1.Pull-in small bile duct's choledochojejunostomy in rabbits

Objective To investigate the effect of Pull in choledochojejunostomy which apllied in rabbits' Small bile duct construction on prevention of anastomosis stenosis and potential mechanisms.Methods A total of 21 rabbits were randomly assigned to three groups(n=7).Group A underwent a simple laparotomy (SL),group B biliary-enteric sutured by mucosa to mucosa Choledochojejunostomy (CJ) and group C Pull-in Choledochojejunostomy(PCJ).TBil and DBil were test in 2,4,8 weeks.The tissue of bile duct and anastomotic stoma were collected after Rabbits was killed in 8 weeks.The diameter of bile duct lumen,anastomotic stoma and the thickness of bile duct were measured respectively.Pathological changes of anastomotic stoma were observed and ki67 expression was studied by Immunohistochemical staining.Results (1)Bilirubin level was nomal in group A and C,but significantly higher in group B(P<0.01).(2)Anastomotic stoma in group C was larger than that in group A and even more than that(completely closed) in group B.There was also statistically significant difference in the diameter of anastomotic stoma,C>A>B(P<0.01).The diameter of bile duct lumen and the thickness of bile duct showed the similar results,B>C>A(P<0.01).(3) As for the inflammatory studies,Group B was observed with significant infiltrated neutrophils compared with group C.Furthermore,cytokine studies showed that the expression of Ki67 index around anastomosis was significantly difference among three groups,B>C>A(P<0.01).Conclusions The studies suggested that pull-in choledochojejunostomy which apllied in rabbits' small bile duct reconstruction could offer some beneficial effect in preventing anastomotic stoma stenosis.The mechanism might be through reducing the inflammatory reaction and restraining excessive hyperplasia in the area around anastomotic stoma.
2.Effect of dexmedetomidine combined with sufentanil on postoperative analgesia and the vascular crisis in patients undergoing free flap transplantation

Objective To investigate the effect of dexmedetomidine combined with sufentanil on postoperative analgesia and the vascular crisis in patients undergoing free flap transplantation. Methods Forty patients with ASA physical statusⅠ-Ⅱhaving underwent free flap transplantation were divided into groupⅠand groupⅡaccording to the random digits table method with 20 cases each. The drugs of patient controlled intravenous analgesia (PCIA) in groupⅠwere sufentanil 1.0μg/(kg·d)+tropisetron 5 mg/100 ml, and the drugs of PCIA in group Ⅱ were sufentanil 1.0 μg/(kg · d) +dexmedetomidine 1.0 μg/(kg · d) + tropisetron 5 mg/100 ml. The PCIA pump was set in 2 groups as follows: continuous perfusion rate 2 ml/h, single bolus 2 ml, lockout time 15 min. The visual analogue score (VAS), Ramsay sedation score, press number of PCIA pump within 48 h after operation, satisfaction score after operation, untoward reaction and vascular crisis were recorded. Results The VAS at 6, 12, 24 and 48 h after operation in group Ⅱ were significantly lower than those in group Ⅰ: (2.18 ± 0.41) scores vs. (3.00 ± 0.63) scores, (2.64 ± 0.51) scores vs. (3.82 ± 0.60) scores, (2.55 ± 0.52) scores vs. (3.36 ± 0.51) scores and (2.27 ± 0.47) scores vs. (3.09 ± 0.70) scores, the Ramsay sedation scores were significantly higher than those in group Ⅰ: (2.42 ± 0.51) scores vs. (2.00 ± 0.58) scores, (2.74 ± 0.45) scores vs. (2.11 ± 0.57) scores, (2.58 ± 0.51) scores vs. (2.05 ± 0.52) scores and (2.53 ± 0.51) scores vs. (2.00 ± 0.47) scores, and there were statistical differences (P<0.05). The press number of PCIA pump within 48 h after operation in groupⅡwas significantly lower than that in groupⅠ:(3.75 ± 1.45) times vs. (8.25 ± 2.61) times, the satisfaction score after operation was significantly higher than that in groupⅠ:(2.47 ± 0.51) scores vs. (1.60 ± 0.50) scores, and there were statistical differences (P<0.05). There were no statistical differences in the incidences of untoward reaction and vascular crisis (P>0.05). Conclusions Dexmedetomidine can relieve anxiety and nervousness, reduce postoperative consumption of sufentanil, and decrease the incidence of nausea and vomiting. In addition, dexmedetomidine does not affect the incidence of vascular crisis.
3.Clinical significance of endothelin-1 and transforming growth factor-β in the newborns with respiratory distress.

