2.Correlation between polymorphism of cytochrome P450 1A2 gene and antidepressant clinical efficacy of duloxetine

Objective To investigate the relationship between C734A and G-2964A polymorphism of cytochrome P450 1A2 gene and clinical efficacy of duloxetine.Methods 223 patients with depression were treated with duloxetine for six weeks.The clinical efficacy was evaluated with the Hamilton rating scale for depression (HAMD) ;single nucleotide polymorphism (SNP) at position C734A and G-2964A of CYP1A2 gene were identified with restriction fragment length polymorphism(RFLPs) ;then one-way ANOVA was adopted to analyze the relationship between SNP and clinical efficacy.Results (1) In 223 patients,the frequency of allele A at locus 734 was 63.64%,while that of allele A at locus-2964 was 26.82%.(2) 220 patient underwent the whole treating course.The conjoint analysis of two locuses indicated that the decreasing ratio of HAMD score of high-activity group,middle-activity group and low-activity after treatment was (56.05± 10.13) %,(66.36± 8.66) % and (73.82± 7.10) % respectively,the differences obtained by pairwise comparison of the three groups were of statistical significance(P<0.01).Conclusion There is close relationship between C734A and G-2964A polymorphism of CYP1A2 gene and clinical efficacy of duloxetine in the treatment for depression.
3.Heart sound classification using energy distribution features extracted with wavelet packet decomposition

Objective To propose a distribution feature extraction algorithm based on wavelet packet coefficients to reconstruct the signal energy sequence for effectively identifying the pathological features of heart sounds,thereby realizing the early screening of heart diseases.Methods The original heart sound signal was decomposed into 10 layers using wavelet packet decomposition algorithm.After obtaining the wavelet packet coefficients of each layer,each coefficient was reconstructed,and the energy of the reconstructed signal was calculated and arranged in the original order to form the energy sequence.The distribution characteristics of the energy sequence of the reconstructed signals at each layer were analyzed,and distribution features were taken as classification features.Support vector machine,K-nearest neighbor,and decision tree were used to classify and recognize normal heart sounds and the heart sound signals of various diseases.Results The combination of the distribution features of the reconstructed signal energy sequence and decision tree classifier had an accuracy of 93.6%for classifying 5 types of heart sounds on the public dataset,and the accuracy was 95.6%for identifying normal heart sounds and hypertrophic cardiomyopathy heart sounds.Conclusion The proposed algorithm can extract the effective pathological information of abnormal heart sounds,providing a reference for clinical cardiac auscultation.
4.Application of meticulous nursing based on enhanced recovery after surgery in patients after meningioma surgery

Objective:To explore the application of meticulous nursing based on enhanced recovery after surgery (ERAS) in patients after meningioma surgery.Methods:From June 2020 to February 2021, convenience sampling was used to select 124 meningioma patients with surgery in the Neurosurgery Department of Henan Cancer Hospital as the research object. The patients were divided into the observation group and the control group, with 62 cases in each group. The control group was given routine nursing during the perioperative period of neurosurgery, and the observation group conducted meticulous nursing based on ERAS on the basis of the control group. This study compared the early postoperative ambulation between the two groups of meningioma patients, including the time to get out of bed for the first time, the time of first ambulation, and the distance to first ambulation. The Self-rating Anxiety Scale (SAS) , Self-rating Depression Scale (SDS) and the Barthel Index for activities of daily living (ADL) were used to compare the anxiety, depression and ADL of the two groups of meningioma patients before and after nursing care. The incidence of complications within 7 days after the surgery of the two groups of meningioma patients was compared.Results:The time to get out of bed for the first time, time of first ambulation, and the distance to first ambulation in the observation group were better than those in the control group, and the differences were statistically significant ( P<0.01) . The ADL scores of patients of the observation group after nursing were higher than those of the control group, and the scores of SAS and SDS were lower than those of the control group, and the differences were statistically significant ( P<0.01) . The incidence of complications in the observation group within 7 days after surgery was 9.7% (6/62) , which was lower than 27.4% (17/62) in the control group, and the difference was statistically significant ( P<0.05) . Conclusions:ERAS-based meticulous nursing can improve early postoperative ambulation in patients with meningioma, help relieve patients' anxiety and depression, increase patients' ADL, and reduce postoperative complications, which is worthy of clinical application.
