1.A new beta-naphthalenecarboxylic acid biglycoside from Chirita longgangensis var. hongyao.

To investigate the chemical constituents in the stems of Chirita longgangensis var. hongyao, methanol extract of the stems was subjected to column chromatography with various chromatographic techniques. One new beta-naphthalenecarboxylic acid biglycoside, 1, 4-dihydroxy-2-naphthalenecarboxylic acid methyl ester-4-O-alpha-L-rhamnopyranosyl-(1-->6)-beta-D-glucopyranoside (1) was isolated, along with two known compounds: isotaxiresinol 4-O-methyl ether (2) and (R)-7-hydroxy-alpha-dunnione (3). Compound 2 was first obtained from Chirita genus and compound 3 was isolated from this plant for the first time. All structures were elucidated on the basis of spectral and chemical evidence, and the NMR spectroscopic data of compound 2 was published for the first time.
2.Comparison of CE-MRA, T 1WI and 3D-T 1WI sequence in evaluation of brain tumor blood supply

Objective:To evaluate the diagnostic efficiency of contrast-enhanced magnetic resonance angiography (CE-MRA), T 1WI and three-dimensional thin-layer T 1WI sequences (3D-T 1WI) in evaluating the blood supply of brain tumors, so as to improve the enhanced magnetic resonance imaging (MRI) scanning scheme of brain tumors. Methods:After the contrast agent was injected, 29 patients with brain tumors were scanned with CE-MRA, T 1WI and 3D-T 1WI. The imaging manifestations of " observing the enhancement of tumor substance" , " observing the small vessels in the tumor body" and " observing the small vessels around the tumor" were qualitatively classified, and the diagnostic differences of the three sequences were analyzed by McNemar test. Results:⑴ In the aspect of " observing the contrast medium in the tumor body" , CE-MRA, T 1WI and 3D-T 1WI sequences diagnosed as grade A1 were 0 cases, 8 cases (27.59%) and 19 cases (65.52%) respectively. The diagnostic efficiency of 3D-T 1WI sequences was better than that of T 1WI sequence ( P<0.05), and T 1WI sequence was better than that of CE-MRA sequence ( P<0.05). ⑵ In the aspect of " observing the small vessels in the tumor" , CE-MRA, T 1WI and 3D-T 1WI sequences diagnosed as grade B1 were 8 cases (27.59%), 5 cases (17.24%) and 16 cases (55.17%), respectively. The diagnostic efficiency of 3D-T 1WI was better than that of T 1WI and CE-MRA ( P<0.05). ⑶ In the aspect of " observing the small vessels around the tumor" , CE-MRA, T 1WI and 3D-T 1WI sequences diagnosed as grade C1 were 18 cases(62.07%), 5 cases (17.24%) and 14 cases (48.28%) respectively. The diagnostic efficiency of CE-MRA and 3D-T 1WI wre better than that of T 1WI ( P<0.05). Conclusions:The combination of 3D-T 1WI and CE-MRA sequence can comprehensively evaluate the blood supply of the tumor, which is of great significance for the localization, characterization and treatment of brain tumors.
3.Effect of Clonorchis sinensis infection on the coagulation indexes of patients with chronic hepatitis B

Abstract: Objective To analyze the changes of coagulation indicators in patients with hepatitis B complicated with Clonorchis sinensis (C. Sinensis), and provide reference value for diagnosis, drug using and prognosis monitoring. Methods The patient samples were collected from the Third Affiliated Hospital of Sun Yat-sen University from June 2018 to February 2022 and divided into six groups. They were 40 healthy patients, 47 patients with simple chronic hepatitis B, 47 patients with post-hepatitis B liver cirrhosis, 40 patients with C. Sinensis mono-infection, 30 patients with chronic hepatitis B patients co-infected with C. Sinensis and 27 patients with post-hepatitis B liver cirrhosis patients coinfected with C. Sinensis. Four coagulation indexes, prothrombin time (PT), activated partial thromboplastin time (APTT), thrombin time (TT) and fibrinogen (FIB), were detected and compared among the groups. Results Compared with the healthy group, C. Sinensis mono-infection group had higher level of FIB and delayed PT, APTT; chronic hepatitis B group and chronic hepatitis B patients co-infected with C. Sinensis group had delayed PT, APTT, TT and significant lower FIB, these differences were statistically significant (all P<0.05). Compared with simple chronic hepatitis B group, post-hepatitis B liver cirrhosis and chronic hepatitis B patients co-infected with C. Sinensis had significant delayed PT, APTT, TT and lower FIB (all P<0.05). Compared with the post-hepatitis B liver cirrhosis and chronic hepatitis B patients co-infected with C. Sinensis, post-hepatitis B liver cirrhosis patients coinfected with C. Sinensis group had significant delayed PT, APTT, TT and lower FIB (all P<0.05). Conclusions The coinfection of C. Sinensis will further aggravate the coagulation dysfunction of HBV patients, leading to poor treatment and prognosis. HBV patients will have worse coagulation function in the process to post-hepatitis B liver cirrhosis; Therefore, it is important to pay attention to C. Sinensis co-infection when treating HBV patients, so that further guidance on clinical use and monitoring of prognosis can be provided.
4.Anastomotic leakage in patients with lower rectal cancer receiving preoperative neoadjuvant therapy