Objective To measure the serum levels of endothelin-1 (ET-1) and transforming growth factor-β(TGF-β) in the newborns with respiratory distress and investigate its clinical significance. Methods Newborns with respiratory distress hospitalized into the Newborn Intensive Care Unit were included. The serum levels of ET-1 and TGF-β were all detected with ELISA in the first six hours,3,7,14 and 28 days after birth. Results The highest levels of ln ( 1 + ET-1 ) and ln ( 1 + TGF-β) were obtained from newborns with diagnosis as meconium aspiration syndrome ( 1.95 ± 1.02) ng/L and ( 1.51 ±0.99) ng/L,respectively) in the samples obtained in the first six hours after birth, and these were statistically different from those of the control group ( P < 0. 05 ). Following were obtained for newborns with respiratory distress syndrome ( ( 1.52 ± 0.74 ) ng/L and ( 1.13 ± 0. 48 ) ng/L, t = 2.28,2. 13,respectively). After oxygen treatment, ET-1 levels obtained in the first six hours of life decreased gradually in the following days (P <0.05 ). Conclusions The measurements of ET-1 and TGF-β levels will help in differentiating diagnosis of the respiratory distress of newborns. The ET-1 levels will help to assess the therapy effectiveness and prognosis.
4.Interpretation of the 2019 Posterior Vitreous Detachment, Retinal Breaks, and Lattice Degeneration Preferred Practice Pattern

Posterior vitreous detachment (PVD), retinal breaks, and lattice degeneration are common problems in ophthalmic clinical practice, which not only cause disturbance to patients' life-quality, but also increase the risk of retinal detachment and vitreoretinal traction.In September 2019, the American Academy of Ophthalmology published Posterior Vitreous Detachment, Retinal Breaks, and Lattice Degeneration Preferred Practice Pattern (PPP). Based on clinical evidence, this PPP provides authoritative guidance for the definition, epidemiological background, diagnosis and treatment of these diseases.This PPP also gives definite solution for treatment and follow-up of different sub-types.This article provides introduction and interpretation of this PPP.
5.Learning curve study in peripherally inserted central catheter switching operation

Objective To investigate the learning curve of peripherally inserted central catheter (PICC) switching operation, for providing a reference basis for the nurses switching operation. Methods The clinical data of 50 cases of switching operation by one nurse of stomach surgery in PICC outpatient service maintenance center were retrospectively analyzed from July to December in 2013. According to the order of operations, every 10 patients was a learning phase, and they were divided into five stages:group A with number 1 to 10, group B with number 11 to 20, group C with number 21 to 30, group D with number 31 to 40, group E with number 41 to 50. The time and score of switching operation were compared among 5 groups. Results The age of the participants was(49.78±14.87) years, and there were no significant differences in sex, age, tube parts, catheter indwelling time among 5 groups, P>0.05. The time of switching operation were (20.17± 0.82),(17.10± 0.47),(15.15± 0.23),(14.27± 0.15),(13.16± 0.09) min in group A, B, C, D, E. The score of switching operation were (80.00 ± 7.88), (91.60 ± 0.56), (93.10 ± 0.53), (93.20 ± 0.44), (93.90 ± 0.43) points in group A, B, C, D, E. There were significant differences in the time and score of switching operation among 5 groups, F=50.978, 23.787,P<0.01. Between two comparison groups, 20 cases, the time of switching operation appeared inflection point (P<0.05), 10 cases, the score of switching operation appeared inflection point (P<0.05). The time of switching operation of the first 20 cases was (19.04±0.51) min,and which of the later 30 cases was (14.45±0.09) min. The score of switching operation of the first 10 cases was (80.00±7.88) points, and which of the later 40 cases was (92.95±1.72) points. Conclusions After a learning curve of 20 switching operation, nurses can overcome the learning curve and master the skills of dressing change of PICC proficiently.
6.Age-associated electrophysiological changes of rat atrial muscle

Objective To investigate the age-associated changes of electrophysiological properties of atrial muscle and the relationship between these changes and atrial fibrillation (AF). Methods According to their ages forty Wistar rats were divided into 4 age groups: young group, adult group, middle-aged group and old group (n=10 each group ). Hearts were isolated and perfused by Langendorff. MAP (monophasic action potential) of atrial muscle was recorded. 7.Efficacy of Motor-development Massage and Neural Development Approach on Children with Cerebral Palsy

Objective To compare the effect of motor development massage and neural development approach on the children with cerebral palsy. Methods The children with cerebral palsy were selected with layered matching as they accepted motor development massage or neural development approach, and reviewed with their score of Gross Motor Function Measure (GMFM-66). Results There were 24 children in each group. The GMFM-66 score increased after treatment in both groups, but there was no statistical difference between these groups. Conclusion The gross motor function can be improved in the children with cerebral palsy with motor-development massage and neural development approach similarly.
8.Activities of daffy living performance in children with cerebral palsy with different gross motor function

