1.Life Quality and Life Satisfaction among the Elderly Population in Ji'nan

Objective:To study quality of life and life satisfaction among the elderly population dwelling in Ji'nan. Method:1386 elderly people were interviewed with“the Short-Form 36 Health Survey Scale”by the investigators who were trained in advance.Results:The prevalence rate of chronic diseases was 65.1%. The main influencing factors of life satisfaction were mental health condition, family harmony, marriage condition, chronic diseases, and income. The main influencing factors of life quality are life satisfaction, chronic diseases, mental health condition, activities of daily life, family harmony, and income.Conclusion:Life satisfaction is closely associated with life quality of elderly people. The physical, psychological, social and economic conditions have significant effect on life quality of elderly people.
2.Risk factors of early renal damage in children with Henoch Schonlein purpura

Objective To analyze the risk factors of early renal damage in children with Henoch Schonlein purpura(HSP).Methods The clinical data of 196 children with HSP admitted to our hospital from April 2012 to January 2016 were analyzed retrospectively.They were divided into the renal damage group and non-renal damage group within 90 d after confirmed diagnosis.The related clinical data such as serum immunoglobulin and urinary microalbumin were compared between the two groups,and the risk factors of early renal damage in children with HSP were screened.Results There were significant differences between the two groups on age,joint symptoms,recurrent purpura,persistent rash,gastrointestinal bleeding and abdominal pain(with χ2 or t of 11.345,16.223,11.275,43.211,12.592,17.771,P<0.05).The white blood cell count,platelet count,immunoglobulin A(IgA) level and urinary albumin level also showed significant differences between the two groups(t=33.750,60.442,9.451,8.458,P<0.05).The multivariate regression analysis showed that the independent risk factors for early renal damage in children with HSP included age(OR=2.703),recurrent purpura(OR=2.721),persistent skin rash(OR=1.782),gastrointestinal bleeding(OR=11.472),abdominal pain(OR=2.046),IgA level(OR=1.221) and urine microalbumin(OR=3.214).Conclusion Age,recurrent purpura,persistent skin rash,gastrointestinal bleeding,abdominal pain,IgA level and urine microalbumin are closely related to early renal damage in children with HSP.
3.Expression of Zonula Occludens-1 in Cerebral Cortex Following Traumatic Brain Injury.

OBJECTIVE:
To observe the time-course expression of zonula occludens-1 (ZO-1) in cerebral cortex after traumatic brain injury (TBI).
METHODS:
The TBI model of mouse was established. The mice were divided in 1 h, 3 h, 6 h, 12 h, 24 h, 3 d, 7 d after TBI, sham and control groups. The permeability of the blood brain barrier was evaluated by measuring the extravasation of Evans blue (EB) dye. The expression of ZO-1 in cerebral cortex in the injured area was detected by Western blotting and immunohistochemistry.
RESULTS:
The extravasation of EB dye of injured cortex gradually increased from 1 h, peaked at 1-3 d and approximately decreased to normal at 7 d after TBI. Western blotting revealed that the expression of ZO-1 gradually decreased after 1 h, was at the lowest at 1-3 d, and then significantly increased after 7 d but was still lower than that of normal and sham groups. The result of immunohistochemistry showed that ZO-1 had strong expression in vessel of normal cortex, gradually decreased after TBI, and almost disappeared at 3 d after TBI and gradually recovered to normal level later.
CONCLUSION
The expression of ZO-1 in the injured cortex after TBI initially decreases and then increases. The negative correlation between ZO-1 expression and EB extravasation after TBI could be used as a new indicator for wound age estimation.

