperative swelling,short duration of pain in chil-dren,as well as the shortened hospitalization time,reduced costs,especially good efficacy for joint function recovery.
3.Transabdominal Preperitoneal Herniorrhaphy versus Tension-Free Hernioplasty for Inguinal Hernia in Adult Patients:Prospective Randomized Controlled Study

Objective To compare the curative effect of tension-free hernioplasty and transabdominal preperitoneal herniorrhaphy(TAPP)in adult patients with inguinal hernia.Methods A prospective randomized controlled study.A total of 240 patients with inguinal hernia were randomly divided into TAPP group and tension-free hernioplasty group(open group),120 patients in each.Results In both the groups,the surgical treatments were completed.The mean operation time in TAPP group was significantly longer than that in open group [(77?15)min vs(48?9)min,t=18.147,P=0.000],and the average cost for surgery in TAPP group was significant higher than that in open group [(7363?351)yuan vs(4635?242)yuan,t=72.663,P=0.000].However,the patients in TAPP group returned to activity and was discharged earlier than those in the open group [(6?1)h vs(10?2)h,t=-19.385,P=0.000;and(3?1)d vs(5?1)d,t=-15.300,P=0.000;respectively].No significant difference was detected in the severity of postoperative pain,complications,and recurrence rate between the two groups.Conclusion Both TAPP and tension-free hernioplasty are worth being wildly used.
4.Expression of Poly ADP-Ribose Polymerase-1 in Human Breast Cancer Tissue and Its Relationship with Prognosis Related Factors

Objective To investigate the expression of poly ADP-ribose polymerase-1(PARP-1) in human breast cancer tissue and its relationship with patient's prognosis related factors.Methods The expression of PARP-1 in breast cancer tissues and their adjacent non-cancerous tissues from resection in 66 operated cases were detected using RT-PCR and immunofluorecence staining(IFS).The relationship between the expression of PARP-1 and the prognosis related factors was analyzed.Results Compared with that of the adjacent non-cancerous tissues,the expression of PARP-1 mRNA increased in 94.9%(62/66) patients,while the expression of PARP-1 protein increased in 90.9%(60/66) ones.The label index(LI) and the expression index(EI) of PARP-1 gene increased especially in the cases in later clinical stage,with poor tumor differentiation grade,of pure breast carcinomas or infiltrative breast carcinomas and of metastasis.Conclusion The expression of PARP-1 in human breast cancer tissue on the level of transcription and translation increases,which may be positively related with the patient's prognosis.
5.Expression of Mutant BRCA2 in Human Breast Cancer Tissue and Its Clinic-healing Significance

Objective To investigate the expression of mutant BRCA2 gene in human breast cancer tissue and its clinic-healing significance.Methods The expression of mutant BRCA2 gene in breast cancer tissues and adjacent non-cancerous tissues of 66 patients' samples was detected using immunofluorostainning(IFS)and the correlation between the expression of mutant BRCA2 and patient's clinic-pathological characteristic was analyzed.Results Among 66 patients' samples,87.8%(58/66)were up-regulated expression of mutant BRCA2 protein in breast cancer tissues and 12.2%(8/66)was no significant difference of the mutant BRCA2 expression between the cancer tissue and its corresponding adjacent noncancerous tissues.The label index(LI)of mutant BRCA2 gene was higher in the cases of breast carcinoma's later clinical stage,poor tumor differentiation grade,breast scirrhus carcinomas,infiltrative breast carcinomas and metastasis.Conclusion The overexpression of mutant BRCA2 gene in human breast cancer tissue is positively related to the breast carcinoma's clinical stage,tumor differentiation grade,the tumor pathological type and metastasis,and it may influence the healing therapy of the breast cancer patients.
6.The experience of the diagnosis and treatment of graft-versus-host disease after liver transplantation in 4 case

ObsjectiveTo investigate the clinical data,and to summarize the clinical experience of diagnosis and treatment of graft-versus-host-disease (GVHD) after liver transplantation.Methods 4 of 480 recipients undergone liver transplantation developed GVHD from Apr.2005 to Sep.2011.The 4 recipients'clinical courses and laboratory tests were recorded.ResultsThe diagnosis of GVHD depended on clinical syndrome involved skin rash,bone marrow depression and diarrhea.Skin biopsy and STR-PCR were matched.Among them,2 with successful treatment have been surviving for 7 and 24 months,and 2 died from infection.One recipient had the donor T lymphocyte microchimerism detected by STR-PCR.ConclusionsGVHD after liver transplantation can cause high mortality due to bone marrow depression.A reasonable treatment can be to reduce immunosuppressant and glucocorticoids and IVIG.
7.Determination of lateral needle insertion point in percutaneous Kirschner wire fixation for pediatric supracondylar fractures of humerus

