1.Titin and Ryanodine receptor epitopes are expressed in thymoma with myasthenia gravis patients

Objective To study the significance of Titin-ab and Ryanodine receptor-ab (RyR-ab) in the myasthenia gravis (MG) patients and the expression of Titin and RyR epitopes in thymoma of myasthenia gravis.Methods Using ELISA methods,the titer of the Titin-ab,RyR-ab in the sera of 62 patients with MG,as well as 45 cases non-MG with other neurologic disorders and 50 case of normal controls were determined,and Titin and RyR were studied with immunohistochemistry stain in the 19 samples of thymic tissues from 9 cases of MG with thymoma (MGT),6 cases of MG with thymic hyperplasia (MGH), 2 cases of MG with thymic atrophy (MGA),and 2 cases of non-MG with thymic carcinoma (NMGTC). Results The positive rate of Titin-ab in MG was 35.5% (22/62),with the highest being 82.3% (14/17) in MGT group.The positive rate of RyR-ab was 24.2% (15/62),the highest being 76.5% (13/17) in MGT group.Titin receptor epitopes were expressed in the transmembrane and cytoplasm region of thymoma epithelial cells of 7 cases of MGT patients,and RyR epitopes in the transmembrane region of thymoma epithelial cells of 6 cases of MGT;but no Titin nor RyR epitopes was identified in controls and such thymic pathological patterns,as MGH,MGA,NMGTC.Conclusions Titin-ab and RyR-ab are mostly found in MGT patients;Titin and RyR epitopes are expressed in neoplasm epithelial cells of thymoma with myasthenia gravis;it's a result of autoimmunization of Titin and RyR epitopes irritated by Titin and RyR specific T cells activated by the change of pathogenic microenvionment inside the thymoma.
2.Fixation of proximal clavicle and sternum through the intramedullary cavity for sternoclavicular joint dislocation with wire rope:an anatomic and clinical research

BACKGROUND:Surgical management of sternoclavicular joint dislocation takes relatively larger risks for its adjacent mediastinal and other important structures. However, the surgical methods are varied and remain controversial.
OBJECTIVE:To investigate the feasibility and validity of intramedul ary fixation of the proximal clavicle and sternum for sternoclavicular joint dislocation.
METHODS:On 22 adult cadaver upper limb specimens, the anatomical features of the sternoclavicular joints and associated ligaments, the size of articular surface between the medial clavicle and manubrium and the morphology of articular disc, costoclavicular ligament and sternoclavicular ligament were observed and measured. From January 2009 to July 2012, three patients with sternoclavicular joint dislocation received the intramedul ary fixation with wire rope or absorbable suture in figure-of-eight shape. They were males, aged 9, 45 and 62 years, with an average age of 38.6 years. The outcome was evaluated by upper limb function DASH score.
RESULTS AND CONCLUSION:Anatomic results demonstrated that the anteroposterior and coronal diameters of the surface of the medial clavicle were greater than that of the sternum, so they did not match each other. The minimum thickness of the manubrium sterni was at the superior margin of articular surface and the largest was at the sternal angle. The minimum width of the manubrium sterni was at the superior margin of articular surface and the largest was at the inferior margin. The articular disc, liked an el ipse and matched with the articulating surface of the manubrium sterni more, whose anteroposterior diameter was greater than its coronal diameter. Central thickness was greater than the edge thickness. The costoclavicular ligament was thicker and denser than the anterior and posterior sternoclavicular ligament. The anterior and posterior sternoclavicular ligaments were close in length, width and thickness. Clinical trial results demonstrated that al cases were fol owed up for 14 to 36 months. DASH score was 10-16, 13.3 in average. Effects of internal fixation were satisfactory. Surgical management of sternoclavicular joint dislocation by the fixation of the proximal clavicle and sternum through the intramedul ary cavity with wire rope or absorbable suture is reliable and effective.
3.Clinical observation on warm needling moxibustion plus tuina for cervical spondylosis of vertebral artery type