4.Nursing care of a patient with hemophilic arthritis and hepatitis C undergoing unilateral hip and knee replacement simultaneously

Objective:To summarize the perioperative nursing experience of a patient with hemophilia arthritis complicated with hepatitis C undergoing unilateral hip and knee arthroplasty at the same time.Methods:In August 2018, a patient with hemophilic arthritis complicated with hepatitis C was admitted to the first hospital of Lanzhou University. Preparations for health education, functional exercises, examination and evaluation before operation was made. Unilateral hip and knee arthroplasty were performed at the same time, strengthen the management of incisions and catheters after the operation, actively prevent complications, and guide patients to perform functional rehabilitation exercises under the conditions of good pain management and safe coagulation factor activity; pay attention to the nursing of coagulation factor replacement treatment in the whole perioperative period.Results:After careful nursing, no serious complications occurred during the patient's hospitalization, and the position and force line of the prosthesis were satisfactory by imaging examination before discharge.Conclusion:The perioperative care of this patient proved that the above-mentioned nursing measures are effective for hemophilic arthritis patients with hepatitis C undergoing unilateral hip and knee arthroplasty at the same time.
5.Application value of spiral CT with 3D reconstruction on diagnosis and treatment of tibial plateau fractures

ObjectiveTo evaluate the value of application of spiral CT with 3D reconstructure on diagnosis and treatment of tibial plateau fractures.Methods15 cases of tibial fractures treated with x-ray and 16-slices spiral CT,3 D images were made,fractures were classified and evaluated with 3D images and treated with correct treatment.ResultsAll of the 15 patients got correct diagnosis by 16-slices spiral CT with 3D reconstructure,1 failed by x-ray.10 fractures were operated under 3D reconstructure guide and operation proved fractures revealed by 3D reconstructure finally.ConclusionSpiral CT with 3D reconstructure could offer more accurate location and classification of tibial plateau fractures,and it had obvious advantage on demonstration of spatial relationship,and it also could provide better planning and management for the tibial plateau fractures.
6.Pathological changes of soft tissues and transforming growth factor beta 1 expression in radiation masseter injury models

BACKGROUND: Long-term radioactive ray accumulation leads to surrounding tissues fibrosis accompanied by limitation of mouth-opening, which seriously affects life quality of tumor patients. OBJECTIVE: To observe pathological change of soft tissues in rats with radiation masseter injury and the level of transforming growth factor β1 (TGF-β1) mRNA expression in vivo.METHODS: A total of 30 Wistar rats were randomly divided into control group (n=10) and radiated group (n=20). Radiated group were radiated with linear accelerator at a dose of 40 Gy. The pathological changes of vessels were observed under light microscope and electron microscope and expression of TGF-β1 was detected by RT-PCR. RESULTS AND CONCLUSION: The microvessel density of radiated group was obviously decreased than that of the control group (P < 0.01), but the mRNA expression of was higher than that in the control group (P < 0.01). It suggests that radiated injury can induce fibrosis to repair the radiated injury.
7.Blockade of OX40/OX40L pathway promotes CD4 + CD25 + T regulatory cells proliferation

Objective To evaluate the feasibility of OX40L gene silence in dentritic cells by RNAi through lentiviral vector, so that to explore the influence of CD4 + CD25 + T regulatory cells by blocking OX40/OX40L costimulation signals. Methods Serial plasmids were constructed, consisting of lentiviral vector framework, containing different OX40L siRNA sequences. The most effective packaged one by 293T cells to be the operating siRNA vector named OX4OL-RNAi-LV was chosen. The negative comparing vector was NC-GFP-LV. DCs isolated from C57BL/6 mice by magnetic selecting system were cultured in vitro and divided into 3 groups. Experimental group and negative control group were transfected with OX40L-RNAi-LV and NC-GFP-LV, respectively, and the blank control group was given the same volume culture solution. The MOI was 25. The status of transfection and GFP expression was monitored by GFP fluorescence. 6 days later, CD86 positive DCs were isolated and were co-cultured with CD4 + CD25 + T regulatory cells isolated from na(y)ve BALB/C. 6 days later, the proliferation and apoptosis of Tregs were evaluated by flow cytometry.Results The most effective siRNA sequence targeted the C,CTCATACAAGAATGAGTA episode of OX40L gene. The suppressive ratio of the OX40L protein expression was 73.1%. While the MOI was 25, the DCs transfected ratio by lentiviral vectors was at the level of 86. 4%. After co-cultured for 6 days, the CD4+CD25 +T regulatory cells of experimental group have a higher proliferation index (respectively, 38.3% vs.24.5 % ,22. 9%, F = 95.40, P = 0. 000) and lower apoptosis percentage ( respectively, 8.7 % vs. 20. 1%,19.8%, F=244.22,P=0. 000) than negative group and blank group. Conclusions The OX40L siRNA lentiviral vetor was constructed. This vector could effectively transfect DCs and block OX40/OX40L pathway, so to promote CD4 + CD25 + T regulatory cells proliferation in vitro.
8.Surgical Treatment of Metastatic Hepatic Cancer (Report of 208 Cases )