Objective To analyze the activities of daily living (ADL) performance in children with cerebral palsy (CP) with different gross motor function and the correlation between gross motor function and ADL perform-ance. Methods A total of 118 children (81 boys and 37 girls; mean age: 47.7 months, SD: 20.1 months, age range: 18-90 months) with CP were recruited from three different rehabilitation centers in Shanghai. Types of CP included hemiplegia (n = 27), spastic diplegia (n = 53), spastic quadriplegia(n = 29), athetoid (n = 4), dystonic (n = 4), ataxic (n = 1). Gross motor function classification system (GMFCS) and Gross motor function measure (GMFM) were adopted to assess and classify the gross motor function. Pediatric evaluation of disability inventory (PEDI) was adopted to assess ADL performance. Differences of PEDI scores were compared among different levels of GMFCS; and the correlation between GMFCS levels, GMFM scores and PEDI scores were analyzed. Resnlts PEDI scores showed significant differences among CP children with different levels of GMFCS. The PEDI score usually de-creased with the increase of the GMFCS level, and mobility ability was the lowest. Significant correlations were found between PEDI scaled scores and GMFCS (rs = 0.85~0. 50, P < 0.001). Various levels of correlations (r = 0. 85~ 0.54, P<0.001) were found between GMFM scores and PEDI scaled scores. Higher correlations were found between GMFCS levels, GMFM scores and mobility ability. Conclusion ADL performance was significantly different among CP children with different gross motor function. The higher the gross motor function, the better the performance of ADL. Mobility would be improved by promoting gross motor function in CP children. The training of self-care and so-cial communication ability should be emphasized.
9.miRNA-7 inhibits proliferation of human nasopharyngeal 5-8F carcinoma cells via EGFR/PI3K/Akt pathway

AIM: To investigate the relationship of microRNA-7 ( miRNA-7 ) over-expression and epidermal growth factor receptor (EGFR)/phosphatidylinositol kinase-3 (PI3K)/protein kinase B (PKB, also called Akt) pathway in human nasopharyngeal carcinoma 5-8F cells.METHODS:The 5-8F cells were transfected with miRNA-7 mimics (car-rying by Lipofectamine 2000).The expression of miRNA-7 was detected by real-time PCR.The cell proliferation was ana-lyzed by CCK-8 assay.The cell colony-forming capability was determined by cell colony formation test.The expression of EGFR/PI3K/Akt at mRNA and protein levels was examined by real-time PCR and Western blotting.RESULTS:The ex-pression level of miRNA-7 was significantly increased in 5-8F cells compared with negative control ( NC) group and control group ( P<0.01) .The proliferation of NPC 5-8F cells was decreased extremely after tansfected with the miRNA-7 mimics (P<0.01), so did the result of the cell colony-formation test.The expression of EGFR/PI3K/Akt at mRNA and protein levels was significantly down-regulated compared with NC group and control group (P<0.01).CONCLUSION:Over-ex-pression of miRNA-7 significantly inhibits the proliferation and colony-forming ability of nasopharyngeal carcinoma 5-8F cells by down-regulation of EGFR/PI3K/Akt pathway.
10.The Rehabilitation Effects of the Cochlear Implantation on Prelingually Deaf Children with Alba Abnormality

Objective To study the rehabilitation effects of the cochlear implants on prelingually deaf children with alba abnormality.Methods A retrospective analysis of the effects of CIs was conducted in the prelingually deaf children of 11 cases of the children with abnormal alba(the research group) and 18 cases of the children who had normal alba(the control group) at the First Affiliated Hospital of Fujian medical university.All the operations were completed by the same doctor.There were no obvious complications during and after the operation.The cochlear implants were turned on after one month and the prelingually deaf children with extremely severe bilateral sensorineural deafness were trained for speech at the rehabilitation centre.The assessment criteria of the categories of auditory performance(CAP) and speech intelligibility rate(SIR) were used.After six months and twelve months of the operation, the family members were followed who have direct contacts with the children.The evaluation of data between the research and the control groups was administered.Results In the research group, the average level of CAP after six months'' post-operation was 2.41±0.47.But in the control group, the average level was 3.28±0.45.In the research group, the average level of SIR after six months'' post-operation was 1.27±0.44.There were 3 children in the research group at level 2, but in the control group, the average level was 1.89±0.31.Two children in the control group were level 1 while the others were level 2.In the research group, the average level of CAP after twelve months'' post-operation was 4.00±0.43 while only one child at level 3.There was one child at level 5 in the research group, the rest were level 4.There was a statistically significant difference in the average level of the CAP after six months'' post-operation(t=4.983, P<0.05) and in the average level of the SIR after six months'' post-operation(t=4.371, P<0.05).There was no statistical difference in the average level of the CAP after twelve months'' post-operation(t=1.033, P>0.05) and in the average level of the SIR after twelve months'' post-operation(t=0.434, P>0.05).Conclusion There is no significant difference of the rehabilitation effects of post-operation between the prelingually deaf children with alba abnormality and those with normal alba.