Objective:To observe the clinical effect of warm needling moxibustion plus Kai Qing Long Suo tuina therapy (opening the Qing Long lock,one type of'Eight and a Half Locks' tuina therapy) for cervical spondylosis of vertebral artery type (CSA).Methods:Sixty patients with CSA were randomly allocated into an observation group or a control group,with 30 cases in each group.The observation group was treated with warm needling moxibustion plus Kai Qing Long Suo tuina therapy,while the control group was treated with warm needling moxibustion alone.Warm needling moxibustion was conducted once every other day and tuina was conducted once a day,7-day treatments for one course.The clinical efficacy and vertebral artery blood flow was observed after one course of treatment.Results:After treatment,the total effective rate was 93.3% in the observation group versus 80.0% in the control group,and there was a significant difference between the two groups (P<0.05).After treatment,the systolic blood flow velocity of vertebral artery increased in both groups,with statistical significance compared with that before treatment (both P<0.05),and the blood flow velocity in the observation group was faster than that in the control group,with statistical significance between the two groups (P<0.05).Conclusion:Both warm needling moxibustion plus Kai Qing Long Suo tuina therapy and warm needling moxibustion alone are both effective for CSA,can improve the systolic blood flow velocity of vertebral artery.The curative effect of warm needling moxibusiton plus Kai Qing Long Suo tuina therapy is better than that of warm needling moxibustion alone.
4.Discussion on adverse reactions monitoring modes of drug manufacturers under new measures for administration of adverse drug reaction report and monitoring.

OBJECTIVETo discuss the modes for smooth progress of ADR monitoring under the new Measures for the Administration of Adverse Drug Reaction Report and Monitoring.
METHODWork modes for ADR monitoring in drug manufacturers were explored by explaining the new Measures and analyzing current state and constrains.
RESULT AND CONCLUSIONAs there is a larger gap between the requirements of new Measures and current status, it is difficult for drug manufacturers to meet all the requirements in short-term. Therefore, drug manufacturers are suggested to gradually complete ADR monitoring under the mode of one platform and four expansions, and thereby finally meeting the requirements of new Measures and fulfilling their duties and missions.

5.Simultaneous Determination of 7 Components in Kuhuang Injection by UPLC-MS/MS

OBJECTIVE:To establish a method for the content determination of matrine,sophocarpine,emodin,rhein,chloro-genic acid,sarkosaponin a,aloe-emodin in Kuhuang injection.METHODS:UPLC-MS/MS was adopted. The determination was per-formed on Phenomenex Kenetix C18 column with mobile phase consisted of methanol-0.1% formic acid(gradient elution)at the flow rate of 0.5 mL/min. The column temperature was set at 20 ℃,and injector temperature was set at 10 ℃. The equilibrium time was 4 min and sample size was 5 μL. Ionization mode was electrospray ionization with spray voltage of 5500 V and -4500 V,at-omizing air pressure of 4.14×105 Pa,heater pressure of 4.48×105 Pa,curtain air of 1.72×105 Pa and ion source temperature of 600 ℃. The work mode was multiple reaction monitoring mode. RESULTS:The linear range were 1.25-80.0 ng/mL for matrine (r=0.9993),1.10-70.0 ng/mL for sophocarpine(r=0.9995),0.16-10.5 ng/mL for emodin(r=0.9993),2.61-168 ng/mL for rhe-in (r=0.9993),1.50-96.0 ng/mL for chlorogenic acid (r=0.9993),1.48-94.5 ng/mL for sarkosaponin a (r=0.9996) and 6.11-391 ng/mL for aloe-emodin(r=0.9991). The limits of quantification were 0.061,0.109,0.041,1.313,0.500,0.492,3.055 ng/mL,limits of detection were 0.025,0.054,0.016,0.656,0.150,0.148,1.528 ng/mL. RSDs of precision,stability and reproducibility tests were all no more than 3%. The recoveries of them were 95.45%-99.45%(RSD=1.43%,n=6),97.50%-101.00%(RSD=1.50%,n=6),95.67%-101.73%(RSD=2.85%,n=6),97.17%-100.57%(RSD=1.16%,n=6),95.19%-98.90%(RSD=1.71%, n=6),95.38%-103.85%(RSD=3.39%,n=6),95.50%-101.17%(RSD=1.20%,n=6),respectively. CONCLUSIONS:The method is simple,effective,accurate,reliable and suitable for simultaneous determination for 7 components in Kuhuang injection.
6.Expression and clinical significance of chemokine receptor 4 in oral squamous cell carcinoma.

