OBJECTIVE To strengthen the hospital supply Division standardized management and prevent the pathogenic microorganism dissemination effectively and safeguard the human health.METHODS Strengthening the vocational study,improving personnel′s knowledge level,formulating control system,strengthening detail and flow management and adopting the effective monitoring method all were the main measures.RESULTS The results were to guarantee the aseptic material quality,and prevent the occurreance of hospital Ainfection.CONCLUSIONS The scientific and effective management measures make our supply division a good quality management institution,with the programming the materiol processing and the standardizing the disinfection and antisepsis.
2.Clinical observation on every 3-week oxaliplatin plus 5-FU/CF regimen as postoperative adjuvant chemotherapy for colorectal cancer

Objective:To investigate the clinical efficacy of every 3-week regimen of oxaliplatin (OXA) plus 5-fluorouracil (5-FU)/leucovorin(CF)as postoperative adjuvant chemotherapy for colorectal cancer.Methods:Ninety eight patients with stage Ⅱ to stage Ⅲ colorectal cancer received every 3-week OXA plus 5-FU/CF adjuvant chemotherapy for six cycles after resection surgery. All the patients received a complete reexamination every 3 months after the end of adjuvant chemotherapy. The disease-free survival period and 1- and 2-year disease-free survival rates were observed.Results:The total 2-year disease-free survival rate was 74.5%. It was 87.0% for stageⅡpatients and 63.5% for stage Ⅲ patients. The main adverse reaction included peripheral neurotoxicity, neutropenia and diarrhea at grades Ⅰ to Ⅱ. Grades Ⅲ to Ⅳ adverse reactions occurred rarely.Conclusion:The every 3-week OXA combined with 5-FU/CF adjuvant chemotherapy regimen had definite efficacy and good tolerance, which was a rational choice of postoperative adjuvant chemotherapy for colorectal cancer.
4.Risk factors for bone mineral density changes in 38 female connective tissue diseases patients who were taking glucocorticoid

Objective To promote preventive and therapeutic measures for osteoporosis by investigating the prevalence of low bone mineral density (BMD) in connective tissue disease patients who were taking corticosteroid and identify the associated risk factors of osteoporosis.Methods Thirty-eight female patients who had connective tissue diseases and were taking corticosteroid in PU MCH rheumatology outpatient clinic were studied and followed up from Oct 2006 to April 2007.Lumber spine and right femoral bone mineral density were determined bv dual energy X ray absortiometry (DEXA).Clinical information was obtained from a questionnaire of history and medical records.Duration and accumulative dosage of glucocorticoid intaking.menopause time were obtained retrospectively.Correlation analysis between BMD and clinical information was conducted.Results ① In 38 patients,23 (61%) patients showed a normal BMD,10 (26%) were osteopenia,3 (8%) were iu the osteoporotic range,while 2 patienLs (53%) had fragile fracture.② Compared with patients with normal BMD,subjects with low BMD had significantly older age,longer period after menopause and higher accumulated dose of corticosteroids.③ Postmenopausal women had significantly lower BMD in lumber and hip than premenopausal women.④ Either of vertebral and right hip BMD correlated negatively with the accumulated dosage of cortieosteroids by simple linear regression.⑤ The correlation between BMD and accumulated dosage of corticosteroids improved after correcting for the effect of age by partial correlation analysis (Pearson partial vertebral r=-0.8,P=0.009;right hip r=-0.3,P=0.010).⑥ A stepwise multivariate linear regression model was constructed to explore the relationship between the different clinicsl factors studied and a low BMD.Two statistically significant variables were menopause status (P=0.0000) and a higher steroid accumulated dosage (P=0.008).Conclusion Low BMD is common in connective tissue disease patients receiving corticosteroid.Risk factors for low BMD are postmenopause.duration and the accumulated dosage of glucocorticosteroid.The high prevalence of low BMD implies that more attention should be paid to the prevention and treatment of osteoporosis and fractures in connective tissue disease patients who are taking corticosteroids.
5.Surgical results of complicated ocular trauma with cyclodialysis

ObjectiveTo explore the surgical techniques, efficacy and the management of complicated ocular trauma with anterior-posterior segment complications such as cyclodialysis. Methods Fifty-five patients (55 eyes) with complicated ocular trauma were enrolled in this study. Among them, there were 35 cases with eyeball contusion and 20 cases of eyeball rupture. Preoperative visual acuity was from no light perception to 0. 15, intraocular pressure (IOP) ranged from one to 10 mm Hg (1 mm Hg=0. 133 kPa).Cyclodialysis, vitreous hemorrhage or retinal detachment were revealed by B-ultrasound and ultrasound biomicroscopy (UBM). Cyclodialysis clefts ranged from one to 12 clock-hours. All patients underwent 3-port pars plana vitrectomy with gas/silicone oil tamponade and ciliary body reattachment by cryotherapy (cyclodialysis clefts ≤ three clock-hours) or suture fixation ( cyclodialysis clefts > three clock-hours).Healing after surgical trauma, visual acuity, intraocular pressure, intraocular hemorrhage, ciliary body and retinal reattachment were followed up. Results In one month after surgery, UBM showed 54 patients out of 55 patients had good ciliary body reattachment. Gonioscopy revealed cyclodialysis still existed in one patient, and this was cured by a second surgical suture fixation. In three months after surgery, ocular trauma healed in all 55 patients, the visual acuity ranged from no light perception to 0. 15, with a best corrected visual acuity of 0.8. The ciliary body and retina had good reattachment. IOP of 52 patients was normal; IOP of three patients was still lower than 10 mm Hg. Three patients had secondary glaucoma which was treated by glaucoma surgery. Conclusions Complicated ocular trauma with cyclodialysis can be treated with vitrectomy and cryotherapy or transscleral sutures. The procedure is safe and effective.
6.Angiogenic effect of platelet-released growth factors from platelet-rich plasma at early stage of transplantation of freeze-dried Achilles tendon in rabbits

