1.Assay and counter measure for the complications of the prosthetic disc nucleus-pulposus(PDN)replacement

[Objective]To analyze the reasons and counter treatment of complication after prosthetic disc nucleus implantation.[Method]From Feb 2002 to Aug 2004,80 patients of lumbar disc herniation were implanted with one single PDN device at our centre.65 patients of them came to the clinical,functional,and radiographic follow-up examinations,magnetic resonance images and follow-up records of all patients were reviewed carefully.They were divided into three groups.Group A:30 cases had at least 24 months follow-up data.Group B:23 cases had at least 36 months follow-up data.Group C:12 cases had at least 48 months follow-up data.[Result]After implantation,device migrations were noticed in 3 patients.24 cases experienced low back pain.In 24~48 month follow-up,the MR images of 39 patients had shown the abnormal signal pattern of the endplate where PDN-solo was implanted,even 32 of them had been found subsidence.In Group A,the height of intervertebral disc at the 24 month follow-up was decreased 4%.In Group B,at 36 month follow-up,it was decreased 12%.In Group C,at 48 month follow-up,it was decreased 18%.[Conclusion]After follow-up,the clinical effect of PDN is preferred.However,the complications of device dislocation,device subsidence remain problems that must be highlighted.This study suggested that the complications of PDN are serious.Therefore,the proper selecting patients for PDN replacement are crucial for good results.The indications for PDN must be restricted.
2.Measuring human arm motion parameters based on high-speed camera.

A sensing method based on high-speed camera is proposed to recognize human arm motion in this paper. A sensing system for human arm motion was established. A fast image processing algorithm was developed to accurately extract marker positions in the image. Angle parameter results were further improved with the instantaneous joint center principle. The human motion information results can serve as the research references of medical treatment, gym, bionics, and so on. The sensing method can also be applied to other fields of the human motion recognition.

4.Biomechanical evaluation of atlantoaxial transarticular screw fixation technique.

OBJECTIVE: To compare the biomechanical differences among the atlantoaxial transarticular screw fixation (Magerl) and other posterior fixation techniques. METHODS: Seven preserved atlantoaxial-complex specimens were harvested and fixated with Magerl, Magerl plus Gallie wiring, Magerl plus Brooks wiring, pure Brooks, pure Gallie, and Halifax interlaminar clamping fixation, respectively. The torque for every fixation technique was measured at the point of five-degree rotation of the atlantoaxial joint. RESULTS: The torque for Magerl was 6.59 Nmplus minus1.14 Nm, which was significantly higher than any other pure posterior techniques including Gallie (1.74 Nmplus minus0.31 Nm). Brooks (4.06 Nmplus minus0.48 Nm) and Halifax (3.44 Nmplus minus0.87 Nm) (P<0.01), but less than Magerl plus brooks (9.94 Nmplus minus1.45 Nm) (P<0.01). No statistically significant difference was found between Magerl and Magerl plus Gallie wiring (7.61 Nmplus minus1.10 Nm) or between Brooks and Halifax. CONCLUSIONS: Compared with other pure posterior fixation techniques the atlantoaxial transarticular screw fixation technique provides more torsion-resistance capacity. It is also suggested that combined Gallie wiring do not add any biomechanical superiority to this technique.
5.The study of HPV infection genotyping in vulva condyloma acuminate tissues of 691 women