Objective To explore the effect of surgical treatment of metastatic hepatic cancer. Methods Two hundred and eight patients with metastatic hepatic cancer received surgical treatment in our department during the past seven years, and their information were analyzed retrospectively in this paper. The ages of these patients ranged from 19 years to 82 years, and 133 of them were male, 75 of them were female. Two of them were complicated with hepatocirrhosis, and 5 with polycystic liver. The metastatic cancer originated from gastrointestinal tract in 121 cases (58.2%), and from other parts of the body in 87 cases (41.8%). One hundred and sixteen of the patients received resection treatment (resection group), and 92 of them received non-resecton treatment (non-resection group). The survival rates of the two groups were compared through Chi square test.Results The 1-, 3- and 5-year survival rates for all patients were 56.3%,23.1% and 13.0%, respectively. The 1-, 3- and 5-year survival rates were 74.1 %,39.7% and 23.3% in the resection group respectively and 33.7%, 2.2% and 0 in the non-resection group, respectively. 9.The role of endothelicyte activation in the development of pulmonary hypertension with congenital heart disease and vascular remodeling

Objective To discuss the role of endothelicyte activation in the development of pulmonary hypertension (PH) and vascular remodeling. Methods A total of 10 patients with simple ventricular septal defect(VSD)of congenital heart disease(CHD)were choosen as controls, and 30 patients with different degree of PH were studied. Plasma levels of interleukin-1(IL-1), endothelin-1 (ET-1) and nitric oxide (NO) which represented the degree of endothelicyte activation were measured by radioimmunoassay and commercial available kit. The specimens taken from right lung before cardiopulmonary bypass were fixed by formaldehyde solution. The percentage of vascular wall thickness/vascular external diameter (WT/D) and the percentage of vascular area/total vascular area (WA/A) which represented vascular remodeling, were measured by QTM970 analysator. Results Compared with patients of simple VSD, the plasma levels of IL-1, ET-1/NO and WA/A, WT/D increased in patients with PH. Conclusion The activation of endothelicytes and imbalance of ET/NO play an important role in the development of PH and vascular remodeling. 10.Effect of decline of hip muscle density on proximal femoral fracture in the elderly

Objective To definite the correlation of hip muscle density with proximal femoral fracture in the elderly.Methods The study included 41 proximal femoral fracture cases [14 males and 27 females,at age of (64.1 ±5.1)years] and 14 normal cases [4 males and 10 females,at age of (64.8 ± 3.9) years] that underwent CT examination for hip joint.In fracture group,19 cases sustained femoral neck fracture and 22 intertrochanteric fracture.CT imaging data were reviewed to analyze the correlation of age with hip muscle density at the level of ischium tubercle.Hip muscle density was compared between fracture side and healthy side in fracture group and between control group and fracture group.Results Hip muscle density showed a downward trend with increased age (P < 0.05).Hip muscle density did not differ significantly between fracture side and healthy side in fracture group (P > 0.05).Regardless of age,gender and other factors,hip anterior muscle density [(34.8 ± 4.1) Hu] and posterior muscle density [(22.1 ±5.5) Hu] were lower in fracture group than in control group [(44.4 ±5.7)Hu,(39.2 ±5.3)Hu respectively,P < 0.05] but there was no significant difference in hip medial muscle density between the two groups (P > 0.05).Conclusions Hip muscle density decreases with age and has no strong correlation with fracture.Deadline of the hip muscle density may be a risk factor to proximal femoral fracture in the elderly.