OBJECTIVETo study the expression of chemokine receptor 4 (CXCR4) protein and mRNA in oral squamous cell carcinoma (OSCC) and to evaluate the relationship between the expression of CXCR4 protein and the clinicopathology factors, and lymph node metastasis of OSCC.
METHODSThe expression of CXCR4 protein in 64 cases of OSCC was examined by immunohistochemical technique (SABC), and the expression CXCR4 mRNA in 40 cases of OSCC studied by in situ hybridization, and its relation to cliniopathological parameter was also analyzed.
RESULTSCXCR4 mRNA and CXCR4 were positively expressed in OSCC, and the positive expression rate of CXCR4 protein was 62.5% (40/64). CXCR4 expression was significantly higher in the patients with lymph node metastasis than those without lymph node metastasis (P = 0.0144), and was also correlated with tumor histological type (P = 0.0042), depth of invasion (P = 0.0023)and TNM stage (P = 0.0012), but not with age at surgery, gender and tumor size.
CONCLUSIONCXCR4 is highly expressed in OSCC, and its expression is associated with the diverse progression of OSCC, including invasion and lymph node metastasis.

7.A new secoiridoid from the flowers of Jasminum officinale L. var. grandiflorum.

To study the chemical constituents of the flowers of Jasminum officinale L. var. grandiflorum, the compounds were isolated and purified by HPLC, recrystallization and chromatography on silica gel and Sephadex LH-20 column. Their structures were elucidated on the basis of physicochemical properties and spectral analysis. Six secoiridoids were identified as jasgranoside (I), jaspolyoside (II), 8-epi-kingiside (III), 10-hydroxy-oleuropein (IV), 10-hydroxy-ligstroside (V), oleoside-7, 11-dimethyl ester (VI). Compound I is a new compound. Compounds II, III, IV, V and VI were isolated from Jasminum officinale L. var. grandiflorum for the first time.

8.Study on the expression of serum bone γ-carboxyglutamic acid-containing protein and osteoprotegerin in skeletal fluorosis patients of coal-burning pollution regions

Objective To explore the mechanism of skeletal fluorosis via observation on the expression of bone γ-carboxyglutamic acid-containing protein (BGP) and osteoprotegerin (OPG) in the residents of coal-burning fluorosis regions, and to provide a basical data for further monitoring and evaluating the effects of fluoride-reducing projects. Methods Stratified sampling was applied, the 6 villages of fluorosis were chosen as focusing areas for investigation. The residents of villages underwent clinical examination of the skeletal fluorosis. And according to the degree of skeletal fluorosis, villages were divided into three groups, namely light, moderate and severe villages. Radio-Immunoassay and Enzyme-linked immunosorbent assay were used to check the expression of BGP and OPG in the serum. Results The levels of serum BGP in the severe skeletal fluorosis cases[(6.78±4.43)μg/L] were significantly higher(P<0.05) than those in the normal, moderate and the severe groups [ (3.58±1.53), (3.44±2.66), (3.41±2.20)μg/L], respectively. The expression of OPG in the light, moderate and the severe groups [(1251.55±998.31), (1265.94±931.77), (1560.55±858.07)ng/L] were significantly higher (P<0.05) than those in the normal[(520.81±385.05)ng/L], respectively. The levels of BGP in mixed type[(6.09±2.62)μg/L] were much higher(P<0.05) than no mixed type[(3.97±1.53), (3.20±2.12)μg/L]. The levels of OPG in the osteosclerosis, osteoporesis and mixed type[(1321.63±1017.00), (1205.42±852.22), (1529.01±402.83)ng/L] were significantly higher(P<0.05) than those of the normal, respectively. The levels of OPG in the light villages [(452.06±338.10)ng/L] were significantly lower(P<0.05) than moderate and severe villages[(1266.30±899.14), (1851.80±956.08 )ng/L], respectively. The levels of OPG in the severe villages were significantly higher(P<0.05) than moderate villages. Conclusions It indicates that OPG can be used as an early indicator in coal-burning pollution endemic fluorosis results in biochemical changes in the composition of bone.
9.Outcome analysis of screening on high arsenic water source in Jingzhou City, Hubei Province

