1.Application of health card service pattern in community health services

Objective To discuss the effects of the health card-based services on community chronic disease management and the utility of community healthcare resources. Methods Follow-up investigation on utilization and satisfaction of card-based health services and coverage of chronic illness management was conducted among cardholder living at Dongcheng District of Beijing during July 1,2006 and December 31,2008. Results The rates of attendance and health education increased from 47.20% or 1.16% to 90.90% and 46.38% ,respectively. The coverage of high blood pressure management and standard hypertension management increased from 47.16% or 45.66% to 80.39% and 78.27%, respectively. The satisfaction of cardholder residents with community healthcare services was improved year by year. Conclusions The health card service pattern plays a significant role in improving chronic disease management,which results in more satisfaction with and utilization of healthcare resources in community.

3.Research progress of peptide drugs for treatment of autoimmune diseases

Recent studies have found that peptide therapies tar-geting specific epitopes can avoid nonspecific immune suppres-sion induced by traditional medicines for the treatment of autoim-mune diseases, and have shown great therapeutic effect in ani-mal models of autoimmune diseases and clinical trials. The pa-per summaries the research progress and trends of peptide drugs for the treatment of autoimmune diseases from candidate peptide sources and their suppression mechanisms, which can provide a theoretical basis for the in-depth understanding of immune toler-ance and allow for discovery of new treatment for autoimmune diseases.
4.Clinical features of noncompaction of ventricular myocardium in children

ObjectiveTo assess the clinical features of noncompaction of ventricular myocardium (NVM) in children. Methods The clinical data from ifve children with NVM were analyzed, including clinical manifestations, ultrasonic cardio-gram(UCG), electrocardiogram, chest X-ray, the process of diagnosis and treatment, and follow up.Results In ifve patients (3 boys and 2 girls) aged 3 months to 12 years, four of them were hospitalized because of pneumonia with heart failure. None of the ifve patients had thromboembolism. Heart enlargement was observed in all ifve patients on chest X-ray. By 2D ultrasonography, 2 patients had cardiac apex involvement, and 3 patients showed involvement of left ventricular wall and interventricular septum. In one patient, congenital partial defect of the endocardial cushion was observed. All patients were followed up for 3 to 36 months. One patient died of heart failure, 3 patients survived, and one patient was lost to follow-up.Conclusions The common clinical manifestation of NVM is the heart failure. UCG is preferred for NVM diagnosis. In children with refractory pneumonia and heart failure, NVM should be suspected.
5.Expression and signifance of TRIM59,Twist and E-cadherin in hepatocellular carcinoma

Objective To investigate the expressions of TRIM59,Twist and E-cadherin in hepatocellular carcinoma and their clinical significance. Methods The expressions of TRIM59,Twist and E-cadherin protein were tested by immunohistochemistry in 80 cases of hepatocellular carcinomas and the adjacent paracancerous tissues. Results The positive rate of TRIM59 in hepatocellular carcinoma was significantly higher than that in the adjacent paracancerous tissue(76.3%vs. 8.0%,P<0.05). Significant difference was also observed in the expres-sion rate of Twist between the hepatocellular carcinomas and the paracancerous tissue(66.3%vs. 6.0%,P<0.05). The positive rate of E-cadherin in hepatocellular carcinoma was significantly lower than that in the adjacent para-cancerous tissue(27.5%vs. 90.0%,P<0.05). The differences of the expression of TRIM59 in hepatocellular carci-noma of pathological grading,tumor differentiation,vascular invasion and clinical TNM stage were significant(P<0.05). The differences of the expression of Twist in hepatocellular carcinoma of pathological grading ,differentia-tion,vascular invasion and clinical TNM stage was also significant(P<0.05,respectively). The differences of the expression of E-cadherin in hepatocellular carcinoma of pathological grading,differentiation,vascular invasion and clinical TNM stage were also significant(P<0.05,respectively). Significantly positive correlation was also found between TRIM59 and Twist by using spearman correlation analysis(P<0.05). Negative correlations were observed between TRIM59 and E-cadherin(P < 0.05),and between Twist and E-cadherin(P < 0.05). Survival analysis showed that TRIM59 expression was an independent prognostic factor in hepatocellular carcinoma. Conclusion TRIM59,Twist and E-cadherin protein expression might be associated with the development,invasion,and metas-tasis of hepatocellular carcinoma. TRIM59 may become a new target gene for the treatment of human hepatocellular carcinoma.
6.The pharmacodynamic research of baijin capsule on depression in chronic unpredictable mild stress rats

