1.Modified Soave procedure for Hirschsprung's disease in neonate and infant

Objective To evaluate modified Soave procedure for the treatment of Hirschsprung's disease for the neonate and infant. Methods Seventeen cases of short and sigmoid types of Hirschsprung's disease were treated by transanal modified Soave procedure. The incision was made 1cm posterioly and 2-3 cm anterioly above the dental line; the dissection progressed from the submucosa layer up to the perineal reflex into the pelvic cavity, and the rectum was pulled through. "V" shap muscular cuff was excised on the posterior wall. The proximal colon was anastomosed to the anal mucosa. Results Mean operative time was 160 min ? 45 min, mean blood loss was 45 ml ?35 ml. No enterocolitis, anastomotic leak, perianal infection and anastomotic stenosis occurred. All patients recovered with normal defecation, and no complication was observed from a follow-up of more than 4 mos. Conclusion Modified Soave procedure is safe and effective for the treatment of Hirschsprung's disease in the neonate and infant.
2.Influence of low calcium dialysate and midodrine hydrochloride on blood pressure in hemodialysis patients

Objective To investigate the influence of low calcium dialysate (DCa1.25) and midodrine hydrochloric (MHC) on blood pressure in hemodialysis patients. Methods Dialysate calcium concentration was changed from 1.5% (DCa1.5) to 1.25% in patients with hypercalcaemia pre- or post-dialysis.For patients with intradialytic hypotension(IDH), pre-dialysis antihypertensive drugs were ceased.If that didn′t work, MHC 2.5 or 5 mg was administered to them 30 minutes before dialysis were ceased.MHC was also administered to patients who had not taken antihypertensive drugs. The blood pressure (BP) and blood volume were recorded during dialysis. UCG and autonomic nerves function test including BP supine and standing test and sustained hand-grip test were measured as well. Results Twenty-one hemodialysis patients were involved in this study including male 9 and female 12. The average age was (54.4?14.2) years old,the time on dialysis (33.04?30.1) months. When DCa1.5 was changed to DCa1.25, 9 cases (42.9%) could maintain stable BP, but IDH occurred in 10 patients(47.6%) with symptoms such as swirl,sweat or cramp, one with lower extremities cramp and one with heart discomfort but without IDH. 3.Expressions of hepatocyte nuclear factor -1α and hepatocyte nuclear factor -4α in human hepatocellular carcinoma

Objective To investigate the expression of hepatocyte nuclear factor-1α (HNF-1α) and hepatocyte nuclear factor-4α (HNF-4α) in human hepatocellular carcinoma (HCC) and explore the function of HNF-1α and HNF-4α during HCC carcinogenesis and development. Methods Twenty-six specimens of hepatocellular carcinoma were collected. The expressions of HNF-1α and HNF-4α in HCC tissues and adjacent non-cancerous tissues were detected by reverse transcription polymerase chain reaction (RT-PCR) and immunohistochemistry staining. Results The mRNA levels of HNF-1α and HNF-4α were significantly lower in HCC tissues than that in adjacent non-cancerous tissues (0.818±0.371 vs. 0.383±0.102 for HNF-1α, P<0.05;0.846±0.384 vs. 0.397±0.105 for HNF-4α, P<0.05).The positive rates of HNF-1α and HNF-4α protein were significantly lower in HCC tissues than in adjacent non-cancerous tissues (92.3% vs. 42.3% for HNF-1α, P<0.05;96.2% vs. 50.0% for HNF-4α, P<0.05). The mRNA and protein expressions of HNF-1α and HNF-4α were correlated with tumor differentiation (P<0.05). There was a negative correlation between HNF-1α and HNF-4α mRNA expressions in HCC tissues.Conclusion The expressions of HNF-1α and HNF-4α are down-regulated in HCC, which might be related to carcinogenesis and development of HCC.
4.Endovascular embolization for the management of cryptogenic massive hemoptysis in long-term smokers

ObjectiveTo determine the effectiveness of endovascular embolization in patients with cryptogenic massive hemoptysis who were all long-term smokers.Methods Aortography and subclavian artery angiography were performed in 21long-term smokers with cryptogenicmassive hemoptysis.Transarterial embolization (TAE) was performed in patients with detectable pathologic systemic arteries.The angiographic findings were reviewed and the clinical and follow-up CT results were observed.ResultsThe pathologic systemic arteries were all bronchial arteries (BAs) and thirty-five arteries were involved.The angiography demonstrated peripheral hyperplasia in all BAs,with 24 pathologic BAs supplying the right lung and 25 supplying the upper lobes.In thirty-five BAs,24 showed hypertrophy and 11 were normal.TAE of the pathologic BAs was successfully performed and cessation of bleeding was achieved in all patients.During follow-up,one patient had episodic bloody sputum after embolization and no recurrence in all patients.The follow-up CTdemonstratednoadditionalabnormalitybesidespre-existingpulmonaryemphysema.Conclusion Cryptogenic massive hemoptysis in long-term smokers efficiently treated by endovascular embolization of the responsible bronchial artery.
5.Retroperitoneal laparoscopic adrenalectomy in 66 cases

