1.Myocardial Damage Presented with Abdominal Pain in 33 Children

Objective To investigate the clinical characteristics,treatment and prognosis of myocardial damage presented with abdominal pain in children.Methods The clinical data of 33 children with myocardial damage presented with abdominal pain in Beijing Anzhen Hospital during the past 4 years were analyzed.Emergency conditions were ruled out in all cases.Results Among 33 cases with myocardial damage presented with abdominal pain,there were 10 cases of pneumonia,9 cases of acute bronchitis,7 cases of upper respiratory tract infection,1 case of allergic purpura,1 case of myocarditis and 5 cases of unknown causes.The duration of abdominal pain lasted 2 days to 3 months.The characteristics and the duration of abdominal pain were irregular.Mycoplasma antibodies positive were 5 cases (2 cases of bronchitis,3 cases of pneumonia).Coxvirus antibodies were 2 cases.There were different symptoms and signs of myocardial damage.Holter:there were ST-T changes in 23 children and 17 frequent premature ventricular complex(pattern of bigeminy and trigeminy in 7 children,premature atrial complex in 4 children).Creatine kinase isoenzyme-MB were high in 21 children(30-124 U?L-1).Cardiac troponin I were high in 19 children(0.8-1.2 ng?L-1).Chest X-ray:cardiomegaly was noted in 7 children,and the ratio of cardiac and chest were 0.55-0.58.Echocardiography:the left ventricular end-diastolic diameter was enlarged in 5 children,the right atrium was enlarged in 1 child.Most abdominal pain was relieved with treatment in 5 to 7 days,and 14 days was the longest.Total treatment course was 2 weeks to 1 month,and 3 months was the longest.But 1 case with myocarditis was recurrent abdominal pain treated nearly 3 months,and lasted to 6 months for recovery.Conclusions It is important to pay attention to cardial examination early,when a child becomes inconsolable from abdominal pain but emergency conditions should be ruled out.
2.Nursing for"super-aged"infants with total transposition of the great artery after Switch operation

Objective To summarize the nursing experience for three cases of above 1-month-oldinfants with total transposition of the great artery after Switch operation.Methods The vital signs of the infants were monitored,including the respiratory system,circulation system,temperature,urine quantity,feeding and delayed sternal closure.Appropriate measures were adopted to cope with changes in the above mentioned aspects.Results All the 3 infants were cured with good nursing effect.Conclusion Switch operation was a complicated operation.The 3 infants missed the-optimum operation time and left great difficulty for nursing.Close monitoring and delicate nursing proved to be the pivotal process to ensure the successful curement.
3.Determination of 2,3,5,4′-Tetrahydroxystilbene-?-O-?-D-Glucoside in Radix polygoni multiflori preparata by HPLC

AIM: To establish HPLC for determination of 2,3,5,4′tetrahydroxystilbene-?-O-?-D-glucoside in Radix polygoni multiflori preparata. METHODS: The column was diamonsil C_(18)(5 ?m,4.6 mm?250 mm).The mobile phase was CH_3CN-CH_3OH-H_2O(10∶20∶70) and ?=320 nm.The flow rate was 1.5 ml/min. RESULTS: The linearity was in the range of 0.0253-0.3542 ?g.The average recovery and RSD were 99.34% and 0.49%,respectively. CONCLUSION: The method is simple,precise and reproduciable and can be used for determination of 2,3,5,4′-tetrahydroxystilbene-?-O-?-D-glucoside in Radix polygoni multiflori preparata effectively.
4.Practicality and repeatability of B-mode ultrasonography on diagnosis and score of carotid atherosclerosis

