1.Functions of practical education in the talents' cultivation of medical English major

As an important segment in the talents' cultivation of medical English major,practical education assumes a positive role in promoting the students' unity of knowing and doing,upgrading their professional kills and improving comprehensive qualities.Taking English major (medical English direction) of Chongqing medical university as an example,the present study attempts to discuss the forms and contents of the practical education,along with the fruitful achievements,in view of students'interests,community service,horizon expanding as well as job hunting and further study pursuing.In addition,some reflections and measures are presented to further boost the practical education.
2.Treatment of bony defects in revision total hip arthroplasty with allograft grafting:midterm follow-up study

[Objective]To explore the intermediate result of the use of allograft grafting for bone defects in revision total hip arthroplasty.[Methods]A retrospective study was carried out on 22 patients(22 hips) with the use of allograft grafting for bone defects in revision of total hip arthroplasty from April 2000 to April 2005.There were 13 male and 9 female with mean age of 65.5 years.According to the AAOS system,the bone defects in acetabulum were classified into type I in 5 hips,type II 14 hips and type III 3 hips,and on the femoral side,they were classified into type I in 3 hips,type II 12 hips and type III 2 hips.The reconstruction of bone defects in acetabulum contained structural bone graft in 5 hips,impaction bone graft in 12 hips,and metal wire mesh with bone graft in 5 hips.On the femoral side,3 hips were dealed with structural bone graft,12 hips were done with impaction bone graft,and 2 hips were repaired by metal wire mesh with bone graft.Harris Hip Score System and radiography were used to evaluate the clinical efficiency.[Results]The mean follow-up time was 61 months(37~97 months).Harris score improved from 45 points(30~65) pre-revision to 86 points(62~96) at the final follow-up.Insion drainage occurred in one hip.One patient remain slight pain in hip.No prothesis loosening,radiolucent lines were observed during the follow-up.None need to second revision.[Conclusion]During the THA revision surgery,adopting suitable allografts to repair the different types of bone defects is feasible and the mid-term result is good.
3.Surgical management of gland and central compartment lymph node in papillary thyroid carcinoma

The incidence of papillary thyroid carcinoma (PTC) has been increasing in the past decades.The overall survival (OS) of most PTC patients is satisfied,however,recurrence is not rare.Surgical management is the basic treatment for PTC.The first step of surgery includes gland resection and lymph node dissection of central compartment.Several controversies remain in the indications and range of surgery.In this article,the progress of gland management,lymph node dissection in central compartment and complications in PTC/papillary thyroid microcarcinoma were reviewed.

5.Preventing the recurrence of condyloma acuminatum with different treatment courses of imiquimod cream

Objective To evaluate the efficacy and safety of imiquimod 5% cream applied with different treatment courses for preventing the recurrence of condyloma acuminatum lesions.Methods A randomized,controlled clinical trial was conducted.Patients with condyloma acuminatum were divided into 3 groups (Group A,B and C).Group A was treated with CO2 laser only.Group B was given topical im- iquimod 5% cream three times a week over 4 weeks after laser treatment of visible warts.Group C was giv- en topical imiquimod 5% cream three times a week over 8 weeks after laser treatment of visible warts.All patients were followed up for 6 months.Results A total of 90 patients with anogenital warts were enrolled in,and finished the study.The recurrence rates of the Group A,B,and C were 64.29%,50%,21.88%,re- spectively,in 6 months of follow-up.The recurrence rate was statistically lower in Group C than in Group A and B (P=0.0033),and in Group C than in Group B (P=0.0457).Adverse reactions occurred in 73.33% and 84.38% of patients in Group B and C,respectively,which included erythema,itching,pain, burning,erosion,ulceration and flu-like symptoms.There was no significant difference of the frequencies of adverse reactions between Group B and C (P>0.05).Conclusion The recurrence rate of anogenital warts declines significantly in patients treated with imiquimod 5% cream for 8 weeks,but not in the 4 weeks group,after laser therapy.The frequencies of adverse reactions are not significantly different in both groups.

7.Efficacy of splenectomy plus selective pericardial devascularization under en-doscope in treatment of advanced schistosomiasis patients with portal hyper-tension and hypersplenism

Objective To evaluate the efficacy of splenectomy plus selective pericardial devascularization under endoscope in the treatment of advanced schistosomiasis patients with portal hypertension and hypersplenism so as to explore the minimally in-vasive and safer surgical treatment. Methods A secure splenectomy was performed with laparoscope and its supporting devices, and at the same time,the ligation of the left gastric vein and the ligation of esophageal vein perforating vertically into the esopha-gus were also performed in 14 advanced schistosomiasis patients with portal hypertension and hypersplenism. Results Among the 14 patients,the splenic artery was separated and clipped before the treatment of splenic pedicle. One patient was of conversion to open laparotomy for the splenic vein rupture bleeding in the separation. There was no death. Conclusion The operation of sple-nectomy plus selective pericardial devascularization under endoscope is effective,truly minimally invasive,and safe in the treat-ment of advanced schistosomiasis patients with portal hypertension and hypersplenism.
9.Optimization of the Formulation of Dextromethorphan Hydrobromide Sustained-release Tablets by Orthogonal Test

OBJECTIVE:To optimize the formulation of dextromethorphan hydrobromide sustained-release tablets. METHODS: The dextromethorphan hydrobromide sustained-release tablets were prepared with HPMC as sustained release matrix. Orthogonal test was performed to optimize the formulation with in vitro accumulative drug release rate as index and the amount of HPMC and lactose as well as ethylcellulose (EC) concentration as factors. Then verification test on the in vitro drug release characteristics of the optimized tablets were performed and the influencing factors (high temperature,high light,and high moisture) were investigated as well. RESULTS: The optimized formulation of dextromethorphan hydrobromide sustained-release tablets was as follows: 30 mg HPMC,50 mg lactose,and 8% EC. The accumulative drug release rate at 8 h was above 70%. In the influencing factor test,the tablets were stable under all conditions except at high moisture condition. CONCLUSION: The optimized formulation of dextromethorphan hydrobromide sustained-release tablets is feasible.
10.Survey on compliance degree of patients with allergic rhinitis receiving specific immunotherapy and its relevant factors and nursing

Objective To survey the compliance degree of patients with allergic rhinitis completing their specific desensitizing therapy and to evaluate the relating factors with this compliance degree, and to explore the nursing countermeasures. Methods A retrospective study was carried out in 186 patients with allergic rhinitis receiving specific immunotherapy from January 2007 to December 2008. Noncompliance patients were contacted with scheduled telephone interviews to investigate the causes. Results Compliance rate was 71.0%,primary reason for non-compliance is poor efficacy (16 patients, 29.6% ), followed by busy work or study (10 patients, 18.5%) and disappearance of symptoms (7 patiens, 13.0%). Conclusions The compliance degree of AR patients receiving specific desensitizing therapy is still poor, but the reasons for non-compliance can be controlled by many factors, compliance is still much room for improvement.