Objective To summarize the operative experience and to study the value and efficacy of retroperitoneal laparoseopie adrenaleetomy.Methods Clinical data of 66 cases undergoing retroperitoneal laparoscopic adrenalectomy in Yijishan Hospital were retrospectively reviewed.There were 22 cases with adrenal adenoms located at left side and 44 cases at fight side.Tumor diameter was from 1.2 to 6.4 cm,with 3.3 cm as the average.There were 36 cases of nonfunctional adrenal adenoma,13 cases of primary hyperaldosteronism,10 cases of adrenal pheochromocytoma,6 cases of adrenal myelolipoma,and 1 case of adrenal cyst.Results 4 cases were converted to open surgery while the other 62 cases were completed successfully.The operation duration was from 35 to 190 min,with 90 min as the average.Intraoperative blood loss was from 20 to 800 ml,with 70 ml as the average.5 cases received blood transfusion because of massive hemorrhage.All cases were followed up for 3to 36 months.None of them had long-term complications or recurrence.Conclusion Retroperitoneal laparoscopic adrenalectomy has advantages of safety,less trauma,short hospitalization time,and quick recovery,making it the flint choice for patients with benign adrenal neoplasms.

Objective To investigate the relationship between the polymorphisms of both aldose reductase (AR) and endothelial nitric oxide synthase (eNOS) genes in chromosome 7q35 and the susceptibility to diabetic nephropathy (DN) in type 2 diabetes of Han nationality in North China. Methods The C(-106)T in the promoter region of AR gene as well as the G894T in exon 7 and the 27 bp repeat polymorphism in intron 4 of the eNOS gene were investigated in 85 healthy controls and 134 type 2 diabetics with or without DN. The C(-106)T as well as the G894T genotype were determined by PCR RFLP method and sequencing the PCR products. The 27 bp repeat polymorphism alleles were determined by PCR combined with agarose gel electrophoresis and sequencing the PCR products. OBJECTIVETo compare the short-term clinical outcome between unilateral fixation fusion (ULF) and minimally invasive spine transforaminal lumbar interbody fusion (MIS-TLIF) in treating lumbar disc herniation (LDH).
METHODSThe clinical data of 39 patients with LDH were retrospectively analyzed from June 2008 to March 2013. There was 22 males and 17 females, aged from 45 to 75 years old with an average of 56.9 years. Therer were 3 cases in L3,4, 15 cases in L4,5, 21 cases in L5S1. Among them, 21 patients underwent unilateral fixation fusion (ULF group) and 18 underwent minimally invasive spine transforaminal lumbar interbody fusion (MIS-TLIF group). Operation time, blood loss, the times of radiographic exposure and hospital stay were noted and compared between two groups. Radiograph informations were regularily accessed and VAS, ODI scores were recorded at 3 days and 3, 6, 12 months after operation, respectively. According to modified Macnab criteria, the clinical effects were evaluated at final follow-up.
RESULTSAll operations were successful without severe complications. The averaged operative time and the times of radiographic exposure in ULF group [(95 ± 25) min and (4.2 ± 0.4) times] were less than that of MIS-TLIF group [(120 ± 35) min and (10.1 ± 3.9) times] (P < 0.05). But, the mean blood loss and hospital stay in MIS-TLIF group [(75 ± 45) ml and (7.2 ± 2.2)d ]were less than that of ULF group [(165 ± 60) ml and (11.0 ± 3.7) d] (P < 0.01). All patients were followed up from 12 to 45 months with an average of 29.5 months. The VAS and ODI score had significantly improved during the follow-up and no significant differences were found between two groups at the same time point (P > 0.05). The postoperative radiographs showed internal fixation position was good. And all patients obtained bone fusion by CT scan at 1 year after operation. There was no significant differences in modified Macnab criteria between two groups at the latest follow-up (P > 0.05).
CONCLUSIONFavorable short-term clinical effects can be achieved in suitable LDH patients with ULF or MIS-TLIF surgical procedures.

10.Cryoprecipitate for external use of anal fistula of postoperative wound repair affect clinical research

Objective To study the cryoprecipitate effect of local external treatment of anal fistula surgical wound repair.Methods A total of 95 cases of low simple anal fistula patients was chosen, and was divided into three groups according to random number table method.The cryoprecipitate group (n =32 cases) began with cryoprecipitate dressing (2 layer) even to fill in a wound, once per day.The Kangfuxin group (n =32 cases) used Kangfuxin solution.The exposed burn ointment (MEBO) group (n =31 cases) used MEBO to cover the wound.The healing time, postoperative wound healing rate, the granulation tissue growth, new epithelial coverage, and adverse reactions were observed.Results The cryoprecipitate group in 14 d and 21 d had postoperative wound healing rate (60.97 ±4.20)% and (97.69 ± 1.79)% ,wound healing time (21.31 ±2.08)days;it had higher healing rate and shorter healing times compared to other two groups (P <0.05).Wound repaired more neatly;and 14d and 21d after the operation, wound granulation tissue growth rate [(61.06 ± 13.29) % and (97.63 ± 2.81) %] and new epithelial coverage [(33.03 ±4.15)% and (94.97 ±4.36)%] were more than the control groups (P <0.05) with less adverse reactions relative to control groups.Conclusions The cryoprecipitate contained in the composition has a strong biological activity.It can promote the wound tissue cell growth, and promote the wound repair.