Objective To explore the clinical distribution states of human papillomavirus genotypes in tissues of 691 women with vulva condyloma acuminates in Nanjing city and Zhenjiang city in Jiangsu Province and genotyping clinical significance.Methods Polymerase chain reaction(PCR)and gene-chips technology were utilized for the detection of 23 kinds of HPV genotypes in tissue specimens from 619 women of vulva condyloma acuminates in Nanjing city and Zhenjiang city in Jiangsu Province.And related materials of all subjects were analyzed.Results In 691 women of vulva condyloma acuminates,597 women of HPV infecton,total infection rate of HPV was 86.40%(597/691),including single genotype infection rate of HPV was 51.38%(355/691),11、6 and 16 genotypes are the most common in single genotypes,they are successively 51.55%(183/355)、41.97%(149/355)and 3.38%(12/355).multiple genotypes infection rate of HPV was 35.02%(242/691),6+11、11+18、6+16 and 11+16 genotypes are the most common in multiple genotypes,they are successively 9.92%(24/242)、9.09%(22/242)、4.96%(12/242)and 4.13%(10/242).Conclusion The low-risk HPV types are the main factors to cause the female vulva CA,a few high-risk HPV types may cause warts as well in tissues of women with vulva condyloma acuminates in Nanjing city and Zhenjiang city in Jiangsu Province.The vulva examine of HPV types should be held to the vulva CA patients.This precaution will has extremely important meaning to the prevention and treatment of the female vulva CA and cervical lesion in our nation.
6. Effect of minimally invasive drilling closed drainage on the expression of HIF-1α, Claudin-5, TGF- β and Smad2/3 in patients with chronic subdural hematoma

Objective:
To investigate the effects of single-hole and double-hole drilling and closed drainage on the expression of Hypoxia inducible factor-1α (HIF-1α), Claudin-5, transforming growth factor-β (TGF-β) and Smad2/3 in the epidermis of patients with chronic subdural hematoma.
Methods:
100 patients with chronic subdural hematoma were randomly divided into single hole and double hole drainage group according to random number table. Immunohistochemical streptavidin-peroxidase (SP) was used to detect the expression of TGF- β protein, and Western blot was used to detect the expression of HIF-1α, Claudin-5 and Smad2/3 in the epihematoma of the two groups before and after operation.
Results:
Compared with pre-operation, the expression of HIF-1α, TGF-β and Smad2/3 protein in adventitia of hematoma in both groups decreased after operation, especially in the double-hole group (
7.The basic design of CBL teaching course in clinical practice on clinical treatment options

Taking case discussion of clinical treatment options of primary liver cancer during clinical internship as an example, the course design of basic elements of case-based learning (CBL) was discussed. The purpose of clinical practice is to cultivate the clinical thinking ability of "selecting the best one from multiple treatment options", which is suitable for taking CBL teaching method. The design of CBL course includes 8 elements: teaching object, purpose, objectives, course content, implementation plan, key points for assessment, course evaluation and reference materials. The core points of the design of CBL course are that: ①The teaching objectives include knowledge, ability and professionalism; ②The course content should includes the training of decision-making organizations and clinical thinking ability of selecting the best one from multiple treatment options; ③The general CBL teaching procedure can be adopted in the implementation of the scheme that focuses on the decision-making issues defined in each decision-making step. The teaching practice of CBL on primary liver cancer cases discussion shows the basic design of CBL course is universal, which is helpful for teachers to design and implement CBL course on clinical treatment options.
8.Application of cypropofol and propofol combined with low-dose alfentanil in gastroenteroscopy

Objective To investigate the anesthetic effects and adverse effects of cypropofol and propofol combined with alfentanil,respectively,for gastroenteroscopy.Methods A total of 162 patients who underwent elective gastroenteroscopy at the Gastrointestinal Endoscopy Center of the First Hospital of Lanzhou University from January to February 2024 were enrolled,including 86 males and 76 females,at an age of 18~65 years old,with a BMI value of 18~30 kg/m2,and ASA grade ≤ Ⅱ.They were randomly divided into propofol group(Group P)and cypropofol group(Group C),with 81 cases in each group.All patients were sedated with 0.7 μg/kg alfentanil,and in 30 s later,2 mg/kg propofol and 0.4 mg/kg cypropofol was intravenously dripped into Group P and Group C,respectively.When the modified alertness/sedation score(MOAA/S)≤1,a gastroscope was started to insert.The related indicators,including total procedure time,successful cases of sedation,induction time and awakening time,heart rate,blood pressure,and pulse oximetry saturation were recorded,occurrence of adverse reactions such as hypotension,respiratory depression,injection pain,intraoperative body movement,nausea and vomiting were observed,and the satisfaction of endoscopists and of patients to anesthesia were recorded and compared between the 2 groups.Results There were no statistical differences in the success rate of sedation,induction time and awakening time between the 2 groups.The patients of the Group C had more stable intraoperative vital signs,statistically lower incidences of injection pain,respiratory depression and hypotension(P<0.05),and increased satisfaction for anesthesia(P<0.05)when compared with those in Group P.No obvious difference were observed in the satisfaction of endoscopist to anesthesia between the 2 groups.Conclusion In combination with small-dose alfentanil,0.4 mg/kg cypropofol shows similar sedation effect as 2 mg/kg propofol in gastroenteroscopy,with comparable induction and awakening time.Cypropofol has more advantages in stable intraoperative vital signs,less adverse effects such as low blood pressure,respiratory depression and injection pain,higher the patient satisfaction,which is worthy of clinical promotion.
9.Clinical effects of free anterolateral thigh perforator flaps in repairing diabetic foot ulcers under a multi-disciplinary team cooperation model