Objective To report our method to determine lateral needle insertion point (intersection point skin marker) in closed reduction and percutaneous Kirschner wire fixation for pediatric supracondylar fracture of humerus.Methods From May 2012 to June 2014,125 children with supracondylar fracture of humerus were treated with closed reduction and percutaneous Kirschner wire fixation.Of them,60 did not use the intersection point method to determine the lateral needle insertion point (group A),including 46 boys and 14 girls,with an average age of 6.3 ±0.6 years.According to Gartland classification,28 cases were type Ⅱ and 32 cases type Ⅲ.In the other 65 children,the intersection point method was used to determine the lateral needle insertion point (group B),including 50 boys and 15 girls,with an average age of 6.4 ± O.9 years.According to Gartland classification,29 cases were type Ⅱ and 36 cases type Ⅲ.The 2 groups were compared in terms of operation time,fluoroscopy times,hospital stay and hospitalization cost.The efficacy was evaluated at the final follow-ups using Flynn criteria.The 2 groups were compatible without significant differences in preoperative general data (P > 0.05).Results All the 125 children obtained successful closed reduction and percutaneous pin fixation,and an average follow-up of 13 months (from 12 to 15 months)as well.There were significant differences between groups A and B in average operation time (23.1 ± 15.3 min versus 17.5 ± 10.3 min) and fluoroscopy times (9.2 ± 1.0 times versus 5.3 ± 1.3 times) (P < 0.05).There was no statistically significant difference between the 2 groups either in Flynn excellent to good rate [(98.3% (59/60) versus 98.5% (64/65)] (P > O.05).Needle irritation occurred in 2 cases and Kirschner wire shift in one in group A while tensile blistering occurred in one in group B.Conclusion In closed reduction and percutaneous Kirschner wire fixation for pediatric supracondylar fracture of humerus,the intersection point skin marker is a simple and reliable method to determine the lateral needle insertion point,leading to considerable reduction in radiographic exposure for both patients and doctors.
8.The clinical effect of dexmedetomidine in early psychosis prevention after liver transplantation

Objective To investigate effects and safety of dexmedetomidine on the prevention of abnormal mental in patients after liver transplantation.Methods 160 patients after liver transplantation were randomly divided into the two groups using the drawing method:midazolam group(n =80),dexmedetomidine group(n =80),compared the two groups of patients with general,mental abnormality and the occurrence of adverse reactions.Results There were no significant differences in the general situation (gender,age,postoperative ventilator time,ICU stay time,oper-ation time,blood FK506 concentration,APACHEII score 24 hours after operation)of the two groups of patients (P >0.05),the spirit of the abnormal rate of the observation group was lower than that of the control group (P =0.009), incidence of hypotension and hypertension in the observation group was higher than that in the control group (P =0.035,0.029).Conclusion Dexmedetomidine can effectively reduce the postoperative mental disorders in patients in the early stage after liver transplantation.
9.Progress of Caveolin and Its Role in Brain

Caveolins are a family of plasmalemmal vesicles caveolae-associated integral membrane proteins and a marker protein of caveolae involved in the formation and localization that associated with vesicular transport, cellular cholesterol homeostasis and signal transduction. Recent years, strong experimental evidences indicated that caveolins play a pivotal role in the brain function such as neural development, synaptic plasticity and neurodegenerative diseases. Recent progress on studies of the structure and functions of caveolins was simply summarized. The regulatory role of caveolins in the brain functions has been reviewed and expected.
10.The influence of epidural analgesia on stress, immune and survival in patients with ovarian cancer undergoing cytoreductive surgery

Objective To explore the influence of epidural analgesia on stress,immune and survival in patients with ovarian cancer undergoing cytoreductive surgery.Methods 72 cases with ovarian cancer undergoing cytoreductive surgery were divided into group A and group B according to random number table method.Patients in group A were treated with epidural analgesia,while patients in group B were treated with intravenous analgesia.The stress index,the plasma cortisol and immune indexes,CD3+,CD4+,CD8+ and CD4+/CD8+ at 30 mins before surgery (T1),right after surgery (T2),12 h after surgery (T3),1 d after surgery (T4) and 3 d after surgery (T5) between the two groups were observed.The overall survival and free survival in the two groups were compared.Results The plasma cortisol levels was lower in group A than in group B (Fgroup=27.784,P=0.000).The plasma cortisol levels in the two groups increased first and then decreased (Ftime=109,268,P-=0.000),and the decrease ratein group A was higher than that in group B (Finteraction,=22.092,P=0.000).The levels of CD3+,CD4+ and CD4+/CD8+ werehigher in group A than in group B,while the level of CD8+ was lower in group A than in group B (Fgroup=5.893,6.110,7.283,5.593,P<0.05).The levels of CD3+,CD4+ and CD4+/CD8+ in the two groups reduced first and then increased while the level of CD8+ increased first and then reduced (Ftime=12.220,14.430,15.592,11.102,P<0.05).CD3+,CD4+ and CD4+/CD8+ in group A increased more while C D8+ reduced more than those in group B (Finteraction=8.251,9.792,8.092,7.103,P<0.05).The overall survival rate and free survival rate of 3 years in group A and group B was 44.44%,22.22% vs 38.89%,16.67%,respectivelyThedifferencehadnostatisticalsignificancebetweenthetwogroups (P>0.05).Conclusion Epidural analgesia may reduce the stress response in patients with ovarian cancer undergoing cytoreductive surgery,and protect the immune function,but the influence on survival is not obvious.