Objective To investigate the prevention and management of anastomotic leakage in patients with middle and lower rectal cancer after neoadjuvant therapy.Methods In this study,50 cases of middle or lower rectal carcinoma underwent anus preserving radical resection and anastomosis after a 4 cycles of chemotherapy with FOLFOX and 46 Gy(2 Gy×23)radiotherapy.Anastomotic leakage,the prevention and management of this complication were analyzed retrospectively.Results All these 50 patients received low anterior resection after chemoradiotherapy,among the 19 cases receiving prophylactic ileac stoma simultaneously,there was no anastomotic leakage,whereas anastomotic leakage developed in 4 out of the 31 patients who did not receive prophylactic ileac stoma,including concomitant rectovaginal fistula in 2 cases.All these fistulae were cured conservatively.Conclusion Concomitant treatment of FOLFOX regimen and RT in neoadjuvant setting of rectal cancer could increase the rate of anal interior sphincter reservation,prophylactic ileac stoma helps to reduce the rate of anastomotic leakage after a anus-preserving low anterior resection of the cancer.
5.The relationship between learning/memory ability and neural cell apoptosis in hippocampal of rats with impaired glucose regulation

Objective To identify the relationship between impaired glucose regulation of neuronal apoptosis in hip-pocampus and the ability of learning/memory in rats.Methods The model rats were made by high fat and sugar diet;The morris water maze was used to detect the learning and memory function;The TUNEL method was used to detect neuronal apoptosis in hippocampus .Expression of Bcl-2/Bax and Bcl-2 mRNA/Bax mRNA factor in hippo-campus neurons was detected with immunohistochemistry and in situ hybridization .Results Compared with NGT group, in IGR group the learning and memory ability were meaningfully decreased (P<0.05);the number of neuro-nal apoptosis in hippocampus was increased significantly ( P<0.05 ); the expression of Bcl-2 and Bcl-2 mRNA in hippocampus decreased significantly ( P<0.05);the expression of Bax and Bax mRNA in hippocampus increased sig-nificantly ( P<0.05 );The ability of learning and memory was positively correlated with expression of Bcl -2 ( P<0.05) and negatively correlated to the expression of Bax (P<0.05).Conclusions There is a relationship between impaired glucose regulation and the ability of learning and memory in rats , its mechanism may be potentially related to hippocampal neuronal apoptosis .
6.The use of TCGA and GEO datasets to analyze clinical significance of glutathione reductase in gastric cancer

Objective To explore the expressions of glutathione reductase in gastric cancer,to investigate the relationship between glutathione reductase (GSR) and clinical pathological characteristics of gastric cancers,to identify the role of GSR in evaluation of the prognosis of gastric cancer patients,and to investigate the role of GSR in the development of gastric cancer.Methods The gastric cancer datasets were searched and downloaded from The Cancer Genome Atlas (TCGA),and chip data were analyzed with clinical information.Gene set enrichment analysis (GSEA) was conducted to explore the gene sets enriched in samples with high GSR expression.Results The expression of GSR was down-regulated in high grade tumors (P < 0.01).No significant difference was found between different age,Shortest tumor diameter,American Joint Committee on Cancer (AJCC) M stage,Barrett's esophagus,family history of gastric cancer,and Helicobacter pylori (H.pylori) infection.Higher expression of GSR indicated poor prognosis in gastric cancer.GSEA indicated that GSR regulates gene sets associated with oxidative phosphorylation,metabolism of nucleotides,mitochondrial protein import,and mitotic G1 S phases.Conclusions GSR can be used as an indicator to predict the prognosis of gastric cancer patients and a target for the treatment of gastric cancer.
7.Construction and expression of anti-glioma single-chain Fv/human tumor necrosis factor α fusion gene

Aim To construct and express the fused gene of anti-glioma single chain Fv(39ScFv)/ human tumor necrosis factor α (TNF-α ). Methods The mature peptide cDNA of TNF-α was fused to 3′ -terminus of ScFv gene. The 39ScFv-TNFα fusion gene was cloned into expression vector pET20b(+ ) and induced to express in E.coli. Results The fusion protein expressed in E.coli account for 20% of the total bacterial protein. It displayed a single band of Mr 44 000 by reducing SDS-PAGE and Western blot, and retained both of its anti-glioma immunoreactivity and biological activity of TNF. Conclusion The fusion gene was constructed and expressed successfully in E.coli. The fusion protein possessed bifunctional activities, it may prove useful in targeting immunotherapy against glioma.