Objective To study the practicality and repeatability of B-mode ultrasonography on diagnosis and score of carotid atherosclerosis. Methods Ninety-nine B-mode ultrasonography pictures of carotid atherosclerosis were obtained from field investigation. According to the established standard(normal scored as 0, other scored as 1 - 7 by the severity), the 99 pictures were diagnosed and scored once together by three trained inexperienced members of the research team. Ten days later, these pictures were diagnosed and scored respectively, by the three members.The diagnostic results were analyzed with statistical methods to test the feasibility and repeatability of this set of diagnostic and scoring system. Results The joint diagnostic results by the three group members were regard as standard with scores 0 in 27 cases, 1 in 27 cases, 2 in 10 cases, 3 in 4 cases, 4 in 13 cases, 5 in 8 cases, 6 in 6 cases, and 7 in 4 cases. Independent diagnostic results by member A was 0 in 29 cases, 1 in 24 cases, 2 in 11 cases, 3 in 2 cases, 4 in 12 cases, 5 in 11 cases, 6 in 6 cases, and 7 in 4 cases. Independent diagnostic results by member B was 0 in 29 cases, 1 in 22 cases, 2 in 14 cases, 3 in 4 cases, 4 in 13 cases, 5 in 5 cases, 6 in 8 cases, and 7 in 4 cases. Independent diagnostic results by member C was 0 in 28 cases, 1 in 25 cases, 2 in 8 cases, 3 in 5 cases, 4 in 14 cases, 5 in 10 cases, 6 in 4 cases, and 7 in 5 cases. Comparison of the 4 diagnostic scores, the difference was not statistically significant(F = 0.019, P > 0.05). Joint diagnostic results of the three members were compared with the standard and correlation coefficient were 0.977,0.987,0.932, respectively(all P < 0.01 ). The correlation coefficient of diagnostic results of each member were 0.969,0.935,0.928, respectively (all P <0.01 ). Diagnostic results of each member were consistent with the standard and the compliance rate were 88.9% (88/99) ,90.9%(90/99) and 86.9%(86/99), respectively. Conclusions B-mode ultrasonography is a non-injure method for diagnosis of carotid atherosclerosis in epidemiology investigation. The method is easy to grasp and has a good repeatability.
6.Applications of global left ventricular myocardial strain in detection of anthracycline-induced cardiotoxicity in breast cancer survivors with postoperative chemotherapy

Objective To assess the clinical significance of three-dimensional speckle tracking imaging (3D-STI) in the evaluation of left ventricular myocardial function in breast cancer survivors with postoperative anthracycline-based chemotherapy.Methods A total of 51 breast cancer survivors with postoperative anthracycline-based chemotherapy were recruited and 31 female healthy volunteers as the controls.Conventional echocardiography and 3D-STI derived parameters [including global longitudinal strain (GLS) and global area strain (GAS)] were measured before and at 3 and 6 anthracycline-based chemotherapeutic cycles.The receiver operating characteristics (ROC) curve was constructed to determine optimal sensitivity and specificity of 3ID-STI derived parameters for the prediction of future cardiotoxicity.Results In comparison with the controls and baseline cases,GLS and GAS deteriorated significantly (P < 0.05 for both).No statistical difference in GCS and conventional parameters showed before and after the initiation of chemotherapy (P <0.05,respectively).The ROC curves showed that GAS as the best 3D-STI predictor of patients who develop cardiotoxicity during the follow-up.The area under ROC(AUC) of GAS was 0.894,and its optimal cut-off value was-28.4%,with a specificity of 88.0% and a sensitivity of 82.9%.AUC of GLS was 0.802,and its optimal cut-off value was-14.3%,with a specificity of 62.0% and a sensitivity of 85.4%.Conclusions Early decreases in GAS and GLS based on 3D-STI may allow the prediction of subsequent cardiotoxic development accurately in breast cancer survivors with postoperative anthracycline-based chemotherapy.
10.The effects of alanyl-glutamine dipeptide-enriched parenteral nutrition on outcome in radiation enteritis rats

Objective:To investigate the impact of alanyl-glutamine(Ala-Gln) on outcome in radiation enteritis rats.Methods: Male SD rats(n=70)were separated randomly into four groups: control group(n=10),AR+pseudosurgery group(n=20),AR+TPN group(n=20) and AR+TPN+Ala-Gln group(n=20).Rats were observed for mortality,changs of body weight,villous hight and area,the bcteriral translocation(BT)in mesenteric lymph nodes(MLNs),liver,spleen and peritoneal cavity.Serum TNF-? and sIL-2R level were determined by sandwich ELISA.Results: When Ala-Gln was administered in radiation enteritis rat,the mortality,body weight loss and bacterial translocation were decreaded,the villous hight and area was increased and the TNF-? and sIL-2R levels were reduced.Conclusion: Parenteral Ala-Gln nutrition can improves the results of radiation enteritis rats.