Objective To observe angiogenic effect of platelet-released growth factors (PRGF) from platelet-rich plasma (PRP) on mierovessel formation at early stage after anterior crueiate ligament (ACL) reconstruction by freeze-dried Achilles tendon. Methods The study involved 14 rabbits, of which 12 rabbits were used as experiment group and the other 2 as control group. In the experiment group, after two sides of rabbit ACL were removed, freeze-dried Achilles tendon treated by PRGF was transplanted into random one side of the knee to substitute the original ACL (PRGF group), while the other side was transplanted with freeze-dried Achilles tendon treated only by normal saline (NS group). Only one side of the knee was removed in the control group. The grafts were observed by HE and immuno- histeehemical staining 2, 4 and 6 weeks after operation. Microvessel density (MVD) was measured by Weidner method. Results Compared with NS group, MVD in PRGF group was significant higher at 2,4 and 6 weeks after operation (P<0.05). MVD of NS group at 2,4 and 6 weeks after operation was 2.52±0.45, 3.41±0.44 and 2.57±0.51 respectively, but that of PRGF group at 2,4 and 6 weeks af- ter operation was 3.56±0.81,4.91±0.46 and 3.01±0.75 respectively (P<0.05). The time of neo- vascular formation and the depth of vascular penetration into the grafts of the PRGF group were superior to those of NS group. Conclusion PRGF can significantly promote microvessel formation at early stage after ACL reconstruction with freeze-dried Achilles tendon.
7.Expression and clinical significance of GPRCSA and SOCS3 in patients with colorectal carcinoma

Objective:To study the expression and clinical significance of GPRC5A and SOCS3 in colorectal carcinoma.Methods:SP immunochemical method was performed to detect the expression of GPRCSA and SOCS3 in 45 cases of colorectal carcinoma,25 cases of colorectal adenomas and 22 cases of normal colorectal tissues.Results:1)Expression of GPRC5A in colorectal cancinoma tissue (22.2%) was significantly lower than that in adenomas tissue (52.0%,P>0.05).The Latter was significantly lower than that in normal colorectal tissue (81.8%,P<0.05).GPRC5Awas closely related to lymph node metastasis,Duke's stages and the deepness of invasion (P<0.05).2) Expression of SOCS3 in colorectal cancinoma tissue (24.4%) was significantly lower than that in adenomas tissue (56.0%,P<0.01).The Latter was significantly lower than that in normal colorectal tissue (86.4%,P<0.05).SOCS3 was closely related to pathological differentiation,the deepness of invasion,Duke's stages and lymph node metastasis (P<0.05).3)The expression of GPRC5A was positive correlated with SOCS3 (P<0.05).Conclusions:The reduced expressions of GPRC5A and SOCS3 may participate in the occurence and development of colorectal carcinorma,suggesting that GPRC5A and SOCS3 may act as biological markers for evaluating the malign degree,prognosis and therapeutic targets of colorectal carcinorma.
8.Detection of HPV-DNA and SCCAg in the Follow-up of Cervical Squamous Cell Carcinoma Patients

Objective To investigate the infection of human papilloma virus(HPV) and expression of serum squamous cell antigen (SCCAg) in the follow - up of 120 cervical squamous carcinoma patients who had received operation or operation combined with radiotherapy or only radiotherapy. Methods 120 cases of therapical cervical squamous carcinoma patients were detected HPV - DNA by HC - Ⅱ and serum SCCAg using immunohistochemical method. Results The positive rate of HPV - DNA and serum SCCAg was 49.17% and 17.50% , with a significant difference between them( P 0.05 ). Conclusion HPV - DNA test with HC - Ⅱ for follow - up of cervical squamous carcinoma patients was feasible. It was more sensitive than serum SCCAg. But it suggested that high risk type HPV -DNA test combined with serum SCCAg may be the independent prognostic factors.
9.Clinical and pathological features of macroscopic hematuria associated acute kidney injury in IgA nephropathy

Objective The current study was to investigate the clinical and pathological features of macroscopic hematuria associated acute kideny injury (MH-AKI) in patients with IgA nephropathy (IgAN) and its associated prognostic factors.Methods We performed an analysis on the clinical and pathological features of biopsy-proven IgAN patients diagnosed during 2004 to August of 2007,who had experienced MH associated AKI.The patients enrolled in the study were followed up.The renal lesions were evaluated with Katafuchi pathological scores of IgAN.Results 5 patients with IgAN had experienced at least one episode of MH-AKI,accounting for 1.3% of IgAN diagnosed during the same period and 13.5% of IgAN patients with macroscopic hematuria.High proportion of tubules filled with red blood cell casts and tubular injury were demonstrated in all biopsy specimens.Renal function of 3 patients completely recovered within 14 days.One patient (patient 4) got quick recovery in the first month of the disease but completely recovered 418 days later.The other patients only had an incomplete recovery during the 20-month follow-up.The incompletely recovered one had relatively an older age,longer persistence of MH,more sclerotic glomeruli and a more severe tubule-interstitial damage.Conclusion AKI is commonly seen during episodes of MH in patients with IgAN.Not all patients with MH-AKI showed a complete recovery.Age,duration of MH,proportion of glomerular sclerosis and the severity of tubular necrosis might be the risk factors of an incomplete recovery of renal function.