Objective To evaluate the effects of baijin capsule on behavioral changes and monoamine neurotransmitters concentration in chronic unpredictable mild stress ( CUMS ) depression rat model.Methods The depression rat model was induced by11-week chronic unpredictable mild stress combining with solitary.After the model were established, rats were given the decoction of baijin capsule ( 12.6 g/kg, 4.2 g/kg, 1.4 g/kg ) or fluoxetine hydrochloride (3.5 mg/kg) by intragastricfor 4 weeks.During the experiment period, sucrose consumption and open-field experiment were conducted to monitor the behavior of rats, such as sucrose consumption percentage, horizontal motion, and vertical motion.At the end of the experiment, the levels of the monoamine neurotransmitters in the cerebral cortex and hippocampus were analyzed by method of high performance liquid chromatography-electrochemistry.Results Compared with the normal group, the weight, horizontal displacement distance, vertical movement times, and sucrose consumption percentage of rats in model group decreased significantly after stimulated with CUMS and solitary for 7 weeks (P<0.05,P<0.01).Compared with model group, consecutively administrated for 4 weeks, horizontal displacement distance, vertical movement times, and the percentage in sugar water consumption significantly increased with the treatment of baijin capsule (P<0.05, P<0.01).Meanwhile, the content of norepinephrine (NE), dopamine (DA), and 5-hydroxytryptamine (5-HT) in the cortex were significantly increased in rats of the baijin capsule ( P <0.05 ) .Conclusions The results indicated that baijin capsule improved the behavioral disturbances in depression rat model, which were related to enhancement of the concentration of monoamine neurotransmitters in the cortex.
7.ACTHR in regulating aldosterone secretion to angiotensin Ⅱ and potassium chloride stimulation

Objective To study CYP11B2 mRNA and aldosterone secretion alteration in human adrenocortical carcinoma H295R cell after angiotensin Ⅱ (AT-Ⅱ) and potassium chloride stimulation,and to investigate the effect of adrenocorticotropic hormone receptor (ACTHR) on them.Methods Lentiviral vector was used to increase ACTHR expression.It was transfected into the H295R cells.Similarly,another H295R cells,without ACTHR vector,was used as the control group.ACTHR alteration was measured by Western blot and real-time polymerase chain reaction (RT-PCR).CYP11B2 mRNA was detected at 24 hours after 100 nmol/L AT-Ⅱ/16 mmol/L KCL stimulation,and the amplification of the two groups was compared.Aldosterone was measured by ELISA kit.Results Compared with those in control ceils,the protein and mRNA level of ACTHR in experimental cells were increased 2.4 times and 18 times respectively (P<0.05).CYP11B2 mRNA of experimental cells was 1.7 times higher than control cells after 24 h stimulation of AT-Ⅱ.Aldosterone production was 121.98+8.31 and 104.05+6.88 ng/L respectively.The former amplification was 2.06 times higher than that of the latter (P<0.05).Similarly,CYP11B2 mRNA of experimental cells was 19.2 times higher than control cells after 24 h stimulation of KCL.Aldosterone production was 137.67±10.35 and 104.05 ± 6.88 ng/L respectively.The former amplification was 3.13 times higher than that of the latter (P<0.05).Conclusion Overexpression of ACTHR increases the sensitivity and response of CYP11B2 mRNA and aldosterone to AT-Ⅱ and KCL stimulation,and ACTHR is expected to become a key protein in understanding primary aldosteronism.

9.Utilization of Narcotic Analgesics in Patients with Cancerous Pain in Our Hospital in 2004~2007

OBJECTIVE:To investigate the utilization of narcotic analgesics in patients with cancerous pain in our hospital. METHODS:The consumption of Morphine and Pethidine in our hospital in 2004~2007 was analyzed statistically. RESULTS:The consumption quantity of Morphine sulfate controlled-release tablets has been on the rise from 2004 to 2007,while that of Morphine Hydrochloride Injection experienced little change and that of Pethidine Hydrochloride Injection showed a downward trend. CONCLUSION:The structure for the use of narcotic analgesics in our hospital tends to be reasonable,which is basically in line with the Guiding Principle of Pain Relief Ladder for Cancer Pain.
10.Expression and significance of HIF-1α, HIF-2αand MT in human papillary thy-roid carcinoma

Objective:To investigate the expression of HIF-1α, HIF-2αand MT in human papillary thyroid carcinoma ( PTC) and the association of their expression with clinicopathological indicators .Methods:Immunohistochemistry was used to analyze the ex-pression of HIF-1α, HIF-2αand MT in 70 PTC samples .The correlations of HIF-1α, HIF-2αand MT expression with one another , and with several clinicopathological indicators were statistically analyzed .Results:In 70 PTC samples, the positive expression rates of HIF-1α, HIF-2αand MT were 52.86%(37/70), 50.00%(35/70) and 44.29%(31/70), respectively.HIF-1α, HIF-2αand MT expression had significant correlations with cervical lymph node metastasis (P=0.034, P=0.022, and P=0.032, respectively). Meanwhile, HIF-1αexpression had a positive correlation with HIF-2α(rs =0.258, P=0.031) and MT (rs =0.266, P=0.026). HIF-2αand MT expression were positively correlated (rs=0.259, P=0.030).Concomitant expression of any two or all of the three molecules had stronger correlation with lymph node metastasis than did each alone ( P=0.004 for HIF-1α/HIF-2α, P=0.024 for HIF-1α/MT, P=0.029 for HIF-2α/MT, P=0.017 for HIF-1α/HIF-2α/MT).Conclusion:HIF-1α, HIF-2αand MT expression in PTC samples have a closely correlation , which are related to cervical lymph node metastasis .Therefore, the expression of HIF-1α, HIF-2αand MT might be used as biomarkers for cervical lymph node metastasis of PTC .