Objective:To explore the clinical effects of free anterolateral thigh perforator flaps in repairing diabetic foot ulcers (DFUs) under a multi-disciplinary team (MDT) cooperation model.Methods:The study was a retrospective observational study. From June 2018 to December 2022, 49 DFU patients who met the inclusion criteria were admitted to the Department of Hand and Foot Microscopy and Wound Repair Surgery of Henan Provincial People's Hospital (People's Hospital of Zhengzhou University), including 28 males and 21 females, aged from 47 to 68 years, with type 2 diabetes history period ranging from 6 months to 21 years. Under a MDT cooperation model, the physicians from department of endocrinology comprehensively assessed the patients, stabilized the patients' general condition, and controlled their complications, the surgeons from department of vascular surgery assessed and improved the patients' lower limb blood supply, the physicians from department of infectious diseases provided anti-infection treatment plans, the physicians from department of anesthesiology and perioperative medicine assessed the patients' perioperative risk and ensured their perioperative safety, and according to the patients' condition, the physicians from departments such as cardiology, neurology, nutrition, and rehabilitation actively and timely participated in the treatment. The surgeons from department of hand and foot microscopy and wound repair surgery prepared the wound base and used free anterolateral thigh perforator flaps to repair the wounds. After once or multiple debridement in the first stage, the wound area ranged from 5.0 cm×4.5 cm to 17.0 cm×10.0 cm. After once or twice vacuum sealing drainage treatment, the free anterolateral thigh perforator flaps were used to repair the wounds with incision area of 6 cm×5 cm to 18 cm×11 cm in the second stage. The descending branches of lateral circumflex femoral artery and the accompanying veins of flaps were anastomosed to the arteries and veins in the recipient sites, respectively. The wounds in the flap donor sites were sutured directly. After surgery, whether the patient's perioperative period was stable, the survival of flaps, the healing of wounds in the flap donor and recipient sites were observed. During the follow-up, the texture and appearance of flaps, whether there was a new ulcer, and the patient's walking ability were observed.Results:All the patients had stable perioperative period. Among them, the flaps in 46 patients survived successfully; the flaps in 2 patients developed complete necrosis, including 1 case whose ulcer was healed after repair of pedicled flap from the lower leg, and 1 case who underwent amputation of the lower leg; the flap in 1 patient developed partial necrosis, which was healed after dressing change and skin grafting. The wounds in the flap donor and recipient sites healed well. During the postoperative follow-up of 6-24 months, the flaps had good texture and appearance with no new ulcers, and the patients had no obvious impairment in daily walk.Conclusions:The MDT cooperation model can sufficiently ensure the perioperative safety of DFU patients. The free anterolateral thigh perforator flaps can repair the DFU wounds achieving good clinical effects with high flap survival rate and decreased amputation rate.
10.Molecular Mechanism of Action of Astragaloside Ⅳ in Modulating Pyroptosis to Attenuate Intestinal Ischemia-reperfusion Injury Based on Experimental Validation