Objective To survey the arsenic content of drinking water in Jingzhou City, to provide basis for prevention and control of endemic arsenic disease. Methods According to historical data, the .arsenic content of water was detected in 357 villages from 6 counties of Jianglin, Songzi, Gongan, Shishou, Jianli, Honghushi in Jinzhou City in 2007 and 2008, The past have been found to have high arsenic water villages, villages known to have high concentration of arsenic were put into census. Villages not found to have high-arsenic wells were sampled 10 percent of the whole water resources at five directions of east, west, south, north and the center. Using sampling investigation, water arsenic was determined by half -quantitative fast reagent kit. All samples of water with arsenic exceeding the standard were re-determined using silver diethyl dithiocarbamate using silver diethyl dithiocarbamate colorimetric mothod. Survey on the disease was carried out in the villages with arsenic exeeeding the standard. Results All 6074 water samples was inspected. Arsenic in 210 samples outnumbered 0.05 mg/L, 51 natural villages were high arsenic areas;The maximum level of arsenic content in drinking water was 0.7 mg/L 3.2% (152/4784) of the wells no deeper that 30 meters and 4.9%(58/1184) between 30 to 100 m had arsenic exceeding the standard The water arsenic content was normal when the wells was deeper that 100 m. The abnormal percentages of water arsenic was related with the depth of wells with a significant difference(χ2 = 12.29,P < 0.01). Medical examination 84 064 residents in 51 villages having high arsenic water 31 neighboring villages was made, No Patients was found suffering from endemic arsenic poisoning. Conclusions High arsenic source has been found in Jingzhou City ,but no endemic arsenic poisoning patient in Jingzhou City. It is suggested that necessary preventive measures should be taken in high arsenic area, low-arsenic water should be spotted or high arsenic water improved. Moreover, wells should be drilled for more than 100 meters or more in depth.
10.Multi-target derogation operation of senile dementia in patients with mental disorder: an efficacy analysis

Objective To explore the clinical effects of stereotactic radiofrequency multitarget lesion on the treatment of senile dementia in patients with mental disorder. Methods Under the CT localization, 18 patients with mental retardation were treated by stereotactic surgery of multi-target lesion.Their therapeutic effects were evaluated by Scale and National Psychosurgery Cooperation Request on Modern Psychosurgery Operation Cure (protocol) in 1990. Results Postoperative follow-up was performed for 12-36 months; 9 patients progressed remarkably; 6 got improvement and only 3 showed no changes; the total effective rate was 83.3%. The scores of brief psychiatric rating scale, social disability screening schedule and scale for assessment of positive symptoms after the treatment were significantly lower than those before the treatment (P<0.05). Mini-mental state examination showed no changes on the intelligence before and after the treatment (P>0.05). Conclusion The stereotactic surgery of multi-targets lesion is an effective method in treating senile dementia in patients with mental disorder,with few complications and little influences on the intelligence.