Objective To study the clinical significance of pulmonary membrane diffusing capacity(Dm) and pulmonary capillary blood volume(Vc) in patients with stable chronic obstructive pulmonary disease(COPD). Methods Spirometry was performed in 38 patients with stable COPD and 35 healthy individuals in resting condition.The changes of pulmonary parameters were obtained and compared between groups. 9.Protection of Electro-acupuncture for Gastric Mucosa of Patients Undergoing Endoscopic Sinus Surgery.

OBJECTIVETo evaluate the effect of electro-acupuncture (EA) on gastric mucosal oxygenation and systemic inflammatory response in patients undergoing endoscopic sinus surgery with controlled hypotension (CH), and to explore its protective effect on gastric mucosa.
METHODSFifty-four patients, 18-65 years old, grade I-II of American Society of Anesthesiology (ASA), who were scheduled for endoscopic sinus surgery were randomly assigned to two groups, group A (general anesthesia group) and group B (general anesthesia combined EA anesthesia group), 27 in each group. Conrolled hypotension was executed during operation, and mean arterial pressure (MAP) was maintained at 55-65 mmHg. After tracheal intubation gastric tesiometer catheter was indwelled through nasal cavity or oral cavity. After successful indwelling, it was connected with gastric mucosa monitoring mode of multifunctional parameters monitor. Patients' MAP and heart rate (HR), pHi, partial pressure of carbon dioxide (PgCO2), arterial partial pressure of carbon dioxide (Pg-aCO2) and endtidal pressure of carbon dioxide (Pg-etCO2) were measured and recorded at T, (immediately before induced hypotension), T, (20 min following induced hypotension to target MAP), T2 (40 min following induced hypotension to target MAP), T3 (20 min after ending induced hypotension), and T4(40 min after ending induced hypotension). Blood samples were intravenously collected, TNF-alpha, IL-1, and IL-6 were detected by ELISA 24 h before operation, during operation (T3), and 24 h after operation.
RESULTSAfter hypotension was induced, Pg-CO2, Pg-aCO2 and Pg-etCO2 increased significantly (P < 0.01, P < 0.05), while pHi decreased significantly (P < 0.01) in both groups at T1-T4 than those at T0. During T1-T4, PgCO2, Pg-aCO2, and Pg-etCO2 were higher (P < 0.01, P < 0.05), while pHi was lower in group A than in group B (P < 0.01). Furthermore, TNF-alpha, IL-1, and IL-6 increased significantly in both groups during operation and 24 h after operation, when compared with those 24 h before operation (P < 0.01, P < 0.05). TNF-alpha and IL-1 in group A were higher than those in group B (P < 0.05) during operation and 24 h after operation, but with no significant difference in the plasma concentration of IL-6 (P > 0.05).
CONCLUSIONEA exerted obvious protective effect of gastric mucosal injury in endoscopic sinus surgery with controlled hypotension, which might be achieved by increasing gastric mucosal blood flow, maintaining oxygen supply and demand, inhibiting inflammatory response, and alleviating injury of gastric mucosal barrier.

10.Developmental status and prospect of musical electroacupuncture.

Through searching domestic and foreign medical journals in CNKI, Wanfang database, VIP database and Pubmed database from January of 2003 to November of 2013, 39 articles regarding musical electroacupuncture (MEA) were analyzed. The result showed that MEA was clinically used to treat neurological and psychotic disorders; because it was combined with musical therapy and overcame the acupuncture tolerability, and MEA was superior to traditional electroacupuncture. However, problems such as low research efficiency and the mechanism of MEA superiority and the musical specificity not being revealed by research design still exist. In future, large-sample multi-center RCT researches should be performed to clarify MEA clinical efficacy. With modern science and technology and optimized study design, guided by five-element theory of TCM, researches on different musical elements and characteristics of musical pulse current as well as MEA's correlation with meridians and organs should be studied, so as to make a further exploration on MEA mechanisms and broaden the range of its clinical application.