ObjectiveTo investigate the mechanism of astragaloside-Ⅳ (AS-Ⅳ) in regulating pyroptosis to alleviate intestinal ischemia-reperfusion injury (IRI) by combining network pharmacology and in vivo experiments. MethodFirstly, the corresponding target genes of AS-Ⅳ were obtained from TraditionalChineseMedicineSystemsPharmacology(TCMSP) database and Swiss Target Prediction database, and the target genes related to intestinal IRI and Pyroptosis were obtained from GeneCards database, and the common target genes of the three were obtained by drawing Venn diagrams through unspiralized website. Protein-protein interaction (PPI) network was constructed by STRING database and Cytoscape software to screen common target genes and imported into Cytoscape software to obtain core target genes. Microbiotics platform was used for gene ontology(GO) and Kyoto encyclopedia of genes and genomes(KEGG) enrichment analysis and prediction of the mechanism of action of AS-Ⅳ in regulating Pyroptosis to alleviate intestinal IRI. Then C57/BL6J mice were randomly divided into 5 groups normal group, model group(IR), drug administration group (IR+AS-Ⅳ), nucleotide-binding oligomerization structural domain-like receptor protein 3 (NLRP3) agonist NSS group (IR+AS-Ⅳ+NSS), and NLRP3 inhibitor MCC950 group (IR+AS-Ⅳ+MCC950) by using a randomized numerical table method. The intestinal IRI model was established by clamping the superior mesenteric artery for 45 min and resuming perfusion for 2 h in the model group, the drug administration group, the NLRP3 agonist NSS group, and the NLRP3 inhibitor MCC950 group, and the normal group was only separated from the vessels without clamping. The administration group, the NLRP3 agonist NSS group, and the NLRP3 inhibitor MCC950 group were gavaged with astragaloside dissolved in 0.1% dimethylsulfoxide (50 mg·kg-1) for 3 consecutive days before modeling, with the last gavage 2 h before modeling, and the remaining two groups were gavaged with equal amounts of saline. The NLRP3 agonist NSS group was injected intraperitoneally with 4 mg·kg-1 of NSS 1 h before modeling, and the NLRP3 inhibitor MCC950 group was injected intraperitoneally with 10 mg·kg-1 of MCC950 1 h before modeling.The mice were put to death by reperfusion for 2 h, and intestinal tissues were obtained. The levels of IL-18 and IL-1β were detected by enzyme linked immunosorbent assay(ELISA), and the protein expression of thioredoxin-binding protein (TXNIP), NLRP3, Caspase-1 and pyrocatechin D (GSDMD) were detected by Western blot, and the pathological changes of intestinal tissues were evaluated by Chiu's score. ResultNetwork pharmacological analysis showed that there were 1599 targets of intestinal IRI, 199 targets of AS-Ⅳ action, 197 targets of pyroptosis, and 20 targets common to all three. There were 10 core targets, including NLRP3, TXNIP, silencing information regulator 1 (SIRT1), high mobility group protein 1 (HMGB1), interleukin-18 (IL-18), GSDMD, and metallo matrix protease-9 (MMP-9),et al. The results of in vivo experiments showed that compared with the normal group, Chiu's score was elevated in the model group, the levels of IL-18,IL-1β inflammatory factors in mouse intestinal tissues were elevated (P<0.05), and the protein expression levels of TXNIP, NLRP3, Caspase-1, and GSDMD were elevated (P<0.05). Compared with the model group,Chiu's score was decreased in the administered group and NLRP3 inhibitor MCC950 group,the level of IL-18,IL-1β inflammatory factors in the intestinal tissue of mice was decreased(P<0.05), and the level of TXNIP,NLRP3,Caspase-1,GSDMD protein expression was decreased(P<0.05). Compared with the administered group, Chiu's score was elevated in the NLRP3 agonist NSS group, the levels of IL-18, IL-1β inflammatory factors in mouse intestinal tissues were elevated (P<0.05), and the protein expression levels of NLRP3, Caspase-1, and GSDMD were elevated (P<0.05). Compared with the NLRP3 inhibitor MCC950 group, the NLRP3 agonist NSS group had elevated Chiu's scores, elevated levels of IL-18,IL-1β inflammatory factors in mouse intestinal tissues (P<0.05), and elevated levels of TXNIP,NLRP3, Caspase-1, and GSDMD protein expression (P<0.05). ConclusionNetwork pharmacological predictions were consistent with the results of in vivo experiments, and astragaloside attenuated intestinal ischemia-reperfusion injury by inhibiting cellular pyroptosis through the TXNIP-NLRP3 signaling pathway.
